    “Test Tube Cetaceans”: From the Evaluation of Susceptibility to the Study of Genotoxic Effects of Different Environmental Contaminants Using Cetacean Fibroblast Cell Cultures

    Population estimates of various species of cetaceans indicate that certain species have declined dramatically this century. Most studies of contamination and biomarker responses in marine mammals have been conducted using animals killed by hunting, tacitly approving this activity. The development of a series of nondestructive techniques to evaluate biomarker responses and residue levels is strongly recommended for the hazard assessment, protection and conservation of endangered species of marine mammals. A non-invasive sampling method, represented by skin biopsy or integument biopsy (epidermis, dermis and blubber), have been developed and validated in cetaceans. In this paper we present the cetacean fibroblast cell cultures obtained from the skin biopsies as the “test tube cetacean” for evaluate both the susceptibility that the genotoxicity of different environmental contaminants. Fibroblast cell cultures were obtained from many species of cetaceans sampled in Mediterranean Sea (Italy) and in the Mar de Cortez (Mexico). Using test tube cetaceans we can study the relationships between contamination and biochemical responses. One of the principal applications of this developed in vitro system was the assessment of interspecies differences in the mixed function oxidase activity (Cyp1A1 and Cyp2B) induced by in vitro treatment of various contaminants, such as some Persistent Organic Pollutants (POPs) but also emerging contaminants (such as bisphenol A (BpA)) and nanoparticles, added at different concentrations. The induction of Cyp1A1 and Cyp2B was evaluated with the indirect immunofluorescence technique. Therefore, another purpose of this work was to evaluate the qualitative and quantitative MICA protein expression in fibroblast cell cultures with immunofluorescence technique as toxicological stress marker of the immune system of different species of cetaceans. Finally, In this paper we will present how to evaluate the presence of DNA damage by comet assay in test tube cetaceans, after treatment with different genotoxic compounds (for example PCBs, DDTs, PAHs, BPA)

    Sometimes Sperm Whales (Physeter macrocephalus) Cannot Find Their Way Back to the High Seas: A Multidisciplinary Study on a Mass Stranding

    BACKGROUND: Mass strandings of sperm whales (Physeter macrocephalus) remain peculiar and rather unexplained events, which rarely occur in the Mediterranean Sea. Solar cycles and related changes in the geomagnetic field, variations in water temperature and weather conditions, coast geographical features and human activities have been proposed as possible causes. In December 2009, a pod of seven male sperm whales stranded along the Adriatic coast of Southern Italy. This is the sixth instance from 1555 in this basin. METHODOLOGY/PRINCIPAL FINDINGS: Complete necropsies were performed on three whales whose bodies were in good condition, carrying out on sampled tissues histopathology, virology, bacteriology, parasitology, and screening of veins looking for gas emboli. Furthermore, samples for age determination, genetic studies, gastric content evaluation, stable isotopes and toxicology were taken from all the seven specimens. The animals were part of the same group and determined by genetic and photo-identification to be part of the Mediterranean population. Causes of death did not include biological agents, or the "gas and fat embolic syndrome", associated with direct sonar exposure. Environmental pollutant tissue concentrations were relatively high, in particular organochlorinated xenobiotics. Gastric content and morphologic tissue examinations showed a prolonged starvation, which likely caused, at its turn, the mobilization of lipophilic contaminants from the adipose tissue. Chemical compounds subsequently entered the blood circulation and may have impaired immune and nervous functions. CONCLUSIONS/SIGNIFICANCE: A multi-factorial cause underlying this sperm whales' mass stranding is proposed herein based upon the results of postmortem investigations as well as of the detailed analyses of the geographical and historical background. The seven sperm whales took the same "wrong way" into the Adriatic Sea, a potentially dangerous trap for Mediterranean sperm whales. Seismic surveys should be also regarded as potential co-factors, even if no evidence of direct impact has been detected

    Variation in Structure and Process of Care in Traumatic Brain Injury: Provider Profiles of European Neurotrauma Centers Participating in the CENTER-TBI Study.

    INTRODUCTION: The strength of evidence underpinning care and treatment recommendations in traumatic brain injury (TBI) is low. Comparative effectiveness research (CER) has been proposed as a framework to provide evidence for optimal care for TBI patients. The first step in CER is to map the existing variation. The aim of current study is to quantify variation in general structural and process characteristics among centers participating in the Collaborative European NeuroTrauma Effectiveness Research in Traumatic Brain Injury (CENTER-TBI) study. METHODS: We designed a set of 11 provider profiling questionnaires with 321 questions about various aspects of TBI care, chosen based on literature and expert opinion. After pilot testing, questionnaires were disseminated to 71 centers from 20 countries participating in the CENTER-TBI study. Reliability of questionnaires was estimated by calculating a concordance rate among 5% duplicate questions. RESULTS: All 71 centers completed the questionnaires. Median concordance rate among duplicate questions was 0.85. The majority of centers were academic hospitals (n = 65, 92%), designated as a level I trauma center (n = 48, 68%) and situated in an urban location (n = 70, 99%). The availability of facilities for neuro-trauma care varied across centers; e.g. 40 (57%) had a dedicated neuro-intensive care unit (ICU), 36 (51%) had an in-hospital rehabilitation unit and the organization of the ICU was closed in 64% (n = 45) of the centers. In addition, we found wide variation in processes of care, such as the ICU admission policy and intracranial pressure monitoring policy among centers. CONCLUSION: Even among high-volume, specialized neurotrauma centers there is substantial variation in structures and processes of TBI care. This variation provides an opportunity to study effectiveness of specific aspects of TBI care and to identify best practices with CER approaches

    Variation in neurosurgical management of traumatic brain injury

    Background: Neurosurgical management of traumatic brain injury (TBI) is challenging, with only low-quality evidence. We aimed to explore differences in neurosurgical strategies for TBI across Europe. Methods: A survey was sent to 68 centers participating in the Collaborative European Neurotrauma Effectiveness Research in Traumatic Brain Injury (CENTER-TBI) study. The questionnaire contained 21 questions, including the decision when to operate (or not) on traumatic acute subdural hematoma (ASDH) and intracerebral hematoma (ICH), and when to perform a decompressive craniectomy (DC) in raised intracranial pressure (ICP). Results: The survey was completed by 68 centers (100%). On average, 10 neurosurgeons work in each trauma center. In all centers, a neurosurgeon was available within 30 min. Forty percent of responders reported a thickness or volume threshold for evacuation of an ASDH. Most responders (78%) decide on a primary DC in evacuating an ASDH during the operation, when swelling is present. For ICH, 3% would perform an evacuation directly to prevent secondary deterioration and 66% only in case of clinical deterioration. Most respondents (91%) reported to consider a DC for refractory high ICP. The reported cut-off ICP for DC in refractory high ICP, however, differed: 60% uses 25 mmHg, 18% 30 mmHg, and 17% 20 mmHg. Treatment strategies varied substantially between regions, specifically for the threshold for ASDH surgery and DC for refractory raised ICP. Also within center variation was present: 31% reported variation within the hospital for inserting an ICP monitor and 43% for evacuating mass lesions. Conclusion: Despite a homogeneous organization, considerable practice variation exists of neurosurgical strategies for TBI in Europe. These results provide an incentive for comparative effectiveness research to determine elements of effective neurosurgical care

    Variation in general supportive and preventive intensive care management of traumatic brain injury: a survey in 66 neurotrauma centers participating in the Collaborative European NeuroTrauma Effectiveness Research in Traumatic Brain Injury (CENTER-TBI) study

    Abstract Background General supportive and preventive measures in the intensive care management of traumatic brain injury (TBI) aim to prevent or limit secondary brain injury and optimize recovery. The aim of this survey was to assess and quantify variation in perceptions on intensive care unit (ICU) management of patients with TBI in European neurotrauma centers. Methods We performed a survey as part of the Collaborative European NeuroTrauma Effectiveness Research in Traumatic Brain Injury (CENTER-TBI) study. We analyzed 23 questions focused on: 1) circulatory and respiratory management; 2) fever control; 3) use of corticosteroids; 4) nutrition and glucose management; and 5) seizure prophylaxis and treatment. Results The survey was completed predominantly by intensivists (n = 33, 50%) and neurosurgeons (n = 23, 35%) from 66 centers (97% response rate). The most common cerebral perfusion pressure (CPP) target was > 60 mmHg (n = 39, 60%) and/or an individualized target (n = 25, 38%). To support CPP, crystalloid fluid loading (n = 60, 91%) was generally preferred over albumin (n = 15, 23%), and vasopressors (n = 63, 96%) over inotropes (n = 29, 44%). The most commonly reported target of partial pressure of carbon dioxide in arterial blood (PaCO2) was 36–40 mmHg (4.8–5.3 kPa) in case of controlled intracranial pressure (ICP) < 20 mmHg (n = 45, 69%) and PaCO2 target of 30–35 mmHg (4–4.7 kPa) in case of raised ICP (n = 40, 62%). Almost all respondents indicated to generally treat fever (n = 65, 98%) with paracetamol (n = 61, 92%) and/or external cooling (n = 49, 74%). Conventional glucose management (n = 43, 66%) was preferred over tight glycemic control (n = 18, 28%). More than half of the respondents indicated to aim for full caloric replacement within 7 days (n = 43, 66%) using enteral nutrition (n = 60, 92%). Indications for and duration of seizure prophylaxis varied, and levetiracetam was mostly reported as the agent of choice for both seizure prophylaxis (n = 32, 49%) and treatment (n = 40, 61%). Conclusions Practice preferences vary substantially regarding general supportive and preventive measures in TBI patients at ICUs of European neurotrauma centers. These results provide an opportunity for future comparative effectiveness research, since a more evidence-based uniformity in good practices in general ICU management could have a major impact on TBI outcome

    Monitoring the impact of litter in large vertebrates in the Mediterranean Sea within the European Marine Strategy Framework Directive (MSFD): Constraints, specificities and recommendations

    In its decision (20101477/EU) relating to the European Marine Strategy Framework Directive (MSFD, 2008/56/EC), the European Commission identified the following points as focuses for monitoring: (i) 10.1.1: Trends in the amount, source and composition of litter washed ashore and/or deposited on coastlines, (ii) 10.1.2: Trends in the amount and composition of litter in the water column and accumulation on the sea floor, (iii) 10.1.3: Trends in the amount, distribution and composition of micro-particles (mainly microplastics), and (iv) 10.2.1: Trends in the amount and composition of litter ingested by marine animals. Monitoring the impacts of litter will be considered further in 2014. At that time, the strategy will be discussed in the context of the Mediterranean Sea, providing information on constraints, protocols, existing harm and research needed to support monitoring efforts. The definition of targets and acceptable levels of harm must take all factors into account, whether entanglement ingestion, the transport and release of pollutants, the transport of alien species and socioeconomic impacts. It must also reflect on the practical deployment of "ingestion" measures (10.2.1). The analysis of existing data will reveal the potential and suitability of some higher trophic level organisms (fish, turtles, birds and mammals) for monitoring the adverse effects of litter. Sea turtles appear to be useful indicator species, but the definition of an ecological quality objective is still needed, as well as research on alternative potential indicator species

    Floating plastic debris tends to fragment into smaller pieces, termed microplastics, which may increase the likelihood of ingestion of plastics by marine organisms entering the food web. This study analyses the amount and spatial distribution of microplastics and zooplankton in an area near Asinara National Park (NW Sardinia) and overlapping the Pelagos Sanctuary (Mediterranean Sea). Analysis showed microplastics in 81% of the 27 samples analysed, with a mean value of 0.17 +/- 0.32 items m(-3). From geographic information system processing of the data, microplastics appeared more abundant (by a factor of four) in the pelagic than in the neritic environment, and showed a size range of the same order as major zooplanktonic taxa determined in the area. These findings suggest a potential risk of mesozooplankton and species preying on plankton mistaking microplastics for food. Further functional and toxicological studies are therefore necessary to assess the hazard associated with microplastics in the marine food web

    To ensure ecologically sustainable development of oil and gas operations, several research projects were promoted to develop and validate an innovative monitoring methodology based on biomarkers, in bioindicator organisms and cell cultures, to assess the environmental quality of on shore and off shore production areas. The biomarker approach, combined with contaminant analysis (hydrocarbons and trace elements), is a suitable strategy for studying effects of pollutants and providing early indications of adverse effects in organisms. The aim of environmental monitoring by means of biomarkers is in fact to identify (diagnosis) and to predict (prognosis) early signs of exposure or effect, so as to avoid undesirable ecological consequences. This monitoring approach has been conducted in different on shore and off-shore sensitive areas: Basilicata Region and Natural Ticino Park (Italy), Mediterranean Sea. Birds, fish, reptiles and invertebrates species were sampled in the studied areas and analyzed for biomarkers and contaminants levels. Laboratory studied were also conducted in a fish model (mosquito fish) and in top predator cell cultures, to evaluate the toxicological potential of oil derivatives (e.g. produced waters) and indentify the most effective depuration procedures. Results led to the identification of the most sensitive biomarkers to typical oil industry contaminants and to the selection of the bioindicators that provided the best responses to the ecotoxicological tests. This methodological approach, that represent an innovative environmental solution, revealed that no significant ecotoxicological effects can be ascribed to monitored on-shore and off-shore activities
