    Gender, trauma type, and PTSD prevalence: a re-analysis of 18 nordic convenience samples

    BACKGROUND: The aim of the study was to examine a possible trauma type related variance in the gender difference of posttraumatic stress disorder (PTSD) prevalence. METHODS: An analysis was conducted on 18 convenience sample studies including data from a total of 5220 participants. The studies all applied the Harvard Trauma Questionnaire – part IV to assess PTSD. Cohen’s d was used to measure variance in gender differences. Trauma types included disasters and accidents, violence, loss, chronic disease and non-malignant diseases. RESULTS: The results showed an overall gender difference in PTSD prevalence similar to previous findings. Thus, women had a two-fold higher prevalence of PTSD than men. Besides categorical analyses, dimensional analyses of PTSD severity were also performed; the latter were associated with twice as large effect sizes. Females were more vulnerable to PTSD after disasters and accidents, followed by loss and non-malignant diseases. In violence and chronic disease, the gender differences were smallest. CONCLUSIONS: The findings support the existence of a trauma type related variance in gender differences in PTSD prevalence

    Bayesian Network Enhanced with Structural Reliability Methods: Methodology

    We combine Bayesian networks (BNs) and structural reliability methods (SRMs) to create a new computational framework, termed enhanced Bayesian network (eBN), for reliability and risk analysis of engineering structures and infrastructure. BNs are efficient in representing and evaluating complex probabilistic dependence structures, as present in infrastructure and structural systems, and they facilitate Bayesian updating of the model when new information becomes available. On the other hand, SRMs enable accurate assessment of probabilities of rare events represented by computationally demanding, physically-based models. By combining the two methods, the eBN framework provides a unified and powerful tool for efficiently computing probabilities of rare events in complex structural and infrastructure systems in which information evolves in time. Strategies for modeling and efficiently analyzing the eBN are described by way of several conceptual examples. The companion paper applies the eBN methodology to example structural and infrastructure systems

    The combined effect of gender and age on post traumatic stress disorder: do men and women show differences in the lifespan distribution of the disorder?

    <p>Abstract</p> <p>Background</p> <p>The aim of the study was to examine the combined effect of gender and age on post traumatic stress disorder (PTSD) in order to describe a possible gender difference in the lifespan distribution of PTSD.</p> <p>Methods</p> <p>Data were collected from previous Danish and Nordic studies of PTSD or trauma. The final sample was composed of 6,548 participants, 2,768 (42.3%) men and 3,780 (57.7%) women. PTSD was measured based on the Harvard Trauma Questionnaire, part IV (HTQ-IV).</p> <p>Results</p> <p>Men and women differed in lifespan distribution of PTSD. The highest prevalence of PTSD was seen in the early 40s for men and in the early 50s for women, while the lowest prevalence for both genders was in the early 70s. Women had an overall twofold higher PTSD prevalence than men. However, at some ages the female to male ratio was nearly 3:1. The highest female to male ratio was found for the 21 to 25 year-olds.</p> <p>Conclusions</p> <p>The lifespan gender differences indicate the importance of including reproductive factors and social responsibilities in the understanding of the development of PTSD.</p

    The Association of Work Stress and Glycemic Status Is Partially Mediated by Autonomic Nervous System Function: Cross-Sectional Results from the Mannheim Industrial Cohort Study (MICS)

    BACKGROUND:Work stress is associated with an increased risk of pre-diabetes, Type 2 diabetes, and inflammation, as well as decreased autonomic nervous system function as measured, for example, via heart rate variability. We investigated the extent to which the association between work stress and glycemic status is mediated by vagally-mediated heart rate variability (vmHRV) and/or inflammation. METHODS:Cross-sectional data from the Mannheim Industrial Cohort Study (MICS) with 9,937 participants were analyzed. The root mean squared successive differences (RMSSD) from long-term heart rate monitoring during work and night time periods was used to index vmHRV. Fasting plasma glucose and glycosylated hemoglobin were assessed to determine glycemic status. High sensitive C-reactive protein levels were observed as a measure of systemic inflammation and the Effort-Reward-Imbalance scale was used to evaluate work stress. Mediation models were adjusted for age, sex, and occupational status, and estimations were bootstrapped (5,000 replications). RESULTS:Effort-Reward-Imbalance was significantly negatively associated with RMSSD and both glycosylated hemoglobin and fasting plasma glucose during both work and night time periods. Effort-Reward-Imbalance was observed to have a significant direct effect on glycosylated hemoglobin and significant indirect effects, through RMSSD, on both glycemic measures during both time periods. Introducing C-reactive protein as a further mediator to the model did not alter the indirect effects observed. C-reactive protein, as an exclusive mediator, was observed to have smaller direct and indirect effects on the glycemic measures as compared to when Effort-Reward-Imbalance was included in the model. CONCLUSIONS:Our results suggest that the association between work stress and glycemic status is partially mediated through vmHRV independent of systemic inflammation as measured by C-reactive protein. We conclude that work stress may be an additional factor that promotes development of hyperglycemic-metabolic states. If supported by prospective evidence, these results may lead to new approaches for primary prevention of hyperglycemia in the workplace
