Inguinal Gravid Hysterocele in Feline
Background: Hysterocele is an inheritance rare in feline, characterized by the protrusion of the uterus by the inguinal channel, a situation related with the congenital abnormality of the inguinal ring, allowing the protrusion of abdominal contents for the subcutaneous space. As differential diagnosis of hysterocele are the breast cancer, abscesses and bruises and image exams such as radiography and ultrasonography have large relevance for identifying the disease and, additionally, clarify the hernia content. The purpose of this study was to describe one case report of the inguinal gravid hysterocele in a cat.Case: A 1-year-old female feline, without history of trauma of increase of volume in the inguinal region was referred for clinical evaluation. Due to the presence of three gestational vesicles, the decision by following-up the fetal development was taken, by means of periodical ultrasound evaluations, for performing operative procedure. At 56 days of gestation, when intestinal peristaltic movements of the three kittens were observed, cesarean was indicated. The procedure began by an oblique incision in the left retro-umbilical region, however, due to adhesions, there was no proper uterine exposition for hysterotomy. Retro-umbilical celiotomy, adhesiolise and return of the uterus for the abdominal cavity were performed in order to execute the cesarean followed by ovariohysterectomy. The hernial ring was debrided and sutured in order to keep patent the inguinal ring. No post-operatory complications were noted indicating that the established conduct allowed the survival of the parturient and its kittens.Discussion: In a casuistic study where data from 1988 until 2007 were collected, from the Faculty of Veterinary Medicine from the University of São Paulo, a higher occurrence of inguinal hernias was observed in female dogs, when compared with male dogs and, until that period, the inguinal hernia was not reported in the feline species. It is known that the inguinal ring may dilate with the obesity and increase of the intra-abdominal pressure, linked with the weakening of the adjacent structures. Besides, the appearance of hysterocele in females is also related with the estrus or gestation, caused by the hormonal imbalance, leading to the weakening of the conjunctive tissue, resulting in the enlargement of the inguinal rings, which is according to what was observed in this report where the animal was pregnant. Additionally, the application of contraceptive (progestin’s) could have accentuated the muscle fragility, favoring the projection of the uterus by the inguinal ring. In most of the cases described in literature, animals have no discomfort to the handling of the herniated content however, for some authors, the prolongation of surgical resolution may aggravate the clinical condition, predisposing the formation of adherences that may cause discomfort or pain, particularly when the bladder or intestinal loops are involved, resulting in difficulty urinating and defecating, respectively. Care must be taken during the reduction of the inguinal ring, in order to avoid compromising the external pudendal artery and vein, and of the genito-femoral nerve, which emerge from the tail-medial aspect of the ring. In this study, it was tried to perform the hernia suture in a way that the inguinal ring remained patent. On the same way that was performed in this study recommend the execution of ovariohysterectomy in order to avoid relapse and because it is an hereditary condition. The inguinal gravid hysterocele in feline may be adequately manage until the ideal moment for cesarean without prejudice to the parturient and to its products
EFEITOS DA REABILITAÇÃO VESTIBULAR NO EQUILÍBRIO POSTURAL EM IDOSOS: UMA REVISÃO SISTEMÁTICA
Introduction: vestibular disorders can be associated with changes such as vertigo, imbalances, dizziness and labyrinthitis. Vestibular rehabilitation has been described as an effective treatment for patients with vertigo due to vestibular disorders. Objectives: systematically review studies on the effects of vestibular rehabilitation on postural balance in the elderly with vestibular disorders. Methods: systematic review, outlined by PRISMA, carried out on the bases: PUBMED, EBSCO, LILACS, MEDLINE, VHL Portal, and SciELO. Search conducted between August and October 2020, using the keywords: “Rehabilitation”, “Vestibular Diseases”, “Postural Balance” and Aged. Included: randomized controlled trials that have evaluated the effects of vestibular rehabilitation in elderly people with postural instability. Such studies should be available in full. There were no restrictions on the language and time of publication of the studies. Methodological quality was assessed using the PEDro scale. Results: searches identified 162 articles, however, after analysis 2 were included. The studies were published between 2016 and 2018. The sample had a total of 188 individuals of both sexes, more than 75% women, who had vestibular disorders. Based on the studies, the interventions lasted from 5 to 50 minutes, from 1 to 2 times a day, from 2 weeks to 2 months. The findings indicate that VR improves balance and reduces the number of falls. The studies showed high methodological quality. Conclusions: individuals who underwent vestibular rehabilitation, had progress in the postural balance of the elderly, consequently reducing the risk of falls. research proposed for this population.Introdução: Os distúrbios vestibulares podem estar associados a alterações como vertigem, desequilíbrios, tonturas e labirintite. A reabilitação vestibular vem sendo descrita como tratamento eficaz para pacientes com vertigem por disfunções vestibulares. Objetivos: revisar sistematicamente estudos sobre os efeitos da reabilitação vestibular no equilíbrio postural no idoso com distúrbios vestibulares. Métodos: revisão sistemática, delineado pela PRISMA, realizada nas bases: PUBMED, EBSCO, LILACS, MEDLINE, Portal da BVS, e SciELO. Busca realizada entre agosto e outubro de 2020, por meio dos descritores: “Rehabilitation”, "Vestibular Diseases", “Postural Balance” e Aged. Incluídos: ensaios clínicos randomizados que tenham avaliado os efeitos da reabilitação vestibular em idosos com instabilidade postural. Tais estudos deveriam estar disponíveis na integra. Não foram realizadas restrições quanto ao idioma e tempo de publicação dos estudos. A qualidade metodológica foi avaliada pela escala PEDro. Resultados: as buscas identificaram 162 artigos, contudo, após análise 2 foram incluídos. Os estudos foram publicados entre 2016 e 2018. A amostra teve como total 188 indivíduos de ambos os sexos, sendo mais de 75% mulheres, que apresentavam distúrbios vestibulares. Com base nos estudos, as intervenções tiveram durações de 5 a 50 minutos, de 1 a 2 vezes por dia, de 2 semanas a 2 meses. Os achados indicam que a RV melhora o equilíbrio e reduz o número de quedas. Os estudos apresentaram alta qualidade metodológica. Conclusões: indivíduos que realizaram reabilitação vestibular, tiveram progresso no equilíbrio postural dos idosos, consequentemente reduzindo o risco de quedas. pesquisas propostas para essa população
A epidemiologia do câncer de mama em Roraima / The epidemiology of breast cancer in Roraima
O câncer de mama é um grande problema de saúde pública, sendo o diagnóstico na sua fase inicial de suma importância para um melhor prognóstico e tratamento da mulher afetada. O presente estudo traz como objetivos traçar um perfil epidemiológico do câncer de mama, no Estado de Roraima entre os anos de 2009 a 2013, bem como verificar se a mamografia influencia na melhoria das taxas de letalidade. Procurou-se, também, comparar a incidência da neoplasia mamária nas regiões do Brasil e entre indivíduos indígenas e não indígenas de Roraima. Para isso, o estudo foi realizado quantitativamente com dados obtidos através da plataforma SISMAMA, INCA e IBGE e complementado com a análise qualitativa de tais informações e de seus efeitos na epidemiologia do Estado. O estudo aponta possível deficiência no sistema preventivo, o que acarreta uma suscetibilidade maior a prognósticos ruins. Ressalta-se, então, a importância da melhoria da rede de atendimento, para a realização de mamografias preventivas, a fim aumentar a probabilidade de tratamento efetivo, cura e estudos mais aprofundados que evidenciem a problemática.
A prevalência de sequelas cognitivas em pacientes diagnosticados com COVID-19 no Brasil – uma revisão de Literatura / The prevalence of cognitive sequelae in patients diagnosed with COVID-19 in Brazil – a Literature Review
OBJETIVOS: Neste artigo propomos um estudo que visa relacionar os déficits cognitivos com pacientes infectados pelo vírus da COVID-19, de acordo com a compreensão de suas fisiopatologias atuais mais prováveis bem como sua prevalência nesses pacientes. MÉTODOS: Revisão de literatura de caráter exploratório, com uma ampla avaliação de estudos e casos da prática clínica, em que se fez uma análise sobre a COVID-19 relacionando-a com as disfunções neurocognitivas apresentadas pelos indivíduos infectados. RESULTADOS: Foram selecionados 40 estudos para compor essa revisão de literatura. Entre as bases de dados selecionadas, estão: PubMed, Google Scholar, Scielo, UpToDate e Scopus. DISCUSSÃO: Desse modo, durante a análise dos artigos foi evidente que há limitação dos estudos observacionais brasileiros que fazem a relação do vírus com a diminuição da cognição. Em resumo, foi analisado que os déficits das funções cognitivas em pacientes que tiveram a doença podem estar intimamente relacionados com a hipóxia e ao perfil inflamatório causados pelo vírus. Nos estudos selecionados, as repercussões cognitivas foram prevalentes entre os pacientes que tiveram COVID-19 e os déficits com maior incidência foram: dificuldade de aprendizado e de concentração, redução da atenção sustentada, perda de memória e diminuição da coordenação motora. Ademais, vale ressaltar que essa redução da cognição está presente tanto em pacientes que tiveram um quadro grave quanto os que tiveram um quadro leve da doença, sendo potencialmente danoso ao cotidiano da maioria desses indivíduos. CONCLUSÃO: Em suma, há claramente uma inerente relação entre os déficits cognitivos e a infecção com o vírus SARS-CoV-2, visto que a maioria dos pacientes infectados possuem pelo menos uma queixa relacionada à diminuição da cognição. Portanto, há necessidade de mais pesquisas e estudos observacionais que associem os déficits cognitivos à COVID-19 no Brasil, visto que é uma doença emergente e prevalente na atual situação global
Epiisopiloturine, an alkaloid from Pilocarpus microphyllus, attenuates LPS-induced neuroinflammation by interfering in the TLR4/NF-κB-MAPK signaling pathway in microglial cells
Neuroinflammation is present in the pathophysiological mechanisms of several diseases that affect the central nervous system (CNS). Microglia have a prominent role in initiating and sustaining the inflammatory process. Epiisopiloturine (EPI) is an imidazole alkaloid obtained as a by-product of pilocarpine extracted from Pilocarpus microphyllus (jaborandi) and has shown promising anti-inflammatory and antinociceptive properties. In the present study, we investigated the effects of EPI on the inflammatory response in microglial cells (BV-2 cells) induced by lipopolysaccharide (LPS) and explored putative underlying molecular mechanisms. Cell viability was not affected by EPI (1-100 μg/mL) as assessed by both LDH activity and the MTT test. Pretreatment with EPI (25, 50, and 100 μg/mL) significantly reduced the proinflammatory response induced by LPS, as observed by a decrease in nitrite oxide production and iNOS protein expression. EPI (25 μg/mL) reduced IL-6 and TNF-α production, by 40% and 34%, respectively. However, no changes were observed in the anti-inflammatory IL-10 production. Mechanistically, EPI inhibited the TLR4 expression and phosphorylation of NF-κB p65 and MAPKs (JNK and ERK1/2) induced by LPS, but no changes were observed in TREM2 receptor expression in LPS-stimulated cells. In conclusion, our data demonstrated the potent anti-inflammatory properties of EPI in microglial cells. These effects are associated with the reduction of TLR4 expression and inhibition of intracellular signaling cascades, including NF-κB and MAPKs (JNK and ERK1/2)
Clonal chromosomal mosaicism and loss of chromosome Y in elderly men increase vulnerability for SARS-CoV-2
The pandemic caused by severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2, COVID-19) had an estimated overall case fatality ratio of 1.38% (pre-vaccination), being 53% higher in males and increasing exponentially with age. Among 9578 individuals diagnosed with COVID-19 in the SCOURGE study, we found 133 cases (1.42%) with detectable clonal mosaicism for chromosome alterations (mCA) and 226 males (5.08%) with acquired loss of chromosome Y (LOY). Individuals with clonal mosaic events (mCA and/or LOY) showed a 54% increase in the risk of COVID-19 lethality. LOY is associated with transcriptomic biomarkers of immune dysfunction, pro-coagulation activity and cardiovascular risk. Interferon-induced genes involved in the initial immune response to SARS-CoV-2 are also down-regulated in LOY. Thus, mCA and LOY underlie at least part of the sex-biased severity and mortality of COVID-19 in aging patients. Given its potential therapeutic and prognostic relevance, evaluation of clonal mosaicism should be implemented as biomarker of COVID-19 severity in elderly people. Goodbye Hartmann trial: a prospective, international, multicenter, observational study on the current use of a surgical procedure developed a century ago
Background: Literature suggests colonic resection and primary anastomosis (RPA) instead of Hartmann's procedure (HP) for the treatment of left-sided colonic emergencies. We aim to evaluate the surgical options globally used to treat patients with acute left-sided colonic emergencies and the factors that leading to the choice of treatment, comparing HP and RPA. Methods: This is a prospective, international, multicenter, observational study registered on ClinicalTrials.gov. A total 1215 patients with left-sided colonic emergencies who required surgery were included from 204 centers during the period of March 1, 2020, to May 31, 2020. with a 1-year follow-up. Results: 564 patients (43.1%) were females. The mean age was 65.9 ± 15.6 years. HP was performed in 697 (57.3%) patients and RPA in 384 (31.6%) cases. Complicated acute diverticulitis was the most common cause of left-sided colonic emergencies (40.2%), followed by colorectal malignancy (36.6%). Severe complications (Clavien-Dindo ≥ 3b) were higher in the HP group (P < 0.001). 30-day mortality was higher in HP patients (13.7%), especially in case of bowel perforation and diffused peritonitis. 1-year follow-up showed no differences on ostomy reversal rate between HP and RPA. (P = 0.127). A backward likelihood logistic regression model showed that RPA was preferred in younger patients, having low ASA score (≤ 3), in case of large bowel obstruction, absence of colonic ischemia, longer time from admission to surgery, operating early at the day working hours, by a surgeon who performed more than 50 colorectal resections. Conclusions: After 100 years since the first Hartmann's procedure, HP remains the most common treatment for left-sided colorectal emergencies. Treatment's choice depends on patient characteristics, the time of surgery and the experience of the surgeon. RPA should be considered as the gold standard for surgery, with HP being an exception
Taking the pulse of Earth's tropical forests using networks of highly distributed plots
Tropical forests are the most diverse and productive ecosystems on Earth. While better understanding of these forests is critical for our collective future, until quite recently efforts to measure and monitor them have been largely disconnected. Networking is essential to discover the answers to questions that transcend borders and the horizons of funding agencies. Here we show how a global community is responding to the challenges of tropical ecosystem research with diverse teams measuring forests tree-by-tree in thousands of long-term plots. We review the major scientific discoveries of this work and show how this process is changing tropical forest science. Our core approach involves linking long-term grassroots initiatives with standardized protocols and data management to generate robust scaled-up results. By connecting tropical researchers and elevating their status, our Social Research Network model recognises the key role of the data originator in scientific discovery. Conceived in 1999 with RAINFOR (South America), our permanent plot networks have been adapted to Africa (AfriTRON) and Southeast Asia (T-FORCES) and widely emulated worldwide. Now these multiple initiatives are integrated via ForestPlots.net cyber-infrastructure, linking colleagues from 54 countries across 24 plot networks. Collectively these are transforming understanding of tropical forests and their biospheric role. Together we have discovered how, where and why forest carbon and biodiversity are responding to climate change, and how they feedback on it. This long-term pan-tropical collaboration has revealed a large long-term carbon sink and its trends, as well as making clear which drivers are most important, which forest processes are affected, where they are changing, what the lags are, and the likely future responses of tropical forests as the climate continues to change. By leveraging a remarkably old technology, plot networks are sparking a very modern revolution in tropical forest science. Identidade na modernidade e na pós-modernidade
Este trabalho visa à tentativa de compreensão das formas de ser e estar no mundo do indivíduo contemporâneo, ou seja, sua identidade. O atual cenário apresenta mudanças intensas, mais que em qualquer outro período da história da sociedade ocidental, mudanças tão radicais que muitos chegam a afirmar o fim dessa história. Diante desse quadro, denominado por muitos como pós-modernidade, busca-se compreender como as identidades estão sendo constituídas, em comparação aos modos de subjetivação modernos, pautados na dialética interior/exterior, verdadeiro/falso. Através de revisão bibliográfica de um grupo de diversos autores, serão abordados alguns fatores influenciadores da identidade pós-moderna, como a compressão espaço-tempo, apontada por David Harvey; a teoria do espetáculo, de Guy Debord, e os fenômenos digitais, como blogs e redes sociais; o conceito de dessublimação repressiva, de Slavoj Zizek; e o advento de uma cultura somática, através do desenvolvimento dos campos da genética e da neurociência. Contrariando análises maniqueístas, cabe pensar em que medida esses processos de mudança ocorrem na sociedade e como é possível, em um mesmo tempo, observar características modernas e pós-modernas em indivíduos que vivem um panorama que não se sabe que fim terá
