    Continuous ultrafiltration during extracorporeal circulation and its effect on lactatemia: A randomized controlled trial

    Introduction. Hyperlactatemia occurs during or after extracorporeal circulation in the form of lactic acidosis, increasing the risk of postoperative complications and the mortality rate. The aim of this study was to evaluate whether continuous high-volume hemofiltration with volume replacement through a polyethersulfone filter during the extracorporeal circulation procedure decreases postoperative lactatemia and its consequences. ||Materials and methods. This was a randomized controlled trial. Patients were randomly divided into two groups of 32: with or without continuous high-volume hemofiltration through a polyethersulfone membrane. Five patients were excluded from each group during the study period. The sociodemographic characteristics, filter effects, and blood lactate levels at different times during the procedure were evaluated. Secondary endpoints were studied, such as the reduction in the intubation time and time spent in ICU. ||Results. Lactatemia measurements performed during the preoperative and intraoperative phases were not significantly different between the two groups. However, the blood lactate levels in the postoperative period and at 24 hours in the intensive care unit showed a significant reduction and a possible clinical benefit in the hemofiltered group. Following extracorporeal circulation, the mean lactate level was higher (difference: 0.77 mmol/L; CI 0.95: 0.01–1.53) in the nonhemofiltered group than in the hemofiltered group (p<0.05). This effect was greater at 24 hours (p = 0.019) in the nonhemofiltered group (difference: 1.06 mmol/L; CI 0.95: 0.18–1.93) than in the hemofiltered group. The reduction of lactatemia is associated with a reduction of inflammatory mediators and intubation time, with an improvement in liver function. || Conclusions. The use and control of continuous high-volume hemofiltration through a polyethersulfone membrane during heart-lung surgery could potencially prevent postoperative complications. The reduction of lactatemia implied a reduction in intubation time, a decrease in morbidity and mortality in the intensive care unit and a shorter hospital stay.14 páginas

    Self-Rated Health Status and Cardiorespiratory Fitness in a Sample of Schoolchildren from Bogotá, Colombia. The FUPRECOL Study

    To evaluate the relationship between Self-Rated Health (SRH) and cardiorespiratory fitness (CRF) in a sample of children and adolescents enrolled in official schools in Bogotá, Colombia. A cross-sectional study was performed with 7402 children and adolescents between 9 and 17 years of age. Participants were asked to rate their health based on eight validated questions, addressing the participants propensity for headache, stomach-ache, backache, feeling-low, irritability/bad mood, nervousness, sleeping-difficulties, and dizziness. The choices were “rarely or never”, “almost every month”, “almost every week”, and “more than once a week/about every day”. Participants performed the international course-navette shuttle run test to estimate CRF, and cut-off points for age and gender were used to categorize the healthy/unhealthy fitness zone according to the FITNESSGRAM® criteria. Overall, 16.4% of those surveyed reported a perception of irritability/bad mood “more than once a week/about every day”, followed by feeling-low and nervousness (both with 9.9%). Dizziness had the lowest prevalence with a percentage of 6.9%. Unhealthy CRF in boys increased the likelihood of headaches by 1.20 times, stomach aches by 1.31 times, feeling-low by 1.29 times, nervousness by 1.24 times, and dizziness by 1.29 times. In girls, unhealthy CRF increased the likelihood of headaches by 1.19 times, backache by 1.26 times, feeling-low by 1.28 times, irritability/bad mood by 1.17 times, sleeping-difficulties by 1.20 times, and dizziness by 1.27 times. SRH was associated with CRF in both genders.     Estrategias docentes para la adaptación de la docencia en Ciencias Jurídicas al entorno on line y semipresencial

    The health situation caused by COVID-19 has forced Spanish universities to adopt tools and measures aimed at adapting e-learning education. The teaching of legal subjects was initially at a disadvantage, as it usually made use of traditional methodologies. To solve this problem, a group of lecturers from the UA and the UMH have designed and implemented a series of strategies and methodologies in order to adapt teaching to the virtual environment in a satisfactory manner. The present work gathers and exposes the experiences deployed during the academic year 2020/2021 in certain subjects in the legal field. The analysis of these experiences shows that the current catalogue of technological tools and methodologies makes it possible to achieve the aforementioned objective of adapting to the virtual environment. However, there are a series of factors beyond the control of the teaching staff and which require the commitment of the rest of the members of the university community.La situación sanitaria provocada por la COVID-19 ha forzado a las universidades españolas a adoptar herramientas y medidas orientadas a una adaptación de la enseñanza e-learning. La docencia en materias jurídicas partía de una situación inicial de desventaja al hacer uso habitualmente de metodologías de carácter tradicional. Para solventar este problema, un grupo de profesoras y profesores de la UA y la UMH ha diseñado e implementado una serie de estrategias y metodologías docentes con el fin de adaptar la enseñanza al entorno virtual de manera satisfactoria. El presente trabajo recoge y expone las experiencias docentes desplegadas durante el curso académico 2020/2021 en determinadas asignaturas del ámbito jurídico. Del análisis de estas experiencias se deriva que el catálogo actual de herramientas tecnológicas y metodologías docentes permite alcanzar el citado objetivo de adaptación al entorno virtual.     ¿Cómo se discute la violencia contra las mujeres en el espacio público digital?: divulgación de resultados investigación-docencia

    Este documento presenta los resultados más relevantes de un proceso de investigación y docencia desarrollado en el marco de los cursos C-1103 Comunicación y Poder y C-1002 Comunicación Inclusiva de la Concentración en Comunicación Social de la Escuela de Ciencias de la Comunicación Colectiva durante el ciclo II-2020, en articulación con el Programa de Narrativas, Género y Comunicación del Centro de Investigación en Comunicación – CICOM-, de la Universidad de Costa Rica (UCR). El proyecto se planteó objetivos en tres ámbitos: el de la investigación, el de la enseñanza-aprendizaje y el de la política pública. En primer lugar, esta investigación responde al esfuerzo de articulación entre el CICOM y el Instituto Nacional de las Mujeres, INAMU, institución que coordina a las 22 instituciones que conforman el Sistema Nacional de prevención y atención de la violencia contra las mujeres e intrafamiliar (Ley 8688), encargado de implementar la Política Nacional para la atención y prevención de la violencia contra las mujeres de todas las edades, PLANOVI 2017-2032 y del cual la UCR es parte. Esta política, particularmente los ejes 1 y 2, centran su interés en la comunicación como vehículo fundamental para conseguir el cambio cultural y propiciar la erradicación de la violencia contra las mujeres y la promoción de las masculinidades no violentas. Para ello, se requiere diseñar e implementar una estrategia integrada de comunicación que exige, como punto de partida, determinar cuáles son los discursos sobre la violencia contra las mujeres que circulan en el espacio público costarricense en la actualidad.     Effectiveness of an intervention for improving drug prescription in primary care patients with multimorbidity and polypharmacy:Study protocol of a cluster randomized clinical trial (Multi-PAP project)

    This study was funded by the Fondo de Investigaciones Sanitarias ISCIII (Grant Numbers PI15/00276, PI15/00572, PI15/00996), REDISSEC (Project Numbers RD12/0001/0012, RD16/0001/0005), and the European Regional Development Fund ("A way to build Europe").Background: Multimorbidity is associated with negative effects both on people's health and on healthcare systems. A key problem linked to multimorbidity is polypharmacy, which in turn is associated with increased risk of partly preventable adverse effects, including mortality. The Ariadne principles describe a model of care based on a thorough assessment of diseases, treatments (and potential interactions), clinical status, context and preferences of patients with multimorbidity, with the aim of prioritizing and sharing realistic treatment goals that guide an individualized management. The aim of this study is to evaluate the effectiveness of a complex intervention that implements the Ariadne principles in a population of young-old patients with multimorbidity and polypharmacy. The intervention seeks to improve the appropriateness of prescribing in primary care (PC), as measured by the medication appropriateness index (MAI) score at 6 and 12months, as compared with usual care. Methods/Design: Design:pragmatic cluster randomized clinical trial. Unit of randomization: family physician (FP). Unit of analysis: patient. Scope: PC health centres in three autonomous communities: Aragon, Madrid, and Andalusia (Spain). Population: patients aged 65-74years with multimorbidity (≥3 chronic diseases) and polypharmacy (≥5 drugs prescribed in ≥3months). Sample size: n=400 (200 per study arm). Intervention: complex intervention based on the implementation of the Ariadne principles with two components: (1) FP training and (2) FP-patient interview. Outcomes: MAI score, health services use, quality of life (Euroqol 5D-5L), pharmacotherapy and adherence to treatment (Morisky-Green, Haynes-Sackett), and clinical and socio-demographic variables. Statistical analysis: primary outcome is the difference in MAI score between T0 and T1 and corresponding 95% confidence interval. Adjustment for confounding factors will be performed by multilevel analysis. All analyses will be carried out in accordance with the intention-to-treat principle. Discussion: It is essential to provide evidence concerning interventions on PC patients with polypharmacy and multimorbidity, conducted in the context of routine clinical practice, and involving young-old patients with significant potential for preventing negative health outcomes.     Clonal chromosomal mosaicism and loss of chromosome Y in elderly men increase vulnerability for SARS-CoV-2

    The pandemic caused by severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2, COVID-19) had an estimated overall case fatality ratio of 1.38% (pre-vaccination), being 53% higher in males and increasing exponentially with age. Among 9578 individuals diagnosed with COVID-19 in the SCOURGE study, we found 133 cases (1.42%) with detectable clonal mosaicism for chromosome alterations (mCA) and 226 males (5.08%) with acquired loss of chromosome Y (LOY). Individuals with clonal mosaic events (mCA and/or LOY) showed a 54% increase in the risk of COVID-19 lethality. LOY is associated with transcriptomic biomarkers of immune dysfunction, pro-coagulation activity and cardiovascular risk. Interferon-induced genes involved in the initial immune response to SARS-CoV-2 are also down-regulated in LOY. Thus, mCA and LOY underlie at least part of the sex-biased severity and mortality of COVID-19 in aging patients. Given its potential therapeutic and prognostic relevance, evaluation of clonal mosaicism should be implemented as biomarker of COVID-19 severity in elderly people. Con un siglo de historia a su espalda, este invento, cuya filiación no se puede adjudicar a un único nombre, ofrece compañía a millones de personas, abre una ventana al mundo desde la que contemplar el devenir de la vida, mantiene vivo el pasado, informa sobre el presente e imagina el futuro. "Lo han dicho en la tele" es una frase de uso común y da una idea de la fuerza de ese medio de comunicación como conformador de una realidad verosímil. Si lo han dicho en la tele, entonces, tiene que ser verdad. Todo el mundo tiene presente algún acontecimiento visto en la pequeña pantalla que ha quedado grabado en su memoria para siempre. La tele es parte de nuestra vida y moldea nuestros recuerdos. Mucho ha cambiado la televisión desde sus orígenes hasta la actualidad. En cuanto al continente, pasa de ser un mamotreto de muchos kilos a un aparato delgado y ligero con una notable definición de imagen. Por no hablar de que ya es posible ver la televisión en otro soporte, el teléfono móvil. Y, respecto al contenido, muta del blanco y negro en diferido al color en riguroso directo. El contenido televisivo evoluciona al ritmo de la tecnología, los usos sociales y los deseos del espectador. Y, en esa forma de hacer televisión destaca, de un tiempo a esta parte, el uso de unas figuras en 3 dimensiones que ni siquiera existen en el plató desde el que se emiten. A esos elementos virtuales, creados con una tecnología llamada realidad aumentada, está dedicado este trabajo...Television is the queen of screens. With a century of history behind it, this invention, whose affiliation cannot be attributed to a single name, offers company to millions of people, opens a window to the world from which to contemplate the evolution of life, keeps the past alive, inform about the present and imagine the future. "They said it onTV" is a commonly used phrase and gives an idea of the strength of this means of communication as a shaper of a plausible reality. If they said it on TV, then it has to be true. Everyone has in mind some event seen on the small screen that has been etched in their memory forever. TV is part of our life and shapes our memories. Television has changed a lot from its origins to the present day. As for the continent, it has gone from being a tome of many kilos to a thin and light device with a remarkable image definition. Not to mention that it is already possible to watch television on another device, the mobile phone. And, regarding the content, it has mutated from blackand white in deferred to rigorously direct color. Television content evolves at the pace of technology, social uses and the wishes of the viewer. And, in this way of making television, for some time now, the use of 3-dimensional figures that do not even exist on the set from which they are broadcast stands out.
