TOLERANCE OF PHYSICAL EFFORT IN PATIENTS WITH SURGICALLY TREATED SCOLIOSIS
Fourty-eight patients were treated operatively for idiopathic scoliosis by means of CotrelDubousset
technique heaving pre-operative angle values of 50°_70°. Exercise test was performed using cycle ergometer both in pre-and postoperative period. Cardiorespiratory parameters were constantly measured throughout the test to estimate ventilation threshold. Following parameters were included: heart rate, oxygen intake, lung ventilation per minute, rate and volume of ventilation, as well as power and work performed. Test was terminated when ventilation threshold was achieved. This is considered noninvasive method to calculate threshold of anaerobic metabolism. Maximal oxygen intake was indicated by means of Astrand-Ryhming nomogram. Body weight and height were also measured. Operative treatment of scoliosis using Cotrel-Dubousset method enhances physical efficiency moderately most probably due to improvement of respiratory mechanics, increase in ventilation per minute during exercise test through deepening of breaths rather than increase in ventilation rate
The role of the peripheral and central nervous systems in rotator cuff disease
Rotator cuff (RC) disease is an extremely common condition associated with shoulder pain, reduced functional capacities and impaired quality of life. It primarily involves alterations in tendon health and mechanical properties that can ultimately lead to tendon failure. RC tendon tears induce progressive muscular changes that negatively impact surgical reparability of the RC tendons and clinical outcomes. At the same time, a significant base of clinical data suggests a relatively weak relationship between RC integrity and clinical presentation, emphasizing the multifactorial aspects of RC disease. This review aims to summarize the potential contribution of peripheral, spinal and supraspinal neural factors that may: (i) exacerbate structural and functional muscle changes induced by tendon tear, (ii) compromise the reversal of these changes during surgery and rehabilitation, (iii) contribute to pain generation and persistence of pain, iv) impair shoulder function through reduced proprioception, kinematics and muscle recruitment, and iv) help to explain interindividual differences and response to treatment. Given the current clinical and scientific interest in peripheral nerve injury in the context of RC disease and surgery, we carefully reviewed this body of literature with a particular emphasis for suprascapular neuropathy that has generated a large number of studies in the past decade. Within this process, we highlight the gaps in current knowledge and suggest research avenues for scientists and clinicians
Application of the Kalman filter for the diagnosis of human biomechanics
W pracy przedstawiono prototyp zdigitalizowanego systemu pomiarowego do diagnostyki biomechaniki ludzkiego ciała. System pomiarowy zostanie zbudowany w ramach projektu dofinansowanego ze środków Unii Europejskiej, pod tytułem „Mobilne urządzenie do ortopedycznej diagnostyki biomechaniki i motoryki ludzkiego ciała”. W pracy przedstawiono dwie metody obliczania orientacji obiektu w przestrzeni 3D: metodę Madgwicka i filtr Kalmana. Zaprezentowano również stanowisko laboratoryjne do testowania zaproponowanych algorytmów oraz uzyskane wyniki badań.This paper presents assumptions and a prototype of a digitized measurement system for the diagnosis of human body biomechanics. The measurement system will be built as part of a project subsidized by the European Union, titled "Mobile device for orthopedic diagnostics of biomechanics and motility of the human body". The paper presents two methods for calculating the orientation of an object in 3D space: the Madgwick method and the Kalman filter. A laboratory stand for testing the proposed algorithms and obtained test results were also presented
