    Systemic Inflammation is Associated with Disease Extent and Survival in Oral Cavity Squamous Cell Carcinoma

    Background: Although systemic immune-inflammation index (SII) correlates with patient survival in various solid malignancies limited information is available in the setting of oral cavity squamous cell carcinoma (OCSCC). Methods: We retrospectively reviewed 222 patients who underwent a resection of curative intent for patients with OCSCC. SII was determined prior to surgical resection as platelet count × neutrophil count/lymphocyte count. Results: At a median follow-up of 30.6 months, 2-year disease-free survival (DFS) and overall survival (OS) rates were 63.9% and 76.4%, respectively. A high SII (\u3e1047) was associated with poor performance status and disease extent. A low SII was independently associated with improved DFS (HR: 0.440, pp=0.007) rates. Conclusions: SII values at diagnosis were associated with patient performance status, disease extent at the time of diagnosis, improved disease control rates, and improved patient survival

    Health and Housing in an Aging Population: Identifying Risk Factors of Seniors Living Independently

    Background: Aging is associated with numerous risk factors for declining physical and mental health. As a result, many elder individuals are forced to relocate to nursing homes, assisted living centers or just closer to adequate medical facilities. Studies have shown: • Relocation of elders is associated with depression, anxiety, memory loss, and decreased social adjustment and life satisfaction • Persistently lonely people exhibit a 2-fold greater risk for developing Alzheimer’s disease (AD) than those who are not chronically lonely • Increased social interaction improves cognitive function in individuals with AD By helping seniors to age in place, many of the deleterious risks of relocation, such as social isolation, depression, and cognitive decline could be avoided. In addition to relocation, other risk factors that affect cognition have been identified: • Physical activity is associated with higher cognitive functioning in elders • Polypharmacy is a risk factor for impaired cognition Thus, simple modifications that allow seniors to age in place may reduce morbidity and enhance quality of life. Cathedral Square Corporation (CSC) is a non-profit organization whose mission is to develop, manage, and own housing that provides community services to promote the health and well-being of elders, low income persons, and persons with disabilities.     Measurement of the cosmic ray spectrum above 4×10184{\times}10^{18} eV using inclined events detected with the Pierre Auger Observatory

    A measurement of the cosmic-ray spectrum for energies exceeding 4×10184{\times}10^{18} eV is presented, which is based on the analysis of showers with zenith angles greater than 6060^{\circ} detected with the Pierre Auger Observatory between 1 January 2004 and 31 December 2013. The measured spectrum confirms a flux suppression at the highest energies. Above 5.3×10185.3{\times}10^{18} eV, the "ankle", the flux can be described by a power law EγE^{-\gamma} with index γ=2.70±0.02(stat)±0.1(sys)\gamma=2.70 \pm 0.02 \,\text{(stat)} \pm 0.1\,\text{(sys)} followed by a smooth suppression region. For the energy (EsE_\text{s}) at which the spectral flux has fallen to one-half of its extrapolated value in the absence of suppression, we find Es=(5.12±0.25(stat)1.2+1.0(sys))×1019E_\text{s}=(5.12\pm0.25\,\text{(stat)}^{+1.0}_{-1.2}\,\text{(sys)}){\times}10^{19} eV.Comment: Replaced with published version.     Predictive Value of Multiparametric Magnetic Resonance Imaging in Risk Group Stratification of Prostate Adenocarcinoma

    Purpose: The aim of this study was to further assess the clinical utility of multiparametric magnetic resonance imaging (MP-MRI) in prostate cancer (PC) staging following 2023 clinical guideline changes, both as an independent predictor of high-stage (>T3a) or high-risk PC and when combined with patient characteristics. Methods and Materials: The present study was a retrospective review of 171 patients from 2008 to 2018 who underwent MP-MRI before radical prostatectomy at a single institution. The accuracy of clinical staging was compared between conventional staging and MP-MRI-based clinical staging. Sensitivity, specificity, positive predictive value, and negative predictive value were compared, and receiver operating characteristic curves were generated. Linear regression analyses were used to calculate concordance (C-statistic). Results: Of the 171 patients, final pathology revealed 95 (55.6%) with T2 disease, 62 (36.3%) with T3a disease, and 14 (8.2%) with T3b disease. Compared with conventional staging, MP-MRI-based staging demonstrated significantly increased accuracy in identifying T3a disease, intermediate risk, and high/very-high-risk PC. When combined with clinical characteristics, MP-MRI-based staging improved the area under the curve from 0.753 to 0.808 (P = .0175), compared with conventional staging. Conclusions: MP-MRI improved the identification of T3a PC, intermediate-risk PC, and high- or very-high-risk PC. Further, when combined with clinical characteristics, MP-MRI-based staging significantly improved risk stratification, compared with conventional staging. These findings represent further evidence to support the integration of MP-MRI into prostate adenocarcinoma clinical staging guidelines
