    7-year follow-up after open reduction and internal screw fixation in Bennett fractures

    Background: Bennett fractures are unstable, and, with inadequate treatment, lead to osteoarthritis, weakness and loss of function of the first carpometacarpal joint. This study focuses on long-term functional and radiological outcomes after open reduction and internal fixation. Methods: Between June 1997 and December 2005, 24 patients with Bennett fractures were treated with open reduction and internal fixation with screws at our center. Radiological and functional assessments including range of motion of the thumb and pinch and grip strength were performed 4months post-procedure and at the long-term follow-up, on average 83months after surgery. Results: Reduction of the Bennett fracture was maintained as it was at the time of the procedure in 96% of the cases when fixation with two lag screws was performed. At the 4-month follow-up, mean pinch and grip strength reached 92±3 and 89±4% of the contralateral side, respectively. Long-term follow-up demonstrated no correlation between the accuracy of the fracture reduction and the development of post-traumatic arthritis. Conclusion: Good clinical results could be observed, if successful reduction of the fracture was achieved and maintained.     National study of adverse reactions after vaccination with bacille Calmette-Guerin

    Few large prospective studies of adverse reactions after bacille Calmette-Guérin (BCG) vaccination are available. In a prospective national study of such adverse reactions among 918 subjects (aged 1 day to 54 years) over a 14-month period, 45 vaccinees (5%) reported 53 adverse reactions (23 injection-site abscesses, 14 severe local reactions, 10 cases of lymphadenitis, and 6 other reactions). Only 1% of vaccinees required medical attention. Reactions, particularly lymphadenitis, were significantly less common in infants <6 months old (but not in subjects aged 6 months) vaccinated by trained (vs. untrained) providers (relative risk [RR], 0.24; 95% confidence interval [CI], 0.090.68). Injection-site abscesses (RR, 2.96; 95% CI, 1.117.90) and severe local reactions (RR, 4.93; 95% CI, 1.1121.90) were significantly more common in older vaccinees. Local reactions were more frequently reported by adult females than by adult males (RR, 7.18; 95% CI, 1.5932.45).     Launching a Peer Supplemental Instruction Program for an Introductory Biology Course

    For the first time at Georgia Gwinnett College, a supplemental instruction (PSI) program was designed to provide peer-led instruction on a) principles of biology, chemistry, mathematics and b) academic skills e.g. self-regulated learning, strategies in studying and test-taking. PSI for Principles of Biology (BIOL1107K) was carried out by PSI leaders who previously earned a grade of ‘A’ or ‘B’ in BIOL1107K, received training on tutoring practices and worked with faculty to develop active learning exercises/worksheets for PSI sessions. PSI was open only to students who earned a grade of ≤ 75% on the first exam across four BIOL1107K sections. Comparison of exam grades revealed that PSI student performance was not significantly different from control (students who earned a grade of ≤ 75% on the first exam but did not enroll in PSI). The challenges at an institution lacking a PSI culture and strategies to encourage student commitment will be discussed

    Interactions of Cathinone NPS with Human Transporters and Receptors in Transfected Cells

    Pharmacological assays carried out in transfected cells have been very useful for describing the mechanism of action of cathinone new psychoactive substances (NPS). These in vitro characterizations provide fast and reliable information on psychoactive substances soon after they emerge for recreational use. Well-investigated comparator compounds, such as methamphetamine, 3,4-methylenedioxymethamphetamine, cocaine, and lysergic acid diethylamide, should always be included in the characterization to enhance the translation of the in vitro data into clinically useful information. We classified cathinone NPS according to their pharmacology at monoamine transporters and receptors. Cathinone NPS are monoamine uptake inhibitors and most induce transporter-mediated monoamine efflux with weak to no activity at pre- or postsynaptic receptors. Cathinones with a nitrogen-containing pyrrolidine ring emerged as NPS that are extremely potent transporter inhibitors but not monoamine releasers. Cathinones exhibit clinically relevant differences in relative potencies at serotonin vs. dopamine transporters. Additionally, cathinone NPS have more dopaminergic vs. serotonergic properties compared with their non-β-keto amphetamine analogs, suggesting more stimulant and reinforcing properties. In conclusion, in vitro pharmacological assays in heterologous expression systems help to predict the psychoactive and toxicological effects of NPS

    Boundary Crossing by a Community of Practice: Tibetan Buddhist Monasteries Engage Science Education

    As a globalized world struggles with division and disinformation, engaging across difference has emerged as a major challenge to communication and collaborative action needed to address growing global challenges. As such, the initiative by Tibetan Buddhist leaders to incorporate western science in curricula for monastic education may serve as an important case study that illuminates the conditions and processes at work in genuine cultural outreach and exchange. That project, spearheaded in the Emory-Tibet Science Initiative (ETSI), involves reaching out across two quite different communities of practice, Tibetan Buddhism and science, and the willingness and ability of individuals to cross the boundaries between them. In the study reported here, we apply existing understandings of communities of practice and of learning mechanisms that mediate boundary crossing to probe for presence of conditions and processes that promote effective outreach among Tibetan Buddhist monastic students. We deploy analysis of qualitative survey, interview, and self-report data from monastic students shortly after ETSI began (2009) and after science education had been rolled out in the monasteries (2019) to, first, identify initial cultural conditions related to outreach and engagement with science, and, second, probe for post-rollout presence of boundary crossing learning mechanisms among monastic students which facilitate communication from one community of practice to another. We found a range of robust initial cultural conditions (e.g., perceived overlap in subjects and methods of inquiry), along with strong presence of mechanisms that facilitate boundary crossing (e.g., reflection, transformation) and operate through time. We observed cascading effects of these conditions and mechanisms on student engagement with science. Furthermore, interactions of these conditions and mechanisms allow monastic students to engage with science on their own Buddhist terms and to regard learning science as potentially beneficial rather than threatening to their personal or collective Buddhist goals

    Feasibility of using a risk assessment tool to predict hospital transfers or death for older people in Australian residential aged care. A retrospective cohort study.

    Residents of Aged Care Facilities (RACF) experience burdensome hospital transfers in the last year of life, which may lead to aggressive and potentially inappropriate hospital treatments. Anticipating these transfers by identifying risk factors could encourage end-of-life discussions that may change decisions to transfer. The aim was to examine the feasibility of identifying an end-of-life risk profile among RACF residents using a predictive tool to better anticipate predictors of hospital transfers, death or poor composite outcome of hospitalisation and/or death after initial assessment. A retrospective cohort study of 373 permanent residents aged 65+ years was conducted using objective clinical factors from records in nine RACFs in metropolitan Sydney, Australia. In total, 26.8% died and 34.3% experienced a composite outcome. Cox proportional hazard regression models confirmed the feasibility of estimating the level of risk for death or a poor composite outcome. Knowing this should provide opportunities to initiate advance care planning in RACFs, facilitating decision making near the end of life. We conclude that the current structure of electronic RACF databases could be enhanced to enable comprehensive assessment of the risk of hospital re-attendance without admission. Automation tools to facilitate the risk score calculation may encourage the adoption of prediction checklists and evaluation of their association with hospital transfers

    Emory-Tibet Science Initiative: Changes in Monastic Science Learning Motivation and Engagement During a Six-Year Curriculum

    Led by His Holiness the Dalai Lama, the initiative taken by the Tibetan Buddhist monastic community to connect with western science and scientists presents a unique opportunity to understand the motivations and engagement behaviors that contribute to monastic science learning. In this study, we draw on quantitative data from two distinct surveys that track motivations and engagement behaviors related to science education among monastic students. The first survey was administered at one monastic university in 2018, and the second follow-up survey was completed by students at two monastic universities in 2019. These surveys assessed the reception of science education related to motivations among monastics and their demonstration of engagement-with-science behaviors. We also tested for variation over time by surveying students in all years of the science curriculum. We identified that monastic students are motivated by their perception that studying science has an overall positive effect and benefits their Buddhist studies, rather than negatively affecting their personal or collective Buddhist goals. In accordance with this finding, monastics behave in ways that encourage fellow scholars to engage with science concepts. Survey responses were disaggregated by years of science study and indicated changes in motivation and engagement during the six-year science curriculum. These insights support the relevance of considering motivation and engagement in a novel educational setting and inform ongoing work to expand the inclusiveness of science education. Our findings provide direction for future avenues of enhancing exchange of knowledge and practice between Buddhism and science

    Willow short-rotation production systems in Canada and Northern United States: A review

    Willow short rotation coppice (SRC) systems are becoming an attractive practice because they are a sustainable system fulfilling multiple ecological objectives with significant environmental benefits. A sustainable supply of bioenergy feedstock can be produced by willow on marginal land using well-adapted or tolerant cultivars. Across Canada and northern U.S.A., there are millions of hectares of available degraded land that have the potential for willow SRC biomass production, with a C sequestration potential capable of offsetting appreciable amount of anthropogenic green-house gas emissions. A fundamental question concerning 1 sustainable SRC willow yields was whether long-term soil productivity is maintained within a multi-rotation SRC system, given the rapid growth rate and associated nutrient exports offsite when harvesting the willow biomass after repeated short rotations. Based on early results from the first willow SRC rotation, it was found willow systems are relatively low nutrient-demanding, with minimal nutrient output other than in harvested biomass. The overall aim of this manuscript is to summarize the literature and present findings and data from ongoing research trials across Canada and northern U.S.A. examining willow SRC system establishment and viability. The research areas of interest presented here are the crop production of willow SRC systems, above- and below-ground biomass dynamics and the C budget, comprehensive soil-willow system nutrient budget, and soil nutrient amendments (via fertilization) in willow SRC systems. Areas of existing research gaps were also identified for the Canadian context

    The purpose of this review is to compile the literature published about different aspects of microwave-assisted extraction (MAE) use and ultrasound-assisted extraction (UAE) applied on jujube worldwide and to compare the results on the antioxidant activity obtained for each extraction method. As a result of the increased consumers demand for natural products, as well as for those of agro-food, nutraceutical, cosmetic industries, and green extraction techniques are nowadays trending to be potential alternatives that can improve antioxidant yield and its quality from an economical and environmental point of view by reducing time, energy, and solvent consumption. Ultrasounds and microwaves are widely used methods in the extraction of active principles due to their cavitation and dipolar rotation effect, respectively. These two techniques provide efficiency of extraction while minimizing the time and preserving the quality of the food matrix, overcoming the disadvantages of conventional techniques characterized by their consumption of large quantities of solvents and providing a sparse quantity of extraction. Jujube, a shrub with a high antioxidant potential, which can be affected by various extraction conditions can be the target of UAE and MAE to increase the antioxidant extraction yield. Exploiting the beneficial properties such as the antioxidant activity can lead to an industrialization process, replacing therefor synthetic antioxidants with natural compounds. These can also help in the development of new nutraceuticals and can be used, for instance, in agro-food industries as preservatives
