    what conditions make inter-country comparisons possible?

    Hospitalizations for ambulatory care sensitive conditions have been extensively used in health services research to assess access, quality and performance of primary health care. Inter-country comparisons can assist policy-makers in pursuing better health outcomes by contrasting policy design, implementation and evaluation. The objective of this study is to identify the conceptual, methodological, contextual and policy dimensions and factors that need to be accounted for when comparing these types of hospitalizations across countries. A conceptual framework for inter-country comparisons was drawn based on a review of 18 studies with inter-country comparison of ambulatory care sensitive conditions hospitalizations. The dimensions include methodological choices; population's demographic, epidemiologic and socio-economic profiles and features of the health services and system. Main factors include access and quality of primary health care, availability of health workforce and health facilities, health interventions and inequalities. The proposed framework can assist in designing studies and interpreting findings of inter-country comparisons of ambulatory care sensitive conditions hospitalizations, accelerating learning and progress towards universal health coverage.publishersversionpublishe

    This article shows the role of knowledge management and some additional factors as triggers for competitive advantage in companies in the TI sector in the Tijuana - San Diego region, in the border region of Baja California and California. The knowledge management through information technologies tries to generate organizational success. The implementation of knowledge management through the development of processes and systems with these factors creates a competitive advantage. One of the limitations on competitiveness is that companies in the it sector have limited use of knowledge management, this creates disadvantages and causes lack of control of intangible assets in the organization. Project management, quality systems and process control are activities that seek continuous improvement and therefore its implementation is necessary. In this first approach, knowledge management and competitiveness are considered, and it is also proposed to continue the study to corroborate this relevance.Este artículo muestra el rol de la gestión de conocimiento y de algunos factores adicionales como detonadores de la ventaja competitiva en empresas del sector de ti en la región Tijuana– San Diego, en la zona fronteriza de Baja California y California.     Microwave-Driven Hexagonal-to-Monoclinic Transition in BiPO4: An In-Depth Experimental Investigation and First-Principles Study

    Present theoretical and experimental work provides an in-depth understanding of the morphological, structural, electronic, and optical properties of hexagonal and monoclinic polymorphs of bismuth phosphate (BiPO4). Herein, we demonstrate how microwave irradiation induces the transformation of a hexagonal phase to a monoclinic phase in a short period of time and, thus, the photocatalytic performance of BiPO4. To complement and rationalize the experimental results, first-principles calculations have been performed within the framework of density functional theory. This was aimed at obtaining the geometric, energetic, and structural parameters as well as vibrational frequencies; further, the electronic properties (band structure diagram and density of states) of the bulk and corresponding surfaces of both the hexagonal and monoclinic phases of BiPO4 were also acquired. A detailed characterization of the low vibrational modes of both the hexagonal and monoclinic polymorphs is key to explaining the irreversible phase transformation from hexagonal to monoclinic. On the basis of the calculated values of the surface energies, a map of the available morphologies of both phases was obtained by using Wulff construction and compared to the observed scanning electron microscopy images. The BiPO4 crystals obtained after 16–32 min of microwave irradiation provided excellent photodegradation of Rhodamine B under visible-light irradiation. This enhancement was found to be related to the surface energy and the types of clusters formed on the exposed surfaces of the morphology. These findings provide details of the hexagonal-to-monoclinic phase transition in BiPO4 during microwave irradiation; further, the results will assist in the design of electronic devices with higher efficiency and reliability

    Activity Inference for Ambient Intelligence Through Handling Artifacts in a Healthcare Environment

    Human activity inference is not a simple process due to distinct ways of performing it. Our proposal presents the SCAN framework for activity inference. SCAN is divided into three modules: (1) artifact recognition, (2) activity inference, and (3) activity representation, integrating three important elements of Ambient Intelligence (AmI) (artifact-behavior modeling, event interpretation and context extraction). The framework extends the roaming beat (RB) concept by obtaining the representation using three kinds of technologies for activity inference. The RB is based on both analysis and recognition from artifact behavior for activity inference. A practical case is shown in a nursing home where a system affording 91.35% effectiveness was implemented in situ. Three examples are shown using RB representation for activity representation. Framework description, RB description and CALog system overcome distinct problems such as the feasibility to implement AmI systems, and to show the feasibility for accomplishing the challenges related to activity recognition based on artifact recognition. We discuss how the use of RBs might positively impact the problems faced by designers and developers for recovering information in an easier manner and thus they can develop tools focused on the user

    Assessment of Platelet REACtivity After Transcatheter Aortic Valve Replacement : The REAC-TAVI Trial

    The REAC-TAVI (Assessment of platelet REACtivity after Transcatheter Aortic Valve Implantation) trial enrolled patients with aortic stenosis (AS) undergoing transcatheter aortic valve replacement (TAVR) pre-treated with aspirin + clopidogrel, aimed to compare the efficacy of clopidogrel and ticagrelor in suppressing high platelet reactivity (HPR) after TAVI. Current recommendations support short-term use of aspirin + clopidogrel for patients with severe AS undergoing TAVR despite the lack of compelling evidence. This was a prospective, randomized, multicenter investigation. Platelet reactivity was measured at 6 different time points with the VerifyNow assay (Accriva Diagnostics, San Diego, California). HPR was defined as (P2Y reaction units (PRU) ≥208. Patients with HPR before TAVR were randomized to either aspirin + ticagrelor or aspirin + clopidogrel for 3 months. Patients without HPR continued with aspirin + clopidogrel (registry cohort). The primary endpoint was non-HPR status (PRU <208) in ≥70% of patients treated with ticagrelor at 90 days post-TAVR. A total of 68 patients were included. Of these, 48 (71%) had HPR (PRU 273 ± 09) and were randomized to aspirin + ticagrelor (n = 24, PRU 277 ± 08) or continued with aspirin + clopidogrel (n = 24, PRU 269 ± 49). The remaining 20 patients (29%) without HPR (PRU 133 ± 12) were included in the registry. Overall, platelet reactivity across all the study time points after TAVR was lower in patients randomized to ticagrelor compared with those treated with clopidogrel, including those enrolled in the registry (p < 0.001). The primary endpoint was achieved in 100% of patients with ticagrelor compared with 21% with clopidogrel (p < 0.001). Interestingly, 33% of clopidogrel responder patients at baseline developed HPR status during the first month after TAVR. HPR to clopidogrel is present in a considerable number of patients with AS undergoing TAVR. Ticagrelor achieves a better and faster effect, providing sustained suppression of HPR to these patients.     Thirty-one years ago, in 1985, Manuel Sacristán died in Barcelona at the age of 59. After the publication in 2014 of a volume with some of his writings translated into English (Llorente 2014), it is time to help non-Spanish-speaking readers to know more about him. Yet it is not easy to explain to generations born after the fall of the Berlin Wall in 1989 that Manuel Sacristán was a most important Marxist philosopher and at the same time one of the few pioneers introducing political ecology and antinuclear peace movement during the last quarter of the 20th century in Spain. Many people believe that Marxism, environmentalism and pacifism are views that exclude each other. Most of what has been said and done on behalf of Marxism since Stalin took over the leadership of the Communist Party of the USSR in the 1930s, up to its dissolution in 1991, contributes to sustaining this belief. The fast industrialization of the Old Russian Empire undertaken by the Soviet State was nowhere near taking into account ecological sustainability. Its socio-environmental impact turned out to be comparable or even worse than the ones caused by capitalist industrialization

    Transiting exoplanets from the CoRoT space mission VIII. CoRoT-7b: the first Super-Earth with measured radius

    We report the discovery of very shallow (DF/F = 3.4 10-4), periodic dips in the light curve of an active V = 11.7 G9V star observed by the CoRoT satellite, which we interpret as due to the presence of a transiting companion. We describe the 3-colour CoRoT data and complementary ground-based observations that support the planetary nature of the companion. Methods. We use CoRoT color information, good angular resolution ground-based photometric observations in- and out- of transit, adaptive optics imaging, near-infrared spectroscopy and preliminary results from Radial Velocity measurements, to test the diluted eclipsing binary scenarios. The parameters of the host star are derived from optical spectra, which were then combined with the CoRoT light curve to derive parameters of the companion. We examine carefully all conceivable cases of false positives, and all tests performed support the planetary hypothesis. Blends with separation larger than 0.40 arcsec or triple systems are almost excluded with a 8 10-4 risk left. We conclude that, as far as we have been exhaustive, we have discovered a planetary companion, named CoRoT-7b, for which we derive a period of 0.853 59 +/- 3 10-5 day and a radius of Rp = 1.68 +/- 0.09 REarth. Analysis of preliminary radial velocity data yields an upper limit of 21 MEarth for the companion mass, supporting the finding.     Los hidratos de gas son compuestos cristalinos donde una molécula de gas, principalmente metano, queda atrapada en una red de moléculas de agua en forma de hielo. La importancia de los hidratos de gas en la naturaleza es muy alta ya que constituye una fuente alternativa de energía y a su vez juegan un papel importante en el delicado equilibrio del clima a nivel global y en los riesgos geológicos en el ámbito marino. La acción COST MIGRATE está diseñada con el fin de integrar la experiencia de un gran número de grupos de investigación europeos y agentes del sector para promover el desarrollo de conocimientos multidisciplinarios sobre el potencial de los hidratos de gas como fuente de energía en Europa. Dos de los objetivos de esta acción son realizar un inventario europeo de hidratos de gas explotables y evaluar los riesgos ambientales. En este trabajo se muestran los principales indicios de hidratos de gas en los márgenes europeos incluida la Península Ibérica, con una primera aproximación sobre el espesor y situación de la zona de estabilidad de hidratos de gas en el margen Ibérico.Gas hydrates are crystalline compounds where a molecule of gas, mainly methane, is trapped in a cage of icewater molecules. The importance of gas hydrates in nature is very high because it is an alternative source of energy and play a major role in the delicate balance of the global climate and in the marine geological risks. MIGRATE COST action is designed to integrate the experience of a large number of European research groups and industrial players to promote the development of multidisciplinary knowledge on the potential of gas hydrates as energy resource in Europe. Two of the objectives of the action aim to estimate the European inventory of exploitable gas hydrates and to assess environmental risks. In this work we show the occurrences of gas hydrates described in European margins including the Iberian Peninsula, with a first approximation on the thickness and location of the area of stability of gas hydrates in the Iberian margin.COST Action ES1405 (MIGRATE)Versión del edito
