Stability of quantized time-delay nonlinear systems: A Lyapunov-Krasowskii-functional approach
Lyapunov-Krasowskii functionals are used to design quantized control laws for
nonlinear continuous-time systems in the presence of constant delays in the
input. The quantized control law is implemented via hysteresis to prevent
chattering. Under appropriate conditions, our analysis applies to stabilizable
nonlinear systems for any value of the quantization density. The resulting
quantized feedback is parametrized with respect to the quantization density.
Moreover, the maximal allowable delay tolerated by the system is characterized

Triangulated Surfaces in Twistor Space: A Kinematical Set up for Open/Closed String Duality
We exploit the properties of the three-dimensional hyperbolic space to
discuss a simplicial setting for open/closed string duality based on (random)
Regge triangulations decorated with null twistorial fields. We explicitly show
that the twistorial N-points function, describing Dirichlet correlations over
the moduli space of open N-bordered genus g surfaces, is naturally mapped into
the Witten-Kontsevich intersection theory over the moduli space of N-pointed
closed Riemann surfaces of the same genus. We also discuss various aspects of
the geometrical setting which connects this model to PSL(2,C) Chern-Simons

Acute kidney disease and renal recovery : consensus report of the Acute Disease Quality Initiative (ADQI) 16 Workgroup
Consensus definitions have been reached for both acute kidney injury (AKI) and chronic kidney disease (CKD) and these definitions are now routinely used in research and clinical practice. The KDIGO guideline defines AKI as an abrupt decrease in kidney function occurring over 7 days or less, whereas CKD is defined by the persistence of kidney disease for a period of > 90 days. AKI and CKD are increasingly recognized as related entities and in some instances probably represent a continuum of the disease process. For patients in whom pathophysiologic processes are ongoing, the term acute kidney disease (AKD) has been proposed to define the course of disease after AKI; however, definitions of AKD and strategies for the management of patients with AKD are not currently available. In this consensus statement, the Acute Disease Quality Initiative (ADQI) proposes definitions, staging criteria for AKD, and strategies for the management of affected patients. We also make recommendations for areas of future research, which aim to improve understanding of the underlying processes and improve outcomes for patients with AKD
Assessing clinical communication skills in physicians: are the skills context specific or generalizable
<p>Abstract</p> <p>Background</p> <p>Communication skills are essential for physicians to practice Medicine. Evidence for the validity and domain specificity of communication skills in physicians is equivocal and requires further research. This research was conducted to adduce evidence for content and context specificity of communication skills and to assess the usefulness of a generic instrument for assessing communication skills in International Medical Graduates (IMGs).</p> <p>Methods</p> <p>A psychometric design was used for identifying the reliability and validity of the communication skills instruments used for high-stakes exams for IMG's. Data were collected from 39 IMGs (19 men – 48.7%; 20 women – 51.3%; Mean age = 41 years) assessed at 14 station OSCE and subsequently in supervised clinical practice with several instruments (patient surveys; ITERs; Mini-CEX).</p> <p>Results</p> <p>All the instruments had adequate reliability (Cronbach's alpha: .54 – .96). There were significant correlations (r range: 0.37 – 0.70, <it>p </it>< .05) of communication skills assessed by examiner with standardized patients, and of mini-CEX with patient surveys, and ITERs. The intra-item reliability across all cases for the 13 items was low (Cronbach's alpha: .20 – .56). The correlations of communication skills within method (e.g., OSCE or clinical practice) were significant but were non-significant between methods (e.g., OSCE and clinical practice).</p> <p>Conclusion</p> <p>The results provide evidence of context specificity of communication skills, as well as convergent and criterion-related validity of communication skills. Both in OSCEs and clinical practice, communication checklists need to be case specific, designed for content validity.</p
Progesterone levels in fat-eree milk of cows during post partum period and early pregnancy diagnosis
Foram avaliados os períodos pós-parto de vacas primíparas mestiças Gir x Caracu e Nelore x Caracu mantidas em regime extensivo de pastagem. Para a determinação dos níveis hormonais de progesterona, amostras de leite foram coletadas semanalmente até que os animais apresentassem o 1º estro perceptível, quando foram inseminadas artificialmente. Decorridos 21 dias da inseminação, amostras foram coletadas e dosadas para fazer o diagnóstico precoce de gestação. Os resultados mostraram início de ciclicidade ovariana com 9,89 semanas após o parto para vacas Gir x Caracu, com valores médios de progesterona de 0,28 nmol/l no dia do estro e 7,62 nmol/1 aos 21 dias após a inseminação. Para vacas Nelore x Caracu, o estro se deu com 12,08 semanas após o parto e com valores médios de progesterona de 0,46 nmol/l e 9.23 nmol/1 para o dia do estro e no 21º dia, respectivamente. Dez gestações de doze diagnosticadas no 21º dia foram confirmadas por palpação retal, tendo ocorrido um caso de mortalidade embrionária precoce. Outras três vacas foram inseminadas durante a fase luteínica. A eficiência na observação de estros foi de 52.94%, pois de 34 identificados pelos níveis hormonais, 18 foram percebidos visualmente. A variação entre os cruzamentos não alterou significativamente o nível de progesterona no dia do estro, tampouco no 21º dia após inseminação e também quanto ao período necessário ao reinício da atividade ovariana pós - parto.Progesterone concentrations were measured by radioimmunoassay in fat-free milk samples obtained weekly from 26 primiparous, suckling cows, raised in range conditions. Samples were collected in post partum period up to observation of first signs of estrus. Profiles of progesterone concentrations for early pregnancy diagnosis were also examined on milk samples taken 21 days after artificial insemination. Results showed ovarian ciclicity starting 9.89 weeks post partum in crossed Gir x Caracu cows, with 0.28 nmol/1 progesterone on the day of estrus anti 7.62 nmol/1 21 days post Al. In crossed Nelore x Caracu cows, these results were 12.08 weeks, 0.46 nmol/1 and 9.23 nmol/1, respectively. Ten pregnancies out of 12 diagnosed were confirmed by rectal palpation. There was one case of embryo mortality and three AI were practiced during luteal phase. Estrus observation efficiency was 52.94%, since out of 34 heats identified through hormonal determinations, 18 were detected visually. Variations between crossings did not significantly alter progesterone profiles on day of estrus, nor on 21th day after AI or as for ovarian activity post partum
A Perspective of Coagulation Dysfunction in Multiple Sclerosis and in Experimental Allergic Encephalomyelitis
A key role of both coagulation and vascular thrombosis has been reported since the first descriptions of multiple sclerosis (MS). Subsequently, the observation of a close concordance between perivascular fibrin(ogen) deposition and the occurrence of clinical signs in experimental allergic encephalomyelitis (EAE), an animal model of MS, led to numerous investigations focused on the role of thrombin and fibrin(ogen). Indeed, the activation of microglia, resident innate immune cells, occurs early after fibrinogen leakage in the pre-demyelinating lesion stage of EAE and MS. Thrombin has both neuroprotective and pro-apoptotic effects according to its concentration. After exposure to high concentrations of thrombin, astrocytes become reactive and lose their neuroprotective and supportive functions, microglia proliferate, and produce reactive oxygen species, IL-1β, and TNFα. Heparin inhibits the thrombin generation and suppresses EAE. Platelets play an important role too. Indeed, in the acute phase of the disease, they begin the inflammatory response in the central nervous system by producing of IL-1alpha and triggering and amplifying the immune response. Their depletion, on the contrary, ameliorates the course of EAE. Finally, it has been proven that the use of several anticoagulant agents can successfully improve EAE. Altogether, these studies highlight the role of the coagulation pathway in the pathophysiology of MS and suggest possible therapeutic targets that may complement existing treatments
Health-Promoting and Health-Risk Behaviors: Theory-Driven Analyses of Multiple Health Behavior Change in Three International Samples
Background: Co-occurrence of different behaviors was investigated using the theoretical underpinnings of the Transtheoretical Model, the Theory of Triadic Influence and the concept of Transfer.
Purpose: To investigate relationships between different health behaviors' stages of change, how behaviors group, and whether study participants cluster in terms of their behaviors.
Method: Relationships across stages for different behaviors were assessed in three studies with N = 3,519, 965, and 310 individuals from the USA and Germany by telephone and internet surveys using correlational analyses, factor analyses, and cluster analyses.
Results: Consistently stronger correlations were found between nutrition and physical activity (r = 0.16-0.26, p < 0.01) than between non-smoking and nutrition (r = 0.08-0.16, p < 0.03), or non-smoking and physical activity (r = 0.01-0.21). Principal component analyses of investigated behaviors indicated two factors: a "health-promoting" factor and a "health-risk" factor. Three distinct behavioral patterns were found in the cluster analyses.
Conclusion: Our results support the assumption that individuals who are in a higher stage for one behavior are more likely to be in a higher stage for another behavior as well. If the aim is to improve a healthy lifestyle, success in one behavior can be used to facilitate changes in other behaviors--especially if the two behaviors are both health-promoting or health-risky. Moreover, interventions should be targeted towards the different behavioral patterns rather than to single behaviors. This might be achieved by addressing transfer between behaviors
Cadmium Induces p53-Dependent Apoptosis in Human Prostate Epithelial Cells
Cadmium, a widespread toxic pollutant of occupational and environmental concern, is a known human carcinogen. The prostate is a potential target for cadmium carcinogenesis, although the underlying mechanisms are still unclear. Furthermore, cadmium may induce cell death by apoptosis in various cell types, and it has been hypothesized that a key factor in cadmium-induced malignant transformation is acquisition of apoptotic resistance. We investigated the in vitro effects produced by cadmium exposure in normal or tumor cells derived from human prostate epithelium, including RWPE-1 and its cadmium-transformed derivative CTPE, the primary adenocarcinoma 22Rv1 and CWR-R1 cells and LNCaP, PC-3 and DU145 metastatic cancer cell lines. Cells were treated for 24 hours with different concentrations of CdCl2 and apoptosis, cell cycle distribution and expression of tumor suppressor proteins were analyzed. Subsequently, cellular response to cadmium was evaluated after siRNA-mediated p53 silencing in wild type p53-expressing RWPE-1 and LNCaP cells, and after adenoviral p53 overexpression in p53-deficient DU145 and PC-3 cell lines. The cell lines exhibited different sensitivity to cadmium, and 24-hour exposure to different CdCl2 concentrations induced dose- and cell type-dependent apoptotic response and inhibition of cell proliferation that correlated with accumulation of functional p53 and overexpression of p21 in wild type p53-expressing cell lines. On the other hand, p53 silencing was able to suppress cadmium-induced apoptosis. Our results demonstrate that cadmium can induce p53-dependent apoptosis in human prostate epithelial cells and suggest p53 mutation as a possible contributing factor for the acquisition of apoptotic resistance in cadmium prostatic carcinogenesis
