    Foreign capital in a growth model

    Within an endogenous growth framework, this paper empirically investigates the impact of financial capital on economic growth for a panel of 60 developing countries, through the channel of domestic capital formation. By estimating the model for different income groups, it is found that while private FDI flows exert beneficial complementarity effects on the domestic capital formation across all income-group countries, the official financial flows contribute to increasing investment in the middle income economies, but not in the low income countries. The latter appears to demonstrate that the aid-growth nexus is supported in the middle income countries, whereas the misallocation of official inflows is more likely to exist in the low income countries, suggesting that aid effectiveness remains conditional on the domestic policy environment

    Regulation of Hemolysin Expression and Virulence of Staphylococcus aureus by a Serine/Threonine Kinase and Phosphatase

    Exotoxins, including the hemolysins known as the alpha (α) and beta (β) toxins, play an important role in the pathogenesis of Staphylococcus aureus infections. A random transposon library was screened for S. aureus mutants exhibiting altered hemolysin expression compared to wild type. Transposon insertions in 72 genes resulting in increased or decreased hemolysin expression were identified. Mutations inactivating a putative cyclic di-GMP synthetase and a serine/threonine phosphatase (Stp1) were found to reduce hemolysin expression, and mutations in genes encoding a two component regulator PhoR, LysR family transcriptional regulator, purine biosynthetic enzymes and a serine/threonine kinase (Stk1) increased expression. Transcription of the hla gene encoding α toxin was decreased in a Δstp1 mutant strain and increased in a Δstk1 strain. Microarray analysis of a Δstk1 mutant revealed increased transcription of additional exotoxins. A Δstp1 strain is severely attenuated for virulence in mice and elicits less inflammation and IL-6 production than the Δstk1 strain. In vivo phosphopeptide enrichment and mass spectrometric analysis revealed that threonine phosphorylated peptides corresponding to Stk1, DNA binding histone like protein (HU), serine-aspartate rich fibrinogen/bone sialoprotein binding protein (SdrE) and a hypothetical protein (NWMN_1123) were present in the wild type and not in the Δstk1 mutant. Collectively, these studies suggest that Stk1 mediated phosphorylation of HU, SrdE and NWMN_1123 affects S. aureus gene expression and virulence

    Evaluation of appendicitis risk prediction models in adults with suspected appendicitis

    Background Appendicitis is the most common general surgical emergency worldwide, but its diagnosis remains challenging. The aim of this study was to determine whether existing risk prediction models can reliably identify patients presenting to hospital in the UK with acute right iliac fossa (RIF) pain who are at low risk of appendicitis. Methods A systematic search was completed to identify all existing appendicitis risk prediction models. Models were validated using UK data from an international prospective cohort study that captured consecutive patients aged 16–45 years presenting to hospital with acute RIF in March to June 2017. The main outcome was best achievable model specificity (proportion of patients who did not have appendicitis correctly classified as low risk) whilst maintaining a failure rate below 5 per cent (proportion of patients identified as low risk who actually had appendicitis). Results Some 5345 patients across 154 UK hospitals were identified, of which two‐thirds (3613 of 5345, 67·6 per cent) were women. Women were more than twice as likely to undergo surgery with removal of a histologically normal appendix (272 of 964, 28·2 per cent) than men (120 of 993, 12·1 per cent) (relative risk 2·33, 95 per cent c.i. 1·92 to 2·84; P < 0·001). Of 15 validated risk prediction models, the Adult Appendicitis Score performed best (cut‐off score 8 or less, specificity 63·1 per cent, failure rate 3·7 per cent). The Appendicitis Inflammatory Response Score performed best for men (cut‐off score 2 or less, specificity 24·7 per cent, failure rate 2·4 per cent). Conclusion Women in the UK had a disproportionate risk of admission without surgical intervention and had high rates of normal appendicectomy.     O tétano é uma doença infecciosa, não contagiosa, causada pelo bacilo Clostridium tetani que penetra no organismo através de ferimento. A disfunção psicomotora facilita acidentes entre os idoso e a cobertura vacinal é baixa nesta população contribuindo para letalidade alta. Este estudo tem como objetivo refletir sobre a situação do homem idoso, com relação ao tétano, dentro da perspectiva da Saúde Coletiva. Trata-se de Estudo de Caso realizado com dois pacientes idosos, do sexo masculino, portadores de tétano acidental, internados num hospital do município de Fortaleza. A coleta de dados se deu entre março e abril de 1998. A análise mostra a ausência de cobertura vacinal e da implantação da profilaxia de emergência. Os dois pacientes evoluíram para óbito confirmando alta mortalidade por tétano entre este grupo etário. A reflexão crítica aponta para urgência de abordagem de saúde coletiva.<br>El tétano es una enfermedad infecciosa, no contagiosa, causada por el bacilo Clostridium Tetani que penetra en el organismo a través de una herida. Este estudio tiene como objetivo la reflexión sobre la situación del adulto mayor, con relación al tétano, dentro de la perspectiva de la Salud Colectiva. Se trata de un estudio de caso con dos pacientes ancianos, del sexo masculino, portadores del tétano accidental, internados en un hospital del municipio de Fortaleza. La recolección de datos de hizo entre marzo y abril de 1998. El análisis muestra la ausencia de cobertura de la vacuna y de una implementación de profilaxis de emergencia. Los dos pacientes fallecieron, lo que confirma la alta mortalidad por el tétano en este grupo de edad. La reflexión crítica, denota la urgencia de un abordaje desde la salud colectiva.<br>Tetanus is an infectious non-contagious disease caused by the bacillus Clostridium tetani, which penetrates in the organism through wounds. Psychomotor dysfunction facilitates accidents among elderly people and vaccinal coverage is low in this population, thus contributing to high lethality. This study aimed at reflecting on the situation faced by elderly people in relation to tetanus in the perspective of Collective Health. It is a Case Study conducted with two elderly males who had accidental tetanus and were hospitalized in a hospital in the municipality of Fortaleza. Data collection took place from March to April, 1998. The analysis showed the absence of vaccinal coverage as well as of the implementation of emergency prophylaxis. The two patients' conditions developed to death, which confirmed the high mortality due to tetanus in this age group. The critical reflection pointed out the urgency of a collective health approach

    Trata-se de uma pesquisa qualitativa, descritiva que teve por objetivo identificar como vem sendo o relacionamento idoso-família, identificando os motivos que levaram os familiares a optarem pela institucionalização do seu idoso, obtendo-se também a visão da família sobre o cuidado no lar. Para coleta dos dados utilizou-se um instrumento com perguntas abertas. A coleta dos dados efetivou-se nos meses de agosto de 2002 a julho de 2003, sendo sujeitos da pesquisa, familiares de idosos de uma instituição asilar em um município da região noroeste do Rio Grande do Sul. Através da análise de conteúdo constatou-se que a maior dificuldade encontrada pela família, visando dispensar cuidados ao idoso é a "falta de tempo para cuidar" do seu familiar idoso, pois a maioria deles exige cuidados em tempo integral
