The Effects of Cardiac Specialty Hospitals on the Cost and Quality of Medical Care
The recent rise of specialty hospitals -- typically for-profit firms that are at least partially owned by physicians -- has led to substantial debate about their effects on the cost and quality of care. Advocates of specialty hospitals claim they improve quality and lower cost; critics contend they concentrate on providing profitable procedures and attracting relatively healthy patients, leaving (predominantly nonprofit) general hospitals with a less-remunerative, sicker patient population. We find support for both sides of this debate. Markets experiencing entry by a cardiac specialty hospital have lower spending for cardiac care without significantly worse clinical outcomes. In markets with a specialty hospital, however, specialty hospitals tend to attract healthier patients and provide higher levels of intensive procedures than general hospitals.
The Core Collapse Supernova Rate from the SDSS-II Supernova Survey
We use the Sloan Digital Sky Survey II Supernova Survey (SDSS-II SNS) data to
measure the volumetric core collapse supernova (CCSN) rate in the redshift
range (0.03<z<0.09). Using a sample of 89 CCSN we find a volume-averaged rate
of (1.06 +/- 0.19) x 10**(-4)/(yr Mpc**3) at a mean redshift of 0.072 +/-
0.009. We measure the CCSN luminosity function from the data and consider the

Photometric Supernova Cosmology with BEAMS and SDSS-II
Supernova cosmology without spectroscopic confirmation is an exciting new
frontier which we address here with the Bayesian Estimation Applied to Multiple
Species (BEAMS) algorithm and the full three years of data from the Sloan
Digital Sky Survey II Supernova Survey (SDSS-II SN). BEAMS is a Bayesian
framework for using data from multiple species in statistical inference when
one has the probability that each data point belongs to a given species,
corresponding in this context to different types of supernovae with their
probabilities derived from their multi-band lightcurves. We run the BEAMS
algorithm on both Gaussian and more realistic SNANA simulations with of order
10^4 supernovae, testing the algorithm against various pitfalls one might
expect in the new and somewhat uncharted territory of photometric supernova
cosmology. We compare the performance of BEAMS to that of both mock
spectroscopic surveys and photometric samples which have been cut using typical
selection criteria. The latter typically are either biased due to contamination
or have significantly larger contours in the cosmological parameters due to
small data-sets. We then apply BEAMS to the 792 SDSS-II photometric supernovae
with host spectroscopic redshifts. In this case, BEAMS reduces the area of the
(\Omega_m,\Omega_\Lambda) contours by a factor of three relative to the case
where only spectroscopically confirmed data are used (297 supernovae). In the
case of flatness, the constraints obtained on the matter density applying BEAMS
to the photometric SDSS-II data are \Omega_m(BEAMS)=0.194\pm0.07. This
illustrates the potential power of BEAMS for future large photometric supernova

Signed zeros of Gaussian vector fields-density, correlation functions and curvature
We calculate correlation functions of the (signed) density of zeros of
Gaussian distributed vector fields. We are able to express correlation
functions of arbitrary order through the curvature tensor of a certain abstract
Riemann-Cartan or Riemannian manifold. As an application, we discuss one- and
two-point functions. The zeros of a two-dimensional Gaussian vector field model
the distribution of topological defects in the high-temperature phase of
two-dimensional systems with orientational degrees of freedom, such as

Type II-P Supernovae from the SDSS-II Supernova Survey and the Standardized Candle Method
We apply the Standardized Candle Method (SCM) for Type II Plateau supernovae
(SNe II-P), which relates the velocity of the ejecta of a SN to its luminosity
during the plateau, to 15 SNe II-P discovered over the three season run of the
Sloan Digital Sky Survey - II Supernova Survey. The redshifts of these SNe -
0.027 < z < 0.144 - cover a range hitherto sparsely sampled in the literature;
in particular, our SNe II-P sample contains nearly as many SNe in the Hubble
flow (z > 0.01) as all of the current literature on the SCM combined. We find
that the SDSS SNe have a very small intrinsic I-band dispersion (0.22 mag),
which can be attributed to selection effects. When the SCM is applied to the
combined SDSS-plus-literature set of SNe II-P, the dispersion increases to 0.29
mag, larger than the scatter for either set of SNe separately. We show that the
standardization cannot be further improved by eliminating SNe with positive
plateau decline rates, as proposed in Poznanski et al. (2009). We thoroughly
examine all potential systematic effects and conclude that for the SCM to be
useful for cosmology, the methods currently used to determine the Fe II
velocity at day 50 must be improved, and spectral templates able to encompass
the intrinsic variations of Type II-P SNe will be needed.Comment: Accepted for publication by ApJ; data used in this paper can be

Single or Double Degenerate Progenitors? Searching for Shock Emission in the SDSS-II Type Ia Supernovae
From the set of nearly 500 spectroscopically confirmed type~Ia supernovae and
around 10,000 unconfirmed candidates from SDSS-II, we select a subset of 108
confirmed SNe Ia with well-observed early-time light curves to search for
signatures from shock interaction of the supernova with a companion star. No
evidence for shock emission is seen; however, the cadence and photometric noise
could hide a weak shock signal. We simulate shocked light curves using SN Ia
templates and a simple, Gaussian shock model to emulate the noise properties of
the SDSS-II sample and estimate the detectability of the shock interaction
signal as a function of shock amplitude, shock width, and shock fraction. We
find no direct evidence for shock interaction in the rest-frame -band, but
place an upper limit on the shock amplitude at 9% of supernova peak flux ( mag). If the single degenerate channel dominates type~Ia progenitors,
this result constrains the companion stars to be less than about 6
on the main sequence, and strongly disfavors red giant companions.Comment: 28 pages, 3 figure
Severe malnutrition with and without HIV-1 infection in hospitalised children in Kampala, Uganda: differences in clinical features, haematological findings and CD4(+ )cell counts
BACKGROUND: The aim of this study was to describe the clinical features, haematological findings and CD4(+ )and CD8(+ )cell counts of severely malnourished children in relation to human immunodeficiency virus (HIV) infection. METHODS: The study was conducted in the paediatric wards of Mulago hospital, which is Uganda's national referral and teaching hospital. We studied 315 severely malnourished children (presence of oedema and/or weight-for-height: z-score < -3) and have presented our findings. At admission, the CD4(+ )and CD8(+ )cells were measured by the flow cytometry and HIV serology was confirmed by Enzyme linked Immunoassay for children >18 months of age, and RNA PCR was performed for those ≤18 months. Complete blood count, including differential counts, was determined using a Beckman Coulter counter. RESULTS: Among the 315 children, 119 (38%) were female; the median age of these children was 17 months (Interquartile range 12–24 months), and no difference was observed in the HIV status with regard to gender or age. The children showed a high prevalence of infections: pneumonia (68%), diarrhoea (38%), urinary tract infection (26%) and bacteraemia (18%), with no significant difference with regard to the HIV status (HIV-positive versus HIV-negative children). However, the HIV-positive children were more likely to have persistent diarrhoea than the HIV-uninfected severely malnourished children (odds ratio (OR) 2.0, 95% confidence interval (CI) 1.2–3.6). When compared with the HIV-negative children, the HIV-positive children showed a significantly lower median white blood cell count (10700 versus 8700) and lymphocyte count (4033 versus 2687). The CD4(+ )cell percentages were more likely to be lower in children with non-oedematous malnutrition than in those with oedematous malnutrition even after controlling for the HIV infection. The novel observation of this study is that the CD4(+ )percentages in both HIV-positive and HIV-negative children without oedema were lower that those in children with oedema. These observations appear to imply that the development of oedema requires a certain degree of immunocompetence, which is an interesting clue to the pathophysiology of oedema in severe malnutrition
Hubble Residuals of Nearby Type Ia Supernovae Are Correlated with Host Galaxy Masses
From Sloan Digital Sky Survey u'g'r'i'z' imaging, we estimate the stellar
masses of the host galaxies of 70 low redshift SN Ia (0.015 < z < 0.08) from
the hosts' absolute luminosities and mass-to-light ratios. These nearby SN were
discovered largely by searches targeting luminous galaxies, and we find that
their host galaxies are substantially more massive than the hosts of SN
discovered by the flux-limited Supernova Legacy Survey. Testing four separate
light curve fitters, we detect ~2.5{\sigma} correlations of Hubble residuals
with both host galaxy size and stellar mass, such that SN Ia occurring in
physically larger, more massive hosts are ~10% brighter after light curve
correction. The Hubble residual is the deviation of the inferred distance
modulus to the SN, calculated from its apparent luminosity and light curve
properties, away from the expected value at the SN redshift. Marginalizing over
linear trends in Hubble residuals with light curve parameters shows that the
correlations cannot be attributed to a light curve-dependent calibration error.
Combining 180 higher-redshift ESSENCE, SNLS, and HigherZ SN with 30 nearby SN
whose host masses are less than 10^10.8 solar masses in a cosmology fit yields
1+w=0.22 +0.152/-0.143, while a combination where the 30 nearby SN instead have
host masses greater than 10^10.8 solar masses yields 1+w=-0.03 +0.217/-0.108.
Progenitor metallicity, stellar population age, and dust extinction correlate
with galaxy mass and may be responsible for these systematic effects. Host

Tailored vs. Standardized Internet-Based Cognitive Behavior Therapy for Depression and Comorbid Symptoms: A Randomized Controlled Trial
Background and Aims:Major depression can be treated by means of cognitive behavior therapy, delivered via the Internet as guided self-help. Individually tailored guided self-help treatments have shown promising results in the treatment of anxiety disorders. This randomized controlled trial tested the efficacy of an Internet-based individually tailored guided selfhelp treatment which specifically targeted depression with comorbid symptoms. The treatment was compared both to standardized (non-tailored) Internet-based treatment and to an active control group in the form of a monitored online discussion group. Both guided self-help treatments were based on cognitive behavior therapy and lasted for 10 weeks. The discussion group consisted of weekly discussion themes related to depression and the treatment of depression. Methods:A total of 121 participants with diagnosed major depressive disorder and with a range of comorbid symptoms were randomized to three groups. The tailored treatment consisted of a prescribed set of modules targeting depression as well as comorbid problems. The standardized treatment was a previously tested guided self-help program for depression. Results:From pre-treatment to post-treatment, both treatment groups improved on measures of depression, anxiety and quality of life. The results were maintained at a 6-month follow-up. Subgroup analyses showed that the tailored treatment was more effective than the standardized treatment among participants with higher levels of depression at baseline and more comorbidity, both in terms of reduction of depressive symptoms and on recovery rates. In the subgroup with lower baseline scores of depression, few differences were seen between treatments and the discussion group. Conclusions:This study shows that tailored Internet-based treatment for depression is effective and that addressing comorbidity by tailoring may be one way of making guided self-help treatments more effective than standardized approaches in the treatment of more severe depression
Towards an Understanding of the Mid-Infrared Surface Brightness of Normal Galaxies
We report a mid-infrared color and surface brightness analysis of IC 10, NGC
1313, and NGC 6946, three of the nearby galaxies studied under the Infrared
Space Observatory Key Project on Normal Galaxies. Images with < 9 arcsecond
(170 pc) resolution of these nearly face-on, late-type galaxies were obtained
using the LW2 (6.75 mu) and LW3 (15 mu) ISOCAM filters. Though their global
I_nu(6.75 mu)/I_nu(15 mu) flux ratios are similar and typical of normal
galaxies, they show distinct trends of this color ratio with mid-infrared
surface brightness. We find that I_nu(6.75 mu)/I_nu(15 mu) ~< 1 only occurs for
regions of intense heating activity where the continuum rises at 15 micron and
where PAH destruction can play an important role. The shape of the
color-surface brightness trend also appears to depend, to the second-order, on
the hardness of the ionizing radiation. We discuss these findings in the
context of a two-component model for the phases of the interstellar medium and
suggest that star formation intensity is largely responsible for the
mid-infrared surface brightness and colors within normal galaxies, whereas
differences in dust column density are the primary drivers of variations in the
