    Plasminogen activator levels are influenced by location and varicosity in greater saphenous vein

    AbstractPurpose: The plasminogen system, which includes tissue type plasminogen activator (tPA), urokinase type plasminogen activator (uPA), and their main inhibitor, plasminogen activator inhibitor type 1 (PAI-1), plays a major role in both fibrinolysis and tissue remodeling. This study compares the levels of tPA, uPA, and PAI-1 at the groin and ankle in normal and varicose greater saphenous vein (GSV).Methods: GSV was collected from patients undergoing varicose vein (VV) removal and from normal vein (NV) from arterial bypass procedures. Portions of the GSV at the groin and the ankle were minced and placed in serum-free media for 48 hours. Assays of the supernatants were obtained for tPA, uPA, and PAI-1 protein by enzyme-linked immunosorbent assay. Cyclohexamide and actinomycin D were also added to the media of the VV tissue explant supernatants to inhibit protein and RNA synthesis, respectively.Results: Levels of tPA were significantly higher at the groin (11 ± 2) than the ankle (5 ± 1) in the VV ( p < 0.005), and this trend was also seen in the NV (groin 10 ± 2 and ankle 7 ± 3). Levels of uPA were significantly higher in the groin VV (14 ± 4.3) than in NV (3.0 ± 0.8, p < 0.05). This difference, although not statistically significant, applied to the ankle as well (VV 14.5 ± 6.3 and NV 5.3 ± 2.7). No significant difference was seen between NV and VV for PAI-1 (NV, groin 155 ± 73 and ankle 113 ± 53, VV, groin 161 ± 20 and ankle 142 ± 38) or tPA. Inhibitor studies revealed no significant difference among control, cyclohexamide, and actinomycin D supernatants for tPA, suggesting release of protein rather than active synthesis. In contrast, inhibitor supernatants were significantly lower for uPA and PAI-1 than control supernatants ( p < 0.05), suggesting that uPA and PAI-1 were actively synthesized.Conclusions: In the tissue explant supernatant model uPA and PAI-1 are actively synthesized, but tPA is not. Levels of PAI-1 were comparable in all four groups. Levels of uPA in the varicose GSV were higher than in NV, suggesting a role for uPA in the pathologic makeup of VV. Levels of tPA were higher at the groin versus the ankle position, potentially explaining the previously described increased fibrinolytic activity seen at the groin. (J Vasc Surg 1996;24:719-24.

    O aumento da percentagem de gravidez na adolescência (Portugal regista 19/1000 na faixa etária dos 15 aos 19 anos), juntando-se a outros riscos ligados à actividade sexual, tornam os jovens um grupo vulnerável em termos de saúde sexual e reprodutiva. Esta investigação avaliou os conhecimentos e atitudes sobre os métodos contraceptivos em jovens estudantes universitários. Administraram-se 436 questioná - rios a 113 rapazes e 323 raparigas, entre os 18 e os 24 anos. Aplicaram-se medidas de conhecimento (CKI; Delcampo e Delcampo, 1976) e de atitudes (CAS; Kyes, 1987) acerca dos métodos contraceptivos. Os resultados obtidos demonstram que a maioria tem pouco conhecimento acerca dos métodos contraceptivos mas revelou uma forte atitude preventiva face a uma gravidez indesejada. Encontraram-se diferenças estatis - tica mente signficativas entre indivíduos de sexo diferentes: as raparigas apresent am mais conhecimentos e preocupação preventiva face à contracepção. ------ ABSTRACT ------The increase in unplanned pregnancies (the rate in Portugal is 19/1000 in 15-19 year-olds), is responsible for ranking young people as an important target group in terms of sexual health prevention. This study tested knowledge and attitude towards contraceptive methods in 18-24 year old college students. Structured selfreported questionnaires were responded by 436 participants (113 college men and 323 college women). Questionnaires assessed knowledge (CKI; Delcampo e Delcampo, 1976) and attitude (CAS; Kyes, 1987) towards contraceptive methods. The findings show the majority has insufficient contraceptive knowledge although they have a strong preventive attitude towards unwanted pregnancy. The results reveal significant variation in responses by gender: college women demonstra ted better knowledge and preventive attitude in relation to contraception

    Philopatry and regional connectivity of the great hammerhead shark, Sphyrna mokarran in the U.S. and Bahamas

    A thorough understanding of movement patterns of a species is critical for designing effective conservation and management initiatives. However, generating such information for large marine vertebrates is challenging, as they typically move over long distances, live in concealing environments, are logistically difficult to capture and, as upper-trophic predators, are naturally low in abundance. Large-bodied, broadly distributed tropical shark typically restricted to coastal and shelf habitats, the great hammerhead shark Sphyrna mokarran epitomizes such challenges. Highly valued for its fins (in target and incidental fisheries), it suffers high bycatch mortality coupled with fecundity conservative life history, and as a result, is vulnerable to over-exploitation and population depletion. Although there are very little species-specific data available, the absence of recent catch records give cause to suspect substantial declines across its range. Here, we used biotelemetry techniques (acoustic and satellite), conventional tagging, laser-photogrammetry, and photo-identification to investigate the level of site fidelity/residency for great hammerheads to coastal areas in the Bahamas and U.S., and the extent of movements and connectivity of great hammerheads between the U.S. and Bahamas. Results revealed large-scale return migrations (3030 km), seasonal residency to local areas (some for 5 months), site fidelity (annual return to Bimini and Jupiter for many individuals) and numerous international movements.     Tsunami hazard assessment of coastal South Africa based on mega-earthquakes of remote subduction zones

    After the mega-earthquakes and concomitant devastating tsunamis in Sumatra (2004) and Japan (2011), we launched an investigation into the potential risk of tsunami hazard to the coastal cities of South Africa. This paper presents the analysis of the seismic hazard of seismogenic sources that could potentially generate tsunamis, as well as the analysis of the tsunami hazard to coastal areas of South Africa. The subduction zones of Makran, South Sandwich Island, Sumatra, and the Andaman Islands were identified as possible sources of mega-earthquakes and tsunamis that could affect the African coast. Numerical tsunami simulations were used to investigate the realistic and worst-case scenarios that could be generated by these subduction zones. The simulated tsunami amplitudes and run-up heights calculated for the coastal cities of Cape Town, Durban, and Port Elizabeth are relatively small and therefore pose no real risk to the South African coast.     Genetic Sharing with Cardiovascular Disease Risk Factors and Diabetes Reveals Novel Bone Mineral Density Loci.

    Bone Mineral Density (BMD) is a highly heritable trait, but genome-wide association studies have identified few genetic risk factors. Epidemiological studies suggest associations between BMD and several traits and diseases, but the nature of the suggestive comorbidity is still unknown. We used a novel genetic pleiotropy-informed conditional False Discovery Rate (FDR) method to identify single nucleotide polymorphisms (SNPs) associated with BMD by leveraging cardiovascular disease (CVD) associated disorders and metabolic traits. By conditioning on SNPs associated with the CVD-related phenotypes, type 1 diabetes, type 2 diabetes, systolic blood pressure, diastolic blood pressure, high density lipoprotein, low density lipoprotein, triglycerides and waist hip ratio, we identified 65 novel independent BMD loci (26 with femoral neck BMD and 47 with lumbar spine BMD) at conditional FDR < 0.01. Many of the loci were confirmed in genetic expression studies. Genes validated at the mRNA levels were characteristic for the osteoblast/osteocyte lineage, Wnt signaling pathway and bone metabolism. The results provide new insight into genetic mechanisms of variability in BMD, and a better understanding of the genetic underpinnings of clinical comorbidity

    Factors Associated with Revision Surgery after Internal Fixation of Hip Fractures

    Background: Femoral neck fractures are associated with high rates of revision surgery after management with internal fixation. Using data from the Fixation using Alternative Implants for the Treatment of Hip fractures (FAITH) trial evaluating methods of internal fixation in patients with femoral neck fractures, we investigated associations between baseline and surgical factors and the need for revision surgery to promote healing, relieve pain, treat infection or improve function over 24 months postsurgery. Additionally, we investigated factors associated with (1) hardware removal and (2) implant exchange from cancellous screws (CS) or sliding hip screw (SHS) to total hip arthroplasty, hemiarthroplasty, or another internal fixation device. Methods: We identified 15 potential factors a priori that may be associated with revision surgery, 7 with hardware removal, and 14 with implant exchange. We used multivariable Cox proportional hazards analyses in our investigation.
