    Quantum Trajectory Analysis of the Two-Mode Three-Level Atom Microlaser

    We consider a single atom laser (microlaser) operating on three-level atoms interacting with a two-mode cavity. The quantum statistical properties of the cavity field at steady state are investigated by the quantum trajectory method which is a Monte Carlo simulation applied to open quantum systems. It is found that a steady state solution exists even when the detailed balance condition is not guaranteed. The differences between a single mode microlaser and a two-mode microlaser are highlighted. The second-order correlation function g^2(T) of a single mode is studied and special attention is paid to the one-photon trapping state, for which a simple formula is derived for its correlation function. We show the effects of the velocity spread of the atoms used to pump the microlaser cavity on the second-order correlation function, trapping states, and phase transitions of the cavity field

    Preventive effect of the flavonoid, quercetin, on hepatic cancer in rats via oxidant/antioxidant activity: molecular and histological evidences

    <p>Abstract</p> <p>Background</p> <p>The incidence of hepatocellular carcinoma is increasing in many countries. The estimated number of new cases annually is over 500,000, and the yearly incidence comprises between 2.5 and 7% of patients with liver cirrhosis. The incidence varies between different geographic areas, being higher in developing areas; males are predominantly affected, with a 2:3 male/female ratio</p> <p>Methods</p> <p>Experiments were designed to examine the effect of <it>N</it>-Nitrosodiethylamine (NDEA) as cancer-inducer compound and to confirm the preventive effect of the flavonoid quercetin on hepatocellular carcinoma in rats. Briefly, thirty six male albino rats of Wistar strain were divided into 3 groups: the 1<sup>st </sup>group was administered NDEA alone (NDEA-treated), the 2<sup>nd </sup>group was treated simultaneously with NDEA and quercetin (NDEA+Q) and the 3<sup>rd </sup>group was used as control (CON). Randomly amplified polymorphic DNA polymerase chain reaction (RAPD-PCR) as well as <it>p53</it>-specifi PCR assays were employed to determine genomic difference between treated, and control animals. Histological confirmation as well as oxidant/antioxidant status of the liver tissue was done.</p> <p>Results</p> <p>RAPD analysis of liver samples generated 8 monomorphic bands and 22 polymorphic bands in a total of 30-banded RAPD patterns. Cluster analysis and statistical analyses of RAPD data resulted in grouping control and NDEA+Q samples in the same group with 80% similarity cut-off value. NDEA-treated samples were clustered in a separate group. Specific PCR assay for polymorphism of <it>P</it><sup>53 </sup>gene revealed a uniform pattern of allele separation in both control and NDEA+Q samples. Quercetin anticancer effect was exhibited in significant decrease of oxidative stress and significant decrease of antioxidant activity. Histopathological studies showed normal liver histology of the NDEA+Q samples. Meanwhile, several cancer-induced features were clearly observable in NDEA-treated samples.</p> <p>Conclusion</p> <p>This paper demonstrated that preventive effect of quercetin on hepatocarcinoma in rats by RAPD-PCR, tracing the effect on <it>p53 </it>gene and by histopathological evidence. Hereby, it was proved that quercetin exerted its preventive effect via decreased oxidative stress and decreased antioxidant activity.</p

    Myomectomy for fibroids during cesarean section: A randomized controlled trial

    Background: There is a considerable debate about the management of myoma during cesarean section (CS). Recently, several studies indicated the safety and feasibility of undertaking myomectomy during CS.Objectives: To evaluate the safety, accessibility, and short-term morbidity of myomectomy for fibroids during cesarean section.Patients and Methods: This was a randomized controlled trial that included 72 patients who were admitted to the Obstetrics &amp; Gynecology Department, Menoufia University Hospital with uterine fibroids during pregnancy; who were randomly allocated equally into a group of cesarean myomectomy (CM; n=36) and another group of CS only (n=36). The operative events and the outcome were recorded and analyzed.Results: CM group showed a longer duration of surgery and longer hospital stay, higher amount of blood loss, and higher mean pain sores, with a highly statistically significant difference (p = 0.000). No cases in both groups required blood transfusion or ICU admission. No statistically significant differences were noted between both groups as regards the fetal outcome measures (p=0.583 &amp; 0.601).Conclusion: CM is safe and applicable in selected cases without deleterious maternal complications. Special precautions ought to be paid during the procedure, particularly in the intramural type and with large fibroids

    Signatures of Chaos in Time Series Generated by Many-Spin Systems at High Temperatures

    Extracting reliable indicators of chaos from a single experimental time series is a challenging task, in particular, for systems with many degrees of freedom. The techniques available for this purpose often require unachievably long time series. In this paper, we explore a new method of discriminating chaotic from multi-periodic integrable motion in many-particle systems. The applicability of this method is supported by our numerical simulations of the dynamics of classical spin lattices at high temperatures. We compared chaotic and nonchaotic regimes of these lattices and investigated the transition between the two. The method is based on analyzing higher-order time derivatives of the time series of a macroscopic observable --- the total magnetization of the spin lattice. We exploit the fact that power spectra of the magnetization time series generated by chaotic spin lattices exhibit exponential high-frequency tails, while, for the integrable spin lattices, the power spectra are terminated in a non-exponential way. We have also demonstrated the applicability limits of the above method by investigating the high-frequency tails of the power spectra generated by quantum spin lattices and by the classical Toda lattice.Comment: This is the final version accepted for publication: 12 pages, 14 figures.     Shisha smokers’ determinants of use, knowledge and perception towards shisha (waterpipe) smoking in Penang, Malaysia

    Background: Tobacco smoking is a serious health problem worldwide. Malaysia as many of south Asian countries faces the challenge of rising tobacco consumption. Despite the apparent spread of Shisha smoking phenomenon among Malaysian population, little is known about the safety concerns and determinants of Shisha smokers in this community. Objectives: This study aimed to determine the social determinants of Shisha smoking among Malaysian population in Penang Island, Malaysia and to explore their health related knowledge and perception toward Shisha smoking. Methods: The survey was conducted using simple random sampling by randomly distributing self-administered questionnaires to consumers in Shisha lounges located in Penang Island. Statistical analyses were performed using the Statistical Package for Social Science (SPSS) version 20. Results: A total number of 171 (42.75%) of respondents participated in this study. Mean age was 21.5±4.4 years. The majority were male, Malay (79.6%, 63.1% respectively). About 97.1% of respondents smoke Shisha either alone or with tobacco cigarettes. The majority (63.1%) started Shisha smoking at an age ≤ 20 years old, always in a café' (72.2%), share Shisha smoking with others (81.6%). Only 35.9% of respondents reported smoking Shisha daily. 71% reported smoking ≤ 7 bowls per week. 50.5% claimed smoking Shisha for more than 60 minutes per session. Boredom, outing and meeting with friends and family ranked first among the motives for smoking Shisha. Regarding knowledge about Shisha smoking, the majority believes that cigarettes are more harmful, more addictive and has more nicotine than waterpipe (60.2%, 71.8% and 67%; respectively). A large proportion (66%) of respondents believes that Shisha smoking doesn't transmit hepatitis infection. The majority of respondents perceived Shisha smoking acceptable by society and parents, compared to cigarettes (67% and 60.2%; respectively). The vast majority of respondents believe that Shisha smoking relieves stress and tension (77.7%), while the minority believes that Shisha smokers have more friends than non-smokers. Regarding access to Shisha, half of respondents agreed that Shisha is easily accessible than cigarettes. The majority of respondent foresees increasing popularity of Shisha smoking in the next 5 years (83.5%). Conclusion: This study showed that there is a high prevalence of Shisha smoking among the Malaysian youth associated with false believes about its health effects. Further studies involving larger population are encouraged

    Semantic segmentation of microbial alterations based on SegFormer

    IntroductionPrecise semantic segmentation of microbial alterations is paramount for their evaluation and treatment. This study focuses on harnessing the SegFormer segmentation model for precise semantic segmentation of strawberry diseases, aiming to improve disease detection accuracy under natural acquisition conditions.MethodsThree distinct Mix Transformer encoders - MiT-B0, MiT-B3, and MiT-B5 - were thoroughly analyzed to enhance disease detection, targeting diseases such as Angular leaf spot, Anthracnose rot, Blossom blight, Gray mold, Leaf spot, Powdery mildew on fruit, and Powdery mildew on leaves. The dataset consisted of 2,450 raw images, expanded to 4,574 augmented images. The Segment Anything Model integrated into the Roboflow annotation tool facilitated efficient annotation and dataset preparation.ResultsThe results reveal that MiT-B0 demonstrates balanced but slightly overfitting behavior, MiT-B3 adapts rapidly with consistent training and validation performance, and MiT-B5 offers efficient learning with occasional fluctuations, providing robust performance. MiT-B3 and MiT-B5 consistently outperformed MiT-B0 across disease types, with MiT-B5 achieving the most precise segmentation in general.DiscussionThe findings provide key insights for researchers to select the most suitable encoder for disease detection applications, propelling the field forward for further investigation. The success in strawberry disease analysis suggests potential for extending this approach to other crops and diseases, paving the way for future research and interdisciplinary collaboration

    Biliary fistula and late recurrence of liver hydatid cyst: Role of cysto-biliary communication: A prospective multicenter study

    Background: Hydatid cyst disease (HCD) is common in certain locations. Surgery is associated with postoperative biliary fistula (POBF) and recurrence. The primary aim of this study was to identify whether occult cysto-biliary communication (CBC) can predict recurrent HCD. The secondary aim was to assess the role of cystic fluid bilirubin and alkaline phosphatase (ALP) levels in predicting POBF and recurrent HCD. Aim: To identify whether occult CBC can predict recurrent HCD. The secondary aim was to assess the role of cystic fluid bilirubin and ALP levels in predicting POBF and recurrent HCD. Methods: From September 2010 to September 2016, a prospective multicenter study was undertaken involving 244 patients with solitary primary superficial stage cystic echinococcosis 2 and cystic echinococcosis 3b HCD who underwent laparoscopic partial cystectomy with omentoplasty. Univariable logistic regression analysis assessed independent factors determining biliary complications and recurrence. Results: There was a highly statistically significant association (P ≤ 0.001) between cystic fluid biochemical indices and the development of biliary complications (of 16 patients with POBF, 15 patients had high cyst fluid bilirubin and ALP levels), where patients with high bilirubin-ALP levels were 3405 times more likely to have biliary complications. There was a highly statistically significant association (P ≤ 0.001) between biliary complications, biochemical indices, and the occurrence of recurrent HCD (of 30 patients with recurrent HCD, 15 patients had high cyst fluid bilirubin and ALP; all 16 patients who had POBF later developed recurrent HCD), where patients who developed biliary complications and high bilirubin-ALP were 244.6 and 214 times more likely to have recurrent hydatid cysts, respectively.     Antimicrobial resistance among migrants in Europe: a systematic review and meta-analysis

    BACKGROUND: Rates of antimicrobial resistance (AMR) are rising globally and there is concern that increased migration is contributing to the burden of antibiotic resistance in Europe. However, the effect of migration on the burden of AMR in Europe has not yet been comprehensively examined. Therefore, we did a systematic review and meta-analysis to identify and synthesise data for AMR carriage or infection in migrants to Europe to examine differences in patterns of AMR across migrant groups and in different settings. METHODS: For this systematic review and meta-analysis, we searched MEDLINE, Embase, PubMed, and Scopus with no language restrictions from Jan 1, 2000, to Jan 18, 2017, for primary data from observational studies reporting antibacterial resistance in common bacterial pathogens among migrants to 21 European Union-15 and European Economic Area countries. To be eligible for inclusion, studies had to report data on carriage or infection with laboratory-confirmed antibiotic-resistant organisms in migrant populations. We extracted data from eligible studies and assessed quality using piloted, standardised forms. We did not examine drug resistance in tuberculosis and excluded articles solely reporting on this parameter. We also excluded articles in which migrant status was determined by ethnicity, country of birth of participants' parents, or was not defined, and articles in which data were not disaggregated by migrant status. Outcomes were carriage of or infection with antibiotic-resistant organisms. We used random-effects models to calculate the pooled prevalence of each outcome. The study protocol is registered with PROSPERO, number CRD42016043681. FINDINGS: We identified 2274 articles, of which 23 observational studies reporting on antibiotic resistance in 2319 migrants were included. The pooled prevalence of any AMR carriage or AMR infection in migrants was 25·4% (95% CI 19·1-31·8; I2 =98%), including meticillin-resistant Staphylococcus aureus (7·8%, 4·8-10·7; I2 =92%) and antibiotic-resistant Gram-negative bacteria (27·2%, 17·6-36·8; I2 =94%). The pooled prevalence of any AMR carriage or infection was higher in refugees and asylum seekers (33·0%, 18·3-47·6; I2 =98%) than in other migrant groups (6·6%, 1·8-11·3; I2 =92%). The pooled prevalence of antibiotic-resistant organisms was slightly higher in high-migrant community settings (33·1%, 11·1-55·1; I2 =96%) than in migrants in hospitals (24·3%, 16·1-32·6; I2 =98%). We did not find evidence of high rates of transmission of AMR from migrant to host populations. INTERPRETATION: Migrants are exposed to conditions favouring the emergence of drug resistance during transit and in host countries in Europe. Increased antibiotic resistance among refugees and asylum seekers and in high-migrant community settings (such as refugee camps and detention facilities) highlights the need for improved living conditions, access to health care, and initiatives to facilitate detection of and appropriate high-quality treatment for antibiotic-resistant infections during transit and in host countries. Protocols for the prevention and control of infection and for antibiotic surveillance need to be integrated in all aspects of health care, which should be accessible for all migrant groups, and should target determinants of AMR before, during, and after migration.     Global economic burden of unmet surgical need for appendicitis

    Background: There is a substantial gap in provision of adequate surgical care in many low-and middle-income countries. This study aimed to identify the economic burden of unmet surgical need for the common condition of appendicitis. Methods: Data on the incidence of appendicitis from 170 countries and two different approaches were used to estimate numbers of patients who do not receive surgery: as a fixed proportion of the total unmet surgical need per country (approach 1); and based on country income status (approach 2). Indirect costs with current levels of access and local quality, and those if quality were at the standards of high-income countries, were estimated. A human capital approach was applied, focusing on the economic burden resulting from premature death and absenteeism. Results: Excess mortality was 4185 per 100 000 cases of appendicitis using approach 1 and 3448 per 100 000 using approach 2. The economic burden of continuing current levels of access and local quality was US 92492millionusingapproach1and92 492 million using approach 1 and 73 141 million using approach 2. The economic burden of not providing surgical care to the standards of high-income countries was 95004millionusingapproach1and95 004 million using approach 1 and 75 666 million using approach 2. The largest share of these costs resulted from premature death (97.7 per cent) and lack of access (97.0 per cent) in contrast to lack of quality. Conclusion: For a comparatively non-complex emergency condition such as appendicitis, increasing access to care should be prioritized. Although improving quality of care should not be neglected, increasing provision of care at current standards could reduce societal costs substantially
