    Atendimentos por queimaduras e corrosões em crianças e adolescentes no período de 2015 a 2019 em território brasileiro

    Introdução: As queimaduras e corrosões são um sério problema de saúde pública que podem causar sequelas físicas, psicológicas e, em muitos casos, mortes. Objetivo: Analisar o número de atendimentos hospitalares por queimaduras e corrosões em indivíduos de 0 a 19 anos durante o período de 2015 a 2019 no Brasil. Metodologia: Trata-se de um estudo descritivo, com abordagem quantitativa. Os dados foram coletados por meio do DATASUS, visando identificar os atendimentos de indivíduos de 0 a 19 anos que sofreram queimaduras e corrosões no período de 2015 a 2019, por residência, nas cinco regiões do Brasil; conforme sexo, faixa etária e o caráter de urgência no atendimento. Resultados: Observou-se um total de 41.676 atendimentos de crianças e adolescentes entre os anos de 2015 a 2019, sendo 14.451 (34,70%) na região Nordeste, 11.104 (26,60%) na Sudeste, 8.548 (20,50%) na Sul, 4.262 (10,23%) na Centro-oeste e 3.311 (7,90%) na Norte. Em relação ao total de atendimentos, em se tratando da faixa etária: 20.117 foram de crianças de 1 a 4 anos e 7.754 de 5 a 9 anos. Além disso, 25.576 atendidos foram do sexo masculino frente a 16.100 do sexo feminino, prevalência em todas as regiões analisadas. Conclusão: Constatou-se que o nordeste brasileiro possui a maior incidência hospitalar de urgência por queimaduras e corrosões em crianças e adolescentes, sendo os indivíduos do sexo masculino e crianças de 1 a 4 anos os mais acometidos em todo país.     Análise dos impactos da pandemia de COVID-19 em pessoas com HIV/AIDS: estudo de caso em uma unidade de saúde em Alagoas

    INTRODUÇÃO: A pandemia de COVID-19 acarretou inúmeros desafios ao sistema de saúde, tanto pelo risco da própria doença, quanto pelos efeitos indiretos dessa patologia. Nesse período, foram necessárias medidas de distanciamento social para conter o avanço do vírus. Isso pode ter impactado de forma negativa na manutenção e no início do tratamento das pessoas com HIV/AIDS. OBJETIVO: Realizar uma análise comparativa entre o fornecimento de medicamentos antirretrovirais para PVHA, bem como analisar o número de usuários que iniciaram o tratamento e fizeram profilaxia pós-exposição ao HIV, antes e depois do período pandêmico. METODOLOGIA: Estudo de natureza exploratória e descritiva, com abordagem qualitativa e quantitativa do tipo documental. Os dados foram obtidos através da análise de prontuários disponibilizados pelo Serviço de Assistência Especializada em Maceió no período de março/2019 a março/2021 e do Boletim Epidemiológico da Secretaria de Estado de Saúde de Alagoas. RESULTADO: Em relação ao número de pacientes que retiraram o antirretroviral, constatou-se que no início da pandemia houve crescimento (55%) e no ápice uma diminuição (13%). Na medida em que foi ocorrendo uma desaceleração dos casos de COVID-19, percebeu-se um crescimento (de 3% a 10%), exceto nos meses de agosto e outubro de 2020. Ademais, 75% do período estudado houve uma diminuição do número de novos usuários que iniciaram o tratamento e atenuação no número de pacientes que fizeram a profilaxia pós-exposição. CONCLUSÕES: Evidencia-se, portanto, o impacto negativo da Pandemia de COVID-19 em das pessoas com HIV/AIDS, sobretudo, na redução da aderência ao tratamento, bem como na profilaxia pós-exposição ao HIV

    A ACURÁCIA DIAGNÓSTICA DAS PATOLOGIAS CIRÚRGICAS NOS EXAMES POR CONGELAÇÃO.

    Introduction: The frozen section is a quick way of histopathological evaluation of tissue fragments during the intraoperative period, being an important tool to help the surgeon in decision making. Objective: To assess the diagnostic accuracy of frozen section exams in different anatomical sites performed in the pathology service of a university hospital. Material and methods: Study of an exploratory nature, with a documentary-type quantitative approach. The sample comprises 803 medical reports of the freezing and definitive paraffin exams from January 1, 2015 to January 1, 2020, available at the Pathological Anatomy Laboratory of the Professor Alberto Antunes University Hospital (HUPAA), excluding 106 reports with data that has been unavailable for whatever reason. Results: It was observed that, of the 697 surgical specimens that make up this research, 540 (77.47%) were indicated for the diagnosis of the lesion and 157 (22.53%) for the assessment of margins. The most analyzed anatomical sites were the breast in 177 cases, lymph node in 120 and skin in 106 cases. Regarding the highest number of discordant results, globally, the breast comes first with 08 cases, followed by the ovary and annexes with 05 cases and, in relation to specific sites, the peritoneum presented 10.00% of its cases, while ovary and annexes 08.47%. The anatomopathological examinations by freezing presented a rate referring to the total accuracy of 91.10%, with the breast, specifically, 94.93%, intra-abdominal 93.75% and lymph node 93.33%. Conclusion: The observed accuracy rate is high and the numbers are in line with several similar studies, which reinforces the importance of the test and the quality of the procedures performed by the service in question.Introdução: O exame por congelação é uma forma rápida de avaliação histopatológica de fragmentos teciduais durante o intra-operatório, tratando-se de uma importante ferramenta no auxílio ao cirurgião à tomada de decisão. Objetivo: Aferir a acurácia diagnóstica dos exames por congelação em diferentes sítios anatômicos efetuados no serviço de patologia de um hospital universitário. Material e métodos: Estudo de natureza exploratória, com abordagem quantitativa do tipo documental. A amostra compreende 803 laudos médicos dos exames de congelação e os definitivos em parafina no período de primeiro de janeiro de 2015 a primeiro de janeiro de 2020, disponíveis no Laboratório de Anatomia Patológica do Hospital Universitário Professor Alberto Antunes (HUPAA), sendo excluídos 106 laudos com dados que estiveram indisponíveis por qualquer motivo. Resultados: Observou-se que, das 697 peças cirúrgicas que compõem esta pesquisa, 540 (77,47%) tiveram como indicação o diagnóstico da lesão e 157 (22,53%) a avaliação de margens. Os sítios anatômicos mais analisados foram a mama em 177 casos, linfonodo em 120 e pele em 106 casos. Em relação ao maior número de resultados discordantes, de modo global, a mama vem em primeiro lugar com 08 casos, seguida de ovário e anexos com 05 casos e, em relação aos sítios específicos, o peritônio apresentou 10,00% dos seus casos, enquanto que ovário e anexos 08,47%. Os exames anatomopatológicos por congelação apresentaram uma taxa referente a acurácia total de 91,10%, tendo a mama, de forma específica, 94,93%, intra-abdominal 93,75% e linfonodo 93,33%. Conclusão: É alta a taxa de acurácia observada e os números estão em consonância com diversos estudos similares, o que reforça a importância do exame e a qualidade dos procedimentos realizados pelo serviço em questão

    Worldwide trends in underweight and obesity from 1990 to 2022: a pooled analysis of 3663 population-representative studies with 222 million children, adolescents, and adults

    Background Underweight and obesity are associated with adverse health outcomes throughout the life course. We estimated the individual and combined prevalence of underweight or thinness and obesity, and their changes, from 1990 to 2022 for adults and school-aged children and adolescents in 200 countries and territories. Methods We used data from 3663 population-based studies with 222 million participants that measured height and weight in representative samples of the general population. We used a Bayesian hierarchical model to estimate trends in the prevalence of different BMI categories, separately for adults (age ≥20 years) and school-aged children and adolescents (age 5–19 years), from 1990 to 2022 for 200 countries and territories. For adults, we report the individual and combined prevalence of underweight (BMI <18·5 kg/m2) and obesity (BMI ≥30 kg/m2). For schoolaged children and adolescents, we report thinness (BMI <2 SD below the median of the WHO growth reference) and obesity (BMI >2 SD above the median). Findings From 1990 to 2022, the combined prevalence of underweight and obesity in adults decreased in 11 countries (6%) for women and 17 (9%) for men with a posterior probability of at least 0·80 that the observed changes were true decreases. The combined prevalence increased in 162 countries (81%) for women and 140 countries (70%) for men with a posterior probability of at least 0·80. In 2022, the combined prevalence of underweight and obesity was highest in island nations in the Caribbean and Polynesia and Micronesia, and countries in the Middle East and north Africa. Obesity prevalence was higher than underweight with posterior probability of at least 0·80 in 177 countries (89%) for women and 145 (73%) for men in 2022, whereas the converse was true in 16 countries (8%) for women, and 39 (20%) for men. From 1990 to 2022, the combined prevalence of thinness and obesity decreased among girls in five countries (3%) and among boys in 15 countries (8%) with a posterior probability of at least 0·80, and increased among girls in 140 countries (70%) and boys in 137 countries (69%) with a posterior probability of at least 0·80. The countries with highest combined prevalence of thinness and obesity in school-aged children and adolescents in 2022 were in Polynesia and Micronesia and the Caribbean for both sexes, and Chile and Qatar for boys. Combined prevalence was also high in some countries in south Asia, such as India and Pakistan, where thinness remained prevalent despite having declined. In 2022, obesity in school-aged children and adolescents was more prevalent than thinness with a posterior probability of at least 0·80 among girls in 133 countries (67%) and boys in 125 countries (63%), whereas the converse was true in 35 countries (18%) and 42 countries (21%), respectively. In almost all countries for both adults and school-aged children and adolescents, the increases in double burden were driven by increases in obesity, and decreases in double burden by declining underweight or thinness. Interpretation The combined burden of underweight and obesity has increased in most countries, driven by an increase in obesity, while underweight and thinness remain prevalent in south Asia and parts of Africa. A healthy nutrition transition that enhances access to nutritious foods is needed to address the remaining burden of underweight while curbing and reversing the increase in obesit

We aimed to measure the prevalence of hypertension and progress in its detection, treatment, and control from 1990 to 2019 for 200 countries and territories. METHODS: We used data from 1990 to 2019 on people aged 30-79 years from population-representative studies with measurement of blood pressure and data on blood pressure treatment. We defined hypertension as having systolic blood pressure 140 mm Hg or greater, diastolic blood pressure 90 mm Hg or greater, or taking medication for hypertension. We applied a Bayesian hierarchical model to estimate the prevalence of hypertension and the proportion of people with hypertension who had a previous diagnosis (detection), who were taking medication for hypertension (treatment), and whose hypertension was controlled to below 140/90 mm Hg (control). The model allowed for trends over time to be non-linear and to vary by age. FINDINGS: The number of people aged 30-79 years with hypertension doubled from 1990 to 2019, from 331 (95% credible interval 306-359) million women and 317 (292-344) million men in 1990 to 626 (584-668) million women and 652 (604-698) million men in 2019, despite stable global age-standardised prevalence. In 2019, age-standardised hypertension prevalence was lowest in Canada and Peru for both men and women; in Taiwan, South Korea, Japan, and some countries in western Europe including Switzerland, Spain, and the UK for women; and in several low-income and middle-income countries such as Eritrea, Bangladesh, Ethiopia, and Solomon Islands for men. Hypertension prevalence surpassed 50% for women in two countries and men in nine countries, in central and eastern Europe, central Asia, Oceania, and Latin America. Globally, 59% (55-62) of women and 49% (46-52) of men with hypertension reported a previous diagnosis of hypertension in 2019, and 47% (43-51) of women and 38% (35-41) of men were treated. Control rates among people with hypertension in 2019 were 23% (20-27) for women and 18% (16-21) for men.     Isolated Peptide from Spider Venom Modulates Dendritic Cells In Vitro: A Possible Application in Oncoimmunotherapy for Glioblastoma

    Dendritic cells (DCs) vaccine is a potential tool for oncoimmunotherapy. However, it is known that this therapeutic strategy has failed in solid tumors, making the development of immunoadjuvants highly relevant. Recently, we demonstrated that Phoneutria nigriventer spider venom (PnV) components are cytotoxic to glioblastoma (GB) and activate macrophages for an antitumor profile. However, the effects of these molecules on the adaptive immune response have not yet been evaluated. This work aimed to test PnV and its purified fractions in DCs in vitro. For this purpose, bone marrow precursors were collected from male C57BL6 mice, differentiated into DCs and treated with venom or PnV-isolated fractions (F1—molecules 10 kDa), with or without costimulation with human GB lysate. The results showed that mainly F1 was able to activate DCs, increasing the activation-dependent surface marker (CD86) and cytokine release (IL-1β, TNF-α), in addition to inducing a typical morphology of mature DCs. From the F1 purification, a molecule named LW9 was the most effective, and mass spectrometry showed it to be a peptide. The present findings suggest that this molecule could be an immunoadjuvant with possible application in DC vaccines for the treatment of GB
