    Reductions in C-reactive protein in older adults with type 2 diabetes are related to improvements in body composition following a randomized controlled trial of resistance training

    Background: Reductions in skeletal muscle mass and increased adiposity are key elements in the aging process and in the pathophysiology of several chronic diseases. Systemic low grade inflammation associated with obesity has been shown to accelerate the age‐related decline in skeletal muscle. The aim of this investigation was to determine the effects of 12 months of progressive resistance training (PRT) on systemic inflammation, and whether reductions in systemic inflammation were associated with changes in body composition. We hypothesized that reductions in systemic inflammation following 12 months of PRT in older adults with type 2 diabetes would be associated with reductions in adiposity and increases in skeletal muscle mass. Methods: Participants (n = 103) were randomized to receive either PRT or sham‐exercise, 3 days a week for 12 months. C‐reactive protein (CRP) was used to assess systemic inflammation. Skeletal muscle mass and total fat mass were determined using bioelectrical impedance. Results: Twelve months of PRT tended to reduce CRP compared to sham exercise (β = −0.25, p = 0.087). Using linear mixed‐effects models, the hypothesized relationships between body composition adaptations and CRP changes were significantly stronger for skeletal muscle mass (p = 0.04) and tended to be stronger for total fat mass (p = 0.07) following PRT when compared to sham‐exercise. Using univariate regression models, stratified by group allocation, reductions in CRP were associated with increases in skeletal muscle mass (p = 0.01) and reductions in total fat mass (p = 0.02) in the PRT group, but not in the sham‐exercise group (p = 0.87 and p = 0.32, respectively). Conclusions: We have shown for the first time that reductions in systemic inflammation in older adults with type 2 diabetes following PRT were associated with increases in skeletal muscle mass. Furthermore, reductions in CRP were associated with reductions in adiposity, but only when associated with PRT. Lifestyle interventions aimed at reducing systemic inflammation in older adults with type 2 diabetes should therefore incorporate anabolic exercise such as PRT to optimize the anti‐inflammatory benefits of favorable body composition adaptations

    Population preference values for treatment outcomes in chronic lymphocytic leukaemia: a cross-sectional utility study

    <p>Abstract</p> <p>Background</p> <p>Given that treatments for chronic lymphocytic leukaemia (CLL) are palliative rather than curative, evaluating the patient-perceived impacts of therapy is critical. To date, no utility (preference) studies from the general public or patient perspective have been conducted in CLL. The objective of this study was to measure preferences for health states associated with CLL treatment.</p> <p>Methods</p> <p>This was a cross-sectional study of 89 members of the general population in the UK (England and Scotland). Using standard gamble, each participant valued four health states describing response status, six describing treatment-related toxicities based on Common Toxicity Criteria, and two describing line of treatment. The health states incorporated standardized descriptions of treatment response (symptoms have "improved," "stabilized," or "gotten worse"), swollen glands, impact on daily activities, fatigue, appetite, and night sweats. Utility estimates ranged from 0.0, reflecting dead, to 1.0, reflecting full health.</p> <p>Results</p> <p>Complete response (CR) was the most preferred health state (mean utility, 0.91), followed by partial response (PR), 0.84; no change (NC), 0.78; and progressive disease (PD), 0.68. Among the toxicity states, grade I/II nausea and nausea/vomiting had the smallest utility decrements (both were -0.05), and grade III/IV pneumonia had the greatest decrement (-0.20). The utility decrements obtained for toxicity states can be subtracted from utilities for CR, PR, NC, and PD, as appropriate. The utilities for second- and third-line treatments, which are attempted when symptoms worsen, were 0.71 and 0.65, respectively. No significant differences in utilities were observed by age, sex, or knowledge/experience with leukaemia.</p> <p>Conclusions</p> <p>This study reports UK population utilities for a universal set of CLL health states that incorporate intended treatment response and unintended toxicities. These utilities can be applied in future cost-effectiveness analyses of CLL treatment.</p

    Stable Isotope Analysis of Human Bone from Ganj Dareh, Iran, Ca. 10,100 calBP

    We report here on stable carbon, nitrogen, and sulfur isotope values from bone collagen of human (n = 20) and faunal (n = 11) remains from the Early Neolithic site of Ganj Dareh, Iran, dating to ca. 10,100 cal. BP. Our focus explores how isotope values of human bone vary by age and sex, and evaluates dietary practices at this site. It also provides a baseline for future studies of subsistence in the early Holocene Central Zagros Mountains, from the site with the first evidence for human ovicaprid management in the Near East. Human remains include individuals of all age groups for dietary reconstruction, as well two Ottoman intrusive burials for temporal and cultural comparison. All analyzed individuals exhibited δ13C and δ15N values consistent with a diet based heavily on C3 terrestrial sources. There is no statistically significant difference between the isotopic compositions of the two sexes, though males appear to show larger variations compared to females. Interesting patterns in the isotopic compositions of the subadults suggested weaning children may be fed with supplements with distinctive δ13C values. Significant difference in sulfur isotope values between humans and fauna could be the earliest evidence of transhumance and could identify one older adult male as a possible transhumant shepherd. Both Ottoman individuals had distinctively different δ13C, δ15N, and δ34S values compared to the Neolithic individuals. This is the first large scale analysis of human stable isotopes from the eastern end of the early Holocene Fertile Crescent. It provides a baseline for future intersite exploration of stable isotopes and insight into the lifeways, health, and processes of neolithisation associated with the origins of goat domestication at Ganj Dareh and the surrounding Central Zagros

    Rock mass loss on a nunatak in Western Dronning Maud Land, Antarctica

    This paper presents the first rock mass loss data for uncut clasts from continental Antarctica. A rock mass loss experiment using doleritic rock samples was conducted over a seven-year period, between 2008 and 2014, at the Vesleskarvet nunataks, Western Dronning Maud Land. The data show that approximately 10% of clasts suffered a mass loss that is an order of magnitude greater than the remaining 90% of clasts. Thus, the observed rock mass loss is suggested to occur in a series of events that are impossible to predict in terms of frequency and/or magnitude. However, extrapolating from the data obtained during the seven-year period indicates that rates of mass loss are slow and of the order of 1% per 100 years. Direct erosion by wind (including abrasion) as well as mechanical and chemical weathering are suggested to be responsible for rock mass loss. Rock properties, the weathering environment, and a lack of available moisture may be contributing factors to the slow rate of rock decay. This paper suggests that in this area of Antarctica, the slow rate of rock mass loss increases the longevity of existing periglacial landforms such as patterned ground and blockfields, but inhibits development of new patterned ground through the slow production of fines.The Department of Environmental Affairs and the National Research Foundation are gratefully acknowledged for logistical and financial support.     Utilizing Public Blockchains for the Sybil-Resistant Bootstrapping of Distributed Anonymity Services

    Distributed anonymity services, such as onion routing networks or cryptocurrency tumblers, promise privacy protection without trusted third parties. While the security of these services is often well-researched, security implications of their required bootstrapping processes are usually neglected: Users either jointly conduct the anonymization themselves, or they need to rely on a set of non-colluding privacy peers. However, the typically small number of privacy peers enable single adversaries to mimic distributed services. We thus present AnonBoot, a Sybil-resistant medium to securely bootstrap distributed anonymity services via public blockchains. AnonBoot enforces that peers periodically create a small proof of work to refresh their eligibility for providing secure anonymity services. A pseudo-random, locally replicable bootstrapping process using on-chain entropy then prevents biasing the election of eligible peers. Our evaluation using Bitcoin as AnonBoot's underlying blockchain shows its feasibility to maintain a trustworthy repository of 1000 peers with only a small storage footprint while supporting arbitrarily large user bases on top of most blockchains.Comment: To be published in the proceedings of the 15th ACM ASIA Conference on Computer and Communications Security (ACM ASIACCS'20

    SoK: hate, harassment, and the changing landscape of online abuse

    We argue that existing security, privacy, and antiabuse protections fail to address the growing threat of online hate and harassment. In order for our community to understand and address this gap, we propose a taxonomy for reasoning about online hate and harassment. Our taxonomy draws on over 150 interdisciplinary research papers that cover disparate threats ranging from intimate partner violence to coordinated mobs. In the process, we identify seven classes of attacks—such as toxic content and surveillance—that each stem from different attacker capabilities and intents. We also provide longitudinal evidence from a three-year survey that hate and harassment is a pervasive, growing experience for online users, particularly for at-risk communities like young adults and people who identify as LGBTQ+. Responding to each class of hate and harassment requires a unique strategy and we highlight five such potential research directions that ultimately empower individuals, communities, and platforms to do so.Accepted manuscrip

    Exploring South Africa’s southern frontier: A 20-year vision for polar research through the South African National Antarctic Programme

    Antarctica, the sub-Antarctic islands and surrounding Southern Ocean are regarded as one of the planet’s last remaining wildernesses, ‘insulated from threat by [their] remoteness and protection under the Antarctic Treaty System’1 . Antarctica encompasses some of the coldest, windiest and driest habitats on earth. Within the Southern Ocean, sub-Antarctic islands are found between the Sub-Antarctic Front to the north and the Polar Front to the south. Lying in a transition zone between warmer subtropical and cooler Antarctic waters, these islands are important sentinels from which to study climate change.2 A growing body of evidence3,4 now suggests that climatically driven changes in the latitudinal boundaries of these two fronts define the islands’ short- and long-term atmospheric and oceanic circulation patterns. Consequently, sub-Antarctic islands and their associated terrestrial and marine ecosystems offer ideal natural laboratories for studying ecosystem response to change.5 For example, a recent study6 indicates that the shift in the geographical position of the oceanic fronts has disrupted inshore marine ecosystems, with a possible impact on top predators. Importantly, biotic responses are variable as indicated by different population trends of these top predators.7,8 When studied collectively, these variations in species’ demographic patterns point to complex spatial and temporal changes within the broader sub-Antarctic ecosystem, and invite further examination of the interplay between extrinsic and intrinsic drivers

    Sex-specific Trans-regulatory Variation on the Drosophila melanogaster X Chromosome

    The X chromosome constitutes a unique genomic environment because it is present in one copy in males, but two copies in females. This simple fact has motivated several theoretical predictions with respect to how standing genetic variation on the X chromosome should differ from the autosomes. Unmasked expression of deleterious mutations in males and a lower census size are expected to reduce variation, while allelic variants with sexually antagonistic effects, and potentially those with a sex-specific effect, could accumulate on the X chromosome and contribute to increased genetic variation. In addition, incomplete dosage compensation of the X chromosome could potentially dampen the male-specific effects of random mutations, and promote the accumulation of X-linked alleles with sexually dimorphic phenotypic effects. Here we test both the amount and the type of genetic variation on the X chromosome within a population of Drosophila melanogaster, by comparing the proportion of X linked and autosomal trans-regulatory SNPs with a sexually concordant and discordant effect on gene expression. We find that the X chromosome is depleted for SNPs with a sexually concordant effect, but hosts comparatively more SNPs with a sexually discordant effect. Interestingly, the contrasting results for SNPs with sexually concordant and discordant effects are driven by SNPs with a larger influence on expression in females than expression in males. Furthermore, the distribution of these SNPs is shifted towards regions where dosage compensation is predicted to be less complete. These results suggest that intrinsic properties of dosage compensation influence either the accumulation of different types of trans-factors and/or their propensity to accumulate mutations. Our findings document a potential mechanistic basis for sex-specific genetic variation, and identify the X as a reservoir for sexually dimorphic phenotypic variation. These results have general implications for X chromosome evolution, as well as the genetic basis of sex-specific evolutionary change

    A review of epidural simulators: Where are we today?

    Thirty-one central neural blockade simulators have been implemented into clinical practice over the last thirty years either commercially or for research. This review aims to provide a detailed evaluation of why we need epidural and spinal simulators in the first instance and then draws comparisons between computer-based and manikin-based simulators. This review covers thirty-one simulators in total; sixteen of which are solely epidural simulators, nine are for epidural plus spinal or lumbar puncture simulation, and six, which are solely lumbar puncture simulators. All hardware and software components of simulators are discussed, including actuators, sensors, graphics, haptics, and virtual reality based simulators. The purpose of this comparative review is to identify the direction for future epidural simulation by outlining necessary improvements to create the ideal epidural simulator. The weaknesses of existing simulators are discussed and their strengths identified so that these can be carried forward. This review aims to provide a foundation for the future creation of advanced simulators to enhance the training of epiduralists, enabling them to comprehensively practice epidural insertion in vitro before training on patients and ultimately reducing the potential risk of harm. © 2013 IPEM
