    E' proposta una applicazione di alcune metodologie basate sulla teoria della dislocazione e sulle leggi di propagazione ondosa, al caso dei terremoti verificatisi tra il 1988 ed il 1991 nella Calabria Meridionale e Sicilia Nordorientale. L'uso dei dati sperimentali dei tempi d'arrivo delle onde sismiche alle stazioni sismometriche ha consentito la localizzazione degli eventi, mentre le caratteristiche di radiazione-propagazione dei primi impulsi relativi alle onde longitudinali hanno reso possibile la definizione dei meccanismi di dislocazione.     Seismic location improvements from an OBS/H temporary network in Southern Tyrrhenian Sea

    We present the first investigation performed on the seismicity of Southern Tyrrhenian Sea, off-shore Sicily with the contribution of data from broad-band ocean bottom seismometers and hydrophones (OBS/H). Offshore data were recorded during the TYrrhenian Deep sea Experiment (TYDE) from December 2000 to May 2001 in the Southern Tyrrhenian Sea. Hypocenter locations of a cluster of 53 seismic events occurred in March 2001 in north-eastern Sicily were estimated by the integration of land (permanent network) and offshore (temporary network) data and compared with locations estimated from land data only. The scatter of the cluster was evaluated by dispersion parameters. The off-shore data significantly reduced the scatter of the swarm hypocenters also restricting the depth range of the cluster. Moreover, space trends of the event distribution originally shown by the land data were only partially confirmed by the land-sea joint data. In order to assess the efficiency in terms of hypocenter mislocations in the subject area, of a land-sea integrated network with respect to a land-based network, we performed simulations by assuming a grid distribution of earthquakes and a recent local 3D velocity model, computing synthetic arrival times of body waves to the stations of both network configurations (integrated and land-based) perturbing the computed times and relocating earthquakes by inversion. The results of the synthetic tests demonstrated that the presence of sea bottom stations in the Tyrrhenian basin can reduce the mislocations of large magnitude and/or superficial earthquakes in the southernmost Calabria and Messina Strait and of low magnitude and/or deep earthquakes in north-eastern Sicily. The major accuracy of synthetic earthquake locations obtained including OBS/H data provides an additional support to the interpretation of the cluster occurred in March 2001 and to the opportunity of long-term installation of an off-shore network like TYDE in the study region

    Cancer Associated Fibroblasts and Senescent Thyroid Cells in the Invasive Front of Thyroid Carcinoma

    Thyroid carcinoma (TC) comprises several histotypes with different aggressiveness, from well (papillary carcinoma, PTC) to less differentiated forms (poorly differentiated and anaplastic thyroid carcinoma, PDTC and ATC, respectively). Previous reports have suggested a functional role for cancer-associated fibroblasts (CAFs) or senescent TC cells in the progression of PTC. In this study, we investigated the presence of CAFs and senescent cells in proprietary human TCs including PTC, PDTC, and ATC. Screening for the driving lesions BRAFV600E and N/H/KRAS mutations, and gene fusions was also performed to correlate results with tumor genotype. In samples with unidentified drivers, transcriptomic profiles were used to establish a BRAF- or RAS-like molecular subtype based on a gene signature derived from The Cancer Genome Atlas. By using immunohistochemistry, we found co-occurrence of stromal CAFs and senescent TC cells at the tumor invasive front, where deposition of collagen (COL1A1) and expression of lysyl oxidase (LOX) enzyme were also detected, in association with features of local invasion. Concurrent high expression of CAFs and of the senescent TC cells markers, COL1A1 and LOX was confirmed in different TC histotypes in proprietary and public gene sets derived from Gene Expression Omnibus (GEO) repository, and especially in BRAF mutated or BRAF-like tumors.     Serum Albumin Is Inversely Associated With Portal Vein Thrombosis in Cirrhosis

    We analyzed whether serum albumin is independently associated with portal vein thrombosis (PVT) in liver cirrhosis (LC) and if a biologic plausibility exists. This study was divided into three parts. In part 1 (retrospective analysis), 753 consecutive patients with LC with ultrasound-detected PVT were retrospectively analyzed. In part 2, 112 patients with LC and 56 matched controls were entered in the cross-sectional study. In part 3, 5 patients with cirrhosis were entered in the in vivo study and 4 healthy subjects (HSs) were entered in the in vitro study to explore if albumin may affect platelet activation by modulating oxidative stress. In the 753 patients with LC, the prevalence of PVT was 16.7%; logistic analysis showed that only age (odds ratio [OR], 1.024; P = 0.012) and serum albumin (OR, -0.422; P = 0.0001) significantly predicted patients with PVT. Analyzing the 112 patients with LC and controls, soluble clusters of differentiation (CD)40-ligand (P = 0.0238), soluble Nox2-derived peptide (sNox2-dp; P < 0.0001), and urinary excretion of isoprostanes (P = 0.0078) were higher in patients with LC. In LC, albumin was correlated with sCD4OL (Spearman's rank correlation coefficient [r(s)], -0.33; P < 0.001), sNox2-dp (r(s), -0.57; P < 0.0001), and urinary excretion of isoprostanes (r(s), -0.48; P < 0.0001) levels. The in vivo study showed a progressive decrease in platelet aggregation, sNox2-dp, and urinary 8-iso prostaglandin F2 alpha-III formation 2 hours and 3 days after albumin infusion. Finally, platelet aggregation, sNox2-dp, and isoprostane formation significantly decreased in platelets from HSs incubated with scalar concentrations of albumin. Conclusion: Low serum albumin in LC is associated with PVT, suggesting that albumin could be a modulator of the hemostatic system through interference with mechanisms regulating platelet activation

    Mortality and pulmonary complications in patients undergoing surgery with perioperative SARS-CoV-2 infection: an international cohort study

    Background: The impact of severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) on postoperative recovery needs to be understood to inform clinical decision making during and after the COVID-19 pandemic. This study reports 30-day mortality and pulmonary complication rates in patients with perioperative SARS-CoV-2 infection. Methods: This international, multicentre, cohort study at 235 hospitals in 24 countries included all patients undergoing surgery who had SARS-CoV-2 infection confirmed within 7 days before or 30 days after surgery. The primary outcome measure was 30-day postoperative mortality and was assessed in all enrolled patients. The main secondary outcome measure was pulmonary complications, defined as pneumonia, acute respiratory distress syndrome, or unexpected postoperative ventilation. Findings: This analysis includes 1128 patients who had surgery between Jan 1 and March 31, 2020, of whom 835 (74·0%) had emergency surgery and 280 (24·8%) had elective surgery. SARS-CoV-2 infection was confirmed preoperatively in 294 (26·1%) patients. 30-day mortality was 23·8% (268 of 1128). Pulmonary complications occurred in 577 (51·2%) of 1128 patients; 30-day mortality in these patients was 38·0% (219 of 577), accounting for 81·7% (219 of 268) of all deaths. In adjusted analyses, 30-day mortality was associated with male sex (odds ratio 1·75 [95% CI 1·28–2·40], p\textless0·0001), age 70 years or older versus younger than 70 years (2·30 [1·65–3·22], p\textless0·0001), American Society of Anesthesiologists grades 3–5 versus grades 1–2 (2·35 [1·57–3·53], p\textless0·0001), malignant versus benign or obstetric diagnosis (1·55 [1·01–2·39], p=0·046), emergency versus elective surgery (1·67 [1·06–2·63], p=0·026), and major versus minor surgery (1·52 [1·01–2·31], p=0·047). Interpretation: Postoperative pulmonary complications occur in half of patients with perioperative SARS-CoV-2 infection and are associated with high mortality. Thresholds for surgery during the COVID-19 pandemic should be higher than during normal practice, particularly in men aged 70 years and older. Consideration should be given for postponing non-urgent procedures and promoting non-operative treatment to delay or avoid the need for surgery.     Analysis of Conditional Stability and Unconditional Stability and Instability for Microwave 3-Ports

    A full and rigorous treatment of conditional and unconditional stability and instability for an active 3-port microwave circuit, represented at a given frequency by S-parameters, is carried out. By terminating a given port with a variable termination one may study at the input and the output of the resulting 2-port circuit the occurrence of unconditional stability and instability, as well as of special cases of conditional stability, defined as reactive unconditional stability or instability. Such conditions are especially relevant in microwave oscillator design and were exhaustively identified for the first time and treated in previous analysis by the authors. Drawing from its conclusions, one obtains in the plane of the variable termination and with a simple algorithm, a single parametric representation, providing at once, in a closed and direct form, the entire collection of boundaries of the regions, wherein the listed conditions occur at ports of the resulting 2-port circuit. Guidelines for the design of microwave amplifiers and oscillators, both in series and shunt configuration, are sketched. Numerical examples, validated by simulations, are also presented
