Thymic tumours and myasthenia gravis: a radiological investigation
2. Tumours occur in the thymus gland in approximately 13% of
patients suffering from myasthenia gravis.3. Some of these tumours are probably present for some years
before myasthenic symptoms occur.4. So far as can be ascertained, these tumours are only
locally malignant, deposits being confined to the mediastinum and
pleura, and rarely within the lung.5. Seventeen cases of thymic tumour in myasthenia gravis are reported, three of which have metastasised locally, i.e. 18%.6. A special radiological technique has been developed to
demonstrate fully the various types of these thymic tumours.1. It appears that many cases previously reported as being
malignant tumours of the thymus are probably secondary tumours
Anakinra reduces blood pressure and renal fibrosis in one kidney/DOCA/salt-induced hypertension
OBJECTIVE: To determine whether a clinically-utilised IL-1 receptor antagonist, anakinra, reduces renal inflammation, structural damage and blood pressure (BP) in mice with established hypertension. METHODS: Hypertension was induced in male mice by uninephrectomy, deoxycorticosterone acetate (2.4mg/d,s.c.) and replacement of drinking water with saline (1K/DOCA/salt). Control mice received uninephrectomy, a placebo pellet and normal drinking water. 10days post-surgery, mice commenced treatment with anakinra (75mg/kg/d, i.p.) or vehicle (0.9% saline, i.p.) for 11 days. Systolic BP was measured by tail cuff while qPCR, immunohistochemistry and flow cytometry were used to measure inflammatory markers, collagen and immune cell infiltration in the kidneys. RESULTS: By 10 days post-surgery, 1K/DOCA/salt-treated mice displayed elevated systolic BP (148.3+/-2.4mmHg) compared to control mice (121.7+/-2.7mmHg; n=18, P\u3c0.0001). The intervention with anakinra reduced BP in 1K/DOCA/salt-treated mice by approximately 20mmHg (n=16, P\u3c0.05), but had no effect in controls. In 1K/DOCA/salt-treated mice, anakinra modestly reduced ( approximately 30%) renal expression of some (CCL5, CCL2; n=7-8; P\u3c0.05) but not all (ICAM-1, IL-6) inflammatory markers, and had no effect on immune cell infiltration (n=7-8, P \u3e 0.05). Anakinra reduced renal collagen content (n=6, P\u3c0.01) but paradoxically appeared to exacerbate the renal and glomerular hypertrophy (n=8-9, P\u3c0.001) that accompanied 1K/DOCA/salt-induced hypertension. CONCLUSION: Despite its anti-hypertensive and renal anti-fibrotic actions, anakinra had minimal effects on inflammation and leukocyte infiltration in mice with 1K/DOCA/salt-induced hypertension. Future studies will assess whether the anti-hypertensive actions of anakinra are mediated by protective actions in other BP-regulating or salt-handling organs such as the arteries, skin and brain
The Malawi National Tuberculosis Programme: an equity analysis

Abstract

Background

Until 2005, the Malawi National Tuberculosis Control Programme had been implemented as a vertical programme. Working within the Sector Wide Approach (SWAp) provides a new environment and new opportunities for monitoring the equity performance of the programme. This paper synthesizes what is known on equity and TB in Malawi and highlights areas for further action and advocacy.

Methods

A synthesis of a wide range of published and unpublished reports and studies using a variety of methodological approaches was undertaken and complemented by additional analysis of routine data on access to TB services. The analysis and recommendations were developed, through consultation with key stakeholders in Malawi and a review of the international literature.

Results

The lack of a prevalence survey severely limits the epidemiological knowledge base on TB and vulnerability. TB cases have increased rapidly from 5,334 in 1985 to 28,000 in 2006. This increase has been attributed to HIV/AIDS; 77% of TB patients are HIV positive. The age/gender breakdown of TB notification cases mirrors the HIV epidemic with higher rates amongst younger women and older men. The WHO estimates that only 48% of TB cases are detected in Malawi. The complexity of TB diagnosis requires repeated visits, long queues, and delays in sending results. This reduces poor women and men's ability to access and adhere to services. The costs of seeking TB care are high for poor women and men – up to 240% of monthly income as compared to 126% of monthly income for the non-poor. The TB Control Programme has attempted to increase access to TB services for vulnerable groups through community outreach activities, decentralising DOT and linking with HIV services.

Conclusion

The Programme of Work which is being delivered through the SWAp is a good opportunity to enhance equity and pro-poor health services. The major challenge is to increase case detection, especially amongst the poor, where we assume most 'missing cases' are to be found. In addition, the Programme needs a prevalence survey which will enable thorough equity monitoring and the development of responsive interventions to promote service access amongst 'missing' women, men, boys and girls.</p
Measurement of the ttbar Production Cross Section in ppbar Collisions at sqrt(s) = 1.96 TeV
We present a measurement of the top quark pair production cross section in
ppbar collisions at sqrt(s)=1.96 TeV using 318 pb^{-1} of data collected with
the Collider Detector at Fermilab. We select ttbar decays into the final states
e nu + jets and mu nu + jets, in which at least one b quark from the t-quark
decays is identified using a secondary vertex-finding algorithm. Assuming a top
quark mass of 178 GeV/c^2, we measure a cross section of 8.7 +-0.9 (stat)
+1.1-0.9 (syst) pb. We also report the first observation of ttbar with
significance greater than 5 sigma in the subsample in which both b quarks are
identified, corresponding to a cross section of 10.1 +1.6-1.4(stat)+2.0-1.3
(syst) pb.Comment: Accepted for publication in Physics Review Letters, 7 page
Polarizations of J/psi and psi(2S) Mesons Produced in ppbar Collisions at 1.96 TeV
We have measured the polarizations of \jpsi and \psiprime mesons as
functions of their transverse momentum \pt when they are produced promptly in
the rapidity range with \pt \geq 5 \pgev. The analysis is performed
using a data sample with an integrated luminosity of about 800 \ipb collected
by the CDF II detector. For both vector mesons, we find that the polarizations
become increasingly longitudinal as \pt increases from 5 to 30 \pgev. These
results are compared to the predictions of nonrelativistic quantum
chromodynamics and other contemporary models. The effective polarizations of
\jpsi and \psiprime mesons from -hadron decays are also reported.Comment: 8 pages, 7 figures, published in Physical Review Letter
Evidence for the exclusive decay Bc+- to J/psi pi+- and measurement of the mass of the Bc meson
We report first evidence for a fully reconstructed decay mode of the
B_c^{\pm} meson in the channel B_c^{\pm} \to J/psi \pi^{\pm}, with J/psi \to
mu^+mu^-. The analysis is based on an integrated luminosity of 360 pb$^{-1} in
p\bar{p} collisions at 1.96 TeV center of mass energy collected by the Collider
Detector at Fermilab. We observe 14.6 \pm 4.6 signal events with a background
of 7.1 \pm 0.9 events, and a fit to the J/psi pi^{\pm} mass spectrum yields a
B_c^{\pm} mass of 6285.7 \pm 5.3(stat) \pm 1.2(syst) MeV/c^2. The probability
of a peak of this magnitude occurring by random fluctuation in the search
region is estimated as 0.012%.Comment: 7 pages, 3 figures. Version 3, accepted by PR

baryon through the decay chain , where

Evidence for observation is based on a signal whose probability of arising from
the estimated background is 6.6 x 10^{-15}, or 7.7 Gaussian standard
deviations. The mass is measured to be (stat.) (syst.) MeV/.Comment: Minor text changes for the second version. Accepted by Phys. Rev.

Search for Third Generation Vector Leptoquarks in p anti-p Collisions at sqrt(s) = 1.96 TeV
We describe a search for a third generation vector leptoquark (VLQ3) that
decays to a b quark and tau lepton using the CDF II detector and 322 pb^(-1) of
integrated luminosity from the Fermilab Tevatron. Vector leptoquarks have been
proposed in many extensions of the standard model (SM). Observing a number of
events in agreement with SM expectations, assuming Yang-Mills (minimal)
couplings, we obtain the most stringent upper limit on the VLQ3 pair production
cross section of 344 fb (493 fb) and lower limit on the VLQ3 mass of 317
GeV/c^2 (251 GeV/c^2) at 95% C.L.Comment: 7 pages, 2 figures, submitted to PR
Top quark mass measurement using the template method at CDF
We present a measurement of the top quark mass in the lepton+jets and
dilepton channels of decays using the template method. The data
sample corresponds to an integrated luminosity of 5.6 fb of
collisions at Tevatron with TeV, collected with the CDF II
detector. The measurement is performed by constructing templates of three
kinematic variables in the lepton+jets and two kinematic variables in the
dilepton channel. The variables are two reconstructed top quark masses from
different jets-to-quarks combinations and the invariant mass of two jets from
the decay in the lepton+jets channel, and a reconstructed top quark mass
and , a variable related to the transverse mass in events with two
missing particles, in the dilepton channel. The simultaneous fit of the
templates from signal and background events in the lepton+jets and dilepton
channels to the data yields a measured top quark mass of Comment: submitted to Phys. Rev.
Searches for Direct Pair Production of Supersymmetric Top and Supersymmetric Bottom Quarks in p-pbar Collisions at sqrt(s)=1.96 TeV
We search for direct pair production of supersymmetric top quarks and
supersymmetric bottom quarks in proton-antiproton collisions at sqrt(s)=1.96
TeV, using 295 pb^-1 of data recorded by the Collider Detector at Fermilab (CDF
II) experiment. The supersymmetric top (supersymmetric bottom) quarks are
selected by reconstructing their decay into a charm (bottom) quark and a
neutralino, which is assumed to be the lightest supersymmetric particle. The
signature of such processes is two energetic heavy-flavor jets and missing
transverse energy. The number of events that pass our selection for each search
process is consistent with the expected standard model background. By comparing
our results to the theoretical production cross sections of the supersymmetric
top and supersymmetric bottom quarks in the minimal supersymmetric standard
model, we exclude, at a 95% confidence level in the frame of that model, a
supersymmetric top quark mass up to 132 GeV/c^2 for a neutralino mass of 48
GeV/c^2, and a supersymmetric bottom quark mass up to 193 GeV/c^2 for a
neutralino mass of 40 GeV/c^2.Comment: 12 pages, 5 figures, submitted to Phys. Rev.
