    Geographical variation in antimicrobial use and multiresistant pathogens in Brazilian intensive care units: a nationwide study

    Introduction: Geographical analyses of antibiotic use identify regions with the highest consumption and help design policies for strategic patient groups.Methodology: We conducted a cross-sectional study based on official data available in July 2022 from Brazilian Health Surveillance Agency (Anvisa). Antibiotics are reported as a defined daily dose (DDD) per 1,000 patient-days, and central line-associated bloodstream infection (CLABSI) is defined according to Anvisa criteria. We also considered multi-drug resistant (MDR) as the critical pathogens the World Health Organization listed. We measured antimicrobial use and CLABSI trends per ICU bed using the compound annual growth rate (CAGR). Results: we evaluated the regional variation in CLABSI by multidrug-resistant pathogens and the antimicrobial use in 1,836 hospital intensive care units (ICUs). In 2020, the leader in use in intensive care units (ICUs) in the North was piperacillin/tazobactam (DDD = 929.7) in the Northeast. Midwest and South were meropenem (DDD = 809.4 and DDD = 688.1, respectively), and Southeast was ceftriaxone (DDD = 751.1). The North has reduced polymyxin use (91.1%), and ciprofloxacin increased (439%) in the South. There was an increase in CLABSI by carbapenem-resistant Pseudomonas aeruginosa in the North region (CAGR = 120.5%). Otherwise, CLABSI by vancomycin-resistant Enterococcus faecium (VRE) increased in all regions except the North (CAGR =-62.2%), while that carbapenem-resistant Acinetobacter baumannii increased in the Midwest (CAGR = 27.3%). Conclusions: we found heterogeneity in antimicrobial use patterns and CLABSI etiology among Brazilian ICUs. Although Gram-negative bacilli were the primary responsible agent, we observed a notable increase trend of CLABSI by VRE

    A before and after study of integrated training sessions for children's health and care services

    Recent UK policy drivers such as the National Collaboration for Integrated Care and Support and Making Every Contact Count prioritise integrated care, an approach that seeks to provide more coordinated and seamless health and social care. In children's services, despite many partners, there are challenges around integrating care. A deprived borough of London ran short training and networking sessions for services supporting children and young people. This study examined whether intersectoral training would improve participants' knowledge of local services and joint working (including communication, navigation and confidence in collaboration). As part of a service evaluation, the study utilised a pre–post Likert scale survey design for each training session, a 1‐month follow‐up survey, and telephone interviews with a subsample of participants. The educational intervention was three sets of 1.5 hr educational workshops from December 2016 to February 2017. There were 302 attendances from 202 individuals from the health (n = 99), education (n = 145), social care (n = 39) and voluntary (n = 19) sectors. The pre and post surveys found significant increases in self‐assessed knowledge of health/education/social care/voluntary services and in some elements of joint working. However, these increases were not sustained in any domain after 1 month of follow‐up. There was also no difference in self‐assessments amongst those who attended three sessions compared to those who attended one or two. Telephone interviewees highlighted networking as being helpful and suggested that informative tasks and diverse attendance would be beneficial in future. To conclude, this study suggests that although short‐learning sessions may seem to improve immediate knowledge and some elements of joint working in the short term, any gains are not sustained in the long term. The cost effectiveness of such schemes is in doubt but may be improved by a more targeted delivery of content

    Modelling the radio to X-ray SED of galaxies

    We present our model to interpret the SED of galaxies. The model for the UV to sub-mm SED is already well established (Silva et al 1998). We remind here its main features and show some applications.     Aim: To evaluate the effects of preventive treatment with low-level laser therapy (LLLT) on progression of dystrophy in mdx mice. Methods: Ten animals were randomly divided into 2 experimental groups treated with superpulsed LLLT (904 nm, 15 mW, 700 Hz, 1 J) or placebo-LLLT at one point overlying the tibialis anterior muscle (bilaterally) 5 times per week for 14 weeks (from 6th to 20th week of age). Morphological changes, creatine kinase (CK) activity and mRNA gene expression were assessed in animals at 20th week of age. Results: Animals treated with LLLT showed very few morphological changes in skeletal muscle, with less atrophy and fibrosis than animals treated with placebo-LLLT. CK was significantly lower (p = 0.0203) in animals treated with LLLT (864.70 U.l−1, SEM 226.10) than placebo (1708.00 U.l−1, SEM 184.60). mRNA gene expression of inflammatory markers was significantly decreased by treatment with LLLT (p<0.05): TNF-α (placebo-control = 0.51 ”g/”l [SEM 0.12], - LLLT = 0.048 ”g/”l [SEM 0.01]), IL-1ÎČ (placebo-control = 2.292 ”g/”l [SEM 0.74], - LLLT = 0.12 ”g/”l [SEM 0.03]), IL-6 (placebo-control = 3.946 ”g/”l [SEM 0.98], - LLLT = 0.854 ”g/”l [SEM 0.33]), IL-10 (placebo-control = 1.116 ”g/”l [SEM 0.22], - LLLT = 0.352 ”g/”l [SEM 0.15]), and COX-2 (placebo-control = 4.984 ”g/”l [SEM 1.18], LLLT = 1.470 ”g/”l [SEM 0.73]). Conclusion: Irradiation of superpulsed LLLT on successive days five times per week for 14 weeks decreased morphological changes, skeletal muscle damage and inflammation in mdx mice. This indicates that LLLT has potential to decrease progression of Duchenne muscular dystrophy

    Assessing the role of CAP for more sustainable and healthier food systems in Europe:A literature review

    Today, the European food system is characterized by unhealthy dietary trends, environmentally unsustainable production, and a dependency on an ageing farming population. The ongoing reform of the Common Agricultural Policy (CAP) represents an opportunity to redress these issues. This literature review highlights trends in how academic and grey literature have received CAP attempts in addressing the (i) environmental issues, (ii) nutritional outcomes, and (iii) rural livelihoods. Additionally, future policy and research directions relating to the CAP have been identified from the selected literature. The reviewed literature varies in approach and perspective. In particular, since the environment and rural development are already part of the CAP, the reviewed studies analyze and propose improvements to existing mechanisms. While for nutrition, the reviewed studies assessed possible policy strategies for integrating this sphere within the CAP, highlighting both the complexity of this task as well as its potential benefits. Despite these differences, a clear commonality emerged from the policy recommendations: the CAP should promote the European Union (EU) policy integration and multi-disciplinary and participatory research as key strategies to meet food system sustainability targets.</p

    Heat source distribution, vertical structure, and coating influences on the temperature of operating 0.98 mu m laser diodes: Photothermal reflectance measurements

    In the present work single-quantum-well laser diodes operating at 0.98 mm are investigated by photothermal reflectance microscopy. Temperature maps were obtained for the output facet of all devices studied. Furthermore, the temperature distribution was determined along the cavity (on the ridge) of lasers soldered with the junction side up. Near the facets, the measured temperature was found to be about seven times the bulk's temperature, indicating the presence of an important surface heat source. The signal phase distribution of the laser facet shows the important role of the vertical structure on the heat confinement. Comparison between experiments and calculations shows that the confinement layers (GaAlAs and GaInP) thermal parameters are the principal responsible for the heat propagation in these structures near the active region. The same calculations show the role of the coating (Al2O3) in the heat propagation, and give a quantitative ratio between surface and bulk heat sources. Measurements made on the facet and on the ridge as a function of injection current were found to present a quite similar behavior, leading to the conclusion that thermal effects are strongly dominant in these measurements, masking any carrier or electroreflectance effects. Finally, measurements made under different light output power conditions and under the same injection current conditions showed that the surface heat source is caused by laser light absorption at the facets.
