    "Disability and Returns to Education in a Developing Country"

    In this paper, we estimate wage returns to investment in education for persons with disabilities in Nepal, using information on the timing of being impaired during school-age years as identifying instrumental variables for years of schooling. We employ unique data collected from persons with hearing, physical, and visual impairments as well as nationally representative survey data from the Nepal Living Standard Survey 2003/2004 (NLSS II). After controlling for endogeneity bias arising from schooling decisions as well as sample selection bias due to endogenous labor participation, the estimated rate of returns to education is very high among persons with disabilities, ranging from 19.4 to 33.2%. The coexistence of these high returns to education and limited years of schooling suggest that supply side constraints in education to accommodate persons with disabilities and/or there are credit market imperfections. Policies to eliminate these barriers will mitigate poverty among persons with disabilities, the largest minority group in the world.

    Disability and barriers to education: evidence from Nepal

    In this article, barriers faced by Nepalese with disabilities in obtaining a school education are discussed. The study employed both quantitative and qualitative method to analyse the barrier using unique data collected from survey and in-depth interviews. It was found that some barriers were impairment-specific, while some were faced commonly irrespective of the type of impairments. People with visual and hearing impairments faced the inadequacy of support systems in schools, whereas participants with physical impairments mainly faced difficulties caused by physical barriers, such as inaccessible buildings and the lack of safe, accessible roads. Additionally, the barriers faced in common by the participants were financial barriers, resulting from parents' poverty; and barriers resulting from parental attitude, social stigma and lack of awareness. Participants with hearing impairments were found to be more vulnerable to the barriers to education, and as a result they had fewer years of schooling than their counterparts with visual and physical impairments. Results suggest that awareness of disability issues at all levels is the first step toward implementing strategies and policies to combat other problems like poverty and the lack of resources. Only with a more comprehensive understanding can effective policies to eliminate these barriers be developed

    Non-Intrusive Program Tracing of Non-Preemptive Multitasking Systems Using Power Consumption

    System tracing, runtime monitoring, execution reconstruction are useful techniques for protecting the safety and integrity of systems. Furthermore, with time-aware or overhead-aware techniques being available, these techniques can also be used to monitor and secure production systems. As operating systems gain in popularity, even in deeply embedded systems, these techniques face the challenge to support multitasking. In this thesis, we propose a novel non-intrusive technique, which efficiently reconstructs the execution trace of non-preemptive multitasking system by observing power consumption characteristics. Our technique uses the Control Flow Graph (CFG) of the application program to identify the most likely block of code that the system is executing at any given point in time. For the purpose of the experimental evaluation, we first instrument the source code to obtain power consumption information of each Basic Block (BB), which is used as the training data for our Dynamic Time Warping (DTW) and k-Nearest Neighbors (k-NN) classifier. Once the system is trained, this technique is used to identify live code-block execution (LCBE). We show that the technique can reconstruct the execution flow of programs in a multi-tasking environment with high accuracy. To aid the classification process, we analyze eight widely used machine learning algorithms with time-series power-traces data and show the comparison of time and computational resources for all the algorithms

    Social inclusion of people with disabilities: a case from Nepal’s decade-long civil war

    This paper discusses the issue of social inclusion of people with disabilities who acquired impairments through Nepal’s decade-long insurgency. In-depth qualitative interviews have been conducted with nine individuals. A disparity in government facilities and services was observed, whereby the combatants reported receiving fewer services than the Nepali army. In relation to their impairments, participants experienced mostly non-discriminatory behaviour from their families, whereas their perceived impression of the community’s behaviour was mixed. Additionally, participants were anxious about their daily living in part resulting from insufficient facilities and inadequate community inclusion strategies, as well as the lack of activities that can help increase opportunities for their economic well-being

    Economics of fish production at Chitwan district, Nepal

    A study was conducted in 2016 to analyze the economics of fish production at Chitwan District of Nepal. Three study sites: East, West and South part of Chitwan were selected purposively. A total of 90 households, 30 from each study site were selected randomly and were interviewed by using pre-tested semi structured questionnaire. Secondary data needed for the study were obtained from DADO, MOAD, NARC and other related organizations working on fisheries and aquaculture sector. Descriptive statistics and extended Cobb Douglas production function was used to accomplish the study objectives for which MS-Excel and SPSS 16 were used. The B/C ratio is obtained dividing the gross return by total variable cost incurred. The total cost of production per ha of the pond area was Rs. 743798 per year with 79 and 21 percent variable and fixed cost components, respectively. Feed cost (28 %) was largest cost item followed by cost for labour (25 %), fingerlings (10 %), maintenance (6 %), manure cum fertilizers (5 %), fuel cum energy (3 %) and limestone and others (2%). The average gross return and net profit realized per ha were Rs. 1223934 and Rs. 480135 respectively. The cost, return and profit were calculated to be highest for east Chitwan with highest B/C ratio followed by west Chitwan and south Chitwan. The B/C ratio for the district was found to be 1.63. The return to scale was found to be decreasing with value of 0.654 indicating that 1 percent increment in all the inputs included in the function will increase income by 0.654 percent. Production function analysis, including five variables, showed significant effect of human labour, fingerlings and fuel cum energy cost but feed and manure cum fertilizers cost were insignificant

    Development and testing the feasibility of a sports-based mental health promotion intervention in Nepal: a protocol for a pilot cluster-randomised controlled trial

    Background: Mental wellbeing encompasses life satisfaction, social connectedness, agency and resilience. In adolescence, mental wellbeing reduces sexual health risk behaviours, substance use and violence; improves educational outcomes; and protects mental health in adulthood. Mental health promotion seeks to improve mental wellbeing and can include activities to engage participants in sport. However, few high-quality trials of mental health promotion interventions have been conducted with adolescents, especially in low- and middle-income countries. We sought to address this gap by testing SMART (Sports-based Mental heAlth pRomotion for adolescenTs) in a pilot cluster-randomised controlled trial (cRCT) in Bardiya, Nepal. // Methods: The objectives of the trial are to assess the acceptability and feasibility of SMART, test trial procedures, explore outcome distributions in intervention and control clusters and calculate the total annual cost of the intervention and unit cost per adolescent. The trial design is a parallel-group, two-arm superiority pilot cRCT with a 1:1 allocation ratio and two cross-sectional census surveys with adolescents aged 12–19, one pre-intervention (baseline) and one post-intervention (endline). The study area is four communities of approximately 1000 population (166 adolescents per community). Each community represents one cluster. SMART comprises twice weekly football, martial arts and dance coaching, open to all adolescents in the community, led by local sports coaches who have received psychosocial training. Sports melas (festivals) and theatre performances will raise community awareness about SMART, mental health and the benefits of sport. Adolescents in control clusters will participate in sport as usual. In baseline and endline surveys, we will measure mental wellbeing, self-esteem, self-efficacy, emotion regulation, social support, depression, anxiety and functional impairment. Using observation checklists, unstructured observation and attendance registers from coaching sessions, and minutes of meetings between coaches and supervisors, we will assess intervention fidelity, exposure and reach. In focus group discussions and interviews with coaches, teachers, caregivers and adolescents, we will explore intervention acceptability and mechanisms of change. Intervention costs will be captured from monthly project accounts, timesheets and discussions with staff members. // Discussion: Findings will identify elements of the intervention and trial procedures requiring revision prior to a full cRCT to evaluate the effectiveness of SMART

    Measuring routine childhood vaccination coverage in 204 countries and territories, 1980-2019 : a systematic analysis for the Global Burden of Disease Study 2020, Release 1

    Background Measuring routine childhood vaccination is crucial to inform global vaccine policies and programme implementation, and to track progress towards targets set by the Global Vaccine Action Plan (GVAP) and Immunization Agenda 2030. Robust estimates of routine vaccine coverage are needed to identify past successes and persistent vulnerabilities. Drawing from the Global Burden of Diseases, Injuries, and Risk Factors Study (GBD) 2020, Release 1, we did a systematic analysis of global, regional, and national vaccine coverage trends using a statistical framework, by vaccine and over time. Methods For this analysis we collated 55 326 country-specific, cohort-specific, year-specific, vaccine-specific, and dosespecific observations of routine childhood vaccination coverage between 1980 and 2019. Using spatiotemporal Gaussian process regression, we produced location-specific and year-specific estimates of 11 routine childhood vaccine coverage indicators for 204 countries and territories from 1980 to 2019, adjusting for biases in countryreported data and reflecting reported stockouts and supply disruptions. We analysed global and regional trends in coverage and numbers of zero-dose children (defined as those who never received a diphtheria-tetanus-pertussis [DTP] vaccine dose), progress towards GVAP targets, and the relationship between vaccine coverage and sociodemographic development. Findings By 2019, global coverage of third-dose DTP (DTP3; 81.6% [95% uncertainty interval 80.4-82 .7]) more than doubled from levels estimated in 1980 (39.9% [37.5-42.1]), as did global coverage of the first-dose measles-containing vaccine (MCV1; from 38.5% [35.4-41.3] in 1980 to 83.6% [82.3-84.8] in 2019). Third- dose polio vaccine (Pol3) coverage also increased, from 42.6% (41.4-44.1) in 1980 to 79.8% (78.4-81.1) in 2019, and global coverage of newer vaccines increased rapidly between 2000 and 2019. The global number of zero-dose children fell by nearly 75% between 1980 and 2019, from 56.8 million (52.6-60. 9) to 14.5 million (13.4-15.9). However, over the past decade, global vaccine coverage broadly plateaued; 94 countries and territories recorded decreasing DTP3 coverage since 2010. Only 11 countries and territories were estimated to have reached the national GVAP target of at least 90% coverage for all assessed vaccines in 2019. Interpretation After achieving large gains in childhood vaccine coverage worldwide, in much of the world this progress was stalled or reversed from 2010 to 2019. These findings underscore the importance of revisiting routine immunisation strategies and programmatic approaches, recentring service delivery around equity and underserved populations. Strengthening vaccine data and monitoring systems is crucial to these pursuits, now and through to 2030, to ensure that all children have access to, and can benefit from, lifesaving vaccines.     The global burden of adolescent and young adult cancer in 2019 : a systematic analysis for the Global Burden of Disease Study 2019

    Background In estimating the global burden of cancer, adolescents and young adults with cancer are often overlooked, despite being a distinct subgroup with unique epidemiology, clinical care needs, and societal impact. Comprehensive estimates of the global cancer burden in adolescents and young adults (aged 15-39 years) are lacking. To address this gap, we analysed results from the Global Burden of Diseases, Injuries, and Risk Factors Study (GBD) 2019, with a focus on the outcome of disability-adjusted life-years (DALYs), to inform global cancer control measures in adolescents and young adults. Methods Using the GBD 2019 methodology, international mortality data were collected from vital registration systems, verbal autopsies, and population-based cancer registry inputs modelled with mortality-to-incidence ratios (MIRs). Incidence was computed with mortality estimates and corresponding MIRs. Prevalence estimates were calculated using modelled survival and multiplied by disability weights to obtain years lived with disability (YLDs). Years of life lost (YLLs) were calculated as age-specific cancer deaths multiplied by the standard life expectancy at the age of death. The main outcome was DALYs (the sum of YLLs and YLDs). Estimates were presented globally and by Socio-demographic Index (SDI) quintiles (countries ranked and divided into five equal SDI groups), and all estimates were presented with corresponding 95% uncertainty intervals (UIs). For this analysis, we used the age range of 15-39 years to define adolescents and young adults. Findings There were 1.19 million (95% UI 1.11-1.28) incident cancer cases and 396 000 (370 000-425 000) deaths due to cancer among people aged 15-39 years worldwide in 2019. The highest age-standardised incidence rates occurred in high SDI (59.6 [54.5-65.7] per 100 000 person-years) and high-middle SDI countries (53.2 [48.8-57.9] per 100 000 person-years), while the highest age-standardised mortality rates were in low-middle SDI (14.2 [12.9-15.6] per 100 000 person-years) and middle SDI (13.6 [12.6-14.8] per 100 000 person-years) countries. In 2019, adolescent and young adult cancers contributed 23.5 million (21.9-25.2) DALYs to the global burden of disease, of which 2.7% (1.9-3.6) came from YLDs and 97.3% (96.4-98.1) from YLLs. Cancer was the fourth leading cause of death and tenth leading cause of DALYs in adolescents and young adults globally. Interpretation Adolescent and young adult cancers contributed substantially to the overall adolescent and young adult disease burden globally in 2019. These results provide new insights into the distribution and magnitude of the adolescent and young adult cancer burden around the world. With notable differences observed across SDI settings, these estimates can inform global and country-level cancer control efforts. Copyright (C) 2021 The Author(s).
