A Qualitative Evaluation of Mental Health Clinic Staff Perceptions of Barriers and Facilitators to Treating Tobacco Use
Introduction: Veterans with mental health disorders smoke at high rates, but encounter low rates of tobacco treatment. We sought to understand barriers and facilitators to treating tobacco use in VA mental health clinics.
Methods: This qualitative study was part of a trial evaluating a telephone care coordination program for smokers using mental health services at six VA facilities. We conducted semi-structured interviews with 14 staff: 12 mental health clinic staff working at the parent study\u27s intervention sites (n = 6 psychiatrists, three psychologists, two social workers, one NP), as well as one psychiatrist and one psychologist on the VA\u27s national tobacco advisory committee. Interviews were transcribed and inductively coded to identify themes.
Results: Five barriers themes emerged: (1) competing priorities, (2) patient challenges/resistance, (3) complex staffing/challenging cross-discipline coordination, (4) mixed perceptions about whether tobacco is a mental health care responsibility, and (5) limited staff training/comfort in treating tobacco. Five facilitators themes emerged: (1) reminding mental health staff about tobacco, (2) staff belief in the importance of addressing tobacco, (3) designating a cessation medication prescriber, (4) linking tobacco to mental health outcomes and norms, and (5) limiting mental health staff burden.
Conclusions: VA mental health staff struggle with knowing that tobacco use is important, but they face competing priorities, encounter patient resistance, are conflicted on their role in addressing tobacco, and lack tobacco training. They suggested strategies at multiple levels that would help overcome those barriers that can be used to design interventions that improve tobacco treatment delivery for mental health patients.
Implications: This study builds upon the existing literature on the high rates of smoking, but low rates of treatment, in people with mental health diagnoses. This study is one of the few qualitative evaluations of mental health clinic staff perceptions of barriers and facilitators to treating tobacco. The study results provide a multi-level framework for developing strategies to improve the implementation of tobacco treatment programs in mental health clinics
The Impact of Parentsâ Categorization of Their Own Weight and Their Childâs Weight on Healthy Lifestyle Promoting Beliefs and Practices
Objective. To evaluate parentsâ beliefs and practices related to childhood obesity and determine if these are influenced by parentâs perception of their own weight or their childâs weight.
Methods. Parents of obese ( = 689) or normal weight ( = 1122) children 4â15 years in Georgia, USA, were randomly selected to complete a telephone survey. Frequency of child obesity-related perceptions, beliefs, and practices were assessed, stratified by parent-perceived self-weight and child weight status, and compared using Chisquared tests and multivariate logistic regression.
Results. Most parents, regardless of perceived child weight, agreed that child overweight/obesity can cause serious illness (95%) but only one-half believed it was a problem in Georgia. Many (42.4%) failed to recognize obesity in their own children. More parents who perceived their child as overweight versus normal weight reported concern about their childâs diet and activity and indicated readiness for lifestyle change. Parentsâ perception of their own weight had little additional impact.
Conclusions. While awareness of child overweight as a modifiable health risk is high, many parents fail to recognize it in their own families and communities, reducing the likelihood of positive lifestyle change. Additional efforts to help parents understand their role in facilitating behavior change and to assist them in identifying at-risk children are required
Telephone care coordination for smokers in VA mental health clinics: protocol for a hybrid type-2 effectiveness-implementation trial
BACKGROUND: This paper describes an innovative protocol for a type-II hybrid effectiveness-implementation trial that is evaluating a smoking cessation telephone care coordination program for Veterans Health Administration (VA) mental-health clinic patients. As a hybrid trial, the protocol combines implementation science and clinical trial methods and outcomes that can inform future cessation studies and the implementation of tobacco cessation programs into routine care. The primary objectives of the trial are (1) to evaluate the process of adapting, implementing, and sustaining a smoking cessation telephone care coordination program in VA mental health clinics, (2) to determine the effectiveness of the program in promoting long-term abstinence from smoking among mental health patients, and (3) to compare the effectiveness of telephone counseling delivered by VA staff with that delivered by state quitlines.
METHODS/DESIGN: The care coordination program is being implemented at six VA facilities. VA mental health providers refer patients to the program via an electronic medical record consult. Program staff call referred patients to offer enrollment. All patients who enroll receive a self-help booklet, mailed smoking cessation medications, and proactive multi-call telephone counseling. Participants are randomized to receive this counseling from VA staff or their state\u27s quitline. Four primary implementation strategies are being used to optimize program implementation and sustainability: blended facilitation, provider training, informatics support, and provider feedback. A three-phase formative evaluation is being conducted to identify barriers to, and facilitators for, program implementation and sustainability. A mixed-methods approach is being used to collect quantitative clinical effectiveness data (e.g., self-reported abstinence at six months) and both quantitative and qualitative implementation data (e.g., provider referral rates, coded interviews with providers). Summative data will be analyzed using the Reach Effectiveness Adoption Implementation Maintenance (RE-AIM) framework.
DISCUSSION: This paper describes the rationale and methods of a trial designed to simultaneously study the clinical effectiveness and implementation of a telephone smoking cessation program for smokers using VA mental health clinics. Such hybrid designs are an important methodological design that can shorten the time between the development of an intervention and its translation into routine clinical care
Monomerization of Far-Red Fluorescent Proteins
Anthozoa-class red fluorescent proteins (RFPs) are frequently used as biological markers, with far-red (λ_(em) ⌠600â700 nm) emitting variants sought for whole-animal imaging because biological tissues are more permeable to light in this range. A barrier to the use of naturally occurring RFP variants as molecular markers is that all are tetrameric, which is not ideal for cell biological applications. Efforts to engineer monomeric RFPs have typically produced dimmer and blue-shifted variants because the chromophore is sensitive to small structural perturbations. In fact, despite much effort, only four native RFPs have been successfully monomerized, leaving the majority of RFP biodiversity untapped in biomarker development. Here we report the generation of monomeric variants of HcRed and mCardinal, both far-red dimers, and describe a comprehensive methodology for the monomerization of red-shifted oligomeric RFPs. Among the resultant variants is mKelly1 (emission maximum, λ_(em) = 656 nm), which, along with the recently reported mGarnet2 [Matela G, et al. (2017) Chem Commun (Camb) 53:979â982], forms a class of bright, monomeric, far-red FPs
Probing Evolutionary Repeatability: Neutral and Double Changes and the Predictability of Evolutionary Adaptation
The question of how organisms adapt is among the most fundamental in evolutionary biology. Two recent studies investigated the evolution of Escherichia coli in response to challenge with the antibiotic cefotaxime. Studying five mutations in the beta-lactamase gene that together confer significant antibiotic resistance, the authors showed a complex fitness landscape that greatly constrained the identity and order of intermediates leading from the initial wildtype genotype to the final resistant genotype. Out of 18 billion possible orders of single mutations leading from non-resistant to fully-resistant form, they found that only 27 (1.5x10(-7)%) pathways were characterized by consistently increasing resistance, thus only a tiny fraction of possible paths are accessible by positive selection. I further explore these data in several ways.Allowing neutral changes (those that do not affect resistance) increases the number of accessible pathways considerably, from 27 to 629. Allowing multiple simultaneous mutations also greatly increases the number of accessible pathways. Allowing a single case of double mutation to occur along a pathway increases the number of pathways from 27 to 259, and allowing arbitrarily many pairs of simultaneous changes increases the number of possible pathways by more than 100 fold, to 4800. I introduce the metric 'repeatability,' the probability that two random trials will proceed via the exact same pathway. In general, I find that while the total number of accessible pathways is dramatically affected by allowing neutral or double mutations, the overall evolutionary repeatability is generally much less affected.These results probe the conceivable pathways available to evolution. Even when many of the assumptions of the analysis of Weinreich et al. (2006) are relaxed, I find that evolution to more highly cefotaxime resistant beta-lactamase proteins is still highly repeatable
Markovian Dynamics on Complex Reaction Networks
Complex networks, comprised of individual elements that interact with each
other through reaction channels, are ubiquitous across many scientific and
engineering disciplines. Examples include biochemical, pharmacokinetic,
epidemiological, ecological, social, neural, and multi-agent networks. A common
approach to modeling such networks is by a master equation that governs the
dynamic evolution of the joint probability mass function of the underling
population process and naturally leads to Markovian dynamics for such process.
Due however to the nonlinear nature of most reactions, the computation and
analysis of the resulting stochastic population dynamics is a difficult task.
This review article provides a coherent and comprehensive coverage of recently
developed approaches and methods to tackle this problem. After reviewing a
general framework for modeling Markovian reaction networks and giving specific
examples, the authors present numerical and computational techniques capable of
evaluating or approximating the solution of the master equation, discuss a
recently developed approach for studying the stationary behavior of Markovian
reaction networks using a potential energy landscape perspective, and provide
an introduction to the emerging theory of thermodynamic analysis of such
networks. Three representative problems of opinion formation, transcription

Scaling and Formulary cross sections for ion-atom impact ionization
The values of ion-atom ionization cross sections are frequently needed for
many applications that utilize the propagation of fast ions through matter.
When experimental data and theoretical calculations are not available,
approximate formulas are frequently used. This paper briefly summarizes the
most important theoretical results and approaches to cross section calculations
in order to place the discussion in historical perspective and offer a concise
introduction to the topic. Based on experimental data and theoretical
predictions, a new fit for ionization cross sections is proposed. The range of
validity and accuracy of several frequently used approximations (classical
trajectory, the Born approximation, and so forth) are discussed using, as
examples, the ionization cross sections of hydrogen and helium atoms by various

Measuring gravitational waves from binary black hole coalescences: I. Signal to noise for inspiral, merger, and ringdown
We estimate the expected signal-to-noise ratios (SNRs) from the three phases
(inspiral,merger,ringdown) of coalescing binary black holes (BBHs) for initial
and advanced ground-based interferometers (LIGO/VIRGO) and for space-based
interferometers (LISA). LIGO/VIRGO can do moderate SNR (a few tens), moderate
accuracy studies of BBH coalescences in the mass range of a few to about 2000
solar masses; LISA can do high SNR (of order 10^4) high accuracy studies in the
mass range of about 10^5 to 10^8 solar masses. BBHs might well be the first
sources detected by LIGO/VIRGO: they are visible to much larger distances (up
to 500 Mpc by initial interferometers) than coalescing neutron star binaries
(heretofore regarded as the "bread and butter" workhorse source for LIGO/VIRGO,
visible to about 30 Mpc by initial interferometers). Low-mass BBHs (up to 50
solar masses for initial LIGO interferometers; 100 for advanced; 10^6 for LISA)
are best searched for via their well-understood inspiral waves; higher mass
BBHs must be searched for via their poorly understood merger waves and/or their
well-understood ringdown waves. A matched filtering search for massive BBHs
based on ringdown waves should be capable of finding BBHs in the mass range of
about 100 to 700 solar masses out to 200 Mpc (initial LIGO interferometers),
and 200 to 3000 solar masses out to about z=1 (advanced interferometers). The
required number of templates is of order 6000 or less. Searches based on merger
waves could increase the number of detected massive BBHs by a factor of order
10 or more over those found from inspiral and ringdown waves, without detailed
knowledge of the waveform shapes, using a "noise monitoring" search algorithm.
A full set of merger templates from numerical relativity could further increase

Magnetoelectric ordering of BiFeO3 from the perspective of crystal chemistry
In this paper we examine the role of crystal chemistry factors in creating
conditions for formation of magnetoelectric ordering in BiFeO3. It is generally
accepted that the main reason of the ferroelectric distortion in BiFeO3 is
concerned with a stereochemical activity of the Bi lone pair. However, the lone
pair is stereochemically active in the paraelectric orthorhombic beta-phase as
well. We demonstrate that a crucial role in emerging of phase transitions of
the metal-insulator, paraelectric-ferroelectric and magnetic disorder-order
types belongs to the change of the degree of the lone pair stereochemical
activity - its consecutive increase with the temperature decrease. Using the
structural data, we calculated the sign and strength of magnetic couplings in
BiFeO3 in the range from 945 C down to 25 C and found the couplings, which
undergo the antiferromagnetic-ferromagnetic transition with the temperature
decrease and give rise to the antiferromagnetic ordering and its delay in
regard to temperature, as compared to the ferroelectric ordering. We discuss
the reasons of emerging of the spatially modulated spin structure and its

Recommendations for a core outcome set for measuring standing balance in adult populations: a consensus-based approach
Standing balance is imperative for mobility and avoiding falls. Use of an excessive number of standing balance measures has limited the synthesis of balance intervention data and hampered consistent clinical practice.To develop recommendations for a core outcome set (COS) of standing balance measures for research and practice among adults.A combination of scoping reviews, literature appraisal, anonymous voting and face-to-face meetings with fourteen invited experts from a range of disciplines with international recognition in balance measurement and falls prevention. Consensus was sought over three rounds using pre-established criteria.The scoping review identified 56 existing standing balance measures validated in adult populations with evidence of use in the past five years, and these were considered for inclusion in the COS.Fifteen measures were excluded after the first round of scoring and a further 36 after round two. Five measures were considered in round three. Two measures reached consensus for recommendation, and the expert panel recommended that at a minimum, either the Berg Balance Scale or Mini Balance Evaluation Systems Test be used when measuring standing balance in adult populations.Inclusion of two measures in the COS may increase the feasibility of potential uptake, but poses challenges for data synthesis. Adoption of the standing balance COS does not constitute a comprehensive balance assessment for any population, and users should include additional validated measures as appropriate.The absence of a gold standard for measuring standing balance has contributed to the proliferation of outcome measures. These recommendations represent an important first step towards greater standardization in the assessment and measurement of this critical skill and will inform clinical research and practice internationally
