    School development through democratic involvement and group-approach in migrant dominated areas: experiences from Raadmandsgades Skole og Heimdalsgades Overbygningsskole in Copenhagen City, Denmark

    Copenhagen City has since 2000 worked to fulfill policy aims and visions for integration in a democratic society. This integration provides equal opportunities in education, where teachers are able to empower pupils with ethnic minority background. The Copenhagen mayor for education and schools Per Bregengaard will focus on this issue. Two schools give examples of how they work. Raadmandsgades School is a primary school for 6-14 years students. They integrate the minority children in language groups. The other school Heimdalsgades Overbygningsskole is a secondary school with 14-17 years old pupils. It has innovated the curriculum and the students take part in the decisions of the school. Finally a consultant from the Development and Planning Unit in the Educational Department in Copenhagen City will explain the efforts to perform a special Copenhagen PISA survey and a massive in-service training for the teacher

    Target-Controlled Infusion of Cefepime in Critically Ill Patients:single center experience

    Attainment of appropriate pharmacokinetic-pharmacodynamic (PK-PD) targets for antimicrobial treatment is challenging in critically ill patients, particularly for cefepime, which exhibits a relative narrow therapeutic-toxic window compared to other beta-lactam antibiotics. Target-controlled infusion (TCI) systems, which deliver drugs to achieve specific target drug concentrations, have successfully been implemented for improved dosing of sedatives and analgesics in anesthesia. We conducted a clinical trial in an intensive care unit (ICU) to investigate the performance of TCI for adequate target attainment of cefepime. Twenty-one patients treated with cefepime according to the standard of care were included. Cefepime was administered through continuous infusion using TCI for a median duration of 4.5 days. TCI was based on a previously developed population PK model incorporating the estimated creatinine clearance based on the Cockcroft-Gault formula as the input variable to calculate cefepime clearance. A cefepime blood concentration of 16 mg/liter was targeted. To evaluate the measured versus predicted plasma concentrations, blood samples were taken (median of 10 samples per patient), and total cefepime concentrations were measured using ultraperformance liquid chromatography-tandem mass spectrometry. The performance of the TCI system was evaluated using Varvel criteria. Half (50.3%) of the measured cefepime concentrations were within +/- 30% around the target value of 16 mg liter(-1). The wobble was 11.4%, the median performance error (MdPE) was 21.1%, the median absolute performance error (MdAPE) was 32.0%, and the divergence was -3.72% h(-1). Based on these results, we conclude that TCI is useful for dose optimization of cefepime in ICU patients

    Responses of bottlenose dolphins and harbor porpoises to impact and vibration piling noise during harbor construction

    The development of risk assessments for the exposure of protected populations to noise from coastal construction is constrained by uncertainty over the nature and extent of marine mammal responses to man-made noise. Stakeholder concern often focuses on the potential for local displacement caused by impact piling, where piles are hammered into the seabed. To mitigate this threat, use of vibration piling, where piles are shaken into place with a vibratory hammer, is often encouraged due to presumed impact reduction. However, data on comparative responses of cetaceans to these different noise sources are lacking. We studied the responses of bottlenose dolphins and harbor porpoises to both impact and vibration pile driving noise during harbor construction works in northeast Scotland, using passive acoustic monitoring devices to record cetacean activity and noise recorders to measure and predict received noise levels. Local abundance and patterns of occurrence of bottlenose dolphins were also compared with a five-year baseline. The median peak-to-peak source level estimated for impact piling was 240 dB re 1 μPa (single-pulse sound exposure level [SEL] 198 dB re 1 μPa2 s), and the r.m.s. source level for vibration piling was 192 dB re 1 μPa. Predicted received broadband SEL values 812 m from the piling site were markedly lower due to high propagation loss: 133.4 dB re 1 μPa2 s (impact) and 128.9 dB re 1 μPa2 s (vibration). Bottlenose dolphins and harbor porpoises were not excluded from sites in the vicinity of impact piling or vibration piling; nevertheless, some small effects were detected. Bottlenose dolphins spent a reduced period of time in the vicinity of construction works during both impact and vibration piling. The probability of occurrence of both cetacean species was also slightly less during periods of vibration piling. This work provides developers and managers with the first evidence of the comparative effects of vibration and impact piling on small cetaceans, enabling more informed risk assessments, policy frameworks, and mitigation plans. In particular, our results emphasize the need for better understanding of noise levels and behavioral responses to vibration piling before recommending its use to mitigate impact piling

    In Vitro Characterization of a Nineteenth-Century Therapy for Smallpox

    In the nineteenth century, smallpox ravaged through the United States and Canada. At this time, a botanical preparation, derived from the carnivorous plant Sarracenia purpurea, was proclaimed as being a successful therapy for smallpox infections. The work described characterizes the antipoxvirus activity associated with this botanical extract against vaccinia virus, monkeypox virus and variola virus, the causative agent of smallpox. Our work demonstrates the in vitro characterization of Sarracenia purpurea as the first effective inhibitor of poxvirus replication at the level of early viral transcription. With the renewed threat of poxvirus-related infections, our results indicate Sarracenia purpurea may act as another defensive measure against Orthopoxvirus infections

    Ovarian cancer risk factors by tumor aggressiveness : An analysis from the Ovarian Cancer Cohort Consortium

    Ovarian cancer risk factors differ by histotype; however, within subtype, there is substantial variability in outcomes. We hypothesized that risk factor profiles may influence tumor aggressiveness, defined by time between diagnosis and death, independent of histology. Among 1.3 million women from 21 prospective cohorts, 4,584 invasive epithelial ovarian cancers were identified and classified as highly aggressive (death in = 35 vs. 20 to <25 kg/m(2), 1.93 [1.46-2.56] and current smoking (vs. never, 1.30 [1.07-1.57]) were associated with increased risk of highly aggressive disease. Results were similar within histotypes. Ovarian cancer risk factors may be directly associated with subtypes defined by tumor aggressiveness, rather than through differential effects on histology. Studies to assess biological pathways are warranted.Peer reviewe

    Plasma levels of leptin and mammographic density among postmenopausal women: a cross-sectional study

    INTRODUCTION: Obesity has been linked to increased risk of breast cancer in postmenopausal women. Increased peripheral production of estrogens has been regarded as the main cause for this association, but other features of increased body fat mass may also play a part. Leptin is a protein produced mainly by adipose tissue and may represent a growth factor in cancer. We examined the association between leptin plasma levels and mammographic density, a biomarker for breast cancer risk. METHODS: We included data from postmenopausal women aged 55 and older, who participated in a cross-sectional mammography study in Tromsø, Norway. Mammograms, plasma leptin measurements as well as information on anthropometric and hormonal/reproductive factors were available from 967 women. We assessed mammographic density using a previously validated computer-assisted method. Multiple linear regression analysis was applied to investigate the association between mammographic density and quartiles of plasma leptin concentration. Because we hypothesized that the effect of leptin on mammographic density could vary depending on the amount of nondense or fat tissue in the breast, we also performed analyses on plasma leptin levels and mammographic density within tertiles of mammographic nondense area. RESULTS: After adjusting for age, postmenopausal hormone use, number of full-term pregnancies and age of first birth, there was an inverse association between leptin and absolute mammographic density (P(trend )= 0.001). When we additionally adjusted for body mass index and mammographic nondense area, no statistically significant association between plasma leptin and mammographic density was found (P(trend )= 0.16). Stratified analyses suggested that the association between plasma leptin and mammographic density could differ with the amount of nondense area of the mammogram, with the strongest association between leptin and mammographic absolute density in the stratum with the medium breast fat content (P(trend )= 0.003, P for interaction = 0.05). CONCLUSION: We found no overall consistent association between the plasma concentration of leptin and absolute mammographic density. Although weak, there was some suggestion that the association between leptin and mammographic density could differ with the amount of fat tissue in the breast
