    El Servicio de Urgencias del C.H.G.U.V. cuenta con una plantilla de 84 Enfermeros/as. Una parte considerable de ellos es personal de nueva incorporación, de ahí que sea prioritaria la elaboración de protocolos de actuación. La administración de medicación intravenosa es sin duda una de las técnicas de mayor aplicación en los pacientes que acuden a Urgencias, que en nuestro Hospital oscila entre 550-600 atenciones diarias. Por ello nos planteamos, mediante la realización de reuniones de grupo, la elaboración del presente trabajo con el objetivo de elaborar un manual informativo sobre la medicación más habitual utilizada en nuestro Servicio de Urgencias; indicaciones, contraindicaciones, diluciones, cuidados y recomendaciones sobre la administración que nos ayude a visionar la practica en nuestro trabajo y la calidad del Servicio que prestamos a nuestros pacientes, y sirva de consulta sobre todo al personal de nueva incorporación.The First aid service of the C.H.G.U.V. Nurse is provided with a staff of 84, a considerable part of them is new staff, and therefore there is a priority of making of performance protocols. The administration of intravenous medication is undoubtedly one of the most used techniques in patients who come to the emergency room, which in our Hospital ranges between 550-600 daily attentions. For this reason, we considered that carrying out group meetings in order to elaborate the present essay. Our main objective is to prepare an informative guidebook about the most common used medicines in our first aid service: indications, contraindications, dilutions, care, and recommendations on the use. In this sense it will contribute to see the practice in our work and the service quality that we give to our patients.     A Spanish-language patient safety questionnaire to measure medical and nursing students' attitudes and knowledge

    Objective. To design and validate a questionnaire for assessing attitudes and knowledge about patient safety using a sample of medical and nursing students undergoing clinical training in Spain and four countries in Latin America. Methods. In this cross-sectional study, a literature review was carried out and total of 786 medical and nursing students were surveyed at eight universities from five countries (Chile, Colombia, El Salvador, Guatemala, and Spain) to develop and refine a Spanish-language questionnaire on knowledge and attitudes about patient safety. The scope of the questionnaire was based on five dimensions (factors) presented in studies related to patient safety culture found in PubMed and Scopus. Based on the five factors, 25 reactive items were developed. Composite reliability indexes and Cronbach''s alpha statistics were estimated for each factor, and confirmatory factor analysis was conducted to assess validity. After a pilot test, the questionnaire was refined using confirmatory models, maximum-likelihood estimation, and the variance-covariance matrix (as input). Multiple linear regression models were used to confirm external validity, considering variables related to patient safety culture as dependent variables and the five factors as independent variables. Results. The final instrument was a structured five-point Likert self-administered survey (the "Latino Student Patient Safety Questionnaire") consisting of 21 items grouped into five factors. Compound reliability indexes (Cronbach''s alpha statistic) calculated for the five factors were about 0.7 or higher. The results of the multiple linear regression analyses indicated good model fit (goodness-of-fit index: 0.9). Item-total correlations were higher than 0.3 in all cases. The convergent-discriminant validity was adequate. Conclusions. The questionnaire designed and validated in this study assesses nursing and medical students'' attitudes and knowledge about patient safety. This instrument could be used to indirectly evaluate whether or not students in health disciplines are acquiring and thus likely to put into practice the professional skills currently considered most appropriate for patient safety

    Broken R-parity, stop decays, and neutrino physics

    We discuss the phenomenology of the lightest stop in models where R-parity is broken by bilinear superpotential terms. In this class of models we consider scenarios where the R-parity breaking two-body decay ~t_1->\tau^+b competes with the leading three-body decays such as ~t_1->W^+b~\chi^0_1. We demonstrate that the R-parity violating decay can be sizable and in some parts of the parameter space even the dominant one. Moreover we discuss the expectations for \~t_1->\mu^+b and ~t_1->e^+b. The recent results from solar and atmospheric neutrinos suggest that these are as important as the tau bottom mode. The \~t_1->l^+b decays are of particular interest for hadron colliders, as they may allow a full mass reconstruction of the lighter stop. Moreover these decay modes allow cross checks on the neutrino mixing angle involved in the solar neutrino puzzle complementary to those possible using neutralino decays.     Testing neutrino mixing at future collider experiments

    Low energy supersymmetry with bilinear breaking of R-parity leads to a weak-scale seesaw mechanism for the atmospheric neutrino scale and a radiative mechanism for the solar neutrino scale. The model has striking implications for collider searches of supersymmetric particles. Assuming that the lightest SUSY particle is the lightest neutralino we demonstrate that (i) The neutralino decays inside the detector even for tiny neutrino masses. (ii) Measurements of the neutrino mixing angles lead to predictions for the ratios of various neutralino branching ratios implying an independent test of neutrino physics at future colliders, such as the Large Hadron Collider or a Linear Collider.Comment: LaTex, 35 pages, 20 figures included, version 2, section on model shortened, Fig. 13 replaced, typos corrected, version to appear in Phys.Rev.

    gamma-rays from annihilating dark matter in galaxy clusters: stacking vs single source analysis

    Clusters of galaxies are potentially important targets for indirect searches for dark matter annihilation. Here we reassess the detection prospects for annihilation in massive halos, based on a statistical investigation of 1743 clusters in the new Meta-Catalog of X-ray Clusters. We derive a new limit for the extra-galactic dark matter annihilation background of at least 20% of that originating from the Galaxy for an integration angle of 0.1 deg. The number of clusters scales as a power law with their brightness, suggesting that stacking may provide a significant improvement over a single target analysis. The mean angle containing 80% of the dark-matter signal for the sample is ~0.15 deg, indicating that instruments with this angular resolution or better would be optimal for a cluster annihilation search based on stacking. A detailed study based on the Fermi-LAT performance and position-dependent background, suggests that stacking may result in a factor ~2 improvement in sensitivity, depending on the source selection criteria. Based on the expected performance of CTA, we find no improvement with stacking, due to the requirement for pointed observations. We note that several potentially important targets: Opiuchius, A2199, A3627 (Norma) and CIZAJ1324.7-5736 may be disfavoured due to a poor contrast with respect to the Galactic dark-matter signal. The use of the homogenised MCXC meta-catalogue provides a robust ranking of the targets, although the absolute value of their signal depends on the exact dark matter substructure content. For conservative assumptions, we find that galaxy clusters (with or without stacking) can probe down to 1e-25-1e-24 cm3/s for dark matter masses in the range 10 GeV-100 GeV. For more favourable substructure configurations, ~1e-26 cm3/s may be reached.Comment: 11 pages, 6+2(new) figures, impact of substructures discussed in new Sec 3.4 (matches accepted MNRAS version).     Disruption of the CCL1-CCR8 axis inhibits vascular Treg recruitment and function and promotes atherosclerosis in mice

    The CC chemokine 1 (CCL1, also called I-309 or TCA3) is a potent chemoattractant for leukocytes that plays an important role in inflammatory processes and diseases through binding to its receptor CCR8. Here, we investigated the role of the CCL1-CCR8 axis in atherosclerosis. We found increased expression of CCL1 in the aortas of atherosclerosis-prone fat-fed apolipoprotein E (Apoe)-null mice; moreover, in vitro flow chamber assays and in vivo intravital microscopy demonstrated an essential role for CCL1 in leukocyte recruitment. Mice doubly deficient for CCL1 and Apoe exhibited enhanced atherosclerosis in aorta, which was associated with reduced plasma levels of the anti-inflammatory interleukin 10, an increased splenocyte Th1/Th2 ratio, and a reduced regulatory T cell (Treg) content in aorta and spleen. Reduced Treg recruitment and aggravated atherosclerosis were also detected in the aortas of fat-fed low-density lipoprotein receptor-null mice treated with CCR8 blocking antibodies.     Interventions in health organisations to reduce the impact of adverse events in second and third victims

    Background Adverse events (AE) are also the cause of suffering in health professionals involved. This study was designed to identify and analyse organization-level strategies adopted in both primary care and hospitals in Spain to address the impact of serious AE on second and third victims. Methods A cross-sectional study was conducted in healthcare organizations assessing: safety culture; health organization crisis management plans for serious AE; actions planned to ensure transparency in communication with patients (and relatives) who experience an AE; support for second victims; and protective measures to safeguard the institution’s reputation (the third victim). Results A total of 406 managers and patient safety coordinators replied to the survey. Deficient provision of support for second victims was acknowledged by 71 and 61 % of the participants from hospitals and primary care respectively; these respondents reported there was no support protocol for second victims in place in their organizations. Regarding third victim initiatives, 35 % of hospital and 43 % of primary care professionals indicated no crisis management plan for serious AE existed in their organization, and in the case of primary care, there was no crisis committee in 34 % of cases. The degree of implementation of second and third victim support interventions was perceived to be greater in hospitals (mean 14.1, SD 3.5) than in primary care (mean 11.8, SD 3.1) (p?<?0.001).
