    DA-CHENG-QI DECOCTION, A TRADITIONAL CHINESE HERBAL FORMULA, FOR INTESTINAL OBSTRUCTION: SYSTEMATIC REVIEW AND META-ANALYSIS

    Background: This study was aimed at determining the effects and safety of Da-Cheng-Qi decoction (DCQD) or DCQD combined with conservative therapy in patients with intestinal obstruction. Materials and Methods: PubMed, EMBASE, Cochrane Controlled Trials Register, and several other databases were searched. Randomised controlled trials (RCTs) of DCQD or DCQD plus conservative therapy in patients with intestinal obstruction were eligible. Therapeutic effect was estimated by the improvement of clinical manifestations and diagnostic imaging; dichotomous/ordinal data assessment of overall response to therapy, adverse effects; or continuous variable were identified, including time to first bowel movement, time to first flatus, length of hospital stay. Results: Sixty eligible RCTs including 6,095 patients were identified. Response rate: (1) DCQD versus conservative therapy (6 RCTs, 361 patients, RR of respond =1.13; 95% CI 0.97 to 1.31). (2) DCQD plus conservative therapy versus conservative therapy (48 RCTs, 4,916 patients, RR of respond =1.25 which favoured DCQD plus conservative therapy; 95% CI 1.20 to 1.30). Treatment effect remained similar when RCTs at high risk of bias were excluded. Time to first flatus postoperatively: (1) DCQD versus conservative therapy (2 RCTs, 240 patients, SMD=-3.65; 95% CI -8.17 to 0.87). (2) DCQD plus conservative therapy versus conservative therapy (11 RCTs, 1,040 patients, SMD=-2.09 which favoured DCQD plus conservative therapy; 95% CI -3.04 to -1.15). Conclusion: DCQD combined with conservative therapy may increase the success rate of conservative therapy for intestinal obstruction significantly and can shorten the duration of postoperative ileus in patients undergoing abdominal surgery compared with conservative therapy alone

    Optimized antimicrobial and antiproliferative activities of titanate nanofibers containing silver

    Titanate nanofibers containing silver have been demonstrated through the experiments reported herein to have effective antifungal and antiproliferative activities in the presence of UV light. The titanate nanofibers containing silver can be fabricated by means of ion exchange followed by a topochemical process in an environment suitable for reductive reactions. Excellent antibacterial, antifungal, and antiproliferative activities could be demonstrated by both Ag2Ti5O11 · xH2O and Ag/titanate (UV light irradiation) due to their unique structures and compositions, which have photocatalytic activities to generate reactive oxygen species and capabilities to continuously release the silver ions. Therefore these materials have the potential to produce a membrane for the treatment of superficial malignant tumor, esophageal cancer, or cervical carcinoma. They may also hold utility if incorporated into a coating on stents in moderate and advanced stage esophageal carcinoma or for endoscopic retrograde biliary drainage. These approaches may significantly reduce infections, inhibit tumor growth, and importantly, improve quality of life and prolong survival time for patients with tumors

    In vitro drug release behavior from a novel thermosensitive composite hydrogel based on Pluronic f127 and poly(ethylene glycol)-poly(Δ-caprolactone)-poly(ethylene glycol) copolymer

    <p>Abstract</p> <p>Background</p> <p>Most conventional methods for delivering chemotherapeutic agents fail to achieve therapeutic concentrations of drugs, despite reaching toxic systemic levels. Novel controlled drug delivery systems are designed to deliver drugs at predetermined rates for predefined periods at the target organ and overcome the shortcomings of conventional drug formulations therefore could diminish the side effects and improve the life quality of the patients. Thus, a suitable controlled drug delivery system is extremely important for chemotherapy.</p> <p>Results</p> <p>A novel biodegradable thermosensitive composite hydrogel, based on poly(ethylene glycol)-poly(Δ-caprolactone)-poly(ethylene glycol) (PEG-PCL-PEG, PECE) and Pluronic F127 copolymer, was successfully prepared in this work, which underwent thermosensitive sol-gel-sol transition. And it was flowing sol at ambient temperature but became non-flowing gel at body temperature. By varying the composition, sol-gel-sol transition and <it>in vitro </it>drug release behavior of the composite hydrogel could be adjusted. Cytotoxicity of the composite hydrogel was conducted by cell viability assay using human HEK293 cells. The 293 cell viability of composite hydrogel copolymers were yet higher than 71.4%, even when the input copolymers were 500 Όg per well. Vitamin B<sub>12 </sub>(VB<sub>12</sub>), honokiol (HK), and bovine serum albumin (BSA) were used as model drugs to investigate the <it>in vitro </it>release behavior of hydrophilic small molecular drug, hydrophobic small molecular drug, and protein drug from the composite hydrogel respectively. All the above-mentioned drugs in this work could be released slowly from composite hydrogel in an extended period. Chemical composition of composite hydrogel, initial drug loading, and hydrogel concentration substantially affected the drug release behavior. The higher Pluronic F127 content, lower initial drug loading amount, or lower hydrogel concentration resulted in higher cumulative release rate.</p> <p>Conclusion</p> <p>The results showed that composite hydrogel prepared in this paper were biocompatible with low cell cytotoxicity, and the drugs in this work could be released slowly from composite hydrogel in an extended period, which suggested that the composite hydrogel might have great potential applications in biomedical fields.</p

    Prognostic value of oxygen inhalation therapy for simple nocturnal hypoxemia in COPD: a meta-analysis

    Objective: The effect of oxygen therapy on the prognosis of chronic obstructive pulmonary disease (COPD) with nocturnal hypoxemia (NOD) has been controversial. Therefore, this study systematically evaluated the relevant literature and included it into randomized controlled studies for meta-analysis to evaluate the efficacy and prognosis.Methods: We searched PubMed, EMBASE, web of science, Cochrane, China HowNet and Wanfang database for the literature on the prognosis of COPD patients with simple NOD from the establishment of the database to 30 June 2022. The outcome indicators were death and aggravation of the disease. The efficacy evaluation measures were pulmonary function and arterial blood gas results. The publication bias and heterogeneity of the included studies were evaluated.Results: A total of 621 patients from 5 studies were included in this meta-analysis, and there was no publication bias in the included studies. The total mortality of long-term oxygen therapy (LTOT) in COPD patients with simple NOD in oxygen therapy group (RR = 1.04; 95% CI: 0.81–1.33, p = 0.77), mortality (RR = 0.87; 95% CI: 0.58–1.31, p = 0.50), risk of progression to LTOT events (RR = 1.07; 95% CI: 0.76–1.51, p = 0.71). PaO2 in patients with COPD and simple NOD in oxygen therapy group was higher than that in non-oxygen therapy group (mean difference (MD) = 13.47; 95% CI: 3.49–23.46, p = 0.008), the decrease of PaCO2 level was not statistically significant (MD = −10.05; 95% CI: −26.36-6.27, p = 0.23).Conclusion: Oxygen therapy can improve the prognosis of blood oxygen partial pressure in COPD patients with simple NOD, but oxygen therapy has no significant effect on the survival rate, controlling the progression of the disease to LTOT and reducing the partial pressure of carbon dioxide

    Endorsement of the CONSORT Statement by High-Impact Medical Journals in China: A Survey of Instructions for Authors and Published Papers

    The CONSORT Statement is a reporting guideline for authors when reporting randomized controlled trials (RCTs). It offers a standard way for authors to prepare RCT reports. It has been endorsed by many high-impact medical journals and by international editorial groups. This study was conducted to assess the endorsement of the CONSORT Statement by high-impact medical journals in China by reviewing their instructions for authors.A total of 200 medical journals were selected according to the Chinese Science and Technology Journal Citation Reports, 195 of which publish clinical research papers. Their instructions for authors were reviewed and all texts mentioning the CONSORT Statement or CONSORT extension papers were extracted. Any mention of the Uniform Requirements for Manuscripts Submitted to Biomedical Journals (URM) developed by the International Committee of Medical Journal Editors (ICMJE) or ‘clinical trial registration’ was also extracted. For journals endorsing the CONSORT Statement, their most recently published RCT reports were retrieved and evaluated to assess whether the journals have followed what the CONSORT Statement required. Out of the 195 medical journals publishing clinical research papers, only six (6/195, 3.08%) mentioned ‘CONSORT’ in their instructions for authors; out of the 200 medical journals surveyed, only 14 (14/200, 7.00%) mentioned ‘ICMJE’ or ‘URM’ in their instructions for authors, and another five journals stated in their instructions for authors that clinical trials should have trial registration numbers and that priority would be given to clinical trials which had been registered. Among the 62 RCT reports published in the six journals endorsing the CONSORT Statement, 20 (20/62, 32.26%) contained flow diagrams and only three (3/62, 4.84%) provided trial registration information.Medical journals in China endorsing either the CONSORT Statement or the ICMJE's URM constituted a small percentage of the total; all of these journals used ambiguous language regarding what was expected of authors

    Palaeoceanographic controls on spatial redox distribution over the Yangtze Platform during the Ediacaran–Cambrian transition

    The Ediacaran–Cambrian interval was an eventful transitional period, when dynamic interactions between the biosphere and its physical environment allowed the Earth System to cross into a new state, characterized by the presence of metazoans, more equable climates and more expansive oxygenation of the oceans. Due to the retreat of widespread sulphidic conditions, redox-sensitive trace-metals could accumulate to a greater extent in ‘black shales’ deposited in localised anoxic/euxinic environments, such as highly productive ocean margins. This study investigates the concentrations of the redox-sensitive trace-metals molybdenum and vanadium in organic-rich sedimentary rocks from seven sections of the Yangtze platform, slope and basin. Iron speciation analyses were carried out in order to distinguish oxic, anoxic-ferruginous and anoxic-sulphidic settings, while sulphur and nitrogen isotope ratios were measured to gain insight into sulphate and nitrate availability, respectively, in the context of changing redox conditions. The data herein demonstrate an overall increase in redox-sensitive trace-metal contents in black shales across the Ediacaran–Cambrian transition, but with marked temporal and spatial variability. Euxinia is evident in South China before 551 Ma in the Ediacaran, and again in the early Cambrian. However, some time-equivalent sections are not enriched in redox-sensitive trace metals, and also exhibit contrasting S-isotope and N-isotope systematics. A more complex configuration of the Yangtze Platform, for example with vast intra-shelf basins, together with changing (generally rising) eustatic sea-level may account for this variability. In this regard, it is proposed that a mid-depth sulphidic wedge, caused by nutrient upwelling over the south-east platform margins, migrated over time (but generally landward), leading to spatially variable redox conditions determined by sea-level, currents and bathymetric constraints. The changing extents of anoxia and euxinia appear to have limited the distribution of emerging Ediacaran and Cambrian ecosystems

    Whole-Genome Resequencing of Worldwide Wild and Domestic Sheep Elucidates Genetic Diversity, Introgression, and Agronomically Important Loci

    Domestic sheep and their wild relatives harbor substantial genetic variants that can form the backbone of molecular breeding, but their genome landscapes remain understudied. Here, we present a comprehensive genome resource for wild ovine species, landraces and improved breeds of domestic sheep, comprising high-coverage (similar to 16.10x) whole genomes of 810 samples from 7 wild species and 158 diverse domestic populations. We detected, in total, similar to 121.2 million single nucleotide polymorphisms, similar to 61 million of which are novel. Some display significant (P < 0.001) differences in frequency between wild and domestic species, or are private to continent-wide or individual sheep populations. Retained or introgressed wild gene variants in domestic populations have contributed to local adaptation, such as the variation in the HBB associated with plateau adaptation. We identified novel and previously reported targets of selection on morphological and agronomic traits such as stature, horn, tail configuration, and wool fineness. We explored the genetic basis of wool fineness and unveiled a novel mutation (chr25: T7,068,586C) in the 3 '-UTR of IRF2BP2 as plausible causal variant for fleece fiber diameter. We reconstructed prehistorical migrations from the Near Eastern domestication center to South-and-Southeast Asia and found two main waves of migrations across the Eurasian Steppe and the Iranian Plateau in the Early and Late Bronze Ages.
