    Estilo de vida saudável e busca de opinião no consumo de suplementos alimentares

    Purpose – This research aimed to verify the influence of healthy lifestyle and the search for opinion in relation to the intention to consume food supplements.Design/methodology/approach – A descriptive research was carried out using a quantitative approach. The field research was developed through an online survey and the data were analyzed using multiple linear regression.Findings – It was found that the higher the level of healthy lifestyle, the greater the intention to resort to food supplementation. Furthermore, the search for opinion, in turn, positively influenced the intention to consume food supplements. However, this search for opinion is intrinsically related to the search for information from sources not indicated to direct the consumption of dietary supplements, which represents a risk to health.Originality/value – The main contribution of this research was that the experience of a healthy lifestyle does not represent the absence of consumption of dietary supplements. The results reveal an alarming situation regarding the health risks to which consumers of dietary supplements are exposed. These risks are also related to people with a high level of healthy lifestyle. Thus, this picture represents a public health problem and refers to the urgent need for information and awareness-raising actions for the population in order to control the incidence of diseases associated with the consumption of these products.Objetivo - Esta pesquisa teve como objetivo verificar a influência do estilo de vida saudável e a busca de opinião em relação à intenção de consumir suplementos alimentares.Desenho / metodologia / abordagem - Foi realizada uma pesquisa descritiva com abordagem quantitativa. A pesquisa de campo foi desenvolvida por meio de um survey online e os dados foram analisados por meio de regressão linear múltipla.Resultados - Constatou-se que quanto maior o nível de estilo de vida saudável, maior a intenção de recorrer à suplementação alimentar. Além disso, a busca de opinião, por sua vez, influenciou positivamente a intenção de consumir suplementos alimentares. No entanto, essa busca de opinião está intrinsecamente relacionada à busca de informações em fontes não indicadas para direcionar o consumo de suplementos alimentares, o que representa risco à saúde.Originalidade / valor - A principal contribuição desta pesquisa foi que a vivência de um estilo de vida saudável não representa a ausência de consumo de suplementos alimentares. Os resultados revelam uma situação alarmante em relação aos riscos para a saúde a que estão expostos os consumidores de suplementos alimentares. Esses riscos também estão relacionados a pessoas com alto nível de estilo de vida saudável. Assim, esse quadro representa um problema de saúde pública e remete à necessidade urgente de informações e ações de conscientização da população a fim de controlar a incidência de doenças associadas ao consumo desses produtos

    Rationale, study design, and analysis plan of the Alveolar Recruitment for ARDS Trial (ART): Study protocol for a randomized controlled trial

    Background: Acute respiratory distress syndrome (ARDS) is associated with high in-hospital mortality. Alveolar recruitment followed by ventilation at optimal titrated PEEP may reduce ventilator-induced lung injury and improve oxygenation in patients with ARDS, but the effects on mortality and other clinical outcomes remain unknown. This article reports the rationale, study design, and analysis plan of the Alveolar Recruitment for ARDS Trial (ART). Methods/Design: ART is a pragmatic, multicenter, randomized (concealed), controlled trial, which aims to determine if maximum stepwise alveolar recruitment associated with PEEP titration is able to increase 28-day survival in patients with ARDS compared to conventional treatment (ARDSNet strategy). We will enroll adult patients with ARDS of less than 72 h duration. The intervention group will receive an alveolar recruitment maneuver, with stepwise increases of PEEP achieving 45 cmH(2)O and peak pressure of 60 cmH2O, followed by ventilation with optimal PEEP titrated according to the static compliance of the respiratory system. In the control group, mechanical ventilation will follow a conventional protocol (ARDSNet). In both groups, we will use controlled volume mode with low tidal volumes (4 to 6 mL/kg of predicted body weight) and targeting plateau pressure <= 30 cmH2O. The primary outcome is 28-day survival, and the secondary outcomes are: length of ICU stay; length of hospital stay; pneumothorax requiring chest tube during first 7 days; barotrauma during first 7 days; mechanical ventilation-free days from days 1 to 28; ICU, in-hospital, and 6-month survival. ART is an event-guided trial planned to last until 520 events (deaths within 28 days) are observed. These events allow detection of a hazard ratio of 0.75, with 90% power and two-tailed type I error of 5%. All analysis will follow the intention-to-treat principle. Discussion: If the ART strategy with maximum recruitment and PEEP titration improves 28-day survival, this will represent a notable advance to the care of ARDS patients.     Objective: Many studies show factors that have a positive impact on the intention to buy food, including the presence of stamps and certified and the endorsement of celebrities, both in isolation and combined with other factors. When it comes to studies that analyze the effects that both factors can have on the purchasing and consumer confidence, the number of studies in the national and international literature becomes incipient. This research aimed to identify the effects of the product origin region (POR) and the Regional Celebrity Appeal (RCP) on the confidence and intention to purchase food. Design/methodology/approach: A descriptive and inferential quantitative approach was used, using experimental research as a research strategy. Findings: From the theoretical point of view, even pointing out that the presence of stamps and certificates and endorsement of celebrity exert direct influence on trust and purchase intention, the research shows that there is no interaction between these constructs when it comes to commodities, indicating that other elections should be considered. Originality/value: The results found show that even if the presence of identification of the region of origin of the product or the endorsement of celebrity congruent with the region generate an identification in the consumer towards the product, this junction is not sufficient when analyzing the intention to purchase food products in the category of commodities.     Consenso brasileiro sobre distrofia muscular de Duchenne, Parte 1 : diagnóstico, corticoterapia e perspectivas

    Significant advances in the understanding and management of Duchenne muscular dystrophy (DMD) took place since international guidelines were published in 2010. Our objective was to provide an evidence-based national consensus statement for multidisciplinary care of DMD in Brazil. A combination of the Delphi technique with a systematic review of studies from 2010 to 2016 was employed to classify evidence levels and grade of recommendations. Our recommendations were divided in two parts. We present Part 1 here, where we describe the guideline methodology and overall disease concepts, and also provide recommendations on diagnosis, steroid therapy and new drug treatment perspectives for DMD. The main recommendations: 1) genetic testing in diagnostic suspicious cases should be the first line for diagnostic confirmation; 2) patients diagnosed with DMD should have steroids prescribed; 3) lack of published results for phase 3 clinical trials hinders, for now, the recommendation to use exon skipping or read-through agents.Avanços na compreensão e no manejo da distrofia muscular de Duchenne (DMD) ocorreram desde a publicação de diretrizes internacionais em 2010. Nosso objetivo foi elaborar um consenso nacional baseado em evidências de cuidado multidisciplinar dos pacientes com DMD no Brasil. Utilizamos a técnica de Delphi combinada com revisão sistemática da literatura de 2010 a 2016 classificando níveis de evidência e graus de recomendação. Nossas recomendações foram divididas em duas partes. Apresentamos aqui a parte 1, descrevendo a metodologia utilizada e conceitos gerais da doença, e fornecemos recomendações sobre diagnóstico, tratamento com corticosteroides e novas perspectivas de tratamentos medicamentosos.     Update of the Brazilian consensus recommendations on Duchenne muscular dystrophy

    In the last few decades, there have been considerable improvements in the diagnosis and care of Duchenne muscular dystrophy (DMD), the most common childhood muscular dystrophy. International guidelines have been published and recently reviewed. A group of Brazilian experts has developed a standard of care based on a literature review with evidence-based graded recommendations in a two-part publication. Implementing best practice management has helped change the natural history of this chronic progressive disorder, in which the life expectancy for children of the male sex in the past used to be very limited. Since the previous publication, diagnosis, steroid treatment, rehabilitation, and systemic care have gained more significant insights with new original work in certain fields. Furthermore, the development of new drugs is ongoing, and some interventions have been approved for use in certain countries. Therefore, we have identified the need to review the previous care recommendations for Brazilian patients with DMD. Our objective was to create an evidence-based document that is an update on our previous consensus on those topics
