    Therapeutisches Drug Monitoring (TDM) von kontinuierlich applizierten Antibiotika zur Behandlung schwerer Infektionen bei Intensivpatienten mit extrakorporaler Membranoxygenierung

    Introduction: Effective antimicrobial treatment is key to reduce the high morbidity and mortality of bacterial sepsis in patients on intensive care units (ICUs). Dose adjustments are frequently required to accommodate for pathophysiological changes or renal replacement therapy. Extracorporeal membrane oxygenation (ECMO) is nowadays increasingly being used for the treatment of respiratory and/or cardiac failure. However, it remains unclear whether specific dose adjustments are necessary to avoid subtherapeutic serum concentrations in septic patients on ECMO support. Objective: To assess, analyze and comparatively evaluate the median serum concentrations of antibiotics used to treat intensive care patients with and without ECMO support in a pneumological intensive care unit at the Saarland University Medical Center in Homburg. Materials and methods: Between November 2018 and December 2019, a total of 105 patients on a pneumological intensive care unit in southwest Germany who received antibiotic treatment with ceftazidime, piperacillin/tazobactam, meropenem, or linezolid due to a suspected or diagnostically confirmed infection, were prospectively enrolled. All antibiotics were applied continuously, and therapeutic drug monitoring of serum concentrations (expressed as mg/L) was carried out using high- performance liquid chromatography. Target concentrations were defined as 4-fold above the minimal inhibitory concentration (MIC) of the pathogen-specific antibiotic susceptibility breakpoints as defined by the European Committee on Antimicrobial Susceptibility Testing (EUCAST). Results: Data from 105 ICU patients, of whom 30 were treated with ECMO, were analyzed. ECMO patients were significantly younger (mean age: 47.73 vs. 61.15 years; p<0.001), required hemodialysis more frequently (53.3% vs. 38.1%; p=0.048) and had an elevated ICU mortality (60% vs. 22.7%; p=0.0005). In the ECMO group, median antibiotic serum concentrations were significantly lower for piperacillin (32.3 vs. 52.9; p=0.029) and standard-dose meropenem (3/d) (15 vs. 17.8; p=0.020). There were high rates of insufficient antibiotic serum concentrations among ECMO patients, which did not reach the pre-specified MIC target (piperacillin: 48.4% vs. 13.0% in Non-ECMO; linezolid: 34.8% vs. 15.0% in Non-ECMO; meropenem standard-dose (3g/d) 6.1% vs. 3.1% in Non-ECMO). Conclusions: ECMO therapy was associated with significantly decreased serum concentrations of certain antibiotics. Additional investigations are required to determine the pharmacokinetic characteristics of ICU patients with ECMO treatment and, subsequently to develop substance-specific dosing recommendations.Einleitung: Eine effektive antimikrobielle Therapie ist entscheidend zur Reduktion der hohen MortalitĂ€t bakterieller SeptikĂ€mien auf Intensivstationen (ICUs). Anpassungen der Antibiotika- Dosierung sind hĂ€ufig notwendig, um pathophysiologischen V erĂ€nderungen oder renalen Ersatzverfahren gerecht zur werden. Die extrakorporale Membranoxygenierung (ECMO) wird immer hĂ€ufiger in der Therapie des Lungen- oder Kreislaufversagens eingesetzt. Es ist jedoch noch weitgehend unklar, ob Anpassungen der Dosierung zur Vermeidung subtherapeutischer Serumkonzentrationen fĂŒr Antibiotika bei septischen Patienten mit ECMO-Therapie vorgenommen werden mĂŒssen. Ziel: Die Auswertung und vergleichende Bewertung der medianen Antibiotika-Serumkonzentrationen von intensivmedizinischen Patienten mit und ohne ECMO-Therapie auf einer pneumologischen ICU am UniversitĂ€tsklinikum des Saarlandes. Material und Methoden: Zwischen November 2018 und Dezember 2019 wurden prospektiv 105 Patienten auf einer pneumologischen Intensivstation eingeschlossen, die aufgrund einer vermuteten oder diagnostisch gesicherten schweren Infektion eine antibiotische Therapie mit Ceftazidim, Piperacillin/Tazobactam, Meropenem oder Linezolid erhielten. Alle Antibiotika wurden kontinuierlich appliziert und das Therapeutic Drug Monitoring (TDM) der Serumkonzentrationen in (mg/l) wurde mittels high-performance liquid chromatography (HPLC) durchgefĂŒhrt. Zielkonzentrationen wurden als 4-fache minimale Hemmkonzentration (MHK) der pathogenspezifischen Antibiotika-Empfindlichkeits- Breakpoints nach den EUCAST-Richtlinien definiert. Ergebnisse: Es wurden Daten von 105 ICU-Patienten analysiert, von denen 30 mit ECMO therapiert wurden. ECMO-Patienten waren signifikant jĂŒnger (mittleres Alter: 47,73 vs. 61,15 Jahre; p<0,001), benötigten hĂ€ufiger HĂ€modialyse (53,3% vs. 38,1%; p=0,048) und hatten eine erhöhte ICU- Sterblichkeit (60% vs. 22,7%; p=0,0005). In der ECMO-Gruppe waren die Serumkonzentrationen signifikant niedriger fĂŒr Piperacillin (32,3 mg/l vs. 52,9 mg/l; p=0,029) und Meropenem in Standarddosis (3g/d) (15 mg/l vs. 17,8 mg/l; p=0,020). Es wurden hohe Raten an insuffizienten Serumkonzentrationen bei ECMO-Patienten festgestellt, welche die definierte Ziel-Konzentration im Serum unterschritten (Piperacillin: 48,4% vs. 13,0% bei Non-ECMO-Patienten; Linezolid: 34,8% vs. 15,0% bei Non-ECMO-Patienten; Meropenem Standarddosis (3g/d) 6,1% vs. 3,1% bei Non-ECMO- Patienten). Schlussfolgerung: Eine ECMO-Therapie war mit signifikant erniedrigten Serumkonzentrationen bestimmter Antibiotika assoziiert. ZusĂ€tzliche Untersuchungen sind erforderlich, um die pharmakokinetischen Charakteristika von ICU-Patienten mit ECMO-Therapie zu ermitteln, und darauf aufbauend substanzspezifische Dosierungsempfehlungen zu erarbeiten

    Antibiotic therapeutic drug monitoring in intensive care patients treated with different modalities of extracorporeal membrane oxygenation (ECMO) and renal replacement therapy: a prospective, observational single-center study

    Background: Efective antimicrobial treatment is key to reduce mortality associated with bacterial sepsis in patients on intensive care units (ICUs). Dose adjustments are often necessary to account for pathophysiological changes or renal replacement therapy.     TNF-related apoptosis-inducing ligand, interferon gamma-induced protein 10, and C-reactive protein in predicting the progression of SARS-CoV-2 infection : a prospective cohort study

    Background: Early prognostication of COVID-19 severity will potentially improve patient care. Biomarkers, such as TNF-related apoptosis-inducing ligand (TRAIL), interferon gamma-induced protein 10 (IP-10), and C-reactive protein (CRP), might represent possible tools for point-of-care testing and severity prediction. Methods: In this prospective cohort study, we analyzed serum levels of TRAIL, IP-10, and CRP in patients with COVID-19, compared them with control subjects, and investigated the association with disease sever ity. Results: A total of 899 measurements were performed in 132 patients (mean age 64 years, 40.2% females). Among patients with COVID-19, TRAIL levels were lower (49.5 vs 87 pg/ml, P = 0.0142), whereas IP-10 and CRP showed higher levels (667.5 vs 127 pg/ml, P <0.001; 75.3 vs 1.6 mg/l, P <0.001) than healthy controls. TRAIL yielded an inverse correlation with length of hospital and intensive care unit (ICU) stay, Simplified Acute Physiology Score II, and National Early Warning Score, and IP-10 showed a positive cor relation with disease severity. Multivariable regression revealed that obesity (adjusted odds ratio [aOR] 5.434, 95% confidence interval [CI] 1.005-29.38), CRP (aOR 1.014, 95% CI 1.002-1.027), and peak IP-10 (aOR 1.001, 95% CI 1.00-1.002) were independent predictors of in-ICU mortality

    Cerebellar gray and white matter volume and their relation with age and manual motor performance in healthy older adults

    ObjectivesFunctional neuroimaging and voxel‐based morphometry studies have confirmed the important role of the cerebellum in motor behavior. However, little is known about the relationship between cerebellar gray (GMv) and white matter (WMv) volume and manual motor performance in aging individuals. This study aims to quantify the relationship between cerebellar tissue volume and manual motor performance.Experimental designTo gain more insight into cerebellar function and how it relates to the role of the primary motor cortex (M1), we related cerebellar GMv, WMv, and M1v to manual motor performance in 217 healthy older individuals. Left and right cerebellar GMv and WMv, and M1v were obtained using FreeSurfer. The following motor measures were obtained: grip force, tapping speed, bimanual visuomotor coordination, and manual dexterity.Principal observationsSignificant positive relationships were observed between cerebellar GMv and WMv and grip strength, right cerebellar WMv and right‐hand tapping speed, right cerebellar WMv and dexterity, M1v and grip strength, and right M1v and left‐hand dexterity, though effect sizes were small.ConclusionsOur results show that cerebellar GMv and WMv are differently associated with manual motor performance. These associations partly overlap with the brain‐behavior associations between M1 and manual motor performance. Not all observed associations were lateralized (i.e., ipsilateral cerebellar and contralateral M1v associations with motor performance), which could point to age‐related neural dedifferentiation. The current study provides new insights in the role of the cerebellum in manual motor performance. In consideration of the small effect sizes replication studies are needed to validate these results.     The Cerebellum Link to Neuroticism: A Volumetric MRI Association Study in Healthy Volunteers

    Prior research suggests an association between reduced cerebellar volumes and symptoms of depression and anxiety in patients with mood disorders. However, whether a smaller volume in itself reflects a neuroanatomical correlate for increased susceptibility to develop mood disorders remains unclear. The aim of the present study was to examine the relationship between cerebellar volume and neurotic personality traits in a non-clinical subject sample. 3T Structural magnetic resonance imaging scans were acquired, and trait depression and anxiety scales of the revised NEO personality inventory were assessed in thirty-eight healthy right-handed volunteers. Results showed that cerebellar volume corrected for total brain volume was inversely associated with depressive and anxiety-related personality traits. Cerebellar gray and white matter contributed equally to the observed associations. Our findings extend earlier clinical observations by showing that cerebellar volume covaries with neurotic personality traits in healthy volunteers. The results may point towards a possible role of the cerebellum in the vulnerability to experience negative affect. In conclusion, cerebellar volumes may constitute a clinico-neuroanatomical correlate for the development of depression- and anxiety-related symptoms

    TRY plant trait database – enhanced coverage and open access

    Plant traits - the morphological, anatomical, physiological, biochemical and phenological characteristics of plants - determine how plants respond to environmental factors, affect other trophic levels, and influence ecosystem properties and their benefits and detriments to people. Plant trait data thus represent the basis for a vast area of research spanning from evolutionary biology, community and functional ecology, to biodiversity conservation, ecosystem and landscape management, restoration, biogeography and earth system modelling. Since its foundation in 2007, the TRY database of plant traits has grown continuously. It now provides unprecedented data coverage under an open access data policy and is the main plant trait database used by the research community worldwide. Increasingly, the TRY database also supports new frontiers of trait‐based plant research, including the identification of data gaps and the subsequent mobilization or measurement of new data. To support this development, in this article we evaluate the extent of the trait data compiled in TRY and analyse emerging patterns of data coverage and representativeness. Best species coverage is achieved for categorical traits - almost complete coverage for ‘plant growth form’. However, most traits relevant for ecology and vegetation modelling are characterized by continuous intraspecific variation and trait–environmental relationships. These traits have to be measured on individual plants in their respective environment. Despite unprecedented data coverage, we observe a humbling lack of completeness and representativeness of these continuous traits in many aspects. We, therefore, conclude that reducing data gaps and biases in the TRY database remains a key challenge and requires a coordinated approach to data mobilization and trait measurements. This can only be achieved in collaboration with other initiatives
