    Kardiologische Rehabilitation mit strukturierter Schulung bei Herzinsuffizienz: Akzeptanz bei Patienten und Veränderungen in Krankheitswissen und Wohlbefinden

    Hintergrund und Fragestellung: Herzinsuffizienz ist ein häufiges klinisches Syndrom mit wachsender Bedeutung und schlechter Prognose. Patientenschulung ist ein zentraler Bestandteil der medizinischen Rehabilitation mit dem Ziel die Folgen chronischer Krankheiten durch gezielte Förderung von Krankheitskompetenz und Selbstmanagement zu verringern und die Lebensqualität von Patienten zu verbessern. Im Rahmen der vorliegenden explorativen Untersuchung wird ein neu entwickeltes Patientenschulungsprogramm für Herzinsuffizienz vorgestellt und die direkte Bewertung des Programms durch Patienten, die Veränderungen des krankheitsbezogenen Wissens, die gesundheits- und krankheitsbezogene Lebensqualität, sowie die psychische Symptombelastung im Verlauf erfasst. Methode: 64 Patienten (79,7% Männer) nahmen im Rahmen einer kardiologischen Rehabilitation an einer kompetenz-fokussierenden Schulung für Herzinsuffizienzpatienten teil. Die Patienten füllten zu Beginn, am Ende sowie sechs Monate nach der Rehabilitation Selbstbeurteilungsfragebögen zum psychischen Wohlbefinden und zur Lebenszufriedenheit, einen auf die Schulungsinhalte bezogenen Wissenstest und einen Evaluationsfragebogen aus. Ergebnisse: Die Bewertung des Schulungsprogramms durch die Teilnehmer war sehr gut. Wissen und Lebensqualität waren am Ende und sechs Monate nach der Rehabilitation signifikant gestiegen, die psychische Symptombelastung (Ängstlichkeit und Depressivität) reduziert. Schlussfolgerung: Während einer kardiologischen Rehabilitation erfährt ein strukturiertes Schulungsprogramm für Patienten mit Herzinsuffizienz eine hohe Akzeptanz seitens der Patienten. Die Patienten erfahren eine Verbesserung ihres Wissens über die Erkrankung, eine Reduktion von Angst- und Depressivitätserleben und eine Verbesserung ihrer Lebensqualität. Die Indikation für eine Schulung muss insbesondere bei Patienten mit Angstneigung sorgfältig abgeklärt werden

    Climate-trait relationships exhibit strong habitat specificity in plant communities across Europe

    Ecological theory predicts close relationships between macroclimate and functional traits. Yet, global climatic gradients correlate only weakly with the trait composition of local plant communities, suggesting that important factors have been ignored. Here, we investigate the consistency of climate-trait relationships for plant communities in European habitats. Assuming that local factors are better accounted for in more narrowly defined habitats, we assigned > 300,000 vegetation plots to hierarchically classified habitats and modelled the effects of climate on the community-weighted means of four key functional traits using generalized additive models. We found that the predictive power of climate increased from broadly to narrowly defined habitats for specific leaf area and root length, but not for plant height and seed mass. Although macroclimate generally predicted the distribution of all traits, its effects varied, with habitat-specificity increasing toward more narrowly defined habitats. We conclude that macroclimate is an important determinant of terrestrial plant communities, but future predictions of climatic effects must consider how habitats are defined

    Evolving and sustaining ocean best practices and standards for the next decade

    The oceans play a key role in global issues such as climate change, food security, and human health. Given their vast dimensions and internal complexity, efficient monitoring and predicting of the planet’s ocean must be a collaborative effort of both regional and global scale. A first and foremost requirement for such collaborative ocean observing is the need to follow well-defined and reproducible methods across activities: from strategies for structuring observing systems, sensor deployment and usage, and the generation of data and information products, to ethical and governance aspects when executing ocean observing. To meet the urgent, planet-wide challenges we face, methods across all aspects of ocean observing should be broadly adopted by the ocean community and, where appropriate, should evolve into “Ocean Best Practices.” While many groups have created best practices, they are scattered across the Web or buried in local repositories and many have yet to be digitized. To reduce this fragmentation, we introduce a new open access, permanent, digital repository of best practices documentation (oceanbestpractices.org) that is part of the Ocean Best Practices System (OBPS). The new OBPS provides an opportunity space for the centralized and coordinated improvement of ocean observing methods. The OBPS repository employs user-friendly software to significantly improve discovery and access to methods. The software includes advanced semantic technologies for search capabilities to enhance repository operations. In addition to the repository, the OBPS also includes a peer reviewed journal research topic, a forum for community discussion and a training activity for use of best practices. Together, these components serve to realize a core objective of the OBPS, which is to enable the ocean community to create superior methods for every activity in ocean observing from research to operations to applications that are agreed upon and broadly adopted across communities. Using selected ocean observing examples, we show how the OBPS supports this objective. This paper lays out a future vision of ocean best practices and how OBPS will contribute to improving ocean observing in the decade to come

    β-Lapachone Induces Acute Oxidative Stress in Rat Primary Astrocyte Cultures that is Terminated by the NQO1-Inhibitor Dicoumarol

    β-lapachone (β-lap) is reduced in tumor cells by the enzyme NAD(P)H: quinone acceptor oxidoreductase 1 (NQO1) to a labile hydroquinone which spontaneously reoxidises to β-lap, thereby generating reactive oxygen species (ROS) and oxidative stress. To test for the consequences of an acute exposure of brain cells to β-lap, cultured primary rat astrocytes were incubated with β-lap for up to 4 h. The presence of β-lap in concentrations of up to 10 µM had no detectable adverse consequences, while higher concentrations of β-lap compromised the cell viability and the metabolism of astrocytes in a concentration- and time-dependent manner with half-maximal effects observed for around 15 µM β-lap after a 4 h incubation. Exposure of astrocytes to β-lap caused already within 5 min a severe increase in the cellular production of ROS as well as a rapid oxidation of glutathione (GSH) to glutathione disulfide (GSSG). The transient cellular accumulation of GSSG was followed by GSSG export. The β-lap-induced ROS production and GSSG accumulation were completely prevented in the presence of the NQO1 inhibitor dicoumarol. In addition, application of dicoumarol to β-lap-exposed astrocytes caused rapid regeneration of the normal high cellular GSH to GSSG ratio. These results demonstrate that application of β-lap to cultured astrocytes causes acute oxidative stress that depends on the activity of NQO1. The sequential application of β-lap and dicoumarol to rapidly induce and terminate oxidative stress, respectively, is a suitable experimental paradigm to study consequences of a defined period of acute oxidative stress in NQO1-expressing cells

    Evaluation of a self-management patient education program for patients with chronic heart failure undergoing inpatient cardiac rehabilitation: study protocol of a cluster randomized controlled trial

    Background Chronic heart failure requires a complex treatment regimen on a life-long basis. Therefore, self-care/self-management is an essential part of successful treatment and comprehensive patient education is warranted. However, specific information on program features and educational strategies enhancing treatment success is lacking. This trial aims to evaluate a patient-oriented and theory-based self-management educational group program as compared to usual care education during inpatient cardiac rehabilitation in Germany. Methods/Design The study is a multicenter cluster randomized controlled trial in four cardiac rehabilitation clinics. Clusters are patient education groups that comprise HF patients recruited within 2 weeks after commencement of inpatient cardiac rehabilitation. Cluster randomization was chosen for pragmatic reasons, i.e. to ensure a sufficient number of eligible patients to build large-enough educational groups and to prevent contamination by interaction of patients from different treatment allocations during rehabilitation. Rehabilitants with chronic systolic heart failure (n = 540) will be consecutively recruited for the study at the beginning of inpatient rehabilitation. Data will be assessed at admission, at discharge and after 6 and 12 months using patient questionnaires. In the intervention condition, patients receive the new patient-oriented self-management educational program, whereas in the control condition, patients receive a short lecture-based educational program (usual care). The primary outcome is patients’ self-reported self-management competence. Secondary outcomes include behavioral determinants and self-management health behavior (symptom monitoring, physical activity, medication adherence), health-related quality of life, and treatment satisfaction. Treatment effects will be evaluated separately for each follow-up time point using multilevel regression analysis, and adjusting for baseline values. Discussion This study evaluates the effectiveness of a comprehensive self-management educational program by a cluster randomized trial within inpatient cardiac rehabilitation in Germany. Furthermore, subgroup-related treatment effects will be explored. Study results will contribute to a better understanding of both the effectiveness and mechanisms of a self-management group program as part of cardiac rehabilitation

    Seed dispersal distance classes and dispersal modes for the European flora

    Motivation Although dispersal ability is one of the key features determining the spatial dynamics of plant populations and the structure of plant communities, it is also one of the traits for which we still lack data for most species. We compiled a comprehensive dataset of seed dispersal distance classes and predominant dispersal modes for most European vascular plants. Our seed dispersal dataset can be used in functional biogeography, dynamic vegetation modelling and ecological studies at local to continental scales.Main Types of Variables Contained Species were classified into seven ordered classes with similar dispersal distances estimated based on the predominant dispersal mode, the morphology of dispersal units (diaspores or propagules), life form, plant height, seed mass, habitat and known dispersal by humans. We evaluated our results by comparing them with dispersal distances calculated using the ‘dispeRsal’ function in R.     Economic Evaluation of Exercise Training in Patients with Pulmonary Hypertension

    Exercise training as an add-on to medical therapy has been shown to improve exercise capacity, quality of life, and possibly prognosis in patients with pulmonary hypertension (PH). The purpose of this study was to analyze the impact of exercise training on healthcare costs in PH. Estimated healthcare costs have been compared between patients with severe PH under optimized medical therapy only (control group) versus patients who received exercise training as an add-on to medical therapy (training group). Cost-analysis included a cost-estimation model of costs for baseline and follow-up visits and all PH-related healthcare events that occurred within the follow-up period. Time to clinical worsening and survival were assessed by clinical records, phone, and/or control visits. At baseline, the training (n = 58) and control group (n = 48) did not differ in age, gender, WHO-functional class, 6-min walking distance, hemodynamic parameters, or PH-targeted medication. During a follow-up of 24 +/- A 12 months, the training group had significantly better survival rates at 1 and 3 years and less worsening events (death, lung transplantation, hospitalization due to PH, new PAH-targeted medication) than the control group (15 vs. 25 events, p < 0.05), which also led to lower estimated healthcare costs of 657a,not sign within a period of 2 years. This is the first study to investigate the cost-effectiveness of exercise training in PH. Due to less worsening events within 2 years, healthcare costs were lower in patients performing exercise training as add-on to medical therapy than in patients with medical treatment only. Further prospective, randomized studies are needed to confirm these findings

    Ellenberg‐type indicator values for European vascular plant species

    Aims: Ellenberg-type indicator values are expert-based rankings of plant species according to their ecological optima on main environmental gradients. Here we extend the indicator-value system proposed by Heinz Ellenberg and co-authors for Central Europe by incorporating other systems of Ellenberg-type indicator values (i.e., those using scales compatible with Ellenberg values) developed for other European regions. Our aim is to create a harmonized data set of Ellenberg-type indicator values applicable at the European scale. Methods: We collected European data sets of indicator values for vascular plants and selected 13 data sets that used the nine-, ten- or twelve-degree scales defined by Ellenberg for light, temperature, moisture, reaction, nutrients and salinity. We compared these values with the original Ellenberg values and used those that showed consistent trends in regression slope and coefficient of determination. We calculated the average value for each combination of species and indicator values from these data sets. Based on species’ co-occurrences in European vegetation plots, we also calculated new values for species that were not assigned an indicator value. Results: We provide a new data set of Ellenberg-type indicator values for 8908 European vascular plant species (8168 for light, 7400 for temperature, 8030 for moisture, 7282 for reaction, 7193 for nutrients, and 7507 for salinity), of which 398 species have been newly assigned to at least one indicator value. Conclusions: The newly introduced indicator values are compatible with the original Ellenberg values. They can be used for large-scale studies of the European flora and vegetation or for gap-filling in regional data sets.     Diversity and distribution of Raunkiær's life forms in European vegetation

    International audienceAims The Raunkiær's system classifies vascular plants into life forms based on the position of renewal buds during periods unfavourable for plant growth. Despite the importance of Raunkiær's system for ecological research, a study exploring the diversity and distribution of life forms on a continental scale is missing. We aim to (i) map the diversity and distribution of life forms in European vegetation and (ii) test for effects of bioclimatic variables while controlling for habitat‐specific responses. Location Europe. Methods We used data on life forms of 8883 species recorded in 546,501 vegetation plots of different habitats (forest, grassland, scrub and wetland). For each plot, we calculated: (i) the proportion of species of each life form and (ii) the richness and evenness of life forms. We mapped these plot‐level metrics averaged across 50 km × 50 km grid cells and modelled their response to bioclimatic variables. Results Hemicryptophytes were the most widespread life form, especially in the temperate zone of Central Europe. Conversely, therophyte and chamaephyte species were more common in the Mediterranean as well as in the dry temperate regions. Moreover, chamaephytes were also more common in the boreal and arctic zones. Higher proportions of phanerophytes were found in the Mediterranean. Overall, a higher richness of life forms was found at lower latitudes while evenness showed more spatially heterogeneous patterns. Habitat type was the main discriminator for most of the responses analysed, but several moisture‐related predictors still showed a marked effect on the diversity of therophytes and chamaephytes. Conclusions Our maps can be used as a tool for future biogeographic and macro‐ecological research at a continental scale. Habitat type and bioclimatic conditions are key for regulating the diversity and distribution of plant life forms, with concomitant consequences for the response of functional diversity in European vegetation to global environmental changes

    Disturbance indicator values for European plants

    Motivation Indicator values are numerical values used to characterize the ecological niches of species and to estimate their occurrence along gradients. Indicator values on climatic and edaphic niches of plant species have received considerable attention in ecological research, whereas data on the optimal positioning of species along disturbance gradients are less developed. Here, we present a new data set of disturbance indicator values identifying optima along gradients of natural and anthropogenic disturbance for 6382 vascular plant species based on the analysis of 736,366 European vegetation plots and using expert-based characterization of disturbance regimes in 236 habitat types. The indicator values presented here are crucial for integrating disturbance niche optima into large-scale vegetation analyses and macroecological studies. Main types of variables contained We set up five main continuous indicator values for European vascular plants: disturbance severity, disturbance frequency, mowing frequency, grazing pressure and soil disturbance. The first two indicators are provided separately for the whole community and for the herb layer. We calculated the values as the average of expert-based estimates of disturbance values in all habitat types where a species occurs, weighted by the number of plots in which the species occurs within a given habitat type.
