    Prenatal Oral Health Education in U.S. Obstetrics and Gynecology Residencies and Dental Schools: Results of a National Survey

    Background: Pregnant women represent a special population within oral health care. Adverse pregnancy outcomes and increased infant caries can occur when prenatal oral disease is not addressed. Currently, medical and dental clinicians are not meeting the oral health needs of pregnant patients. Objective: Medical and dental providers are not addressing prenatal oral health (POH) with patients despite knowledge of the risks. The objective of this study was to determine how training in dental schools and OB/Gyn residencies may contribute to this paradox. Methods: We conducted a national survey of 60 dental school deans and 240 obstetrics and gynecology residency program directors. Questions assessed the number of hours of POH education, topics addressed, awareness of guidelines, and barriers to including more POH training. Results: Response rates were 53% and 40% for dental schools and OB/Gyn residencies, respectively. 94% of dental schools provide some POH education, with 61% of schools offering 3+ hours. Only 39% of OB/Gyn residencies provide some POH education, most only 1-2 hours. 65% of dental programs and 45% of OB/Gyn residencies are aware of current POH evidence-based guidelines. Those OB/Gyn residency programs with POH training were three times as likely to expose their residents to these guidelines. A similar trend was observed for dental schools. Barriers to POH education include space in the curriculum and competing clinical priorities. 76% of OB/Gyn directors affirmed the importance of addressing oral health needs among prenatal patients; however, only 23% agreed that the ACGME should add POH competencies. The majority of respondents agreed they would add more POH education if the American College of Obstetrics and Gynecology issued a policy statement or practice bulletin. Conclusions: The majority of dental schools teach POH but clinical exposure is limited. Less than half of OB/Gyn residencies include POH training. Future efforts should include distribution of POH guidelines/consensus statements to educators and learners, increasing exposure of dental students to pregnant patients, and developing faculty expertise in residencies

    Slow dynamics, aging, and glassy rheology in soft and living matter

    We explore the origins of slow dynamics, aging and glassy rheology in soft and living matter. Non-diffusive slow dynamics and aging in materials characterised by crowding of the constituents can be explained in terms of structural rearrangement or remodelling events that occur within the jammed state. In this context, we introduce the jamming phase diagram proposed by Liu and Nagel to understand the ergodic-nonergodic transition in these systems, and discuss recent theoretical attempts to explain the unusual, faster-than-exponential dynamical structure factors observed in jammed soft materials. We next focus on the anomalous rheology (flow and deformation behaviour) ubiquitous in soft matter characterised by metastability and structural disorder, and refer to the Soft Glassy Rheology (SGR) model that quantifies the mechanical response of these systems and predicts aging under suitable conditions. As part of a survey of experimental work related to these issues, we present x-ray photon correlation spectroscopy (XPCS) results of the aging of laponite clay suspensions following rejuvenation. We conclude by exploring the scientific literature for recent theoretical advances in the understanding of these models and for experimental investigations aimed at testing their predictions.Comment: 22 pages, 5 postscript figures; invited review aricle, to appear in special issue on soft matter in Solid State Communication

    Characterizing the conformational dynamics of metal-free PsaA using molecular dynamics simulations and electron paramagnetic resonance spectroscopy

    Prokaryotic metal-ion receptor proteins, or solute-binding proteins, facilitate the acquisition of metal ions from the extracellular environment. Pneumococcal surface antigen A (PsaA) is the primary Mn2+-recruiting protein of the human pathogen Streptococcus pneumoniae and is essential for its in vivo colonization and virulence. The recently reported high-resolution structures of metal- free and metal-bound PsaA have provided the first insights into the mechanism of PsaA-facilitated metal binding. However, the conformational dynamics of metal-free PsaA in solution remain unknown. Here, we use continuous wave electron paramagnetic resonance (EPR) spectroscopy and molecular dynamics (MD) simulations to study the relative flexibility of the structural domains in metal-free PsaA and its distribution of conformations in solution. The results show that the crystal structure of the metal-free PsaA is a good representation of the dominant conformation in solution, but the protein also samples structurally distinct conformations that are not captured by the crystal structure. Further, these results suggest that the metal binding site is larger and more solvent exposed than indicated by the metal-free crystal structure. Collectively, this study provides atomic-resolution insight into the conformational dynamics of PsaA prior to metal binding and lays the groundwork for future EPR and MD based studies of PsaA in solution

    Beetle and plant arrow poisons of the Ju|’hoan and Hai||om San peoples of Namibia (Insecta, Coleoptera, Chrysomelidae; Plantae, Anacardiaceae, Apocynaceae, Burseraceae)

    The use of archery to hunt appears relatively late in human history. It is poorly understood but the application of poisons to arrows to increase lethality must have occurred shortly after developing bow hunting methods; these early multi-stage transitions represent cognitive shifts in human evolution. This paper is a synthesis of widely-scattered literature in anthropology, entomology, and chemistry, dealing with San (“Bushmen”) arrow poisons. The term San (or Khoisan) covers many indigenous groups using so-called ‘click languages’ in southern Africa. Beetles are used for arrow poison by at least eight San groups and one non-San group. Fieldwork and interviews with Ju|’hoan and Hai||om hunters in Namibia revealed major differences in the nature and preparation of arrow poisons, bow and arrow construction, and poison antidote. Ju|’hoan hunters use leaf-beetle larvae of Diamphidia Gerstaecker and Polyclada Chevrolat (Chrysomelidae: Galerucinae: Alticini) collected from soil around the host plants Commiphora africana (A. Rich.) Engl. and Commiphora angolensis Engl. (Burseracaeae). In the Nyae Nyae area of Namibia, Ju|’hoan hunters use larvae of Diamphidia nigroornata Ståhl. Larvae and adults live above-ground on the plants and eat leaves, but the San collect the underground cocoons to extract the mature larvae. Larval hemolymph is mixed with saliva and applied to arrows. Hai||om hunters boil the milky plant sap of Adenium bohemianum Schinz (Apocynaceae) to reduce it to a thick paste that is applied to their arrows. The socio-cultural, historical, and ecological contexts of the various San groups may determine differences in the sources and preparation of poisons, bow and arrow technology, hunting behaviors, poison potency, and perhaps antidotes

    I felt this creative project should be a culmination of my education in both graphic design and fine art at Ball State University. To incorporate both of these aspects, I chose to apply my knowledge of typography and design to the printing and bookmaking techniques I have learned in my printmaking classes. After conducting research on general guidelines of typography and layout, I decided to illustrate and define ten basic principles of design. I felt these ten ideals represented the core foundation of most design. A page that demonstrates each principle typographically precedes the definitions.This project has given me a better understanding of the sensitivity of typography.     Meta-Analysis of VTE Risk: Ovarian Cancer Patients by Stage, Histology, Cytoreduction, and Ascites at Diagnosis

    Venous thromboembolisms (VTEs) have been a leading secondary cause of death among ovarian cancer patients, prompting multiple studies of risk factors. The objective of this meta-analysis is to quantify the associations between VTE and the most commonly reported risk factors among ovarian cancer patients. PubMed, Embase, and Cumulative Index to Nursing and Allied Health Literature (CINAHL) were used to identify observational studies. Two reviewers independently abstracted data and assessed quality via the Newcastle–Ottawa tool. A random effects model was used to calculate the pooled odds ratios for VTE with each of the following exposures: advanced cancer stage, clear cell histology, serous histology, ascites at diagnosis, and complete cytoreduction. The I2 and Q tests were used to evaluate heterogeneity. Twenty cohort studies with 6,324 total ovarian cancer patients, 769 of whom experienced a VTE, were included. The odds of VTE in ovarian cancer patients were higher among patients with cancer stage III/IV (versus cancer stage I/II, pooled odds ratio (OR) 2.73; 95% CI 1.84–4.06; I2= 64%), clear cell (versus nonclear cell) histology (OR 2.11; 95% CI 1.55–2.89; I2 = 6%), and ascites (versus no ascites) at diagnosis (OR 2.12; 95% CI 1.51–2.96; I2 = 32%). Serous (versus nonserous) histology (OR 1.26; 95% CI 0.91–1.75; I2 = 42%) and complete (versus incomplete) cytoreduction (OR 1.05; 95% CI 0.27–4.11; I2 = 88%) were not associated with VTE. This meta-analysis quantifies the significantly elevated odds of VTE in ovarian cancer patients with advanced stage at diagnosis, clear cell histology, and ascites at diagnosis. Further studies are needed to account for confounders and inform clinical decision-making tools

    Connect Fred: Enhancing Trail Connectivity through Historic Fredericksburg

    The students of HISP 469: Laboratory in Preservation Planning worked under Dr. Smith to develop new bike and pedestrian paths for historic downtown Fredericksburg in order to improve the wayfinding and connectivity of existing trails. Students researched successful trails in urban settings comparable to Fredericksburg to gain an understanding of the positive and negative qualities of those trails, and how they could be applied or avoided in the new trails created by this project. Research into local and state ordinances regarding signage, bike paths, and similar details was also completed. Using GIS, Survey123, and Google Maps, students conducted extensive surveying of the existing trails and downtown area. The class selected the most ideal pedestrian and bike trails based on the data collected. After these trail paths were tentatively established, the class carried out two interviews with Fredericksburg locals to gain a better understanding of how users interact with the existing infrastructure and how to effectively integrate our plan in a way that benefits the city. For the plan proposed by the students to effectively connect to the existing network of trails, designs for signage that will aid in wayfinding while creating a unified aesthetic for each path were included. With these trails, the class hopes to further improve the walkability of downtown Fredericksburg and increase accessibility to local resources both historic and natural
