BACKGROUND: Herd immunity is important in the effectiveness of conjugate polysaccharide vaccines against encapsulated bacteria. A large multicenter study investigated the effect of meningococcal serogroup C conjugate vaccine introduction on the meningococcal population. METHODS: Carried meningococci in individuals aged 15-19 years attending education establishments were investigated before and for 2 years after vaccine introduction. Isolates were characterized by multilocus sequence typing, serogroup, and capsular region genotype and changes in phenotypes and genotypes assessed. RESULTS: A total of 8462 meningococci were isolated from 47 765 participants (17.7%). Serogroup prevalence was similar over the 3 years, except for decreases of 80% for serogroup C and 40% for serogroup 29E. Clonal complexes were associated with particular serogroups and their relative proportions fluctuated, with 12 statistically significant changes (6 up, 6 down). The reduction of ST-11 complex serogroup C meningococci was probably due to vaccine introduction. Reasons for a decrease in serogroup 29E ST-254 meningococci (from 1.8% to 0.7%) and an increase in serogroup B ST-213 complex meningococci (from 6.7% to 10.6%) were less clear. CONCLUSIONS: Natural fluctuations in carried meningococcal genotypes and phenotypes a can be affected by the use of conjugate vaccines, and not all of these changes are anticipatable in advance of vaccine introduction

Energy injustice and Nordic electric mobility: inequality, elitism, and externalities in the electrification of vehicle-to-grid (V2G) transport

Much research on electric mobility transitions has been descriptive or positive, rather than normative or critical, assessing the deeper ethical, justice, or moral issues that arise. To address this gap, this study qualitatively assesses the ongoing transition to Nordic electric vehicles (EVs) and vehicle-to-grid (V2G) systems. It does so through the various lenses of distributive justice, procedural justice, cosmopolitan justice, and recognition justice. It asks: what are the types of injustices associated with electric mobility and V2G? In what ways do emerging patterns of electric mobility worsen socio-environmental risks or vulnerabilities? Based on original primary data collected from 257 experts across Denmark, Finland, Iceland, Norway, and Sweden, the study finds that electric mobility can erode elements of distributive justice for being accessible only to the rich, and for raising risks related to privacy, hacking, and cyberterrorism. Electric mobility may contravene aspects of procedural justice by reinforcing exclusion and elitism in national planning. It can erode cosmopolitan justice by producing negative environmental externalities, and exacerbating rural (and global) vulnerability. It may threaten recognition justice through unemployment, disruption to traditional businesses, and the entrenchment of patriarchy. Thankfully, the study also proposes a suite of policy mechanisms to address many of these concerns

Valuation of depression co-occurring with a somatic condition:feasibility of the time trade-off task

Background Health state valuations obtained from the general population are used for cost-utility analyses of health-care interventions. Currently, most studies have focused on valuations of somatic conditions, to a much lesser extent of mental states, that is, depression and even less on valuations of depression co-occurring with somatic conditions. Objective We tested the feasibility of the time trade-off (TTO) task to elicit valuations for depression solitary or co-occurring with a somatic condition. Moreover, we explored person-and state-related factors that may affect valuations. Design During semi-structured interviews, 10 individuals (five women, mean age: 36 years) used a TTO task to value vignettes describing mild and severe depression; and mild depression co-occurring with moderate and severe states of cancer, diabetes or heart disease. During valuations, participants were thinking aloud. Feasibility criteria were successful completion and difficulty/concentration (1-10); logical consistency of values; and comprehension of the TTO, based on qualitative analysis of think aloud data. Factors influencing valuations were generated from think aloud data. Results Participants reported satisfactory levels of difficulty (mean: 1.9) and concentration (mean: 8.3) and assigned consistent values. Qualitative analysis revealed difficulties with imagining: living with depression for lifetime (n = 4); reaching the age of 80 (n = 6); and living with a somatic condition and mentally healthy (n = 6). Person-and state-related factors, for example perceived susceptibility to depression (n = 4), appeared to affect valuations. Conclusion Quantitative findings supported feasibility of the valuation protocol, yet qualitative findings indicated that certain task aspects should be readdressed. Factors influencing valuations can be explored to better understand valuations

Effects of Anacetrapib in Patients with Atherosclerotic Vascular Disease

BACKGROUND: Patients with atherosclerotic vascular disease remain at high risk for cardiovascular events despite effective statin-based treatment of low-density lipoprotein (LDL) cholesterol levels. The inhibition of cholesteryl ester transfer protein (CETP) by anacetrapib reduces LDL cholesterol levels and increases high-density lipoprotein (HDL) cholesterol levels. However, trials of other CETP inhibitors have shown neutral or adverse effects on cardiovascular outcomes. METHODS: We conducted a randomized, double-blind, placebo-controlled trial involving 30,449 adults with atherosclerotic vascular disease who were receiving intensive atorvastatin therapy and who had a mean LDL cholesterol level of 61 mg per deciliter (1.58 mmol per liter), a mean non-HDL cholesterol level of 92 mg per deciliter (2.38 mmol per liter), and a mean HDL cholesterol level of 40 mg per deciliter (1.03 mmol per liter). The patients were assigned to receive either 100 mg of anacetrapib once daily (15,225 patients) or matching placebo (15,224 patients). The primary outcome was the first major coronary event, a composite of coronary death, myocardial infarction, or coronary revascularization. RESULTS: During the median follow-up period of 4.1 years, the primary outcome occurred in significantly fewer patients in the anacetrapib group than in the placebo group (1640 of 15,225 patients [10.8%] vs. 1803 of 15,224 patients [11.8%]; rate ratio, 0.91; 95% confidence interval, 0.85 to 0.97; P=0.004). The relative difference in risk was similar across multiple prespecified subgroups. At the trial midpoint, the mean level of HDL cholesterol was higher by 43 mg per deciliter (1.12 mmol per liter) in the anacetrapib group than in the placebo group (a relative difference of 104%), and the mean level of non-HDL cholesterol was lower by 17 mg per deciliter (0.44 mmol per liter), a relative difference of -18%. There were no significant between-group differences in the risk of death, cancer, or other serious adverse events. CONCLUSIONS: Among patients with atherosclerotic vascular disease who were receiving intensive statin therapy, the use of anacetrapib resulted in a lower incidence of major coronary events than the use of placebo. Journey Through the Blues

VR can help core text programs expand the idea of “text” and enrich their content, but the humanities have perhaps been slower to embrace this technology than our peers in STEM fields. Coming from a background in English literature and teaching in a first-year seminar program, I have been seeking ways to engage my students more fully in understanding art and culture, formats that move beyond the limitations of text. Using VR technology, I have undertaken the development of a three-dimensional “blues joint,” with the help of Andrew Grace, Senior Applications Developer for the CAVE at Villanova University. We have created an immersive experience to share both blues poems by Langston Hughes (visually and orally) and performances of the blues, music that inspired his work. Nearly 100 years after the publication of The Weary Blues in 1925, Hughes’ first volume of verse, we can celebrate anew the then-novelty of the multiple modality of his work, which cannot be confined to the page. This range of modality is more accessible to students through the multimedia experience of the CAVE, which situates Hughes’ poems within the cultural context of the Harlem Renaissance and its aims, creating an exchange between Hughes, two other poets, and an array of blues musicians who are showcased in the project. This presentation will discuss how the CAVE program named Journey Through the Blues aids students in considering not only the musical quality that is inherent in word choice and lyricism of composition, but also a more direct celebration of the musical form that underlies the poem: the blues, a musical form with which most students are not familiar

Trends in Temperature, Secchi Depth, and Dissolved Oxygen Depletion Rates in the Central Basin of Lake Erie, 1983–2002

    No full text
We examined temperature trends in a 20-year set of monitoring records collected at multiple deep-water stations in the central basin of Lake Erie. Data collected were statistically corrected (“deseasonalized”) to remove biases resulting from irregular sampling intervals within years. Depthintegrated summer temperature has increased by an average (±SE) of 0.037 ± 0.01°C per year. An observed reduction of Secchi depth (SD) by 7±3 cm/y seems to be unrelated to variation in either total phosphorus (TP) or chlorophyll a concentrations. Midsummer midbasin SD values varied widely between 4 and 10 m, possibly depending on whether phytoplankton were concentrated in the epilimnion (giving shallow SD), or whether phytoplankton had settled out of the epilimnion into the lower layers, giving deeper SD values. Hypolimnetic volume-corrected oxygen depletion (HVOD) rates have also been highly variable, ranging from 2.68 to 4.72 mg/L/mo. These rates are sensitive to production of oxygen in the thermocline and hypolimnion by photosynthetically active phytoplankton that have settled from the epilimnion. The HVOD rate in any year was correlated with the previous year\u27s TP loading into Lake Erie. Since TP loading trends largely reflect the consequences of improving water treatment through the 1980s and increasing contributions from tributary run-off sediments through the 1990s, there is little direct evidence to suggest that the appearance of dreissenids has directly influenced hypolimnial oxygen depletion rates in the central basin. The observation that central-basin HVOD tracked the reductions in TP loadings through the 1980s may be the first affirmation that central basin hypolimnetic oxygen dynamics can be regulated by phosphorus inputs. This implies that TP loads must continue to be regulated if we wish to minimize oxygen depletion rates as a strategy to reduce the frequency of episodic central basin anoxia
