    Exercise for people living with frailty and receiving haemodialysis:a mixed-methods randomised controlled feasibility study

    Objectives Frailty is highly prevalent in haemodialysis (HD) patients, leading to poor outcomes. This study aimed to determine whether a randomised controlled trial (RCT) of intradialytic exercise is feasible for frail HD patients, and explore how the intervention may be tailored to their needs.Design Mixed-methods feasibility.Setting and participants Prevalent adult HD patients of the CYCLE-HD trial with a Clinical Frailty Scale Score of 4–7 (vulnerable to severely frail) were eligible for the feasibility study.Interventions Participants in the exercise group undertook 6 months of three times per week, progressive, moderate intensity intradialytic cycling (IDC).Outcomes Primary outcomes were related to feasibility. Secondary outcomes were falls incidence measured from baseline to 1 year following intervention completion, and exercise capacity, physical function, physical activity and patient-reported outcomes measured at baseline and 6 months. Acceptability of trial procedures and the intervention were explored via diaries and interviews with n=25 frail HD patients who both participated in (n=13, 52%), and declined (n=12, 48%), the trial.Results 124 (30%) patients were eligible, and of these 64 (52%) consented with 51 (80%) subsequently completing a baseline assessment. n=24 (71% male; 59±13 years) dialysed during shifts randomly assigned to exercise and n=27 (81% male; 65±11 years) shifts assigned to usual care. n=6 (12%) were lost to follow-up. The exercise group completed 74% of sessions. 27%–89% of secondary outcome data were missing. Frail HD patients outlined several ways to enhance trial procedures. Maintaining ability to undertake activities of daily living and social participation were outcomes of primary importance. Participants desired a varied exercise programme.Conclusions A definitive RCT is feasible, however a comprehensive exercise programme may be more efficacious than IDC in this population.</div

    Structured group education programme and accompanying mHealth intervention to promote physical activity in women with a history of gestational diabetes : a randomised controlled trial

    Aims: Assess effectiveness of a hybrid intervention targeting physical activity in women with prior gestational diabetes. Methods: Randomised controlled trial with parallel arms. 293 women (35.1 ± 5.1 years; 40% ethnic minority) recruited from two hospitals and randomised to routine care or hybrid lifestyle intervention comprising two group sessions and access to a mobile web app. Primary outcome was a change in objectively measured physical activity at 12 months. Secondary outcomes included self‐efficacy for exercise, quality of life and anxiety and depression. Linear regression compared outcome measures between groups. Results: 83% of intervention participants attended at least one group session, of who 66% registered to use the app. There was a non‐significant increase in physical activity at 12 months (between‐group difference of 0.95 mg [95% CI: −0.46 to 2.37]), equivalent to approximately 500 steps per day. Intervention participants reported higher self‐efficacy for exercise (0.54, 95% CI: 0.05 to 1.102; p = 0.029), lower anxiety (−0.91, 95% CI: −1.74 to −0.09; p = 0.031), and higher quality of life (0.05, 95% CI: 0.004 to 0.09; p = 0.032), compared to controls. Conclusions: The intervention improved confidence in exercise and quality of life. Further research is needed to improve participant engagement with physical activity interventions in multi‐ethnic populations with a history of gestational diabetes

    A Comparison of Dietary Intake Between Individuals Undergoing Maintenance Hemodialysis in the United Kingdom and China

    OBJECTIVE: Protein-energy wasting is highly prevalent in people with end-stage kidney disease receiving regular hemodialysis. Currently, it is unclear what the optimal nutritional recommendations are, which is further complicated by differences in dietary patterns between countries. The aim of the study was to understand and compare dietary intake between individuals receiving hemodialysis in Leicester, UK and Nantong, China. METHODS: The study assessed 40 UK and 44 Chinese participants' dietary intake over a period of 14 days using 24-hour diet recall interviews. Nutritional blood parameters were obtained from medical records. Food consumed by participants in the UK and China was analyzed using the Nutritics and Nutrition calculator to quantify nutritional intake. RESULTS: Energy and protein intake were comparable between UK and Chinese participants, but with both below the recommended daily intake. Potassium intake was higher in UK participants compared to Chinese participants (2,115 [888] versus 1,159 [861] mg/d; P < .001), as was calcium (618 [257] versus 360 [312] mg/d; P < .001) and phosphate intake (927 [485] versus 697 [434] mg/d; P = .007). Vitamin C intake was lower in UK participants compared to their Chinese counterparts (39 [51] versus 64 [42] mg/d; P = .024). Data are reported here as median (interquartile range). CONCLUSION: Both UK and Chinese hemodialysis participants have insufficient protein and energy in their diet. New strategies are required to increase protein and energy intakes. All participants had inadequate daily intake of vitamins C and D; there may well be a role in the oral supplementation of these vitamins, and further studies are urgently needed

    An overview of burst, buckling, durability and corrosion analysis of lightweight FRP composite pipes and their applicability

All rights reserved.The main aim of this review article was to address the performance of filament wound fibre reinforced polymer (FRP) composite pipes and their critical properties, such as burst, buckling, durability and corrosion. The importance of process parameters concerning merits and demerits of the manufacturing methods was discussed for the better-quality performance. Burst analysis revealed that the winding angle of ±55° was observed to be optimum with minimum failure mechanisms, such as matrix cracking, whitening, leakage and fracture. The reduction of buckling effect was reported in case of lower hoop stress value in the hoop to axial stress ratio against axial, compression and torsion. A significant improvement in energy absorption was observed in the hybrid composite pipes with the effect of thermal treatment. However, the varying winding angle in FRP pipe fabrication was reported as an influencing factor affecting all the aforementioned properties. Almost 90% of the reviewed studies was done using E-glass/epoxy materials for the composite pipe production. By overcoming associated limitations, such as replacing synthetic materials, designing new material combinations and cost-benefit analysis, the production cost of the lightweight FRP composite pipes can be decreased for the real-time applications.Peer reviewe

    Microparticle responses to aerobic exercise and meal consumption in healthy men

    PURPOSE: Microparticles (MPs) are shed extracellular vesicles that express the pro-thrombotic tissue factor (TF). Aerobic exercise may reduce MP count and TF expression. This study investigated the impact of acute running or rest followed by standardised meal consumption on MP phenotypes and TF expression. METHODS: 15 males (age: 22.9 ± 3.3 years; body mass: 81.9 ± 11.4 kg; V[Combining Dot Above]O2 max 54.9 ± 6.5 mL·kg·min; mean ± SD) completed 1h of running (70% V[Combining Dot Above]O2max) or rest at 9am, and consumed a standardised meal (1170 kcal, 43% CHO, 17% PRO, 40% fat) at 10:45am. Venous blood samples were taken at 9am, 10am and 11:30am. MP concentration, diameter, phenotypes and TF-expression were assessed using nanoparticle tracking analysis (NTA) and flow cytometry. RESULTS: NTA identified no changes in MP concentration or diameter in response to time or trial. Flow cytometry revealed total MP count increased from 9am to 10am (1.62 ± 2.28 to 1.74 ± 2.61 x10/L, p = .016, effect size (η) = .105), but was unaffected by trial. TF platelet-derived MP % reduced from 9am to 10am (44.0 ± 21.2 to 21.5 ± 9.3%, p = .001, η = .582) after exercise only (control: 36.8 ± 18.2 to 34.9 ± 11.9%, p = .972). TF neutrophil-derived MP % reduced from 9am to 11:30am (42.3 ± 17.2 to 25.1 ± 14.9%, p = 0.048, η = .801) in the exercise trial only (control: 28.5 ± 15.7 to 32.2 ± 9.6%, p = .508). CONCLUSION: Running induced a significant reduction in %TF platelet and neutrophil MP, suggesting a transient reduction in cardiovascular risk via reduced TF-stimulated thrombosis. This requires further investigation over longer time periods in cardiovascular disease populations

    Physical activity and the 'pediatric inactivity triad' in children living with chronic kidney disease: a narrative review.

    The 'paediatric inactivity triad' (PIT) framework consists of three complex inter-related conditions that influence physical inactivity and related health risks. In those living with chronic kidney disease (CKD), a multi-factorial milieu of components likely confound the PIT elements, resulting in a cycle of decreased physical functioning and reduced physical activity. In this review, we explore and summarize previous research on each of the three principal PIT components (exercise deficit disorder, dynapenia, and physical illiteracy) in the pediatric CKD population. We found those living with CKD are significantly physically inactive compared to their peers. Physical inactivity occurs early in the disease process and progressively gets worse as disease burden increases. Although physical activity appears to increase post-transplantation, it remains lower compared to healthy controls. There is limited evidence on interventions to increase physical activity behaviour in this population, and those that have attempted have had negligible effects. Studies reported profound reductions in muscle strength, physical performance, and cardiorespiratory fitness. A small number of exercise-based interventions have shown favourable improvements in physical function and cardiorespiratory fitness, although small sample sizes and methodological issues preclude the generalization of findings. Physical activity must be adapted and individualized to the needs and goals of the children, particularly those with acute and chronic medical needs as is the case in CKD, and further work is needed to define optimal interventions across the life course in this population if we aim to prevent physical activity declining further.</p

    The influence of acute moderate-to-high intensity aerobic exercise on markers of immune function and microparticles in renal transplant recipients.

    Renal transplant recipients (RTRs) and non-dialysis chronic kidney disease (ND-CKD) patients display elevated circulating microparticle (MP) counts, whilst RTRs display immunosuppression-induced infection susceptibility. The impact of aerobic exercise on circulating immune cells and microparticles is unknown in RTRs. Fifteen RTRs (age 52.8±14.5 years, estimated glomerular filtration rate [eGFR] 51.7±19.8 ml/min/1.73m2 [mean ± SD]), 16 ND-CKD patients (54. ± 6.3 years, eGFR 61.9±21.0 ml/min/1.73m2, acting as a uremic control group), and 16 HCs (52.2±16.2 years, eGFR 85.6±6.1 ml/min/1.73m2) completed 20 minutes of walking at 60-70% VO2 peak. Venous blood samples were taken pre, post, and 1h post-exercise. Leukocytes and MPs were assessed using flow cytometry. Exercise increased classical (p = 0.001) and non-classical (p = 0.002) monocyte subset proportions but decreased the intermediate subset (p < 0.001) in all groups. Exercise also decreased the percentage of platelet-derived MPs that expressed tissue factor (TF+) in all groups (p = 0.01), though no other exercise-dependent effects were observed. The exercise-induced reduction in intermediate monocyte percentage suggests an anti-inflammatory effect, though this requires further investigation. The reduction in the percentage of TF+ platelet-derived MPs suggests reduced pro-thrombotic potential, though further functional assays are required. Exercise did not cause aberrant immune cell activation, suggesting its safety from an immunological standpoint (ISRCTN38935454)
