    Epidemiologie, prevence a léčba kolorektálního karcinomu dle dostupných českých a mezinárodních dat

    Nádory tlustého střeva a konečníku patří mezi nejčastější onkologické diagnózy. V České republice je každoročně nově diagnostikováno téměř 8 500 pacientů s kolorektálním karcinomem a přibližně 3 900 osob tomuto onemocnění každým rokem podlehne. Celková prevalence přesáhne v roce 2012 hranici 55 000 osob. Společenské, etické i ekonomické důsledky vyplývající z takto vysoké zátěže jsou zřejmé. Publikace „Epidemiologie, prevence a léčba kolorektálního karcinomu dle dostupných českých a mezinárodních dat“ přináší ucelený přehled epidemiologické a léčebné zátěže české populace touto chorobou a věnuje se krátkodobým a dlouhodobým predikcím dalšího vývoje. Situace je rovněž hodnocena v mezinárodním srovnání, neboť v hodnotách incidence a mortality kolorektálního karcinomu obsazuje ČR přední příčky evropských i světových statistik. Z mezinárodních studií však také vyplývá pozitivní poznatek, že většině nádorů tlustého střeva a konečníku lze předejít účinnou prevencí. Kromě primární prevence je zde hlavním nástrojem organizovaný populační screening, který je založen na testech krvácení do stolice a na kolonoskopii. Otázkám prevence a výsledkům screeningu kolorektálního karcinomu jsou v publikaci věnovány zvláštní kapitoly. Ačkoli nejnovější data dokládají rostoucí výkonnost českého screeningu, celkově dosažené pokrytí populace bohužel stále není dostatečné. To platí pro včasný záchyt kolorektálního karcinomu obecně, neboť v ČR je setrvale téměř 50 % nových onemocnění diagnostikováno v pokročilých klinických stadiích, a tedy s výrazně sníženou šancí na vyléčení nebo dlouhodobé přežití pacienta. Publikace rovněž hodnotí přežití dosahované u českých pacientů jakožto zásadní ukazatel výsledků léčebné péče, u kterého v posledních 15 letech zaznamenáváme statisticky významné zlepšení. Pravděpodobnost 5letého relativního přežití je u včasně diagnostikovaného kolorektálního karcinomu v klinickém stadiu i vyšší než 88%. Tato čísla opět potvrzují nutnost účinné prevence a posílení screeningu tohoto preventabilního onemocnění. V tomto směru má publikace ambici informovat odbornou i laickou veřejnost, a proto věnuje značný prostor přehledu dostupných informačních zdrojů.The book aims to inform expert and general public about epidemiology of colorectal carcinoma in the Czech Republic and priorities resulting from high population burden for prevention, diagnostics, and treatment of this severe disease.     Performance of the CMS Cathode Strip Chambers with Cosmic Rays

    The Cathode Strip Chambers (CSCs) constitute the primary muon tracking device in the CMS endcaps. Their performance has been evaluated using data taken during a cosmic ray run in fall 2008. Measured noise levels are low, with the number of noisy channels well below 1%. Coordinate resolution was measured for all types of chambers, and fall in the range 47 microns to 243 microns. The efficiencies for local charged track triggers, for hit and for segments reconstruction were measured, and are above 99%. The timing resolution per layer is approximately 5 ns

    The Feasibility and Impact of Delivering a Mind-Body Intervention in a Virtual World

    Introduction: Mind-body medical approaches may ameliorate chronic disease. Stress reduction is particularly helpful, but face-to-face delivery systems cannot reach all those who might benefit. An online, 3-dimensional virtual world may be able to support the rich interpersonal interactions required of this approach. In this pilot study, we explore the feasibility of translating a face-to-face stress reduction program into an online virtual setting and estimate the effect size of the intervention. Methods and Findings: Domain experts in virtual world technology joined with mind body practitioners to translate an existing 8 week relaxation response-based resiliency program into an 8-week virtual world-based program in Second Life™ (SL). Twenty-four healthy volunteers with at least one month's experience in SL completed the program. Each subject filled out the Perceived Stress Scale (PSS) and the Symptom Checklist 90- Revised (SCL-90-R) before and after taking part. Participants took part in one of 3 groups of about 10 subjects. The participants found the program to be helpful and enjoyable. Many reported that the virtual environment was an excellent substitute for the preferred face-to-face approach. On quantitative measures, there was a general trend toward decreased perceived stress, (15.7 to 15.0), symptoms of depression, (57.6 to 57.0) and anxiety (56.8 to 54.8). There was a significant decrease of 2.8 points on the SCL-90-R Global Severity Index (p<0.05). Conclusions: This pilot project showed that it is feasible to deliver a typical mind-body medical intervention through a virtual environment and that it is well received. Moreover, the small reduction in psychological distress suggests further research is warranted. Based on the data collected for this project, a randomized trial with less than 50 subjects would be appropriately powered if perceived stress is the primary outcome

    Performance and Operation of the CMS Electromagnetic Calorimeter

    The operation and general performance of the CMS electromagnetic calorimeter using cosmic-ray muons are described. These muons were recorded after the closure of the CMS detector in late 2008. The calorimeter is made of lead tungstate crystals and the overall status of the 75848 channels corresponding to the barrel and endcap detectors is reported. The stability of crucial operational parameters, such as high voltage, temperature and electronic noise, is summarised and the performance of the light monitoring system is presented

    JWST-TST DREAMS: Quartz Clouds in the Atmosphere of WASP-17b

    Clouds are prevalent in many of the exoplanet atmospheres that have been observed to date. For transiting exoplanets, we know if clouds are present because they mute spectral features and cause wavelength-dependent scattering. While the exact composition of these clouds is largely unknown, this information is vital to understanding the chemistry and energy budget of planetary atmospheres. In this work, we observe one transit of the hot Jupiter WASP-17b with JWST's MIRI LRS and generate a transmission spectrum from 5-12 μ\rm{\mu}m. These wavelengths allow us to probe absorption due to the vibrational modes of various predicted cloud species. Our transmission spectrum shows additional opacity centered at 8.6 μ\rm{\mu}m, and detailed atmospheric modeling and retrievals identify this feature as SiO2_2(s) (quartz) clouds. The SiO2_2(s) clouds model is preferred at 3.5-4.2σ\sigma versus a cloud-free model and at 2.6σ\sigma versus a generic aerosol prescription. We find the SiO2_2(s) clouds are comprised of small 0.01{\sim}0.01 μ\rm{\mu}m particles, which extend to high altitudes in the atmosphere. The atmosphere also shows a depletion of H2_2O, a finding consistent with the formation of high-temperature aerosols from oxygen-rich species.     Characteristics of contralateral carcinomas in patients with differentiated thyroid cancer larger than 1 cm

    Purpose: Traditionally, total thyroidectomy has been advocated for patients with tumors larger than 1 cm. However, according to the ATA and NCCN guidelines (2015, USA), patients with tumors up to 4 cm are now eligible for lobectomy. A rationale for adhering to total thyroidectomy might be the presence of contralateral carcinomas. The purpose of this study was to describe the characteristics of contralateral carcinomas in patients with differentiated thyroid cancer (DTC) larger than 1 cm. Methods: A retrospective study was performed including patients from 17 centers in 5 countries. Adults diagnosed with DTC stage T1b-T3 N0-1a M0 who all underwent a total thyroidectomy were included.     Growth factor concentrations and their placental mRNA expression are modulated in gestational diabetes mellitus: possible interactions with macrosomia

    <p>Abstract</p> <p>Background</p> <p>Gestational diabetes mellitus (GDM) is a form of diabetes that occurs during pregnancy. GDM is a well known risk factor for foetal overgrowth, termed macrosomia which is influenced by maternal hypergycemia and endocrine status through placental circulation. The study was undertaken to investigate the implication of growth factors and their receptors in GDM and macrosomia, and to discuss the role of the materno-foeto-placental axis in the <it>in-utero </it>regulation of foetal growth.</p> <p>Methods</p> <p>30 women with GDM and their 30 macrosomic babies (4.75 ± 0.15 kg), and 30 healthy age-matched pregnant women and their 30 newborns (3.50 ± 0.10 kg) were recruited in the present study. Serum concentrations of GH and growth factors, <it>i.e</it>., IGF-I, IGF-BP3, FGF-2, EGF and PDGF-B were determined by ELISA. The expression of mRNA encoding for GH, IGF-I, IGF-BP3, FGF-2, PDGF-B and EGF, and their receptors, <it>i.e</it>., GHR, IGF-IR, FGF-2R, EGFR and PDGFR-β were quantified by using RT-qPCR.</p> <p>Results</p> <p>The serum concentrations of IGF-I, IGF-BP3, EGF, FGF-2 and PDGF-B were higher in GDM women and their macrosomic babies as compared to their respective controls. The placental mRNA expression of the growth factors was either upregulated (FGF-2 or PDGF-B) or remained unaltered (IGF-I and EGF) in the placenta of GDM women. The mRNA expression of three growth factor receptors, <it>i.e</it>., IGF-IR, EGFR and PDGFR-β, was upregulated in the placenta of GDM women. Interestingly, serum concentrations of GH were downregulated in the GDM women and their macrosomic offspring. Besides, the expression of mRNAs encoding for GHR was higher, but that encoding for GH was lower, in the placenta of GDM women than control women.</p> <p>Conclusions</p> <p>Our results demonstrate that growth factors might be implicated in GDM and, in part, in the pathology of macrosomia via materno-foeto-placental axis.</p
