Teaching English as a Global Language for Intercultural Communication in Higher Education
With the development of globalization and internationalization, teaching English language, as lingua franca has been shifted from developing communicative competence to intercultural communicative competence. In this aspect, this research paper aims to study the perceptions of students and teachers concerning using English language as a global language for intercultural communication. It bridges the gap between student's perceptions and teachers' practices to help them design activities that prepare EFL learners to communicate with English as a global language for intercultural communication. 60 high and university students shared in a survey that evaluates students' motives for learning English as a global Language. Moreover, 30 high school and university English teachers shared also in another survey that evaluates their practices in teaching English for Intercultural education. Then a comparative analysis between both views was done. The results of the research paper revealed that students mostly learn English to get better job, and they need to learn the basic words and expressions to communicate. Students also are equipped with intercultural skills as most of them affirm that learning another language is related to learning its culture. On the other hand, the perceptions of English language teachers revealed that their main aim is to help the students communicate effectively. They also asserted the role of intercultural communication is to help learners have good conversation with nonnative speakers. Therefore, this research paper affirmed the differences in attitudes between the students and teachers' perception which creates a problem in real practice of teaching English. Accordingly, the results of this research raise awareness of EFL teachers and curriculum designers to design intercommunicative tasks that help EFL learners achieve appropriate communication in the intercultural context
A Fuzzy-Based Context-Aware Misbehavior Detecting Scheme for Detecting Rogue Nodes in Vehicular Ad Hoc Network
A vehicular ad hoc network (VANET) is an emerging technology that improves road safety, traffic efficiency, and passenger comfort. VANETs’ applications rely on co-operativeness among vehicles by periodically sharing their context information, such as position speed and acceleration, among others, at a high rate due to high vehicles mobility. However, rogue nodes, which exploit the co-operativeness feature and share false messages, can disrupt the fundamental operations of any potential application and cause the loss of people’s lives and properties. Unfortunately, most of the current solutions cannot effectively detect rogue nodes due to the continuous context change and the inconsideration of dynamic data uncertainty during the identification. Although there are few context-aware solutions proposed for VANET, most of these solutions are data-centric. A vehicle is considered malicious if it shares false or inaccurate messages. Such a rule is fuzzy and not consistently accurate due to the dynamic uncertainty of the vehicular context, which leads to a poor detection rate. To this end, this study proposed a fuzzy-based context-aware detection model to improve the overall detection performance. A fuzzy inference system is constructed to evaluate the vehicles based on their generated information. The output of the proposed fuzzy inference system is used to build a dynamic context reference based on the proposed fuzzy inference system. Vehicles are classified into either honest or rogue nodes based on the deviation of their evaluation scores calculated using the proposed fuzzy inference system from the context reference. Extensive experiments were carried out to evaluate the proposed model. Results show that the proposed model outperforms the state-of-the-art models. It achieves a 7.88% improvement in the overall performance, while a 16.46% improvement is attained for detection rate compared to the state-of-the-art model. The proposed model can be used to evict the rogue nodes, and thus improve the safety and traffic efficiency of crewed or uncrewed vehicles designed for different environments, land, naval, or air
Saudi mothers' preferences about breaking bad news concerning newborns: a structured verbal questionnaire
<p>Abstract</p> <p>Background</p> <p>Breaking bad news (BBN) to parents whose newborn has a major disease is an ethical dilemma. In Saudi Arabia, BBN about newborns is performed according to the parental preferences that have been reported from non-Arabic/non-Islamic countries. Saudi mothers' preferences about BBN have not yet been studied. Therefore, we aimed to elicit the preferences of Saudi mothers about BBN concerning newborns.</p> <p>Methods</p> <p>We selected a convenience sample of 402 Saudi mothers, aged 18-50 years, who had no previous experience with BBN. We selected them via a simple number-randomization scheme from the premises of a level III Saudi hospital between October of 2009 and January of 2011. We used a hypothetical situation (BBN about trisomy 21) to elicit their preferences about BBN concerning newborns via a structured verbal questionnaire composed of 12 multiple-choice questions. We expressed their preferences as percentages (95% confidence interval), and we used the Kendall's W test (W) to assess the degree of agreement in preferences.</p> <p>Results</p> <p>The Saudi mothers preferred that BBN be conducted with both parents together (64% [60-69]), albeit with weak levels of agreement (W = 0.29). They showed moderate agreement in their preferences that BBN should be conducted early (79% [75-83], W = 0.48), in detail (81% [77-85], W = 0.52), in person (88% [85-91], W = 0.58), and in a quiet setting (86% [83-90], W = 0.53). With extremely weak agreement, they preferred to have a known person present for support during BBN (56% [51-61], W = 0.01), to have close bodily contact with their babies (66% [61-70], W = 0.10), and to have no another patients present (64% [59-68], W = 0.08). They showed moderate levels of agreement in their desires to detail, in advance, their preferences about process of BBN by giving a reversible, written informed consent that could be utilized for guidance, if needed (80% [76-84], W = 0.36).</p> <p>Conclusions</p> <p>In our experience, Saudi mothers' preferences about BBN concerning newborns are varied, suggesting that a "one-size-fits-all" approach is inappropriate. A reversible, written informed consent detailing their preferences about BBN that would be kept in their medical records and utilized for guidance, if needed, may be the best solution, given this level of diversity. These findings merit further study.</p
Isolation of filter passing bacteria from a range of dental clinic surfaces
Filter passing bacteria have been isolated from a variety of natural environments, appearing as a mixture of Gram-positive and Gram-negative, as well as nano-forms and wall-free species. In this study, filter passing bacteria were isolated from surfaces located in various dental departments at the College of Dentistry, King Saud University Hospital. Surface samples were obtained by using Q-tip swabs, with ten different surfaces being sampled in each clinic during pre-patient and post-patient visits. Filterable bacteria (using 0.4 and 0.2 micron filters, but not 0.1 micron filter) were isolated, being mainly Gram-positive cocci. Isolation results of filterable bacteria were compared before and after patient treatment in the clinic. More frequently, filter passing bacteria were isolated on clinic surfaces after patient treatment. The results show that dental settings are contaminated with filterable bacteria which may act as a reservoir for the wider contamination of hospital environments
Hepatoprotective Role of Carvedilol against Ischemic Hepatitis Associated with Acute Heart Failure via Targeting miRNA-17 and Mitochondrial Dynamics-Related Proteins: An In Vivo and In Silico Study
Acute heart failure (AHF) is one of the most common diseases in old age that can lead to mortality. Systemic hypoperfusion is associated with hepatic ischemia–reperfusion injury, which may be irreversible. Ischemic hepatitis due to AHF has been linked to the pathogenesis of liver damage. In the present study, we extensively investigated the role of mitochondrial dynamics-related proteins and their epigenetic regulation in ischemic liver injury following AHF and explored the possible hepatoprotective role of carvedilol. The biochemical analysis revealed that the ischemic liver injury following AHF significantly elevated the activity of alanine aminotransferase (ALT), aspartate aminotransferase (AST), and alkaline phosphatase (ALP) enzymes, the level of total and direct bilirubin, and the expression of hepatic mitogen-activated protein kinase (MAPK), dynamin-1-like protein (DNM1L), and hepatic miRNA-17. At the same time, it significantly reduced the serum albumin level, the activity of hepatic superoxide dismutase (SOD), and the expression of mitochondrial peroxisome proliferator-activated receptor-1α (PGC-1α), and mitofusin 2 (Mtf2). The histological examination of the liver tissue revealed degenerated hepatocytes. Interestingly, administration of carvedilol either prior to or after isoprenaline-induced AHF significantly improved the liver function and reversed the deterioration effect of AHF-induced ischemic hepatitis, as demonstrated by biochemical, immunohistochemical, and histological analysis. Our results indicated that the hepatoprotective effect of carvedilol in ameliorating hepatic ischemic damage could be attributed to its ability to target the mitochondrial dynamics-related proteins (Mtf2, DNM1L and PGC-1α), but also their epigenetic regulator miRNA-17. To further explore the mode of action of carvedilol, we have investigated, in silico, the ability of carvedilol to target dynamin-1-like protein and mitochondrial dynamics protein (MID51). Our results revealed that carvedilol has a high binding affinity (−14.83 kcal/mol) toward the binding pocket of DNM1L protein. Antimicrobial resistance among migrants in Europe: a systematic review and meta-analysis
BACKGROUND: Rates of antimicrobial resistance (AMR) are rising globally and there is concern that increased migration is contributing to the burden of antibiotic resistance in Europe. However, the effect of migration on the burden of AMR in Europe has not yet been comprehensively examined. Therefore, we did a systematic review and meta-analysis to identify and synthesise data for AMR carriage or infection in migrants to Europe to examine differences in patterns of AMR across migrant groups and in different settings. METHODS: For this systematic review and meta-analysis, we searched MEDLINE, Embase, PubMed, and Scopus with no language restrictions from Jan 1, 2000, to Jan 18, 2017, for primary data from observational studies reporting antibacterial resistance in common bacterial pathogens among migrants to 21 European Union-15 and European Economic Area countries. To be eligible for inclusion, studies had to report data on carriage or infection with laboratory-confirmed antibiotic-resistant organisms in migrant populations. We extracted data from eligible studies and assessed quality using piloted, standardised forms. We did not examine drug resistance in tuberculosis and excluded articles solely reporting on this parameter. We also excluded articles in which migrant status was determined by ethnicity, country of birth of participants' parents, or was not defined, and articles in which data were not disaggregated by migrant status. Outcomes were carriage of or infection with antibiotic-resistant organisms. We used random-effects models to calculate the pooled prevalence of each outcome. The study protocol is registered with PROSPERO, number CRD42016043681. FINDINGS: We identified 2274 articles, of which 23 observational studies reporting on antibiotic resistance in 2319 migrants were included. The pooled prevalence of any AMR carriage or AMR infection in migrants was 25·4% (95% CI 19·1-31·8; I2 =98%), including meticillin-resistant Staphylococcus aureus (7·8%, 4·8-10·7; I2 =92%) and antibiotic-resistant Gram-negative bacteria (27·2%, 17·6-36·8; I2 =94%). The pooled prevalence of any AMR carriage or infection was higher in refugees and asylum seekers (33·0%, 18·3-47·6; I2 =98%) than in other migrant groups (6·6%, 1·8-11·3; I2 =92%). The pooled prevalence of antibiotic-resistant organisms was slightly higher in high-migrant community settings (33·1%, 11·1-55·1; I2 =96%) than in migrants in hospitals (24·3%, 16·1-32·6; I2 =98%). We did not find evidence of high rates of transmission of AMR from migrant to host populations. INTERPRETATION: Migrants are exposed to conditions favouring the emergence of drug resistance during transit and in host countries in Europe. Increased antibiotic resistance among refugees and asylum seekers and in high-migrant community settings (such as refugee camps and detention facilities) highlights the need for improved living conditions, access to health care, and initiatives to facilitate detection of and appropriate high-quality treatment for antibiotic-resistant infections during transit and in host countries. Protocols for the prevention and control of infection and for antibiotic surveillance need to be integrated in all aspects of health care, which should be accessible for all migrant groups, and should target determinants of AMR before, during, and after migration. Elevated serum neutrophil elastase is related to prehypertension and airflow limitation in obese women
<p>Abstract</p> <p>Background</p> <p>Neutrophil elastase level/activity is elevated in a variety of diseases such as atherosclerosis, systolic hypertension and obstructive pulmonary disease. It is unknown whether obese individuals with prehypertension also have elevated neutrophil elastase, and if so, whether it has a deleterious effect on pulmonary function. Objectives: To determine neutrophil elastase levels in obese prehypertensive women and investigate correlations with pulmonary function tests.</p> <p>Methods</p> <p>Thirty obese prehypertensive women were compared with 30 obese normotensive subjects and 30 healthy controls. The study groups were matched for age. Measurements: The following were determined: body mass index, waist circumference, blood pressure, lipid profile, high sensitivity C-reactive protein, serum neutrophil elastase, and pulmonary function tests including forced expiratory volume in one second (FEV<sub>1</sub>), forced vital capacity (FVC) and FEV<sub>1</sub>/FVC ratio.</p> <p>Results</p> <p>Serum neutrophil elastase concentration was significantly higher in both prehypertensive (405.8 ± 111.6 ng/ml) and normotensive (336.5 ± 81.5 ng/ml) obese women than in control non-obese women (243.9 ± 23.9 ng/ml); the level was significantly higher in the prehypertensive than the normotensive obese women. FEV1, FVC and FEV1/FVC ratio in both prehypertensive and normotensive obese women were significantly lower than in normal controls, but there was no statistically significant difference between the prehypertensive and normotensive obese women. In prehypertensive obese women, there were significant positive correlations between neutrophil elastase and body mass index, waist circumference, systolic blood pressure, diastolic blood pressure, total cholesterol, triglyceride, low density lipoprotein cholesterol, high sensitivity C-reactive protein and negative correlations with high density lipoprotein cholesterol, FEV1, FVC and FEV1/FVC.</p> <p>Conclusion</p> <p>Neutrophil elastase concentration is elevated in obese prehypertensive women along with an increase in high sensitivity C-reactive protein which may account for dyslipidemia and airflow dysfunction in the present study population.</p
