Asimismo, da respuesta a la necesidad del alumnado de crear espacios de aprendizaje donde desarrollar propuestas que favorezcan el impulso de la carrera universitaria y donde se pueda colaborar de forma horizontal con otras personas de forma que se potencie la creatividad y la iniciativa

    Treatment with tocilizumab or corticosteroids for COVID-19 patients with hyperinflammatory state: a multicentre cohort study (SAM-COVID-19)

    Objectives: The objective of this study was to estimate the association between tocilizumab or corticosteroids and the risk of intubation or death in patients with coronavirus disease 19 (COVID-19) with a hyperinflammatory state according to clinical and laboratory parameters. Methods: A cohort study was performed in 60 Spanish hospitals including 778 patients with COVID-19 and clinical and laboratory data indicative of a hyperinflammatory state. Treatment was mainly with tocilizumab, an intermediate-high dose of corticosteroids (IHDC), a pulse dose of corticosteroids (PDC), combination therapy, or no treatment. Primary outcome was intubation or death; follow-up was 21 days. Propensity score-adjusted estimations using Cox regression (logistic regression if needed) were calculated. Propensity scores were used as confounders, matching variables and for the inverse probability of treatment weights (IPTWs). Results: In all, 88, 117, 78 and 151 patients treated with tocilizumab, IHDC, PDC, and combination therapy, respectively, were compared with 344 untreated patients. The primary endpoint occurred in 10 (11.4%), 27 (23.1%), 12 (15.4%), 40 (25.6%) and 69 (21.1%), respectively. The IPTW-based hazard ratios (odds ratio for combination therapy) for the primary endpoint were 0.32 (95%CI 0.22-0.47; p < 0.001) for tocilizumab, 0.82 (0.71-1.30; p 0.82) for IHDC, 0.61 (0.43-0.86; p 0.006) for PDC, and 1.17 (0.86-1.58; p 0.30) for combination therapy. Other applications of the propensity score provided similar results, but were not significant for PDC. Tocilizumab was also associated with lower hazard of death alone in IPTW analysis (0.07; 0.02-0.17; p < 0.001). Conclusions: Tocilizumab might be useful in COVID-19 patients with a hyperinflammatory state and should be prioritized for randomized trials in this situatio

    La posición, identidad y rol social determinan los niveles de interacción positiva con nuestro entorno y, por ende, la capacidad del individuo para construir un tejido social estable que repercuta en los recursos psicosociales de la persona para afrontar sucesos de índole estresante. El estigma, el aislamiento y el desajuste de los roles sociales positivos fomentan la creación de espacios de exclusión en los que las personas con discapacidad intelectual ven imposibilitado el desarrollo de un plan de vida autónomo y de calidad. Residir en una comunidad no significa ser parte de ella, estar integrado en la misma o participar de forma activa. El trabajo en pro de la calidad de vida debe estar ligado al incremento de las redes sociales de apoyo estables y funcionalmente positivas, de la integración y la participación comunitarias y de la creación de espacios inclusivos que aborden de forma crítica el feedback sociedad-persona con discapacidad intelectual. Los resultados obtenidos a través de la realización de 125 entrevistas a personas tuteladas residentes en centros de atención a personas con discapacidad y/o residencias para personas mayores muestran déficits en los niveles de bienestar psicosocial que correlacionan de forma significativa con la calidad de vida. Además, se analizarán los citados resultados en función del sexo, el grado reconocido de dependencia y el tipo de alojamiento

    Influence of the EULAR recommendations for the use of imaging in large vessel vasculitis in the diagnosis of giant cell arteritis: results of the ARTESER register

    Objective The main study objective was to determine how giant cell arteritis (GCA) is diagnosed in our clinical practice and whether the EULAR recommendations have influenced the diagnostic procedures used.Methods ARTEritis of the Rheumatology Spanish Society -Sociedad Española de Reumatología (ARTESER) is a multicentre observational retrospective study conducted in 26 hospitals with support from the Spanish Society of Rheumatology. All patients diagnosed with GCA between 1 June 2013 and 29 March 2019 were included. The gold standard for the diagnosis of GCA was the judgement of the physician in charge, according to clinical criteria, supported by data available from laboratory tests, imaging studies (ultrasound, positron emission tomography (PET) and MRI/CT angiography) and temporal artery biopsy (TAB) when available.Results We included 1675 patients with GCA (mean age±SD (76.9±8.1) years, 1178 women (70.3%)). Of these, 776 patients had a positive TAB (46.3%), 503 (30.0%) positive ultrasound, 245 positive PET (14.6%) and 64 positive MRI/CT angiography (3.8%). These percentages changed substantially over the study. From 2013 to 2019, the use of ultrasound in diagnosis grew from 25.8% to 52.9% and PET from 12.3% to 19.6%, while use of TAB decreased from 50.3% to 33.3%.Conclusions Biopsy was the most widely used diagnostic test for confirming GCA, but use of imaging as a diagnostic tool has grown in recent years. Following publication of the 2018 EULAR recommendations, ultrasound has displaced biopsy as the first-line diagnostic test; TAB was performed in a third and PET in a fifth of cases

    Coping with poachers in European stalked barnacle fisheries: Insights from a stakeholder workshop

    International audienceIn January 2020, a stakeholder workshop was organized as a knowledge sharing strategy among European stalked barnacle fisheries. Management of this fishery differs greatly among regions and ranges from less organized and governed at large scales (>100 km, coasts of SW Portugal and Brittany in France) to highly participatory systems which are co-managed at small spatial scales (10′s km and less, Galicia and Asturias). Discussions revealed that poaching is ubiquitous, hard to eradicate, and adapts to all types of management. The stakeholders identified some key management initiatives in the fight against poaching: granting professional harvesters with exclusive access to the resource, increasing social capital among harvesters through tenure systems (e.g. Territorial Use Rights in Fisheries) that empower them as stewards of their resource and intensification of surveillance with the active participation of the harvesters. Furthermore, increased cooperation between fishers associations and regional fisheries authorities, improved legal frameworks, adoption of new technologies and the implementation of market-based solutions can also help coping with this systemic problem

    Descripción: Ética y Ciudadanía es un curso de formación general, de carácter teórico-práctico, dirigido a estudiantes de ciclos iniciales de distintas carreras; que busca contribuir al desarrollo una de las competencias generales de nuestro modelo educativo: Ciudadanía en el nivel 1. Propósito: El curso pretende contribuir con el aprendizaje ético y ciudadano de los estudiantes invitándolos a conocer y explicar problemas éticos y de su vida cotidiana. Teniendo en cuenta que este curso se realiza bajo la modalidad semipresencial, se proponen diversas actividades educativas de tipo individual y grupal, tanto en las sesiones presenciales como en las sesiones virtuales, lo que supone una participación activa, organizada y permanente por parte de los estudiantes. Las estrategias a utilizarse incluyen: lectura y discusión de textos, análisis y evaluación de películas y documentales, resolución de casos, debates y exposiciones, foros virtuales. Siendo el docente la pieza clave para este proceso de aprendizaje, cumple con el rol de promover un ambiente colaborativo a través del diálogo, el respeto y la reflexión, partiendo del conocimiento previo y las fortalezas de los estudiantes para generar nuevas capacidades y desarrollar la competencia asociada al curso
