    Impact of treatment planning target volumen (PTV) size on radiation induced diarrhoea following selenium supplementation in gynecologic radiation oncology - a subgroup analysis of a multicenter, phase III trial

    Background: In a previous analysis (Int J Radiat Oncol Biol Phys 70:828-835,2010), we assessed whether an adjuvant supplementation with selenium (Se) improves Se status and reduces the radiation-induced side-effects of patients treated by adjuvant radiotherapy (RT) for cervical and uterine cancer. Now, a potential relation between the planning target volume (PTV) of the RT and the Se effect concerning radiation induced diarrhoea was evaluated in detail. Methods: Whole blood Se concentrations had been measured in patients with cervical (n=11) and uterine cancer (n=70) after surgical treatment, during, and at the end of RT. Patients with initial Se concentrations of less than 84 μg/l were categorized as Se-deficient and randomized before RT to receive Se (as sodium selenite) per os on the days of RT, or to receive no supplement during RT. Diarrhoea was graded according to the Common Toxicity Criteria system (CTC, Version 2a). The evaluation of the PTV of the RT was ascertained with the help of a specialised computer-assisted treatment planning software used for radiation planning procedure. Results: A total of 81 patients had been randomized for the initial supplementation study, 39 of which received Se [selenium group, SeG] and 42 serving as controls [control group, CG]. Mean Se levels did not differ between SeG and CG upon study initiation, but were significantly higher in the SeG compared to the CG at the end of RT. The actuarial incidence of at least CTC 2 radiation induced diarrhoea in the SeG was 20.5% compared to 44.5% in the CG (p=0.04). The median PTV in both groups was 1302 ml (916–4608). With a PTV of 1302 ml (n=40) the actuarial incidence of at least CTC 2 diarrhoea in the SeG was 19.1% (4 of 21 patients) versus 52.6% (10 of 19 patients) in the CG (p=0.046). Conclusions: Se supplementation during RT was effective to improve blood Se status in Se-deficient cervical and uterine cancer patients, and reduces episodes and severity of RT-induced diarrhoea. This effect was most pronounced and significant in patients with large PTV (> 1302 ml)

    Cholesterol Synthesis Is Associated with Hepatic Lipid Content and Dependent on Fructose/Glucose Intake in Healthy Humans

    Visceral obesity and fatty liver have been related to high synthesis and low absorption of cholesterol. This study aimed to investigate the associations of cholesterol metabolism with liver and visceral fat content in healthy humans. Another objective was to explore the effects of very-high-fructose and very-high-glucose diets on cholesterol homeostasis. We report on a cohort of 20 people (12 males, 8 females; age 30.5 ± 2.0 years; body mass index 25.9 ± 0.5 kg/m2) who completed a four-week dietary intervention study. Between the baseline and the followup examination the study participants in addition to a balanced weight-maintaining diet received 150 g of either fructose or glucose per day. Visceral and liver fat were measured with magnetic resonance (MR) imaging and 1H-MR spectroscopy, respectively. Cholesterol absorption and synthesis were estimated from the serum noncholesterol sterol concentrations. Performing cross-sectional analyses the lanosterol and desmosterol to cholesterol ratios were positively correlated with visceral and liver fat content (all P < .03). The lathosterol to cholesterol ratio decreased in response to high-fructose diet (P = .006) but not in response to high-glucose diet. To conclude, visceral and liver fat content are associated with cholesterol synthesis in healthy humans. Furthermore, cholesterol synthesis appears to be dependent on fructose/glucose intake

    Three-dimensional printing (3DP) represents an emerging field of surgery. 3DP can facilitate the plastic surgeon’s workflow, including preoperative planning, intraoperative assistance, and postoperative follow-up. The broad clinical application spectrum stands in contrast to the paucity of research on the legal framework of 3DP. This imbalance poses a potential risk for medical malpractice lawsuits. To address this knowledge gap, we aimed to summarize the current body of legal literature on medical 3DP in the US legal system. By combining the promising clinical use of 3DP with its current legal regulations, plastic surgeons can enhance patient safety and outcomes

    Introduction Horizontal gene transfer (HGT) is an important driver for resistance- and virulence factor accumulation in pathogenic bacteria such as Staphylococcus aureus. Methods Here, we have investigated the downstream region of the bacterial chromosomal attachment site (attB) for the staphylococcal cassette chromosome mec (SCCmec) element of a commensal mecC- positive Staphylococcus stepanovicii strain (IMT28705; ODD4) with respect to genetic composition and indications of HGT. S. stepanovicii IMT28705 was isolated from a fecal sample of a trapped wild bank vole (Myodes glareolus) during a screening study (National Network on “Rodent-Borne Pathogens”) in Germany. Whole genome sequencing (WGS) of IMT28705 together with the mecC- negative type strain CM7717 was conducted in order to comparatively investigate the genomic region downstream of attB (GenBank accession no. KR732654 and KR732653). Results The bank vole isolate (IMT28705) harbors a mecC gene which shares 99.2% nucleotide (and 98.5% amino acid) sequence identity with mecC of MRSA_LGA251. In addition, the mecC-encoding region harbors the typical blaZ-mecC-mecR1-mecI structure, corresponding with the class E mec complex. While the sequences downstream of attB in both S. stepanovicii isolates (IMT28705 and CM7717) are partitioned by 15 bp direct repeats, further comparison revealed a remarkable low concordance of gene content, indicating a chromosomal “hot spot” for foreign DNA integration and exchange. Conclusion Our data highlight the necessity for further research on transmission routes of resistance encoding factors from the environmental and wildlife resistome

    On the seismic age and heavy-element abundance of the Sun

    We estimate the main-sequence age and heavy-element abundance of the Sun by means of an asteroseismic calibration of theoretical solar models using only low-degree acoustic modes from the BiSON. The method can therefore be applied also to other solar-type stars, such as those observed by the NASA satellite Kepler and the planned ground-based Danish-led SONG network. The age, 4.60+/-0.04 Gy, obtained with this new seismic method, is similar to, although somewhat greater than, today's commonly adopted values, and the surface heavy-element abundance by mass, Zs=0.0142+/-0.0005, lies between the values quoted recently by Asplund et al. (2009) and by Caffau et al. (2009). We stress that our best-fitting model is not a seismic model, but a theoretically evolved model of the Sun constructed with `standard' physics and calibrated against helioseismic data.Comment: 16 pages, 11 figures, 5 tables, accepted for publication in MNRA

    Exoplanets around G-K Giants

    G and K giants are a class of radial velocity (RV) variables. One reason for this variability are planetary companions which are indicated in time series of stellar spectra. Since 2004 these spectra in the visual range were obtained with the high resolution coud\'e \'echelle spectrograph mounted on the 2m telescope of the Th\"uringer Landessternwarte Tautenburg (TLS) for a northern sample of 62 very bright K giants. In the South around 300 G and K giants were observed with HARPS mounted on the 3.6m telescope on La Silla. The TLS sample contains at least 11 stars (18 %) which show low-amplitude, long-period RV variations most likely due to planets. This percentage of planet frequency is confirmed by preliminary results of the HARPS study. Moreover the TLS survey seems to indicate that giant planets do not favour metal-rich stars, are more massive, and have longer periods than those found around solar-type host stars.Comment: Part of PlanetsbeyondMS/2010 proceedings http://arxiv.org/html/1011.660

    Modeling beta Virginis using seismological data

    This paper presents the modeling of the F9 V star beta Virginis based on seismological measurements. Using the Geneva evolution code including rotation and atomic diffusion, we find that two distinct solutions reproduce all existing asteroseismic and non-asteroseismic observational constraints well: a main-sequence model with a mass of 1.28 +- 0.03 Msun and an age t = 3.24 +- 0.20 Gyr, or a model in the post-main sequence phase of evolution with a lower mass of 1.21 +- 0.02 Msun and an age t = 4.01 +- 0.30 Gyr. The small spacings and the ratio between small and large spacings are sensitive to the differences in the structure of the central layers between these two solutions and are also sensitive to the structural changes due to the rotational mixing. They can therefore be used to unambiguously determine the evolutionary state of beta Vir and to study the effects of rotation on the inner structure of the star. Unfortunately, existing asteroseismic data do not enable such precise determination. We also show that the scatter in frequencies introduced by the rotational splittings can account for the larger dispersion of the observed large spacings for the non-radial modes than for the radial modes.Comment: 11 pages, 12 figures, accepted for publication in A&

    Accurate fundamental parameters for 23 bright solar-type stars

    We combine results from interferometry, asteroseismology and spectroscopy to determine accurate fundamental parameters of 23 bright solar-type stars, from spectral type F5 to K2 and luminosity classes III to V. For some stars we can use direct techniques to determine the mass, radius, luminosity and effective temperature, and we compare with indirect methods that rely on photometric calibrations or spectroscopic analyses. We use the asteroseismic information available in the literature to infer an indirect mass with an accuracy of 4-15 percent. From indirect methods we determine luminosity and radius to 3 percent. For Teff we find a slight offset of -40+-20 K between the spectroscopic method and the direct method, meaning the spectroscopic temperatures are too high. From the spectroscopic analysis we determine the detailed chemical composition for 13 elements, including Li, C and O. We find no significant offset between the spectroscopic surface gravity and the value from combining asteroseismology with radius estimates. From the spectroscopy we also determine vsini and we present a new calibration of macro- and microturbulence. From the comparison between the results from the direct and spectroscopic methods we claim that we can determine Teff, log g, and [Fe/H] with absolute accuracies of 80 K, 0.08 dex, and 0.07 dex. The indirect methods are important to obtain reliable estimates of the fundamental parameters of relatively faint stars when interferometry cannot be used. Our study is the first to compare direct and indirect methods for a large sample of stars, and we conclude that indirect methods are valid, although slight corrections may be needed.Comment: Accepted by MNRAS.
