Managing Patient Service in a Diagnostic Medical Facility
Hospital diagnostic facilities, such as magnetic resonance imaging centers, typically provide service to several diverse patient groups: outpatients, who are scheduled in advance; inpatients, whose demands are generated randomly during the day; and emergency patients, who must be served as soon as possible. Our analysis focuses on two interrelated tasks: designing the outpatient appointment schedule, and establishing dynamic priority rules for admitting patients into service.
We formulate the problem of managing patient demand for diagnostic service as a finite-horizon dynamic program and identify properties of the optimal policies. Using empirical data from a major urban hospital, we conduct numerical studies to develop insights into the sensitivity of the optimal policies to the various cost and probability parameters and to evaluate the performance of several heuristic rules for appointment acceptance and patient scheduling

Coping with Time-Varying Demand When Setting Staffing Requirements for a Service System
We review queueing-theory methods for setting staffing requirements in service systems where customer demand varies in a predictable pattern over the day. Analyzing these systems is not straightforward, because standard queueing theory focuses on the long-run steady-state behavior of stationary models. We show how to adapt stationary queueing models for use in nonstationary environments so that time-dependent performance is captured and staffing requirements can be set. Relatively little modification of straightforward stationary analysis applies in systems where service times are short and the targeted quality of service is high. When service times are moderate and the targeted quality of service is still high, time-lag refinements can improve traditional stationary independent period-by-period and peak-hour approximations. Time-varying infinite-server models help develop refinements, because closed-form expressions exist for their time-dependent behavior. More difficult cases with very long service times and other complicated features, such as end-of-day effects, can often be treated by a modified-offered-load approximation, which is based on an associated infinite-server model. Numerical algorithms and deterministic fluid models are useful when the system is overloaded for an extensive period of time. Protein kinase B controls transcriptional programs that direct cytotoxic T cell fate but is dispensable for T cell metabolism
SummaryIn cytotoxic TÂ cells (CTL), Akt, also known as protein kinase B, is activated by the TÂ cell antigen receptor (TCR) and the cytokine interleukin 2 (IL-2). Akt can control cell metabolism in many cell types but whether this role is important for CTL function has not been determined. Here we have shown that Akt does not mediate IL-2- or TCR-induced cell metabolic responses; rather, this role is assumed by other Akt-related kinases. There is, however, a nonredundant role for sustained and strong activation of Akt in CTL to coordinate the TCR- and IL-2-induced transcriptional programs that control expression of key cytolytic effector molecules, adhesion molecules, and cytokine and chemokine receptors that distinguish effector versus memory and naive TÂ cells. Akt is thus dispensable for metabolism, but the strength and duration of Akt activity dictates the CTL transcriptional program and determines CTL fate
The \u3cem\u3eChlamydomonas\u3c/em\u3e Genome Reveals the Evolution of Key Animal and Plant Functions
Chlamydomonas reinhardtii is a unicellular green alga whose lineage diverged from land plants over 1 billion years ago. It is a model system for studying chloroplast-based photosynthesis, as well as the structure, assembly, and function of eukaryotic flagella (cilia), which were inherited from the common ancestor of plants and animals, but lost in land plants. We sequenced the ∼120-megabase nuclear genome of Chlamydomonas and performed comparative phylogenomic analyses, identifying genes encoding uncharacterized proteins that are likely associated with the function and biogenesis of chloroplasts or eukaryotic flagella. Analyses of the Chlamydomonas genome advance our understanding of the ancestral eukaryotic cell, reveal previously unknown genes associated with photosynthetic and flagellar functions, and establish links between ciliopathy and the composition and function of flagella
Genome-wide analyses for personality traits identify six genomic loci and show correlations with psychiatric disorders
Personality is influenced by genetic and environmental factors1
and associated with mental health. However, the underlying
genetic determinants are largely unknown. We identified six
genetic loci, including five novel loci2,3, significantly associated
with personality traits in a meta-analysis of genome-wide
association studies (N = 123,132–260,861). Of these genomewide
significant loci, extraversion was associated with variants
in WSCD2 and near PCDH15, and neuroticism with variants
on chromosome 8p23.1 and in L3MBTL2. We performed a
principal component analysis to extract major dimensions
underlying genetic variations among five personality traits
and six psychiatric disorders (N = 5,422–18,759). The first
genetic dimension separated personality traits and psychiatric
disorders, except that neuroticism and openness to experience
were clustered with the disorders. High genetic correlations
were found between extraversion and attention-deficit–
hyperactivity disorder (ADHD) and between openness and
schizophrenia and bipolar disorder. The second genetic
dimension was closely aligned with extraversion–introversion
and grouped neuroticism with internalizing psychopathology
(e.g., depression or anxiety)
PTF10iya: A short-lived, luminous flare from the nuclear region of a star-forming galaxy
We present the discovery and characterisation of PTF10iya, a short-lived (dt
~ 10 d, with an optical decay rate of ~ 0.3 mag per d), luminous (M_g ~ -21
mag) transient source found by the Palomar Transient Factory. The
ultraviolet/optical spectral energy distribution is reasonably well fit by a
blackbody with T ~ 1-2 x 10^4 K and peak bolometric luminosity L_BB ~ 1-5 x
10^44 erg per s (depending on the details of the extinction correction). A
comparable amount of energy is radiated in the X-ray band that appears to
result from a distinct physical process. The location of PTF10iya is consistent
with the nucleus of a star-forming galaxy (z = 0.22405 +/- 0.00006) to within
350 mas (99.7 per cent confidence radius), or a projected distance of less than
1.2 kpc. At first glance, these properties appear reminiscent of the
characteristic "big blue bump" seen in the near-ultraviolet spectra of many
active galactic nuclei (AGNs). However, emission-line diagnostics of the host
galaxy, along with a historical light curve extending back to 2007, show no
evidence for AGN-like activity. We therefore consider whether the tidal
disruption of a star by an otherwise quiescent supermassive black hole may
account for our observations. Though with limited temporal information,
PTF10iya appears broadly consistent with the predictions for the early
"super-Eddington" phase of a solar-type star disrupted by a ~ 10^7 M_sun black
hole. Regardless of the precise physical origin of the accreting material, the
large luminosity and short duration suggest that otherwise quiescent galaxies
can transition extremely rapidly to radiate near the Eddington limit; many such
outbursts may have been missed by previous surveys lacking sufficient cadence.Comment: 18 pages, 8 figures; revised following referee's comment
Discovery and Genomic Characterization of Noroviruses from a Gastroenteritis Outbreak in Domestic Cats in the US
Norovirus (NoV) RNA was detected in the stools of 6 out 14 (42.8%) 8–12-week-old cats with enteritis from a feline shelter, in New York State. Upon sequence analysis of the complete capsid, the six NoVs were found to be identical, suggesting the spread of a unique NoV strain in the shelter. The full-length genomic sequence (7839 nt) of one feline NoV, CU081210E/2010/US, was determined. In the capsid protein VP1 region, the virus displayed the highest amino acid identity to animal genogroup IV genotype 2 (GIV.2) NoVs: lion/Pistoia-387/06/IT (97.9%) and dog/Bari-170/07/IT (90.4%). These findings document the discovery of a novel feline calicivirus, different from vesiviruses, and extend the spectrum of NoV host range. Epidemiological studies using feline NoV-specific diagnostic tools and experimental infection of cats are required to understand whether NoVs have a pathogenic role in this species
