BACKGROUND: Cardiovascular (CV) disease is the leading cause of morbidity and mortality in patients with type 2 diabetes mellitus (T2DM). Furthermore, patients with T2DM and acute coronary syndrome (ACS) have a particularly high risk of CV events. The glucagon-like peptide 1 receptor agonist, lixisenatide, improves glycemia, but its effects on CV events have not been thoroughly evaluated. METHODS: ELIXA (www.clinicaltrials.gov no. NCT01147250) is a randomized, double-blind, placebo-controlled, parallel-group, multicenter study of lixisenatide in patients with T2DM and a recent ACS event. The primary aim is to evaluate the effects of lixisenatide on CV morbidity and mortality in a population at high CV risk. The primary efficacy end point is a composite of time to CV death, nonfatal myocardial infarction, nonfatal stroke, or hospitalization for unstable angina. Data are systematically collected for safety outcomes, including hypoglycemia, pancreatitis, and malignancy. RESULTS: Enrollment began in July 2010 and ended in August 2013; 6,068 patients from 49 countries were randomized. Of these, 69% are men and 75% are white; at baseline, the mean ± SD age was 60.3 ± 9.7 years, body mass index was 30.2 ± 5.7 kg/m(2), and duration of T2DM was 9.3 ± 8.2 years. The qualifying ACS was a myocardial infarction in 83% and unstable angina in 17%. The study will continue until the positive adjudication of the protocol-specified number of primary CV events. CONCLUSION: ELIXA will be the first trial to report the safety and efficacy of a glucagon-like peptide 1 receptor agonist in people with T2DM and high CV event risk

International audienceBACKGROUND:Patients with peripheral artery disease have an increased risk of cardiovascular morbidity and mortality. Antiplatelet agents are widely used to reduce these complications.METHODS:This was a multicentre, double-blind, randomised placebo-controlled trial for which patients were recruited at 602 hospitals, clinics, or community practices from 33 countries across six continents. Eligible patients had a history of peripheral artery disease of the lower extremities (previous peripheral bypass surgery or angioplasty, limb or foot amputation, intermittent claudication with objective evidence of peripheral artery disease), of the carotid arteries (previous carotid artery revascularisation or asymptomatic carotid artery stenosis of at least 50%), or coronary artery disease with an ankle-brachial index of less than 0·90. After a 30-day run-in period, patients were randomly assigned (1:1:1) to receive oral rivaroxaban (2·5 mg twice a day) plus aspirin (100 mg once a day), rivaroxaban twice a day (5 mg with aspirin placebo once a day), or to aspirin once a day (100 mg and rivaroxaban placebo twice a day). Randomisation was computer generated. Each treatment group was double dummy, and the patient, investigators, and central study staff were masked to treatment allocation. The primary outcome was cardiovascular death, myocardial infarction or stroke; the primary peripheral artery disease outcome was major adverse limb events including major amputation. This trial is registered with ClinicalTrials.gov, number NCT01776424, and is closed to new participants.FINDINGS:Between March 12, 2013, and May 10, 2016, we enrolled 7470 patients with peripheral artery disease from 558 centres. The combination of rivaroxaban plus aspirin compared with aspirin alone reduced the composite endpoint of cardiovascular death, myocardial infarction, or stroke (126 [5%] of 2492 vs 174 [7%] of 2504; hazard ratio [HR] 0·72, 95% CI 0·57-0·90, p=0·0047), and major adverse limb events including major amputation (32 [1%] vs 60 [2%]; HR 0·54 95% CI 0·35-0·82, p=0·0037). Rivaroxaban 5 mg twice a day compared with aspirin alone did not significantly reduce the composite endpoint (149 [6%] of 2474 vs 174 [7%] of 2504; HR 0·86, 95% CI 0·69-1·08, p=0·19), but reduced major adverse limb events including major amputation (40 [2%] vs 60 [2%]; HR 0·67, 95% CI 0·45-1·00, p=0·05). The median duration of treatment was 21 months. The use of the rivaroxaban plus aspirin combination increased major bleeding compared with the aspirin alone group (77 [3%] of 2492 vs 48 [2%] of 2504; HR 1·61, 95% CI 1·12-2·31, p=0·0089), which was mainly gastrointestinal. Similarly, major bleeding occurred in 79 (3%) of 2474 patients with rivaroxaban 5 mg, and in 48 (2%) of 2504 in the aspirin alone group (HR 1·68, 95% CI 1·17-2·40; p=0·0043).INTERPRETATION:Low-dose rivaroxaban taken twice a day plus aspirin once a day reduced major adverse cardiovascular and limb events when compared with aspirin alone. Although major bleeding was increased, fatal or critical organ bleeding was not. This combination therapy represents an important advance in the management of patients with peripheral artery disease. Rivaroxaban alone did not significantly reduce major adverse cardiovascular events compared with asprin alone, but reduced major adverse limb events and increased major bleeding

The research purpose is geomechanical substantiation of such parameters of combined system of lining and support on the roadhead of roadway and longwall through which is possible and economically feasible to ensure the reuse of development workings under the mining and geological conditions of "Selidovugol" mines. As a result of the in–situ measurements revealed that the value of the remaining area can be a criteria that characterizes the feasibility for belt road reusing to adjacent longwall. Such value is the remaining area in the light that equal to 8.5 m2. On the basis of justified criteria the influence of various support systems on stress–deformed state of the geomechanical system "lining – support – array" was solved. It is established that a residual inside half–timbers of roadway can act the criteria that characterize suitable for reuse in development workings adjacent to the longwall. Economic rationality criteria for reuse roadway is a residual inside half–timbers, equal to 8.5 m2. It was found that by using a support system in the form of sawed stick crib, with the two – tier system of frame – bolt lining the criteria for development workings reuse was performed over the longwall. The influence of the angle of the bed at the geomechanical system parameters was adopted. The developments were successfully implemented and approbated. Economic effect is 2422 UAH on 1m

    International audienceThe ratios of the branching fractions of the decays Λc+_{c}^{+}  → pπ^{−}π+^{+}, Λc+_{c}^{+}  → pK^{−}K+^{+}, and Λc+_{c}^{+}  → pπ^{−}K+^{+} with respect to the Cabibbo-favoured Λc+_{c}^{+}  → pK^{−}π+^{+} decay are measured using proton-proton collision data collected with the LHCb experiment at a 7 TeV centre-of-mass energy and corresponding to an integrated luminosity of 1.0 fb1^{−1}: \begin{array}{l}\frac{\mathrm{\mathcal{B}}\left({\varLambda}_c^{+}\to p{\pi}^{-}{\pi}^{+}\right)}{\mathrm{\mathcal{B}}\left({\varLambda}_c^{+}\to p{K}^{-}{\pi}^{+}\right)}=\left(7.44\pm 0.08\pm 0.18\right)\%,\hfill \\ {}\frac{\mathrm{\mathcal{B}}\left({\varLambda}_c^{+}\to p{K}^{-}{K}^{+}\right)}{\mathrm{\mathcal{B}}\left({\varLambda}_c^{+}\to p{K}^{-}{\pi}^{+}\right)}=\left(1.70\pm 0.03\pm 0.03\right)\%\hfill \\ {}\frac{\mathrm{\mathcal{B}}\left({\varLambda}_c^{+}\to p{\pi}^{-}{K}^{+}\right)}{\mathrm{\mathcal{B}}\left({\varLambda}_c^{+}\to p{K}^{-}{\pi}^{+}\right)}=\left(0.165\pm 0.015\pm 0.005\right)\%,\hfill \end{array} where the uncertainties are statistical and systematic, respectively. These results are the most precise measurements of these quantities to date. When multiplied by the world-average value for B(Λc+pKπ+) \mathrm{\mathcal{B}}\left({\varLambda}_c^{+}\to p{K}^{-}{\pi}^{+}\right) , the corresponding branching fractions are \begin{array}{l}\mathrm{\mathcal{B}}\left({\varLambda}_c^{+}\to p{\pi}^{-}{\pi}^{+}\right)=\left(4.72\pm 0.05\pm 0.11\pm 0.25\right)\times {10}^{-3},\hfill \\ {}\mathrm{\mathcal{B}}\left({\varLambda}_c^{+}\to p{K}^{-}{K}^{+}\right)=\left(1.08\pm 0.02\pm 0.02\pm 0.06\right)\times {10}^{-3},\hfill \\ {}\mathrm{\mathcal{B}}\left({\varLambda}_c^{+}\to p{\pi}^{-}{K}^{+}\right)=\left(1.04\pm 0.09\pm 0.03\pm 0.05\right)\times {10}^{-4},\hfill \end{array} where the final uncertainty is due to B(Λc+pKπ+) \mathrm{\mathcal{B}}\left({\varLambda}_c^{+}\to p{K}^{-}{\pi}^{+}\right)

Search for Baryon-Number Violating Ξb0{\mathrm{\Xi}}_{b}^{0} Oscillations

    No full text
International audienceA search for baryon-number violating Ξb0 oscillations is performed with a sample of pp collision data recorded by the LHCb experiment, corresponding to an integrated luminosity of 3  fb-1. The baryon number at the moment of production is identified by requiring that the Ξb0 come from the decay of a resonance Ξb*-→Ξb0π- or Ξb′-→Ξb0π-, and the baryon number at the moment of decay is identified from the final state using the decays Ξb0→Ξc+π-,Ξc+→pK-π+. No evidence of baryon-number violation is found, and an upper limit at the 95% confidence level is set on the oscillation rate of ω<0.08  ps-1, where ω is the associated angular frequency

First Observation of the Rare Purely Baryonic Decay B0ppˉB^0\to p\bar p

    No full text
International audienceThe first observation of the decay of a B0 meson to a purely baryonic final state, B0→pp¯, is reported. The proton-proton collision data sample used was collected with the LHCb experiment at center-of-mass energies of 7 and 8 TeV and corresponds to an integrated luminosity of 3.0  fb-1. The branching fraction is determined to be B(B0→pp¯)=(1.25±0.27±0.18)×10-8, where the first uncertainty is statistical and the second systematic. The decay mode B0→pp¯ is the rarest decay of the B0 meson observed to date. The decay Bs0→pp¯ is also investigated. No signal is seen and the upper limit B(Bs0→pp¯)<1.5×10-8 at 90% confidence level is set on the branching fraction

The 6-m timed hop test is a prognostic factor for outcomes in patients with meniscal tears treated with exercise therapy or arthroscopic partial meniscectomy: a secondary, exploratory analysis of the Odense–Oslo meniscectomy versus exercise (OMEX) trial

    Get PDF
Purpose: To identify the prognostic factors for 2-year patient-reported outcomes in middle-aged patients with degenerative meniscal tears treated with exercise therapy (ET) or arthroscopic partial meniscectomy (APM). Methods: One hundred and seven patients, with mean age 49.6 (SD 6.2) years and BMI 25.7 (SD 3.7), were included in this analysis of data from the OMEX trial (http://www.clinicaltrials.gov NCT01002794). Linear and Poisson regression models were built to explore the associations between potential prognostic factors (patient characteristics, knee function-related and disease-related factors) and 2-year patient-reported outcomes: the Knee Injury and Osteoarthritis Outcome Score (KOOS) subscales Pain, Symptoms, ADL, Sport/Rec, QoL and 5-point Global Rating of Change scales for knee pain (GRC Pain) and function (GRC Function). Analyses were performed for the whole cohort and for the two treatment groups (n = 55 and 52) with adjustments for age, sex, BMI and baseline KOOS. Results: For the whole cohort, a 1-s better baseline 6-m timed hop test result was associated with 3.1–7.1 points better 2-year scores for all KOOS subscales (95% CIs 1.1–5.2 to 4.1–10.1 points). A 1.61–2.80 s better test was associated with scores equivalent to previously calculated clinical relevant differences for each KOOS subscale. For the groups of patients treated with ET and APM, respectively, 2.09–3.60 s and 0.63–1.99 s better tests were associated with clinical relevant differences. For the whole cohort, a 1-s better test was associated with 26% (95% CI 15–38%) and 22% (95% CI 11–34%) higher possibility for better or much better GRC Pain and Function scores. Patients treated with ET had 17% (95% CI 2–33%) increased possibility for better or much better GRC Pain score, and patients treated with APM had 65% (95% CI 32–108%) and 70% (95% CI 38–109%) increased possibility for better or much better GRC Pain and Function scores. Conclusions: The 6-m timed hop test result was a significant prognostic factor for 2-year patient-reported outcomes in middle-aged patients with degenerative meniscal tears, especially in those treated with APM
