    External validation of EORTC risk scores to predict recurrence after transurethral resection of brazilian patients with non -muscle invasive bladder cancer stages Ta and T1

    Validate the EORTC risk tables in Brazilian patients with NMIBC. Methods: 205 patients were analyzed. The 6 parameters analyzed were: histologic grading, pathologic stage, size and number of tumors, previous recurrence rate and concomitant CIS. The time for first recurrence (TFR), risk score and probability of re\uadcurrence were calculated and compared to the probabilities obtained from EORTC risk tables. C-index was calculated and accuracy of EORTC tables was analyzed. Results: pTa was presented in 91 (44.4%) patients and pT1 in 114 (55.6%). Ninety-seven (47.3%) patients had solitary tumor, and 108 (52.7%) multiple tumors. One hundred and three (50.2%) patients had tumors smaller than 3 cm and 102 (40.8%) had bigger than 3 cm. Concomitant CIS was observed in 21 (10.2%) patients. Low grade was presented in 95 (46.3%) patients, and high grade in 110 (53.7%). Intravesical therapy was utilized in 105 (56.1%) patients. Recurrence was observed in 117 (57.1%) patients and the mean TFR was 14,2 \ub1 7,3 months. C-index was 0,72 for 1 year and 0,7 for 5 years. The recurrence risk was 28,8% in 1 year and 57,1% in 5 years, independently of the scoring risk. In our population, the EORTC risk tables overestimated the risk of recurrence in 1 year and underestimated in 5 years. Conclusion: The validation of the EORTC risk tables in Brazilian patients with NMIBC was satisfactory and should be stimulated to predict recurrence, although these may overestimated the risk of recurrence in 1 year and underestimated in 5 years

    Patients' motives for choosing a physician: comparison between conventional and complementary medicine in Swiss primary care

    <p>Abstract</p> <p>Background</p> <p>The study is part of a nationwide evaluation of complementary and alternative medicine (CAM) in primary care in Switzerland. The Objective was to identify patients' expectations and reasons governing the choice of complementary medicine compared with conventional primary care (CONV).</p> <p>Methods</p> <p>The data were derived from the PEK study (Programm Evaluation Komplementärmedizin), which was conducted in 2002–2003 with 7879 adult patients and parents of 1291 underage patients, seeking either complementary (CAM) or conventional (CONV) primary care. The study was performed as a cross-sectional survey. The respondents were asked to document their (or their children's) self-perceived health status, reasons governing their choice, and treatment expectations. Physicians were practicing conventional medicine and/or complementary methods (homeopathy, anthroposophic medicine, neural therapy, and traditional Chinese medicine). Reasons governing the choice of physician were evaluated on the basis of a three-part classification (physician-related, procedure-related, and pragmatic/other reasons)</p> <p>Results and Discussion</p> <p>Patients seeing CAM physicians tend to be younger and more often female. CAM patients referred to procedure-related reasons more frequently, whereas pragmatic reasons dominated among CONV patients. CAM respondents expected fewer adverse side effects compared to conventional care patients.</p> <p>Conclusion</p> <p>The majority of alternative medicine users appear to have chosen CAM mainly because they wish to undergo a certain procedure; additional reasons include desire for more comprehensive treatment, and expectation of fewer side-effects.</p

    Study of Zγ events and limits on anomalous ZZγ and Zγγ couplings in pp̄ collisions at s=1.96TeV

    We present a measurement of the Zγ production cross section and limits on anomalous ZZγ and Zγγ couplings for form-factor scales of Λ=750 and 1000 GeV. The measurement is based on 138 (152) candidates in the eeγ (μμγ) final state using 320(290)pb-1 of pp̄ collisions at s=1.96TeV. The 95% C.L. limits on real and imaginary parts of individual anomalous couplings are |h10,30Z|<0.23, |h20,40Z|<0.020, |h10,30γ|<0.23, and |h20,40γ|<0.019 for Λ=1000GeV. © 2005 The American Physical Society

    Precision Measurement of the Neutron Spin Asymmetry A1nA_1^n and Spin-Flavor Decomposition in the Valence Quark Region

    We have measured the neutron spin asymmetry A1nA_1^n with high precision at three kinematics in the deep inelastic region at x=0.33x=0.33, 0.47 and 0.60, and Q2=2.7Q^2=2.7, 3.5 and 4.8 (GeV/c)2^2, respectively. Our results unambiguously show, for the first time, that A1nA_1^n crosses zero around x=0.47x=0.47 and becomes significantly positive at x=0.60x=0.60. Combined with the world proton data, polarized quark distributions were extracted. Our results, in general, agree with relativistic constituent quark models and with perturbative quantum chromodynamics (pQCD) analyses based on the earlier data. However they deviate from pQCD predictions based on hadron helicity conservation.Comment: 5 pages, 2 figures, this is the final version appeared in Phys. Rev. Let

    Patient satisfaction and side effects in primary care: An observational study comparing homeopathy and conventional medicine

    <p>Abstract</p> <p>Background</p> <p>This study is part of a nationwide evaluation of complementary medicine in Switzerland (Programme Evaluation of Complementary Medicine PEK) and was funded by the Swiss Federal Office of Public Health. The main objective of this study is to investigate patient satisfaction and perception of side effects in homeopathy compared with conventional care in a primary care setting.</p> <p>Methods</p> <p>We examined data from two cross-sectional studies conducted in 2002–2003. The first study was a physician questionnaire assessing structural characteristics of practices. The second study was conducted on four given days during a 12-month period in 2002/2003 using a physician and patient questionnaire at consultation and a patient questionnaire mailed to the patient one month later (including Europep questionnaire).</p> <p>The participating physicians were all trained and licensed in conventional medicine. An additional qualification was required for medical doctors providing homeopathy (membership in the Swiss association of homeopathic physicians SVHA).</p> <p>Results</p> <p>A total of 6778 adult patients received the questionnaire and 3126 responded (46.1%). Statistically significant differences were found with respect to health status (higher percentage of chronic and severe conditions in the homeopathic group), perception of side effects (higher percentage of reported side effects in the conventional group) and patient satisfaction (higher percentage of satisfied patients in the homeopathic group).</p> <p>Conclusion</p> <p>Overall patient satisfaction was significantly higher in homeopathic than in conventional care. Homeopathic treatments were perceived as a low-risk therapy with two to three times fewer side effects than conventional care</p

    Reproductive Performance, Udder Health, and Antibiotic Resistance in Mastitis Bacteria isolated from Norwegian Red cows in Conventional and Organic Farming

    <p>Abstract</p> <p>Background</p> <p>The objectives of this study were to investigate whether there were differences between Norwegian Red cows in conventional and organic farming with respect to reproductive performance, udder health, and antibiotic resistance in udder pathogens.</p> <p>Methods</p> <p>Twenty-five conventional and 24 organic herds from south-east and middle Norway participated in the study. Herds were matched such that geographical location, herd size, and barn types were similar across the cohorts. All organic herds were certified as organic between 1997 and 2003. All herds were members of the Norwegian Dairy Herd Recording System. The herds were visited once during the study. The relationship between the outcomes and explanatory variables were assessed using mixed linear models.</p> <p>Results</p> <p>There were less > 2nd parity cows in conventional farming. The conventional cows had higher milk yields and received more concentrates than organic cows. Although after adjustment for milk yield and parity, somatic cell count was lower in organic cows than conventional cows. There was a higher proportion of quarters that were dried off at the herd visit in organic herds. No differences in the interval to first AI, interval to last AI or calving interval was revealed between organic and conventional cows. There was no difference between conventional and organic cows in quarter samples positive for mastitis bacteria from the herd visit. Milk yield and parity were associated with the likelihood of at least one quarter positive for mastitis bacteria. There was few <it>S. aureus </it>isolates resistance to penicillin in both management systems. Penicillin resistance against Coagulase negative staphylococci isolated from subclinically infected quarters was 48.5% in conventional herds and 46.5% in organic herds.</p> <p>Conclusion</p> <p>There were no large differences between reproductive performance and udder health between conventional and organic farming for Norwegian Red cows.</p

    Precision Measurement of the Neutron Spin Asymmetries and Spin-dependent Structure Functions in the Valence Quark Region

    We report on measurements of the neutron spin asymmetries A1,2nA_{1,2}^n and polarized structure functions g1,2ng_{1,2}^n at three kinematics in the deep inelastic region, with x=0.33x=0.33, 0.47 and 0.60 and Q2=2.7Q^2=2.7, 3.5 and 4.8 (GeV/c)2^2, respectively. These measurements were performed using a 5.7 GeV longitudinally-polarized electron beam and a polarized 3^3He target. The results for A1nA_1^n and g1ng_1^n at x=0.33x=0.33 are consistent with previous world data and, at the two higher xx points, have improved the precision of the world data by about an order of magnitude. The new A1nA_1^n data show a zero crossing around x=0.47x=0.47 and the value at x=0.60x=0.60 is significantly positive. These results agree with a next-to-leading order QCD analysis of previous world data. The trend of data at high xx agrees with constituent quark model predictions but disagrees with that from leading-order perturbative QCD (pQCD) assuming hadron helicity conservation. Results for A2nA_2^n and g2ng_2^n have a precision comparable to the best world data in this kinematic region. Combined with previous world data, the moment d2nd_2^n was evaluated and the new result has improved the precision of this quantity by about a factor of two. When combined with the world proton data, polarized quark distribution functions were extracted from the new g1n/F1ng_1^n/F_1^n values based on the quark parton model. While results for Δu/u\Delta u/u agree well with predictions from various models, results for Δd/d\Delta d/d disagree with the leading-order pQCD prediction when hadron helicity conservation is imposed.Comment: A typing error in A_\parallel(3He) at x=0.47 in Table VII of Phys.     Measurements of fiducial and differential cross sections for Higgs boson production in the diphoton decay channel at s√=8 TeV with ATLAS

    Measurements of fiducial and differential cross sections are presented for Higgs boson production in proton-proton collisions at a centre-of-mass energy of s√=8 TeV. The analysis is performed in the H → γγ decay channel using 20.3 fb−1 of data recorded by the ATLAS experiment at the CERN Large Hadron Collider. The signal is extracted using a fit to the diphoton invariant mass spectrum assuming that the width of the resonance is much smaller than the experimental resolution. The signal yields are corrected for the effects of detector inefficiency and resolution. The pp → H → γγ fiducial cross section is measured to be 43.2 ±9.4(stat.) − 2.9 + 3.2 (syst.) ±1.2(lumi)fb for a Higgs boson of mass 125.4GeV decaying to two isolated photons that have transverse momentum greater than 35% and 25% of the diphoton invariant mass and each with absolute pseudorapidity less than 2.37. Four additional fiducial cross sections and two cross-section limits are presented in phase space regions that test the theoretical modelling of different Higgs boson production mechanisms, or are sensitive to physics beyond the Standard Model. Differential cross sections are also presented, as a function of variables related to the diphoton kinematics and the jet activity produced in the Higgs boson events. The observed spectra are statistically limited but broadly in line with the theoretical expectations
