    El siguiente trabajo es la presentación de un caso ingresado en el servicio de Medicina Interna del Hospital Provincial Abel Santamaría Cuadrado, provincia Pinar del Río, en septiembre del 2007, con el diagnóstico de Gota Tofácea crónica. Este paciente presentaba una artritis invalidante y la presencia de tofos con grandes dimensiones que han provocado la anquilosis articular y deformaciones significativas. Considerando que es de gran interés semiológico para estudiantes de pre y postgrado, así como personal médico en general, pues en la actualidad es difícil encontrar pacientes con las manifestaciones encontradas al examen físico de este caso, debido a la prontitud del diagnóstico, la sistematicidad del tratamiento que se realiza en estos momentos.     Trastornos del sueño asociados a la hipertensión arterial / Sleeping Disorders associated to High Blood Pressure

    Se realizó un estudio transversal con el objetivo de establecer la relación existente entre los trastornos del sueño y la Hipertensión Arterial. La muestra estuvo formada por el total de pacientes hipertensos pertenecientes a tres consultorios médicos del Policlínico Universitario Turcios Lima en el municipio de Pinar del Río, en el periodo comprendido de enero 2006 a diciembre de 2006. Para dicho estudio se aplicó una encuesta donde se recogen datos relacionados con los trastornos del sueño más frecuentes que presentaban los pacientes. Se utilizó el método estadístico de la prueba de hipótesis de comparación de proporciones para los niveles de significación hasta 1%. Se obtuvo como resultado un elevado por ciento (81,07%) de pacientes con trastornos del sueño, el insomnio y el ronquido resultaron los trastornos más frecuentes, un tercio de los pacientes refirió síntomas subjetivos de apnea obstructiva del sueño y la mitad de los pacientes ingerían psicofármacos para dormir. A survey was applied for compiling the data related to the most frequent sleeping disorders in patients .The statistical method of the hypothesis testing of comparison of proportions was used with significant levels up 1%.It was obtained a high percentage of patients with sleeping disorders (81, 07) being insomnia and snore the most frequent disorders presented, a third of patients had subjective symptoms of obstructive sleeping apnea and the half of patients used sleeping drugs Key words: High blood pressure, sleeping disorders, circadian cycle sleeping-vigil, sleeping apnea, hypersomnia, insomni

    First Observation of the Cabibbo-suppressed Decays Xi_c+ -> Sigma+ pi- pi+ and Xi_c+ -> Sigma- pi+ pi+ and Measurement of their Branching Ratios

    We report the first observation of two Cabibbo-suppressed decay modes, Xi_c+ -> Sigma+ pi- pi+ and Xi_c+ -> Sigma- pi+ pi+. We observe 59+/-14 over a background of 87, and 22+/-8 over a background of 13 events, respectively, for the signals. The data were accumulated using the SELEX spectrometer during the 1996-1997 fixed target run at Fermilab, chiefly from a 600GeV/c Sigma- beam. The branching ratios of the decays relative to the Cabibbo--favored Xi_c+ -> Xi- pi+ pi+ are measured to be B(Xi_c+ -> Sigma+ pi- pi+)/B(Xi_c+ -> Xi- pi+ pi+) = 0.48+/-0.20, and B(Xi_c+ -> Sigma- pi+ pi+)/B(Xi_c+ -> Xi- pi+ pi+) = 0.18+/-0.09, respectively.     Precision Measurement of the Boron to Carbon Flux Ratio in Cosmic Rays from 1.9 GV to 2.6 TV with the Alpha Magnetic Spectrometer on the International Space Station

    Knowledge of the rigidity dependence of the boron to carbon flux ratio (B/C) is important in understanding the propagation of cosmic rays. The precise measurement of the B/C ratio from 1.9 GV to 2.6 TV, based on 2.3 million boron and 8.3 million carbon nuclei collected by AMS during the first 5 years of operation, is presented. The detailed variation with rigidity of the B/C spectral index is reported for the first time. The B/C ratio does not show any significant structures in contrast to many cosmic ray models that require such structures at high rigidities. Remarkably, above 65 GV, the B/C ratio is well described by a single power law R[superscript Δ] with index Δ=-0.333±0.014(fit)±0.005(syst), in good agreement with the Kolmogorov theory of turbulence which predicts Δ=-1/3 asymptotically.National Science Foundation (U.S.) (Grants 1455202 and 1551980)Wyle Research (Firm) (Grant 2014/T72497)United States.     Antiproton Flux, Antiproton-to-Proton Flux Ratio, and Properties of Elementary Particle Fluxes in Primary Cosmic Rays Measured with the Alpha Magnetic Spectrometer on the International Space Station

    International audienceA precision measurement by AMS of the antiproton flux and the antiproton-to-proton flux ratio inprimary cosmic rays in the absolute rigidity range from 1 to 450 GV is presented based on 3.49 × 105antiproton events and 2.42 × 109 proton events. The fluxes and flux ratios of charged elementary particlesin cosmic rays are also presented. In the absolute rigidity range ∼60 to ∼500 GV, the antiproton ¯p, protonp, and positron eþ fluxes are found to have nearly identical rigidity dependence and the electron e− fluxexhibits a different rigidity dependence. Below 60 GV, the ( ¯ p=p), ( ¯ p=eþ), and (p=eþ) flux ratios eachreaches a maximum. From ∼60 to ∼500 GV, the ( ¯ p=p), ( ¯ p=eþ), and (p=eþ) flux ratios show no rigiditydependence. These are new observations of the properties of elementary particles in the cosmos

    Iron Behaving Badly: Inappropriate Iron Chelation as a Major Contributor to the Aetiology of Vascular and Other Progressive Inflammatory and Degenerative Diseases

    The production of peroxide and superoxide is an inevitable consequence of aerobic metabolism, and while these particular "reactive oxygen species" (ROSs) can exhibit a number of biological effects, they are not of themselves excessively reactive and thus they are not especially damaging at physiological concentrations. However, their reactions with poorly liganded iron species can lead to the catalytic production of the very reactive and dangerous hydroxyl radical, which is exceptionally damaging, and a major cause of chronic inflammation. We review the considerable and wide-ranging evidence for the involvement of this combination of (su)peroxide and poorly liganded iron in a large number of physiological and indeed pathological processes and inflammatory disorders, especially those involving the progressive degradation of cellular and organismal performance. These diseases share a great many similarities and thus might be considered to have a common cause (i.e. iron-catalysed free radical and especially hydroxyl radical generation). The studies reviewed include those focused on a series of cardiovascular, metabolic and neurological diseases, where iron can be found at the sites of plaques and lesions, as well as studies showing the significance of iron to aging and longevity. The effective chelation of iron by natural or synthetic ligands is thus of major physiological (and potentially therapeutic) importance. As systems properties, we need to recognise that physiological observables have multiple molecular causes, and studying them in isolation leads to inconsistent patterns of apparent causality when it is the simultaneous combination of multiple factors that is responsible.     The Seventeenth Data Release of the Sloan Digital Sky Surveys: Complete Release of MaNGA, MaStar and APOGEE-2 Data

    This paper documents the seventeenth data release (DR17) from the Sloan Digital Sky Surveys; the fifth and final release from the fourth phase (SDSS-IV). DR17 contains the complete release of the Mapping Nearby Galaxies at Apache Point Observatory (MaNGA) survey, which reached its goal of surveying over 10,000 nearby galaxies. The complete release of the MaNGA Stellar Library (MaStar) accompanies this data, providing observations of almost 30,000 stars through the MaNGA instrument during bright time. DR17 also contains the complete release of the Apache Point Observatory Galactic Evolution Experiment 2 (APOGEE-2) survey which publicly releases infra-red spectra of over 650,000 stars. The main sample from the Extended Baryon Oscillation Spectroscopic Survey (eBOSS), as well as the sub-survey Time Domain Spectroscopic Survey (TDSS) data were fully released in DR16. New single-fiber optical spectroscopy released in DR17 is from the SPectroscipic IDentification of ERosita Survey (SPIDERS) sub-survey and the eBOSS-RM program. Along with the primary data sets, DR17 includes 25 new or updated Value Added Catalogs (VACs). This paper concludes the release of SDSS-IV survey data. SDSS continues into its fifth phase with observations already underway for the Milky Way Mapper (MWM), Local Volume Mapper (LVM) and Black Hole Mapper (BHM) surveys
