    Contrasting multi-taxa diversity patterns between abandoned and non-intensively managed forests in the southern Dolomites

    The abandonment of silvicultural activities can lead to changes in species richness and composition of biological communities, when compared to those found in managed forests. The aim of this study was to compare the multi-taxonomical diversity of two mature silver fir-beech-spruce forests in the southern Dolomites (Italy), corresponding to the European Union habitat type 9130. The two sites share similar ecological and structural characteristics, but differ in their recent management histories. In the last 50 years, one site underwent non-intensive management, while the other was left unmanaged and was included in a forest reserve. The species richness and composition of eight taxa were surveyed in the two sites between 2009 and 2011. The difference in mean species richness between the two forest management types was tested through permutation tests, while differences in species composition were tested by principal coordinates analysis and the permutational multivariate analysis of variance. Mean species richness of soil macrofungi, deadwood lichens, bark beetles, and longhorn beetles were significantly higher in the abandoned than in the non-intensively managed forests. Deadwood fungi and epiphytic lichens did not differ in mean species richness between the two study sites, while mean species richness of ground beetles and birds were higher in the non-intensively managed than in the abandoned forest. Significant differences in species composition between the two sites were found for all the taxa, except for longhorn beetles. These results indicate that improving forest landscape heterogeneity through the creation of a mosaic of abandoned and extensively managed forests should better fulfill the requirements of ecologically different taxa

    Editorial: Non-native tree species: Impacts and management

    Non-native tree species are an important component of many forests around the world. These species were introduced for various reasons: the provision of wood and non-timber forest products, soil protection or rehabilitation, ornamental purposes, etc. In fact, they are important for sectoral economies, especially locally and in rural landscapes. However, there are multiple contexts in which some of these trees may have undesired negative effects, such as declines of biodiversity, increases of disturbance frequency, or disruptions of ecological balance of the native forests, greatly affecting the provision of ecosystem services. This Research Topic includes articles focusing on different aspects of relevance in the management of non-native tree species, ranging from identifying knowledge gaps and future implications, investigating the future use of non-native trees, providing governance approaches, assessment of impacts, and applying novel monitoring approaches

    Oxidative Stress and Dementia in Alzheimer’s Patients: Effects of Synbiotic Supplementation

    Alzheimer’s disease (AD) is the most common cause of dementia in elderly patients. Recently, several studies have shown that inflammation and oxidative stress precede the cardinal neuropathological manifestations of AD. In view of the proven antioxidant effects of probiotics, we proposed that continuous dietary supplementation with milk fermented with kefir grains might improve cognitive and metabolic and/or cellular disorders in the AD patients. Methods. This study was designed as an uncontrolled clinical investigation to test the effects of probiotic-fermented milk supplementation (2 mL/kg/daily) for 90 days in AD patients exhibiting cognitive deficit. Cognitive assessment, cytokine expression, systemic oxidative stress levels, and blood cell damage biomarkers were evaluated before (T0) and after (T90) kefir synbiotic supplementation. Results. When the patients were challenged to solve 8 classical tests, the majority exhibit a marked improvement in memory, visual-spatial/abstraction abilities, and executive/language functions. At the end of the treatment, the cytometric analysis showed an absolute/relative decrease in several cytokine markers of inflammation and oxidative stress markers (⋅ O2 – , H2O2, and ONOO− , ~30%) accompanied by an increase in NO bioavailability (100%). In agreement with the above findings by using the same technique, we observed in a similar magnitude an improvement of serum protein oxidation, mitochondrial dysfunction, DNA damage/repair, and apoptosis. Conclusion. In conclusion, we demonstrated that kefir improves cognitive deficits, which seems to be linked with three important factors of the AD—systemic inflammation, oxidative stress, and blood cell damage—and may be a promising adjuvant therapy against the AD progression.This study was supported by the National Council for Scientific and Technological Development (CNPq) and the State Agency for the Development of Science and Technology (FAPES) through the Edital 24/2018 -PRONEx #84321148, TO 569/2018S

    Boschi di neoformazione in Italia: approfondimenti conoscitivi e orientamenti gestionali

    Nelle regioni meridionali, e in Sicilia in particolare, la fisionomia della vegetazione forestale post-abbandono è quella della macchia o arbusteto che difficilmente evolve verso un bosco propriamente detto. Il processo evolutivo della vegetazione spesso non raggiunge lo stadio di bosco non perché le condizioni ambientali non lo consentano ma per due ragioni fondamentali: i disturbi, in particolare gli incendi e il pascolo, e le limitazioni nell’arrivo di propaguli, causate dalla mancanza di piante madri, dei dispersori dei semi o entrambi. Infatti, in contesti favorevoli (assenza di disturbi e arrivo dei propaguli) la vegetazione evolve sino al bosco. Le formazioni preforestali frutto dei processi di successione secondaria occupano superfici significative e sono conseguenza del fenomeno dell’abbandono dell’agricoltura iniziato alla seconda metà del secolo scorso e che ha interessato tutta l’Europa. Alcune azioni per ridurre i fattori negativi ed esaltare invece i vantaggi ambientali sono possibili per i boschi degli ambienti mediterranei. Tra queste, la trasformazione dei boschi di neoformazione in sistemi agroforestali, e più propriamente silvopastorali, utilizzando la parte aerea delle specie arboree e arbustive e mantenendo piccoli nuclei di specie spontanee utili alla fauna selvatica (a esempio, specie con frutti carnosi) senza lasciare che esse dominino la vegetazione erbacea. Ciò consentirebbe una valorizzazione delle superfici innanzitutto come pascoli, ma non si esclude l’utilizzazione come legna da ardere della componente arborea e, in certi casi, dei prodotti ottenibili (a esempio, manna, mandorle, carrube, nocciole). Altra possibilità per non disperdere i vantaggi consisterebbe nel mettere a coltura gli ex coltivi adottando tecniche alternative che non disperdano il carbonio accumulato. Su questo aspetto esistono già esperienze: ciò potrebbe inoltre ridurre la conflittualità tra gli enti gestori delle aree protette, restii a consentire un ritorno alla coltivazione, e gli agricoltori nonché i sostenitori della necessità di tutelare i paesaggi agrari

    Antimicrobial resistance among migrants in Europe: a systematic review and meta-analysis

    BACKGROUND: Rates of antimicrobial resistance (AMR) are rising globally and there is concern that increased migration is contributing to the burden of antibiotic resistance in Europe. However, the effect of migration on the burden of AMR in Europe has not yet been comprehensively examined. Therefore, we did a systematic review and meta-analysis to identify and synthesise data for AMR carriage or infection in migrants to Europe to examine differences in patterns of AMR across migrant groups and in different settings. METHODS: For this systematic review and meta-analysis, we searched MEDLINE, Embase, PubMed, and Scopus with no language restrictions from Jan 1, 2000, to Jan 18, 2017, for primary data from observational studies reporting antibacterial resistance in common bacterial pathogens among migrants to 21 European Union-15 and European Economic Area countries. To be eligible for inclusion, studies had to report data on carriage or infection with laboratory-confirmed antibiotic-resistant organisms in migrant populations. We extracted data from eligible studies and assessed quality using piloted, standardised forms. We did not examine drug resistance in tuberculosis and excluded articles solely reporting on this parameter. We also excluded articles in which migrant status was determined by ethnicity, country of birth of participants' parents, or was not defined, and articles in which data were not disaggregated by migrant status. Outcomes were carriage of or infection with antibiotic-resistant organisms. We used random-effects models to calculate the pooled prevalence of each outcome. The study protocol is registered with PROSPERO, number CRD42016043681. FINDINGS: We identified 2274 articles, of which 23 observational studies reporting on antibiotic resistance in 2319 migrants were included. The pooled prevalence of any AMR carriage or AMR infection in migrants was 25·4% (95% CI 19·1-31·8; I2 =98%), including meticillin-resistant Staphylococcus aureus (7·8%, 4·8-10·7; I2 =92%) and antibiotic-resistant Gram-negative bacteria (27·2%, 17·6-36·8; I2 =94%). The pooled prevalence of any AMR carriage or infection was higher in refugees and asylum seekers (33·0%, 18·3-47·6; I2 =98%) than in other migrant groups (6·6%, 1·8-11·3; I2 =92%). The pooled prevalence of antibiotic-resistant organisms was slightly higher in high-migrant community settings (33·1%, 11·1-55·1; I2 =96%) than in migrants in hospitals (24·3%, 16·1-32·6; I2 =98%). We did not find evidence of high rates of transmission of AMR from migrant to host populations. INTERPRETATION: Migrants are exposed to conditions favouring the emergence of drug resistance during transit and in host countries in Europe. Increased antibiotic resistance among refugees and asylum seekers and in high-migrant community settings (such as refugee camps and detention facilities) highlights the need for improved living conditions, access to health care, and initiatives to facilitate detection of and appropriate high-quality treatment for antibiotic-resistant infections during transit and in host countries. Protocols for the prevention and control of infection and for antibiotic surveillance need to be integrated in all aspects of health care, which should be accessible for all migrant groups, and should target determinants of AMR before, during, and after migration.     Surgery for Bismuth-Corlette Type 4 Perihilar Cholangiocarcinoma: Results from a Western Multicenter Collaborative Group

    Background Although Bismuth-Corlette (BC) type 4 perihilar cholangiocarcinoma (pCCA) is no longer considered a contraindication for curative surgery, few data are available from Western series to indicate the outcomes for these patients. This study aimed to compare the short- and long-term outcomes for patients with BC type 4 versus BC types 2 and 3 pCCA undergoing surgical resection using a multi-institutional international database. Methods Uni- and multivariable analyses of patients undergoing surgery at 20 Western centers for BC types 2 and 3 pCCA and BC type 4 pCCA. Results Among 1138 pCCA patients included in the study, 826 (73%) had BC type 2 or 3 disease and 312 (27%) had type 4 disease. The two groups demonstrated significant differences in terms of clinicopathologic characteristics (i.e., portal vein embolization, extended hepatectomy, and positive margin). The incidence of severe complications was 46% for the BC types 2 and 3 patients and 51% for the BC type 4 patients (p = 0.1). Moreover, the 90-day mortality was 13% for the BC types 2 and 3 patients and 12% for the BC type 4 patients (p = 0.57). Lymph-node metastasis (N1; hazard-ratio [HR], 1.62), positive margins (R1; HR, 1.36), perineural invasion (HR, 1.53), and poor grade of differentiation (HR, 1.25) were predictors of survival (all p ≤0.004), but BC type was not associated with prognosis. Among the N0 and R0 patients, the 5-year overall survival was 43% for the patients with BC types 2 and 3 pCCA and 41% for those with BC type 4 pCCA (p = 0.60). Conclusions In this analysis of a large Western multi-institutional cohort, resection was shown to be an acceptable curative treatment option for selected patients with BC type 4 pCCA although a more technically challenging surgical approach was required

    Laparoscopic right hemicolectomy: a SICE (Società Italiana di Chirurgia Endoscopica e Nuove tecnologie) network prospective study on the approach to right colon lymphadenectomy in Italy: is there a standard?—CoDIG 2 (ColonDx Italian Group)

    Background: Colon cancer is a disease with a worldwide spread. Surgery is the best option for the treatment of advanced colon cancer, but some aspects are still debated, such as the extent of lymphadenectomy. In Japanese guidelines, the gold standard was D3 dissection to remove the central lymph nodes (203, 213, and 223), but in 2009, Hoenberger et al. introduced the concept of complete mesocolic excision, in which surgical dissection follows the embryological planes to remove the mesentery entirely to prevent leakage of cancer cells and collect more lymph nodes. Our study describes how lymphadenectomy is currently performed in major Italian centers with an unclear indication on the type of lymphadenectomy that should be performed during right hemicolectomy (RH). Methods: CoDIG 2 is an observational multicenter national study that involves 76 Italian general surgery wards highly specialized in colorectal surgery. Each center was asked not to modify their usual surgical and clinical practices. The aim of the study was to assess the preference of Italian surgeons on the type of lymphadenectomy to perform during RH and the rise of any new trends or modifications in habits compared to the findings of the CoDIG 1 study conducted 4 years ago. Results: A total of 788 patients were enrolled. The most commonly used surgical technique was laparoscopic (82.1%) with intracorporeal (73.4%), side-to-side (98.7%), or isoperistaltic (96.0%) anastomosis. The lymph nodes at the origin of the vessels were harvested in an inferior number of cases (203, 213, and 223: 42.4%, 31.1%, and 20.3%, respectively). A comparison between CoDIG 1 and CoDIG 2 showed a stable trend in surgical techniques and complications, with an increase in the robotic approach (7.7% vs. 12.3%). Conclusions: This analysis shows how lymphadenectomy is performed in Italy to achieve oncological outcomes in RH, although the technique to achieve a higher lymph node count has not yet been standardized.     Where are we now with European forest multi-taxon biodiversity and where can we head to?

    The European biodiversity and forest strategies rely on forest sustainable management (SFM) to conserve forest biodiversity. However, current sustainability assessments hardly account for direct biodiversity indicators. We focused on forest multi-taxon biodiversity to: i) gather and map the existing information; ii) identify knowledge and research gaps; iii) discuss its research potential. We established a research network to fit data on species, standing trees, lying deadwood and sampling unit description from 34 local datasets across 3591 sampling units. A total of 8724 species were represented, with the share of common and rare species varying across taxonomic classes: some included many species with several rare ones (e.g., Insecta); others (e.g., Bryopsida) were represented by few common species. Tree-related structural attributes were sampled in a subset of sampling units (2889; 2356; 2309 and 1388 respectively for diameter, height, deadwood and microhabitats). Overall, multi-taxon studies are biased towards mature forests and may underrepresent the species related to other developmental phases. European forest compositional categories were all represented, but beech forests were over-represented as compared to thermophilous and boreal forests. Most sampling units (94%) were referred to a habitat type of conservation concern. Existing information may support European conservation and SFM strategies in: (i) methodological harmonization and coordinated monitoring; (ii) definition and testing of SFM indicators and thresholds; (iii) data-driven assessment of the effects of environmental and management drivers on multi-taxon forest biological and functional diversity, (iv) multi-scale forest monitoring integrating in-situ and remotely sensed information
