    Learnings From Longitudinal Patient-Reported and Clinical Outcomes in Palliative Head and Neck Cancer Care

    Objective: Patients with palliative head and neck cancer experience many symptoms in a short period of time. Longitudinal data on patient-reported outcomes in this phase are lacking. The aim of this study is to use structurally obtained patient-reported outcome data combined with clinical patient data and obtain insight in patient-reported outcomes, survival, circumstances of death, and interventions and treatment during the palliative phase in order to improve the quality of end-of-life care and patient-centered counseling. Study Design: Longitudinal observational cohort study. Setting: Tertiary cancer center. Method: Quality of life was prospectively collected using the European Organization for Research and Treatment of Cancer QLQ-C15-PAL. Tumor- and patient-specific data were retrospectively collected. Descriptive statistics, linear mixed models, and regression analyses were performed. Results: A significant deterioration was found in global health status, physical functioning, fatigue, dyspnea, appetite loss, and constipation over time. However, emotional functioning improved. Median survival was 5.1 months, and only a low percentage of in-hospital death was observed (7.8%). Higher global health status at intake was associated with prolonged survival. Conclusion: Structural measurement of patient-reported outcome together with clinical outcomes provides unique insight, which enables improvement of patient-centered counseling and care.</p

    Individualized Dynamic Prediction Model for Patient-Reported Voice Quality in Early-Stage Glottic Cancer

    Objective: Early-stage glottic cancer (ESGC) is a malignancy of the head and neck. Besides disease control, preservation and improvement of voice quality are essential. To enable expectation management and well-informed decision-making, patients should be sufficiently counseled with individualized information on expected voice quality. This study aims to develop an individualized dynamic prediction model for patient-reported voice quality. This model should be able to provide individualized predictions at every time point from intake to the end of follow-up. Study Design: Longitudinal cohort study. Setting: Tertiary cancer center. Methods: Patients treated for ESGC were included in this study (N = 294). The Voice Handicap Index was obtained prospectively. The framework of mixed and joint models was used. The prognostic factors used are treatment, age, gender, comorbidity, performance score, smoking, T-stage, and involvement of the anterior commissure. The overall performance of these models was assessed during an internal cross-validation procedure and presentation of absolute errors using box plots. Results: The mean age in this cohort was 67 years and 81.3% are male. Patients were treated with transoral CO2 laser microsurgery (57.8%), single vocal cord irradiation up to (24.5), or local radiotherapy (17.5%). The mean follow-up was 43.4 months (SD 21.5). Including more measurements during prediction improves predictive performance. Including more clinical and demographic variables did not provide better predictions. Little differences in predictive performance between models were found. Conclusion: We developed a dynamic individualized prediction model for patient-reported voice quality. This model has the potential to empower patients and professionals in making well-informed decisions and enables tailor-made counseling.</p

    Serum magnesium and calcium levels in relation to ischemic stroke : Mendelian randomization study

    ObjectiveTo determine whether serum magnesium and calcium concentrations are causally associated with ischemic stroke or any of its subtypes using the mendelian randomization approach.MethodsAnalyses were conducted using summary statistics data for 13 single-nucleotide polymorphisms robustly associated with serum magnesium (n = 6) or serum calcium (n = 7) concentrations. The corresponding data for ischemic stroke were obtained from the MEGASTROKE consortium (34,217 cases and 404,630 noncases).ResultsIn standard mendelian randomization analysis, the odds ratios for each 0.1 mmol/L (about 1 SD) increase in genetically predicted serum magnesium concentrations were 0.78 (95% confidence interval [CI] 0.69-0.89; p = 1.3 7 10-4) for all ischemic stroke, 0.63 (95% CI 0.50-0.80; p = 1.6 7 10-4) for cardioembolic stroke, and 0.60 (95% CI 0.44-0.82; p = 0.001) for large artery stroke; there was no association with small vessel stroke (odds ratio 0.90, 95% CI 0.67-1.20; p = 0.46). Only the association with cardioembolic stroke was robust in sensitivity analyses. There was no association of genetically predicted serum calcium concentrations with all ischemic stroke (per 0.5 mg/dL [about 1 SD] increase in serum calcium: odds ratio 1.03, 95% CI 0.88-1.21) or with any subtype.ConclusionsThis study found that genetically higher serum magnesium concentrations are associated with a reduced risk of cardioembolic stroke but found no significant association of genetically higher serum calcium concentrations with any ischemic stroke subtype

    Associations of autozygosity with a broad range of human phenotypes

    In many species, the offspring of related parents suffer reduced reproductive success, a phenomenon known as inbreeding depression. In humans, the importance of this effect has remained unclear, partly because reproduction between close relatives is both rare and frequently associated with confounding social factors. Here, using genomic inbreeding coefficients (F-ROH) for >1.4 million individuals, we show that F-ROH is significantly associated (p <0.0005) with apparently deleterious changes in 32 out of 100 traits analysed. These changes are associated with runs of homozygosity (ROH), but not with common variant homozygosity, suggesting that genetic variants associated with inbreeding depression are predominantly rare. The effect on fertility is striking: F-ROH equivalent to the offspring of first cousins is associated with a 55% decrease [95% CI 44-66%] in the odds of having children. Finally, the effects of F-ROH are confirmed within full-sibling pairs, where the variation in F-ROH is independent of all environmental confounding.Peer reviewe

    A multi-platform comparison of efficient probabilistic methods in the prediction of total knee replacement mechanics

    Explicit finite element (FE) and multi-body dynamics (MBD) models have been developed to evaluate total knee replacement (TKR) mechanics as a complement to experimental methods. In conjunction with these models, probabilistic methods have been implemented to predict performance bounds and identify important parameters, subject to uncertainty in component alignment and experimental conditions. Probabilistic methods, such as advanced mean value (AMV) and response surface method (RSM), provide an efficient alternative to the gold standard Monte Carlo simulation technique (MCST). The objective of the current study was to benchmark models from three platforms (two FE and one MBD) using various probabilistic methods by predicting the influence of alignment variability and experimental parameters on TKR mechanics in simulated gait. Predicted kinematics envelopes were on average about 2.6 mm for tibial anterior-posterior translation, 2.9° for tibial internal-external rotation and 1.9 MPa for tibial peak contact pressure for the various platforms and methods. Based on this good agreement with the MCST, the efficient probabilistic techniques may prove useful in the fast evaluation of new implant designs, including considerations of uncertainty, e.g. misalignment

    Inleiding: De opleiding van basisartsen en medisch specialisten vindt zowel in universitaire klinieken als perifere opleidingsziekenhuizen plaats. In dit artikel is onderzocht of er een verschil bestaat tussen academische en niet-academische centra in de waardering van onderwijsitems. Methode: Hiertoe zijn twee groepen onderzocht. Allereerst de co-assistenten (n=90) die middels een enquete hun mening gaven over het ervaren onderwijsklimaat tijdens hun co-schap gynaecologie en verloskunde. Daarnaast is (voormalige) agio's verloskunde en gynaecologie (n=326) gevraagd waar zij de meeste vaardigheden en kennis hebben opgedaan. Resultaten: De co-assistenten vonden het onderwijsklimaat in de periferie beter dan in de academie (p<0.05). Agio's en pas afgestudeerde gynaecologen deden vaardigheden en kennis het meest op in niet-academische ziekenhuizen (p<0.05). Discussie en conclusie: Oorzaken voor deze uitkomsten worden gezocht in (1) de beperkte prioriteit die de medische opleiding in academische ziekenhuizen krijgt en in (2) veranderende patiëntenstromen die het onderwijsklimaat benadelen. Er wordt voor gepleit om academische en niet-academische ziekenhuizen verder te laten integreren om de verschillende taken als onderwijs en onderzoek optimaal te laten vervullen.     Timing of Group B Streptococcus Screening in Pregnancy: A Systematic Review

    Background: Group B streptococcus (GBS) is an important cause of neonatal sepsis. Guidelines advise to collect cultures at 35-37 weeks' gestation and to administer intrapartum antibiotic prophylaxis in case of GBS-positive cultures, as well as in all preterm deliveries. Improved effectiveness of antenatal cultures might help to further decrease GBS early-onset disease. Objective: To determine the best timing of antenatal cultures, which may help establish optimal prevention of perinatal GBS infection in both term and preterm neonates. Methods: PubMed and EMBASE databases were searched for relevant articles published from 1966 to February 2009. Nine articles were included. Information about study features and predictive values of antenatal cultures were abstracted. Results: Positive predictive values for antenatal GBS cultures ranged from 43 to 100% (mean 69%) and negative predictive values from 80 to 100% (mean 94%). GBS cultures collected in late pregnancy had high positive predictive values for colonization during delivery. The negative predictive value was high and relatively constant regardless of GA. Conclusions: This systematic review confirms recommendations to screen pregnant women for colonization of GBS at 35-37 weeks' gestation, but one should be aware of the limitations of screening, with 6% of GBS carriers remaining undetected in antenatal cultures.
