    Investigation of physical activity, sleep, and mental health recovery in treatment resistant depression (TRD) patients receiving repetitive transcranial magnetic stimulation (rTMS) treatment

    Background : Repetitive transcranial magnetic stimulation (rTMS) is effective in treating depression; however, the effect on physical activity, sleep and recovery is unclear. This study investigated rTMS effect on physical activity and sleep through providing patients with a Fitbit and software apps; and reports the impact of rTMS on depression, anxiety and mental health recovery. Methods : Study design was a pre and post data collection without a control, with twenty-four participants with treatment-resistant depression (TRD). Measures used were Fitbit activity and sleep data, and patient-rated Recovering Quality of Life (ReQoL-20), Patient Health Questionnaire (PHQ-9) and Generalised Anxiety Disorder (GAD-7). Results : Response and remission rates were, respectively: 34.8% and 39% for PHQ-9; 34.8% and 47.8% for GAD-7. ReQoL-20 response and reliable improvement were 29.4% and 53%. PHQ-9, GAD-7 and ReQol-20 scores significantly improved, with large effect sizes. Analysis of Fitbit activity and sleep data yielded non-significant results. The Fitbit data machine learning model classified two levels of depression to 82% accuracy. Limitations : rTMS treatment was open-label and adjunct to existing antidepressant medication. No control group. Female patients were overrepresented. Conclusions : Improvements on the ReQoL-20 and aspects of sleep and activity indicate the positive impact of rTMS on the individual’s real world functioning and quality of life. A wearable activity tracker can provide feedback to patients and clinicians on sleep, physical activity and depression levels. Further research could be undertaken through a sufficiently powered RCT comparing rTMS versus rTMS with use of a Fitbit, its software applications, and sleep and physical activity advice

    Kinematics of Nearby Subdwarf Stars

    We present an analysis of the space motions of 742 subdwarf stars based on the sample of Carney et al. (1994, CLLA). Hipparcos parallaxes, TYC2+HIP proper motions and Tycho2 proper motions were combined with radial velocities and metallicities from CLLA. The kinematical behavior is discussed in particular in relation to their metallicities. The majority of these sample stars have metal abundances of [Fe/H] >-1 and represent the thick disk population. The halo component, with [Fe/H] <-1.6, is characterized by a low mean rotation velocity and a radially elongated velocity ellipsoid. In the intermediate metallicity range (-1.6 < [Fe/H] <-1), we find a significant number of subdwarfs with disklike kinematics.     The role of the clinical nurse specialist from the perspective of parents of children born with cleft lip and/or palate in the United Kingdom:a qualitative study

    This qualitative study explores parents' perspectives of the clinical nurse specialist (CNS) after diagnosis of cleft lip and/or palate and while preparing for/after lip or palate closure and other events on the cleft treatment pathway in the United Kingdom.Parents with children on the "cleft treatment pathway" were recruited from multiple regions across the United Kingdom. In-depth interviews were conducted with 24 parents of children born with cleft lip and/or palate.Thematic analysis identified 4 key themes: relationship with CNS, information provision, preoperative care and multidisciplinary team (MDT), and postoperative support.Clinical nurse specialists are a highly valued point of contact and act as a trusted mediator between parents and other professionals in the MDT. Clinical nurse specialists are important gatekeepers in providing support and information and developing trust after diagnosis and birth and throughout the cleft treatment pathway.The present findings affirm the emotional, social, and informational support that the CNS can offer parents whose children are on a cleft treatment pathway. The CNS is a vital member of the cleft service MDT

    Spatial variation in exhumation rates across Ladakh and the Karakoram: New apatite fission track data from the Eastern Karakoram, NW India

    Characterization of low-temperature cooling histories and associated exhumation rates is critical for deciphering the recent evolution of orogenic regions. However, these may vary significantly over relatively short distances within orogens. It is pertinent therefore to constrain cooling histories and hence exhumation rates across major tectonic boundaries. We report the first apatite fission track ages from the Karakoram Fault Zone in the Eastern Karakoram range, which forms part of the western margin of the Tibetan Plateau. Ten samples, from elevations of 3477–4875m, have apatite fission track dates from 3.3 ± 0.3 Ma to 7.4± 1.1Ma. The ages correspond to modeled average erosional exhumation rates of 0.67+ 0.27-0.18mm/yr across the Eastern Karakoram. The results are consistent with a trend northward from the Indus suture zone, across the Ladakh terrane and into the Karakoram, in which tectonic uplift associated with crustal thickening increases toward the north, raising elevation and promoting glaciation and generation of extreme relief. As a result, erosion and exhumation rates increase south to north. Present-day precipitation on the other hand varies little within the study area and on a larger scale decreases southwest to northeast across this portion of the orogen.     Chemical abundances of distant extremely metal-poor unevolved stars

    Aims: The purpose of our study is to determine the chemical composition of a sample of 16 candidate Extremely Metal-Poor (EMP) dwarf stars, extracted from the Sloan Digital Sky Survey (SDSS). There are two main purposes: in the first place to verify the reliability of the metallicity estimates derived from the SDSS spectra; in the second place to see if the abundance trends found for the brighter nearer stars studied previously also hold for this sample of fainter, more distant stars. Methods: We used the UVES at the VLT to obtain high-resolution spectra of the programme stars. The abundances were determined by an automatic analysis with the MyGIsFOS code, with the exception of lithium, for which the abundances were determined from the measured equivalent widths of the Li I resonance doublet. Results: All candidates are confirmed to be EMP stars, with [Fe/H]<= -3.0. The chemical composition of the sample of stars is similar to that of brighter and nearer samples. We measured the lithium abundance for 12 stars and provide stringent upper limits for three other stars, for a fourth star the upper limit is not significant, owing to the low signal-to noise ratio of the spectrum. The "meltdown" of the Spite plateau is confirmed, but some of the lowest metallicity stars of the sample lie on the plateau. Conclusions: The concordance of the metallicities derived from high-resolution spectra and those estimated from the SDSS spectra suggests that the latter may be used to study the metallicity distribution of the halo. The abundance pattern suggests that the halo was well mixed for all probed metallicities and distances. The fact that at the lowest metallicities we find stars on the Spite plateau suggests that the meltdown depends on at least another parameter, besides metallicity. (abridged)Comment: A&A in pres

    Biological maturation, relative age and self-regulation in male professional academy soccer players: A test of the underdog hypothesis

    ObjectivesThe main and interactive effects of biological maturity status and relative age upon self-regulation in male academy soccer players are considered. Consistent with the ‘underdog’ hypothesis, whereby relatively younger players may benefit from competitive play with older peers, it was predicted later maturing and/or relatively younger players would report more adaptive self-regulation.DesignCross-sectional study.MethodPlayers (n = 171, aged 11–16 years) from four English professional soccer academies completed the modified Soccer Self-Regulation Scale. Date of birth, height, weight and parental height were obtained. Relative age was based on birth quarter for the selection year. Maturity status was based upon percentage of predicted adult height attained.ResultsLinear regression models showed later maturation was inversely associated with adaptive self-regulation, while relative age was unrelated to self-regulation.ConclusionsIn partial support of the underdog hypothesis, later maturing players appear to possess a psychological advantage

    On the alleged duality of the Galactic halo

    We examine the kinematics of the Galactic halo based on SDSS/SEGUE data by Carollo et al. (2007, 2010). We find that their claims of a counter-rotating halo are the result of substantial biases in distance estimates (of order 50%): the claimed retrograde component, which makes up only a tiny fraction of the entire sample, prone to contaminations, is identified as the tail of distance overestimates. The strong overestimates also result in a lift in the vertical velocity component, which explains the large altitudes those objects were claimed to reach. Errors are worst for the lowest metallicity stars, which explains the metal-poor nature of the artificial component. We also argue that measurement errors were not properly accounted for and that the use of Gaussian fitting on intrinsically non-Gaussian Galactic components invokes the identification of components that are distorted or even artificial. Our evaluation of the data leads to a revision of the estimated velocity ellipsoids and does not yield any reliable evidence for a counterrotating halo component. If a distinct counterrotating halo component exists it must be far weaker than claimed by Carollo et al. Finally we note that their revised analysis presented in Beers et al. The central problems of time pressure, language difficulties, a lack of critical analysis and a prevalence of personal problems among international students are discussed. This paper makes recommendations for the improvement of language and critical thinking skills, and questions the future policy of language requirements at HE for international Masters students

    Constraining cosmic scatter in the Galactic halo through a differential analysis of metal-poor stars

    © ESO 2017.Context. The chemical abundances of metal-poor halo stars are important to understanding key aspects of Galactic formation and evolution. Aims. We aim to constrain Galactic chemical evolution with precise chemical abundances of metal-poor stars (−2.8 ≀ [Fe/H] ≀ −1.5). Methods. Using high resolution and high S/N UVES spectra of 23 stars and employing the differential analysis technique we estimated stellar parameters and obtained precise LTE chemical abundances. Results. We present the abundances of Li, Na, Mg, Al, Si, Ca, Sc, Ti, V, Cr, Mn, Co, Ni, Zn, Sr, Y, Zr, and Ba. The differential technique allowed us to obtain an unprecedented low level of scatter in our analysis, with standard deviations as low as 0.05 dex, and mean errors as low as 0.05 dex for [X/Fe]. Conclusions. By expanding our metallicity range with precise abundances from other works, we were able to precisely constrain Galactic chemical evolution models in a wide metallicity range (−3.6 ≀ [Fe/H] ≀ −0.4). The agreements and discrepancies found are key for further improvement of both models and observations. We also show that the LTE analysis of Cr II is a much more reliable source of abundance for chromium, as Cr I has important NLTE effects. These effects can be clearly seen when we compare the observed abundances of Cr I and Cr II with GCE models. While Cr I has a clear disagreement between model and observations, Cr II is very well modeled. We confirm tight increasing trends of Co and Zn toward lower metallicities, and a tight flat evolution of Ni relative to Fe. Our results strongly suggest inhomogeneous enrichment from hypernovae. Our precise stellar parameters results in a low star-to-star scatter (0.04 dex) in the Li abundances of our sample, with a mean value about 0.4 dex lower than the prediction from standard Big Bang nucleosynthesis; we also study the relation between lithium depletion and stellar mass, but it is difficult to assess a correlation due to the limited mass range. We find two blue straggler stars, based on their very depleted Li abundances. One of them shows intriguing abundance anomalies, including a possible zinc enhancement, suggesting that zinc may have been also produced by a former AGB companion.Peer reviewedFinal Published versio
