    La presente tesis lleva por título “Implementación de las 5S para mejorar la productividad del área de producción de la empresa L&R Corporación Huaraz SAC, Carabayllo, 2022”. El principal objetivo fue determinar de qué manera la implementación de las 5S mejora la productividad del área de producción. La investigación fue del tipo aplicada, diseño preexperimental y enfoque cuantitativo. El periodo de análisis fue de 30 días antes y 30 días después de la implementación. Se emplearon las técnicas de observación y recolección de datos. El instrumento check list, para obtener información de la variable independiente y la ficha de registro de datos para la variable dependiente. En el análisis inicial se encontró que la eficiencia fue 66.78%, la eficacia igual a 68.67% y la productividad 46.80%. Al verificar las mejoras en base al check list se evaluó la variable dependiente donde se obtuvo que la eficiencia obtuvo un valor de 81%, en eficacia se obtuvo 73.33% y en productividad el 60.12%. En conclusión, luego de la implementación de las 5S se logró mejorar la productividad del área de producción de la empresa L&R Corporación Huaraz SAC, Carabayllo, 2022 en un 28.46%

    El objetivo de la presente investigación fue conocer el significado psicológico de "ciudadano" entre universitarios. Se trabajó con una muestra de 60 sujetos, distribuidos equitativamente por sexo. Las aplicaciones fueron grupales, utilizando la técnica de las redes semánticas naturales (Figueroa, González y Solís, 1981; Valdez, 1998). Los resultados obtenidos permiten observar de manera general que la tendencia de los grupos, al definir al ciudadano, fue con base en muchos elementos que giran en torno de él, pero que no necesariamente hablan de sus derechos y obligaciones. Por tanto, lo encontrado difiere de las propuestas idealistas de algunos autores, como el pensar que el ciudadano vive en sociedad buscando ayudarse los unos a los otros, lo cual, al parecer, sigue siendo una utopía

    Factors associated with the development of septic shock in patients with candidemia: a post hoc analysis from two prospective cohorts

    Background: Almost one third of the patients with candidemia develop septic shock. The understanding why some patients do and others do not develop septic shock is very limited. The objective of this study was to identify variables associated with septic shock development in a large population of patients with candidemia. Methods: A post hoc analysis was performed on two prospective, multicenter cohort of patients with candidemia from 12 hospitals in Spain and Italy. All episodes occurring from September 2016 to February 2018 were analyzed to assess variables associated with septic shock development defined according to The Third International Consensus Definition for Sepsis and Septic Shock (Sepsis-3). Results: Of 317 candidemic patients, 99 (31.2%) presented septic shock attributable to candidemia. Multivariate logistic regression analysis identifies the following factors associated with septic shock development: age > 50 years (OR 2.57, 95% CI 1.03–6.41, p = 0.04), abdominal source of the infection (OR 2.18, 95% CI 1.04–4.55, p = 0.04), and admission to a general ward at the time of candidemia onset (OR 0.21, 95% CI, 0.12–0.44, p = 0.001). Septic shock development was independently associated with a greater risk of 30-day mortality (OR 2.14, 95% CI 1.08–4.24, p = 0.02). Conclusions: Age and abdominal source of the infection are the most important factors significantly associated with the development of septic shock in patients with candidemia. Our findings suggest that host factors and source of the infection may be more important for development of septic shock than intrinsic virulence factors of organisms

    European warm-season temperature and hydroclimate since 850 CE

    The long-term relationship between temperature and rainfall variables (hydroclimate) remains uncertain due to the short length of instrumental measurements and inconsistent results from climate model simulations. This lack of understanding is critical with regard to projecting future drought and flood risks. Here we assess northern Hemisphere summertime co-variability patterns between temperature and rainfall, over Europe back to 850 CE using instrumental measurements, tree-ring reconstructions, and climate model simulations. We find the temperature–hydroclimate relationship, in both the instrumental and proxt data to be more positive at lower frequencies, but less so in model simulations. In comp[arison to instrumental climate data, climate model simulations reveal a more negative co-variability between temperature and hydroclimate, across all timescales both lower and higher frequency. The reconstructions exhibit more positive co-variability. Despite observed differences in the temperature–hydroclimate co-variability patterns in instrumental, reconstructed and model simulated data, all data types share similar phase-relationships between temperature and hydroclimate, all of which indicate the common influence of external forcing of the climate system. The co-variability between temperature and soil moisture in the model simulations is overestimated, implying a possible overestimation of temperature-driven future drought risks

    Combining MRI and clinical data to detect high relapse risk after the first episode of psychosis

    Detecting patients at high relapse risk after the first episode of psychosis (HRR-FEP) could help the clinician adjust the preventive treatment. To develop a tool to detect patients at HRR using their baseline clinical and structural MRI, we followed 227 patients with FEP for 18–24 months and applied MRIPredict. We previously optimized the MRI-based machine-learning parameters (combining unmodulated and modulated gray and white matter and using voxel-based ensemble) in two independent datasets. Patients estimated to be at HRR-FEP showed a substantially increased risk of relapse (hazard ratio = 4.58, P < 0.05). Accuracy was poorer when we only used clinical or MRI data. We thus show the potential of combining clinical and MRI data to detect which individuals are more likely to relapse, who may benefit from increased frequency of visits, and which are unlikely, who may be currently receiving unnecessary prophylactic treatments. We also provide an updated version of the MRIPredict software

    A Customized Pigmentation SNP Array Identifies a Novel SNP Associated with Melanoma Predisposition in the SLC45A2 Gene

    As the incidence of Malignant Melanoma (MM) reflects an interaction between skin colour and UV exposure, variations in genes implicated in pigmentation and tanning response to UV may be associated with susceptibility to MM. In this study, 363 SNPs in 65 gene regions belonging to the pigmentation pathway have been successfully genotyped using a SNP array. Five hundred and ninety MM cases and 507 controls were analyzed in a discovery phase I. Ten candidate SNPs based on a p-value threshold of 0.01 were identified. Two of them, rs35414 (SLC45A2) and rs2069398 (SILV/CKD2), were statistically significant after conservative Bonferroni correction. The best six SNPs were further tested in an independent Spanish series (624 MM cases and 789 controls). A novel SNP located on the SLC45A2 gene (rs35414) was found to be significantly associated with melanoma in both phase I and phase II (P<0.0001). None of the other five SNPs were replicated in this second phase of the study. However, three SNPs in TYR, SILV/CDK2 and ADAMTS20 genes (rs17793678, rs2069398 and rs1510521 respectively) had an overall p-value<0.05 when considering the whole DNA collection (1214 MM cases and 1296 controls). Both the SLC45A2 and the SILV/CDK2 variants behave as protective alleles, while the TYR and ADAMTS20 variants seem to function as risk alleles. Cumulative effects were detected when these four variants were considered together. Furthermore, individuals carrying two or more mutations in MC1R, a well-known low penetrance melanoma-predisposing gene, had a decreased MM risk if concurrently bearing the SLC45A2 protective variant.     Association between convalescent plasma treatment and mortality in COVID-19: a collaborative systematic review and meta-analysis of randomized clinical trials.

    Funder: laura and john arnold foundationBACKGROUND: Convalescent plasma has been widely used to treat COVID-19 and is under investigation in numerous randomized clinical trials, but results are publicly available only for a small number of trials. The objective of this study was to assess the benefits of convalescent plasma treatment compared to placebo or no treatment and all-cause mortality in patients with COVID-19, using data from all available randomized clinical trials, including unpublished and ongoing trials (Open Science Framework, https://doi.org/10.17605/OSF.IO/GEHFX ). METHODS: In this collaborative systematic review and meta-analysis, clinical trial registries (ClinicalTrials.gov, WHO International Clinical Trials Registry Platform), the Cochrane COVID-19 register, the LOVE database, and PubMed were searched until April 8, 2021. Investigators of trials registered by March 1, 2021, without published results were contacted via email. Eligible were ongoing, discontinued and completed randomized clinical trials that compared convalescent plasma with placebo or no treatment in COVID-19 patients, regardless of setting or treatment schedule. Aggregated mortality data were extracted from publications or provided by investigators of unpublished trials and combined using the Hartung-Knapp-Sidik-Jonkman random effects model. We investigated the contribution of unpublished trials to the overall evidence. RESULTS: A total of 16,477 patients were included in 33 trials (20 unpublished with 3190 patients, 13 published with 13,287 patients). 32 trials enrolled only hospitalized patients (including 3 with only intensive care unit patients). Risk of bias was low for 29/33 trials. Of 8495 patients who received convalescent plasma, 1997 died (23%), and of 7982 control patients, 1952 died (24%). The combined risk ratio for all-cause mortality was 0.97 (95% confidence interval: 0.92; 1.02) with between-study heterogeneity not beyond chance (I2 = 0%). The RECOVERY trial had 69.8% and the unpublished evidence 25.3% of the weight in the meta-analysis. CONCLUSIONS: Convalescent plasma treatment of patients with COVID-19 did not reduce all-cause mortality. These results provide strong evidence that convalescent plasma treatment for patients with COVID-19 should not be used outside of randomized trials. Evidence synthesis from collaborations among trial investigators can inform both evidence generation and evidence application in patient care
