    Reverse Hyperextension Device

    The Reverse Hyperextension Device is an exercise device typically used to aid in the recovery of those with lower back problems. Its main use is to substitute squats for those who cannot sustain the strain that comes with doing regular squats. A typical Reverse Hyperextension device is a bulky and immovable device, which poses a problem if the placement area has limited space. With our innovative design, the device will be foldable so it can be stored away when not in use and still as stable as needed to serve its purpose without causing injury. Another benefit coming from the redesign is the ability to easily move the device when needed. The redesign of the device will be made out of aluminum, with a few components out of steel for added strength. Finally, something not seen on ordinary Reverse Hyperextension devices that our redesign has is a real time angle readout. The readout is intended to keep track of the patients’ progress along their recovery process. With the redesign of the Reverse Hyperextension device, one will have everything found on the typical device along with many added features to make the device more user friendly and flexible to the users

    Guidance of sentinel lymph node biopsy decisions in patients with T1-T2 melanoma using gene expression profiling.

    AIM: Can gene expression profiling be used to identify patients with T1-T2 melanoma at low risk for sentinel lymph node (SLN) positivity? PATIENTS & METHODS: Bioinformatics modeling determined a population in which a 31-gene expression profile test predicted \u3c5% SLN positivity. Multicenter, prospectively-tested (n = 1421) and retrospective (n = 690) cohorts were used for validation and outcomes, respectively. RESULTS: Patients 55-64 years and β‰₯65 years with a class 1A (low-risk) profile had SLN positivity rates of 4.9% and 1.6%. Class 2B (high-risk) patients had SLN positivity rates of 30.8% and 11.9%. Melanoma-specific survival was 99.3% for patients β‰₯55 years with class 1A, T1-T2 tumors and 55.0% for class 2B, SLN-positive, T1-T2 tumors. CONCLUSION: The 31-gene expression profile test identifies patients who could potentially avoid SLN biopsy

    Pattern formation in spatially heterogeneous Turing reaction-diffusion models

    The Turing reaction–diffusion model [Phil. Trans. R. Soc. 237 (1952) 37–72] for self-organised spatial pattern formation has been the subject of a great deal of study for the case of spatially homogeneous parameters. The case of parameters which vary spatially has received less attention. Here, we show that a simple step function heterogeneity in a kinetic parameter can lead to spatial pattern formation outside the classical Turing space parameter regime for patterning. This reduces the constraints on the model parameters, extending possible applications. Furthermore, it highlights the potential importance of boundaries during pattern formation

    Evaluating the Comparative Advantage in Producing Soybean Under the Current Socio-Economic Structure in Ghana

    Ghana, like most developing countries, has pursued a policy of food self-sufficiency as a result of its very limited foreign exchange and uncertainty in international prices of agricultural commodities. There is the assumption that producing a commodity locally will use less foreign exchange than importing it. This assumption overlooks the real cost of saving foreign exchange. If a country does not have a comparative advantage in the production of a commodity, then substantial losses are borne by the country. It is therefore necessary to evaluate the efficiency with which foreign exchange can be saved or earned from a particular production process. The economic efficiency of producing soybean in Ghana under the current socioeconomic structure has been evaluated in this study, using the domestic resource cost (DRC) approach, via the Policy Analysis Matrix (PAM) framework. The results of the analysis reveal that under existing prices, technology and government policies, soybean production in Ghana is financially viable. However, the country has a comparative disadvantage in producing the commodity. Results of sensitivity tests on major variables in the economic analysis suggest that it is possible for Ghana to gain comparative advantage in soybean production in the long run if the current fiscal and monetary reforms are stepped up. The major policy implication from the study is that, unless the f.o.b. price of soybean in Ghana or the official exchange rate (OER) rise to a very high level, the production of the crop cannot be justified on grounds of comparative advantage

    The writing on the wall? John Ingram’s verse and the dissemination of Catholic prison writing

    The strong association between prison writing and writing on walls, whether by graffiti or carving, is as true of Tudor and Stuart England as of other times and places. Yet even if prison-writers associated themselves with the idea of writing on a wall, they need not have done so in reality. This article considers the topos in the writings and afterlife of the Catholic priest, poet and martyr John Ingram, and asks whether it is to be taken at face value. Ingram’s verse, composed in Latin and mostly epigrammatic, survives in two contemporary manuscripts. The notion that the author carved his verses with a blunt knife on the walls of the Tower of London while awaiting death derives from a previous editorial interpretation of a prefatory sentence within the more authoritative manuscript of the two, traditionally held to be autograph. However, though several Tudor and Stuart inscriptions survive to this day on the walls of the Tower of London, no portions of Ingram’s verse are among them, nor any inscriptions of similar length and complexity. Ingram might instead have written his verse down in the usual way, using wall-carving as a metaphor for the difficulty of writing verse when undergoing incarceration and torture

    Creative learning environments in education-A systematic literature review

    This paper reports on a systematic review of 210 pieces of educational research, policy and professional literature relating to creative environments for learning in schools, commissioned by Learning and Teaching Scotland (LTS). Despite the volume of academic literature in this field, the team of six reviewers found comparatively few empirical studies published in the period 2005–2011 providing findings addressing the review objectives. There was, however a reasonable weight of research evidence to support the importance of the following factors in supporting creative skills development in children and young people: flexible use of space and time; availability of appropriate materials; working outside the classroom/school; β€˜playful’ or β€˜games-bases’ approaches with a degree of learner autonomy; respectful relationships between teachers and learners; opportunities for peer collaboration; partnerships with outside agencies; awareness of learners’ needs; and non-prescriptive planning. The review also found evidence for impact of creative environments on pupil attainment and the development of teacher professionalism. LTS intend to use the review as a basis for recommendations to Scottish schools in promoting creativity within Curriculum for Excellence. However, the findings of the review and methodological gaps in the reviewed studies have implications for policy, practice and research internationally

    Standardized and reproducible methodology for the comprehensive and systematic assessment of surgical resection margins during breast-conserving surgery for invasive breast cancer

    <p>Abstract</p> <p>Background</p> <p>The primary goal of breast-conserving surgery (BCS) is to completely excise the tumor and achieve "adequate" or "negative" surgical resection margins while maintaining an acceptable level of postoperative cosmetic outcome. Nevertheless, precise determination of the adequacy of BCS has long been debated. In this regard, the aim of the current paper was to describe a standardized and reproducible methodology for comprehensive and systematic assessment of surgical resection margins during BCS.</p> <p>Methods</p> <p>Retrospective analysis of 204 BCS procedures performed for invasive breast cancer from August 2003 to June 2007, in which patients underwent a standard BCS resection and systematic sampling of nine standardized re-resection margins (superior, superior-medial, superior-lateral, medial, lateral, inferior, inferior-medial, inferior-lateral, and deep-posterior). Multiple variables (including patient, tumor, specimen, and follow-up variables) were evaluated.</p> <p>Results</p> <p>6.4% (13/204) of patients had positive BCS specimen margins (defined as tumor at inked edge of BCS specimen) and 4.4% (9/204) of patients had close margins (defined as tumor within 1 mm or less of inked edge but not at inked edge of BCS specimen). 11.8% (24/204) of patients had at least one re-resection margin containing additional disease, independent of the status of the BCS specimen margins. 7.1% (13/182) of patients with negative BCS specimen margins (defined as no tumor cells seen within 1 mm or less of inked edge of BCS specimen) had at least one re-resection margin containing additional disease. Thus, 54.2% (13/24) of patients with additional disease in a re-resection margin would not have been recognized by a standard BCS procedure alone (P < 0.001). The nine standardized resection margins represented only 26.8% of the volume of the BCS specimen and 32.6% of the surface area of the BCS specimen.</p> <p>Conclusion</p> <p>Our methodology accurately assesses the adequacy of surgical resection margins for determination of which individuals may need further resection to the affected breast in order to minimize the potential risk of local recurrence while attempting to limit the volume of additional breast tissue excised, as well as to determine which individuals are not realistically amendable to BCS and instead need a completion mastectomy to successfully remove multifocal disease.</p

    Towards an understanding of neuroscience for science educators

    Advances in neuroscience have brought new insights to the development of cognitive functions. These data are of considerable interest to educators concerned with how students learn. This review documents some of the recent findings in neuroscience, which is richer in describing cognitive functions than affective aspects of learning. A brief overview is presented here of the techniques used to generate data from imaging and how these findings have the possibility to inform educators. There are implications for considering the impact of neuroscience at all levels of education – from the classroom teacher and practitioner to policy. This relatively new cross-disciplinary area of research implies a need for educators and scientists to engage with each other. What questions are emerging through such dialogues between educators and scientists are likely to shed light on, for example, reward, motivation, working memory, learning difficulties, bilingualism and child development.     Identification of Lysine 37 of Histone H2B as a Novel Site of Methylation

    Recent technological advancements have allowed for highly-sophisticated mass spectrometry-based studies of the histone code, which predicts that combinations of post-translational modifications (PTMs) on histone proteins result in defined biological outcomes mediated by effector proteins that recognize such marks. While significant progress has been made in the identification and characterization of histone PTMs, a full appreciation of the complexity of the histone code will require a complete understanding of all the modifications that putatively contribute to it. Here, using the top-down mass spectrometry approach for identifying PTMs on full-length histones, we report that lysine 37 of histone H2B is dimethylated in the budding yeast Saccharomyces cerevisiae. By generating a modification-specific antibody and yeast strains that harbor mutations in the putative site of methylation, we provide evidence that this mark exist in vivo. Importantly, we show that this lysine residue is highly conserved through evolution, and provide evidence that this methylation event also occurs in higher eukaryotes. By identifying a novel site of histone methylation, this study adds to our overall understanding of the complex number of histone modifications that contribute to chromatin function

    Release of neuroactive substances by exocytosis from dendrites is surprisingly widespread and is not confined to a particular class of transmitters: it occurs in multiple brain regions, and includes a range of neuropeptides, classical neurotransmitters and signaling molecules such as nitric oxide, carbon monoxide, ATP and arachidonic acid. This review is focused on hypothalamic neuroendocrine cells that release vasopressin and oxytocin and midbrain neurons that release dopamine. For these two model systems, the stimuli, mechanisms and physiological functions of dendritic release have been explored in greater detail than is yet available for other neurons and neuroactive substances
