Late noninvasive evaluation of cardiac performance in mildly symptomatic older patients with Ebstein's anomaly of tricuspid valve: Role of radionuclide imaging
AbstractTen patients 8 to 54 years of age with isolated Ebstein's anomaly of the tricuspid valve were evaluated by electrocardiography, maximal exercise treadmill testing, 24 h electrocardiographs (ECG) monitoring, echocardiography and rest radionuclide imaging of the left ventricle. The patients presented after the 1st year of life and had not undergone surgical intervention. All except one were in functional class II. No patient had preexcitation on the surface ECG, but abnormal tachyarrhythmias or bradyarrhythmias were seen in five patients on 24 h ECG monitoring. Subnormal exercise performance was observed in five patients.Echocardiography demonstrated typical variable tricuspid valve displacement and paradoxic interventricular septal motion. Left ventricular end-diastolic dimensions were normal in all patients, but posterior wall motion was reduced in two. Moderate to severe tricuspid regurgitation with a Doppler jet velocity <2.5 m/s was demonstrated in eight patients. Left ventricular radionuclide scintigraphy revealed a subnormal ejection fraction (<50%) in 5 of 10 patients; these 5 had previously shown suboptimal exercise performance.The two youngest patients (<15 years) had no arrhythmia, normal exercise performance and normal left ventricular ejection fraction. There was no correlation between the degree of tricuspid valve displacement or régurgitation and the presence of rhythm disturbance, exercise performance or radionuclide left ventricular function.Late evaluation of patients with Ebstein's anomaly may demonstrate significant unsuspected abnormalities in cardiac rhythm, exercise performance and left ventricular function. Radionuclide scintigraphy is a useful noninvasive technique for assessing left ventricular dysfunction in these patients
Assessing cognitive dysfunction in Parkinson's disease: An online tool to detect visuo-perceptual deficits.
BackgroundPeople with Parkinson's disease (PD) who develop visuo-perceptual deficits are at higher risk of dementia, but we lack tests that detect subtle visuo-perceptual deficits and can be performed by untrained personnel. Hallucinations are associated with cognitive impairment and typically involve perception of complex objects. Changes in object perception may therefore be a sensitive marker of visuo-perceptual deficits in PD.ObjectiveWe developed an online platform to test visuo-perceptual function. We hypothesised that (1) visuo-perceptual deficits in PD could be detected using online tests, (2) object perception would be preferentially affected, and (3) these deficits would be caused by changes in perception rather than response bias.MethodsWe assessed 91 people with PD and 275 controls. Performance was compared using classical frequentist statistics. We then fitted a hierarchical Bayesian signal detection theory model to a subset of tasks.ResultsPeople with PD were worse than controls at object recognition, showing no deficits in other visuo-perceptual tests. Specifically, they were worse at identifying skewed images (P < .0001); at detecting hidden objects (P = .0039); at identifying objects in peripheral vision (P < .0001); and at detecting biological motion (P = .0065). In contrast, people with PD were not worse at mental rotation or subjective size perception. Using signal detection modelling, we found this effect was driven by change in perceptual sensitivity rather than response bias.ConclusionsOnline tests can detect visuo-perceptual deficits in people with PD, with object recognition particularly affected. Ultimately, visuo-perceptual tests may be developed to identify at-risk patients for clinical trials to slow PD dementia. © 2018 The Authors. Some doubts on the validity of the foreground Galactic contribution subtraction from microwave anisotropies
The Galactic foreground contamination in CMBR anisotropies, especially from
the dust component, is not easily separable from the cosmological or
extragalactic component. In this paper, some doubts will be raised concerning
the validity of the methods used to date to remove Galactic dust emission in
order to show that none of them achieves its goal.
First, I review the recent bibliography on the topic and discuss critically
the methods of foreground subtraction: the cross-correlation with templates,
analysis assuming the spectral shape of the Galactic components, the "maximum
entropy method", "internal linear combination", and "wavelet-based high
resolution fitting of internal templates". Second, I analyse the galactic
latitude dependence from WMAP data. The frequency dependence is discussed with
the data in the available literature. The result is that all methods of
subtracting the Galactic contamination are inaccurate. The galactic latitude
dependence analysis or the frequency dependence of the anisotropies in the
range 50-250 GHz put a constraint on the maximum Galactic contribution in the
power spectrum to be less than a ~10% (68% C. L.) for a ~1 degree scale, and
possibly higher for larger scales.
The origin of most of the signal in the CMBR anisotropies is not Galactic. In
any case, the subtraction of the Galaxy is not accurate enough to allow a
"precision Cosmology"; other sources of contamination (extragalactic, solar

Adapting agricultural water use to climate change in a post-Soviet context: challenges and opportunities in southeast Kazakhstan
The convergence of climate change and post-Soviet
socio-economic and institutional transformations has been
underexplored so far, as have the consequences of such convergence on crop agriculture in Central Asia. This paper provides a place-based analysis of constraints and opportunities for adaptation to climate change, with a specific focus on water use, in two districts in southeast Kazakhstan. Data were collected by 2 multi-stakeholder participatory workshops, 21 semi-structured in-depth interviews, and secondary statistical data. The present-day agricultural system is characterised by
enduring Soviet-era management structures, but without state inputs that previously sustained agricultural productivity. Low margins of profitability on many privatised farms mean that attempts to implement integrated water management have produced water users associations unable to maintain and upgrade a deteriorating irrigation infrastructure. Although actors
engage in tactical adaptation measures, necessary structural adaptation of the irrigation system remains difficult without significant public or private investments. Market-based water management models have been translated ambiguously to this region, which fails to encourage efficient water use and hinders adaptation to water stress. In addition, a mutual interdependence of informal networks and formal institutions characterises both state governance and everyday life in Kazakhstan. Such interdependence simultaneously facilitates
operational and tactical adaptation, but hinders structural adaptation, as informal networks exist as a parallel system that achieves substantive outcomes while perpetuating the inertia and incapacity of the state bureaucracy. This article has relevance for critical understanding of integrated water management in practice and adaptation to climate change in post-Soviet institutional settings more broadly
The Afterglows of Swift-era Gamma-Ray Bursts II.: Type I GRB versus Type II GRB Optical Afterglows
We use a large sample of GRB afterglow and prompt-emission data (adding
further GRB afterglow observations in this work) to compare the optical
afterglows (or the lack thereof) of Type I GRBs with those of Type II GRBs. In
comparison to the afterglows of Type II GRBs, we find that those of Type I GRBs
have a lower average luminosity and show an intrinsic spread of luminosities at
least as wide. From late and deep upper limits on the optical transients, we
establish limits on the maximum optical luminosity of any associated supernova,
confirming older works and adding new results. We use deep upper limits on Type
I GRB optical afterglows to constrain the parameter space of possible mini-SN
emission associated with a compact-object merger. Using the prompt emission
data, we search for correlations between the parameters of the prompt emission
and the late optical afterglow luminosities. We find tentative correlations
between the bolometric isotropic energy release and the optical afterglow
luminosity at a fixed time after trigger (positive), and between the host
offset and the luminosity (negative), but no significant correlation between
the isotropic energy release and the duration of the GRBs. We also discuss
three anomalous GRBs, GRB 060505, GRB 060614, and GRB 060121, in the light of

Design of CubeSats for Formation Flying & for Extreme Low Earth Orbit
This project handled the design and analysis of the attitude determination and control, command and data handling, and structural subsystems of CubeSats designed for two distinct missions. One mission was to fly a CubeSat in extreme low earth orbit, while the other was to fly two CubeSats in formation. Attitude sensors and an onboard computer selected in previous years' designs were reviewed and updated. Structural analysis simulations were conducted in ANSYS. Additionally, the test bed for an attitude determination and control system experiment designed and constructed by the students in MAD-1701 was improved upon, and an attitude control system was designed. Finally, recommendations were provided for future teams looking to further develop this experimental setup
