    Health ecology: factors of spread and consequences of viral hepatitis manifestation

    The features of viral hepatitis B (HB) and C (HC) in the initial making of these diagnoses in three age groups of men and women (25-30, 31-55 years old for women and 31-60 years old for men, over 55 for women and 60 years old for men) were studied. High levels of bilirubin in the blood serum in all groups indicated damage of the functions of hepatocytes, especially in men with hepatitis B, in women with hepatitis C. The high ALT activity indicated an increase in cytolytic processes in liver hepatocytes in all groups with viral HB and HC, especially in men of the younger group, in women of the younger and middle-aged grou

    Bioactive compounds in blackberry fruits under the conditions of Central Russia

    Background. Blackberry is becoming increasingly popular in Russia for its rapid early-fruiting, high self-fertility and late flowering, contributing to longer consumption of fresh berries, a source of antioxidant compounds. There is a need to study the biochemical composition of blackberries.Materials and methods. Fruits of 25 blackberry cultivars and hybrid seedlings from the Institute of Fruit Crop Breeding (VNIISPK) were analyzed for the contents of catechins, leucoanthocyanins, anthocyanins, and total phenolic compounds using the photometric method, and the content of ascorbic acid using the titrimetric method. Statistical data processing was carried out using the methods of variation and correlation analyses.Results. Bioactive compounds (BAC) accumulating in blackberry fruits were studied: ascorbic acid, catechins, leukoanthocyanins, anthocyanins, and total phenolics, with medium and significant variability of the studied indicators (V > 10%). Among the genotypes, 73% accumulated more than 20.0 mg/100 g of ascorbic acid in fruits. All genotypes were identified as sources of anthocyanins. About 50% of the accessions accumulated catechins in fruits above the average level. Leucoanthocyanins were stored in blackberry fruits in slightly larger amounts than catechins. In 10 accessions the level of phenolics was higher than 1000.0 mg/100 g.Conclusion. Genotypes with high BAC content were identified. Correlation coefficients were obtained: they indicate the presence of an medium degree of rectilinear relationship between the content of ascorbic acid and anthocyanin compounds, ascorbic acid and total phenolics. Cvs. ‘Black Satin’, ‘Cacanska Bestrna’, ‘Chester’, ‘Natchez’ and ‘Triple Crown’, plus hybrid seedlings LN-14, LN-1 and LN-7, were identified according to a set of indicators (ascorbic acid and phenolics) and recommended for breeding to improve the quality of blackberry fruits

    The role of active herpesvirus infection in the formation of atopic dermatitis and psoriasis

    The results of previous studies suggest the involvement of herpes viruses in the development of atopic dermatitis and psoriasis, but there is no convincing evidence. Aim of the study was to evaluate the role of active herpesvirus infection in exacerbation of atopic dermatitis and psoriasis. Material and methods. The analysis of the incidence of infectious mononucleosis, genital herpes, cytomegalovirus infection, skin and subcutaneous tissue diseases, atopic dermatitis, psoriasis in Russia in 2000–2020 was carried out. 92 blood donors and 97 patients with skin and subcutaneous tissue diseases (44 with atopic dermatitis and 53 with psoriasis) were examined for the presence of immunoglobulins M and G to herpes simplex viruses (HSV1,2), Epstein-Barr (EBV), cytomegalovirus (CMV), human herpes type 6 (HНV6). Results and discussion. A significant strong direct correlation was revealed between the incidence of genital herpes and diseases of the skin and subcutaneous tissue (p = 0.85), atopic dermatitis (p = 0.85); infectious mononucleosis and psoriasis (p = 0.85). The frequency of detection of IgM to HSV1,2, in total, IgG EA and IgM VCA EBV in the group of patients is significantly higher than in donors (p < 0.05). In patients with atopic dermatitis, IgM to HSV1 were detected significantly more often than in individuals with psoriasis, and markers of active EBV infection were significantly less common. The presence of IgM HSV-1 statistically significantly increases the incidence of atopic dermatitis (relative risk (RR) = 2.3, 95 % confidence interval (95 % CI) 1.6–3.3)), IgM VCA and IgG EA EBV – the incidence of psoriasis (RR = 2.3, 95 % CI 1.5–3.3). Conclusions. It has been shown for the first time that active HSV1,2 infection is a trigger factor for the development of atopic dermatitis, EBV infection is psoriasis

    There is no standard care for metastatic renal cell carcinoma (MRCC). High and intermediate IL-2 regimens are difficult to recommend due to their high toxicity. A combination of low-dose cytokines is assumed to be effective and safe in MRCC patients. A prospective randomized study was started in 2003.Methods: The eligibility criteria included histopathologically confirmed MRCC, ECOG PS 0—2, no autoimmune diseases, no brain metastases, and normal organ function. All patients were randomized in three arms: IL-2 alone, 1.5 MIU, iv, t.i.w., weeks 1—3 or IL-2 1.0 MIU, iv, t.i.w., weeks 1—3 plus IFN 5 MIU, sc, t.i.w, weeks 1—3 or biochemotherapy group 5-FU, 500 mg/m 2 , iv, once a week, weeks 1—3 plus IL-2 1.0 MIU, iv, t.i.w., weeks 1—3 plus IFN 5 MIU, sc, t.i.w., weeks 1—3. Cycles were repeated every three weeks. A response was evaluated according to RECIST every 2 cycles.Results: 64 patients were enrolled of whom 63 were analyzed. The patients’ median age was 55.4 years (range 16—74). 55.6% of the patients had a poor prognosis (as described by Motzer et al., 2002). Bone metastases were present in 52.4%. Sixteen patients treated with IL-2 alone showed no CR, no PRs, 2 SDs, and 14 PDs. Of the 23 patients in IL-2+IFN group, 5 PRs, 8 SDs, and 10 PDs were observed, with a response rate of 21.7%. Amongst the 24 patients in 5-FU+IL-2+IFN group, 1 CR, 3 PRs, 10 SDs, and 10 PDs were shown, with a response rate of 16.7%. One-year survival was 20.0%, 81.3% and 81.0%, respectively. Flu-like syndrome was the most common side effect in the patients receiving IFN (89.1%, grade 1, CTC). Hypotension associated with IL-2 (all groups) was seen in 56.3% (grade 1 in 50% and grade 2 in 6.3%). The other toxicities were 12.7% grade 1 neutropenia and vomiting in 4.7% (group 3).Conclusions: Small-dose IL-2 in combination with IFN and 5-FU in MRCC is feasible, tolerable, with the efficacy comparable to other more toxic therapeutic modalities.

    New V-shaped 2,4-di(hetero)arylpyrimidine push-pull systems: Synthesis, solvatochromism and sensitivity towards nitroaromatic compounds

    Novel D–π–A–π–D type pyrimidine-based dyes, possessing (hetero)aryl electron-donating groups in 2,4-positions were synthesized, and their photophysical properties were investigated by using absorption and emission spectral analyses. All dyes proved to exhibit a strong emission solvatochromism with quantum yields up to 0.96, depending on their molecular structure and solvent polarity. The compounds have been established to undergo a reversible protonation, directed at nitrogen atoms of the pyrimidine ring, and these phenomena are associated with dramatic color changes. In addition, fluorophores 5a-d show a high sensitive response for nitroaromatic traces in solutions or real-time detection of their vapors in air.     Динамика выживаемости больных муковисцидозом в Москве и Московской области за периоды 1992–2001 и 2002–2011 гг.

    Summary. The aim of this study was to evaluate changes in survival of patients with cystic fibrosis (CF) in Moscow and Moscow region and to determine factors influencing the survival. We analyzed outpatient medical records of patients followed-up in Moscow CF centers by the 1st of January, 2002, and by the 1st of January, 2012.Median survival for 2002–2012 was 37.2 years; this was significantly higher that the similar value for the previous decade (25.9 years). A total number of CF patients was more than doubled due to increased survival and improved diagnosis of the disease.Improved survival was due to improved work of CF centers and to implementation of effective medications, primarily dornase alfa (Pulmozyme) and some inhaled and systemic antibiotics and macrolides in subinhibitory concentrations, in routine clinical practice. Gram-negative infection, especially Burkholderia cepacia, was shown to decrease survival in CF patients in contrary to "mild" mutations that are better prognostic factors.Резюме. Целью работы явилось определение динамики числа и выживаемости больных муковисцидозом (МВ) в Москве и Московской области, а также определение факторов, влияющих на выживаемость. Были проанализированы амбулаторные карты больных, состоящих на учете в московских центрах муковисцидоза на 01.01.2002 и 01.01.2012.Медиана выживаемости больных за периоды 2002–2011 гг. составила 37,2 года, что достоверно выше, чем аналогичный показатель за предыдущее 10-летие – 25,9 года. Отмечено более чем 2-кратное увеличение общего количества больных, обусловленное ростом выживаемости и улучшением диагностики.Увеличение выживаемости обусловлено оптимизацией работы центров МВ и началом рутинного применения различных эффективных медикаментозных препаратов, прежде всего дорназы альфа (Пульмозим), а также некоторых ингаляционных и системных антибиотиков, макролидов в субингибирующих концентрациях. Показано, что инфицирование грамотрицательной инфекцией, особенно Burkholderia cepacia, снижает выживаемость пациентов с МВ, а наличие "мягкого" генотипа является более благоприятным прогностическим фактором

    Spatial, temporal, and demographic patterns in prevalence of smoking tobacco use and attributable disease burden in 204 countries and territories, 1990-2019 : a systematic analysis from the Global Burden of Disease Study 2019

    Background Ending the global tobacco epidemic is a defining challenge in global health. Timely and comprehensive estimates of the prevalence of smoking tobacco use and attributable disease burden are needed to guide tobacco control efforts nationally and globally. Methods We estimated the prevalence of smoking tobacco use and attributable disease burden for 204 countries and territories, by age and sex, from 1990 to 2019 as part of the Global Burden of Diseases, Injuries, and Risk Factors Study. We modelled multiple smoking-related indicators from 3625 nationally representative surveys. We completed systematic reviews and did Bayesian meta-regressions for 36 causally linked health outcomes to estimate non-linear dose-response risk curves for current and former smokers. We used a direct estimation approach to estimate attributable burden, providing more comprehensive estimates of the health effects of smoking than previously available. Findings Globally in 2019, 1.14 billion (95% uncertainty interval 1.13-1.16) individuals were current smokers, who consumed 7.41 trillion (7.11-7.74) cigarette-equivalents of tobacco in 2019. Although prevalence of smoking had decreased significantly since 1990 among both males (27.5% [26. 5-28.5] reduction) and females (37.7% [35.4-39.9] reduction) aged 15 years and older, population growth has led to a significant increase in the total number of smokers from 0.99 billion (0.98-1.00) in 1990. Globally in 2019, smoking tobacco use accounted for 7.69 million (7.16-8.20) deaths and 200 million (185-214) disability-adjusted life-years, and was the leading risk factor for death among males (20.2% [19.3-21.1] of male deaths). 6.68 million [86.9%] of 7.69 million deaths attributable to smoking tobacco use were among current smokers. Interpretation In the absence of intervention, the annual toll of 7.69 million deaths and 200 million disability-adjusted life-years attributable to smoking will increase over the coming decades. Substantial progress in reducing the prevalence of smoking tobacco use has been observed in countries from all regions and at all stages of development, but a large implementation gap remains for tobacco control. Countries have a dear and urgent opportunity to pass strong, evidence-based policies to accelerate reductions in the prevalence of smoking and reap massive health benefits for their citizens.     This paper presents the study multiplier effects of government regulation of industrial development of the macro-region by the example of the Russian Arctic (Russian Arctic), made in the process of writing a doctoral dissertation Lapochkin LV on «Industrial policy of the Arctic zone of Russia: theoretical and methodological basis» and its discussion in the St. Petersburg State University of Economics. The novelty of the study is: first, in the view of the Russian Arctic as a macro-region; secondly, forming a conceptual model of a multiplier based on the criterion of national economic efficiency and system constraints, reflecting the direction of the structural industrial policy with regard to the development of non-oil economy; in the third, the formulation of multiplier effects in the fields and methods of exposure.
