    Producción CientíficaEs ya un lugar común y está social y políticamente asumido desde hace algunos años, que Valladolid se enfrenta a grandes retos en la primera década del siglo XXI que pueden convertirla en una de las ciudades españolas con más expectativas de crecimiento y desarrollo. Si supera estos retos, habrá sabido poner en valor entonces su localización, su posición estratégica en la red de transportes, su tradición industrial, el rango de capital regional y la capacidad para la prestación de servicios avanzados en el ámbito de la automoción, las telecomunicaciones y la investigación I+D+I. Las últimas decisiones estratégicas en materia de infraestructuras de transporte y comunicación que han afectado a la ciudad, dibujan un horizonte ciertamente alentador que viene a reforzar el carácter estratégico de su privilegiada situación. Ello será sin duda preludio de un incremento de actividad económica –más industria, más terciario-servicios- cuyo emplazamiento y relación con otros usos hay que ordenar de forma adecuada. Las DOTVAENT y el avance de la revisión y adaptación del PGOU hacen a este respecto un diagnóstico correcto de la situación: identifican las áreas estratégicas, califican el suelo por áreas de actividad y anticipan una estructura somera de interconexión a través de las nuevas infraestructuras que habrán de inaugurarse en los próximos años.Departamento de Geografí

    Prevention of Periprosthetic Joint Infection in Total Hip and Knee Replacement:One European Consensus

    Periprosthetic joint infection (PJI) is a devastating complication in total hip and knee replacement. Its prevention is key to decrease the incidence and avoid some consequences that seriously impact patients and health systems. In view of the variety of recommendations and guidelines, we decided to conduct an expert, peer-reviewed European consensus analysis about the pre-, intra-, and postoperative prevention of PJI. A multinational group of practicing orthopedic experts developed a series of 47 consensus statements in 6 main groups of intervention, and a 2-stage Delphi approach was launched with a threshold for agreement at 75% and for very high agreement at more than 90%. A total of 306 orthopedic surgeon responses were gathered from 9 countries. Consensus was reached for 42/47 statements, 31/47 of which achieved a very high consensus. Many preoperative actions gathered strong consensus, although areas like the use of alcoholic chlorhexidine or the timing of hair removal did not attain strong consensus, despite available evidence. Intra- and postoperative actions showed more variability regarding incise drapes, skin suturing techniques, and wound follow-up. This study confirms an important consensus among orthopedic surgeons across Europe in many areas well known to contribute to the prevention of PJI; however, there are still grounds for improvement

    Arcilla bentonítica cubana: composición, estructura y caracterización textural

    The physic and chemical characterization of the bentonite clay from the Chiqui Gómez deposit, central Cuba, (Cuban bentonite clay) shows that it is mainly constituted by sodium montmorillonite (>90%), with a structural formula for one-layer unit determined as (Na3.99Al0.01)(Al1.11Fe3+0.49Mg0.18Ti0.07)(Ca0.24Na0.15K0.01)O10(OH)2. The Cuban bentonite clay has a specific surface area of 80m2g-1, a pore volume of about 0.0776 cm3g-1, 61 % porosity and both N2 adsorption?desorption isotherms exhibited a hysteresis loop of IV type. The thermogravimetric analyses (TGA) of studied mineral presents the first endothermic peak, characteristic of montmorillonite, in 48.1 ºC and others less accentuated (80.8, 94.0, 119.8 ºC) characteristic of sodium montmorillonite, that corresponds to the loss of water, and can be extended up to 250 ºC. The FT-IR spectra showed the existence of Si-OH, Al-Al-OH, Al-Fe-OH, Al-Mg-OH and Si-O-Si functional groups in all clay samples, bands between 1120 and 461 cm-1 correspond to phyllosilicate structures and OH stretching vibrations were observed. pH at the point of zero charge (pHPZC) obtained has a value of 8.1, which allows montmorillonite to be classified as basic.La caracterización física y química de la arcilla bentonítica del depósito Chiqui Gómez, en el centro de Cuba, muestra que está constituida mayoritariamente (>90%) por montmorillonita sódica, con una formula estructural para una capa unitaria establecida como (Na3.99Al0.01)(Al1.11Fe3+ 0.49Mg0.18Ti0.07)(Ca0.24Na0.15K0.01)O10(OH)2. Las arcillas de bentonita cubana tienen una superficie específica de 80 m2 g -1 , un volumen de poro de aproximadamente 0,0776 cm3 g -1 y una porosidad de 61%. Ambas isotermas de desorción-desorción de N2 exhibieron una isoterma de histéresis de tipo IV. El análisis TGA muestra que la bentonita estudiada presenta el primer pico endotérmico, característico de la montmorillonita, en 48,1 ºC y otros menos acentuados (80.8, 94.0, 119.8 ºC) característicos de la montmorillonita sódica, que corresponde a la pérdida de agua, pudiendo extenderse hasta 250 ºC. Los espectros FT-IR mostraron la existencia de grupos funcionales Si-OH, Al-Al-OH, Al-Fe-OH, Al-Mg-OH y Si-OSi en todas las muestras de arcilla, las bandas entre 1120 y 461 cm-1 corresponden a estructuras de filosilicatos y se observaron vibraciones de estiramiento OH. El pH en el punto de carga cero (pHPZC) obtenido tiene un valor de 8.1, lo que permite clasificar la montmorillonita como básica.Fil: Quero Jiménez, Pedro César.     [EN] The incidence of prosthetic joint infection (PJI) is expected to increase in the coming years. PJI has serious consequences for patients, and high costs for the health system. The complexity of these infections makes it necessary to organize the vast quantity of information published in the last several years. The indications for the choice of a given surgical strategy and the corresponding antimicrobial therapy are specifically reviewed. The authors selected clinically relevant questions and then reviewed the available literature in order to give recommendations according to a pre-determined level of scientific evidence. The more controversial aspects were debated, and the final composition was agreed at an ad hoc meeting. Before its final publication, the manuscript was made available online in order that all SEIMC members were able to read it and make comments and suggestions.[ES] Se prevé un incremento de la incidencia de infección de las prótesis articulares (IPA) en los próximos años. Las IPA plantean graves consecuencias para los pacientes y un alto coste el sistema sanitario. La complejidad de estas infecciones hace que sea necesario organizar la inmensa cantidad de información publicada en los últimos años. En estas guías se revisan específicamente las indicaciones para la elección de una estrategia quirúrgica dada y el tratamiento antimicrobiano correspondiente. Los autores seleccionaron las preguntas clínicamente relevantes y revisaron la literatura disponible con el fin de proporcionar recomendaciones de acuerdo con un grado de evidencia científica predeterminada. Los aspectos más controvertidos fueron debatidos y la redacción final se acordó en una reunión ad hoc.     The 13th Data Release of the Sloan Digital Sky Survey: First Spectroscopic Data from the SDSS-IV Survey Mapping Nearby Galaxies at Apache Point Observatory

    The fourth generation of the Sloan Digital Sky Survey (SDSS-IV) began observations in July 2014. It pursues three core programs: APOGEE-2,MaNGA, and eBOSS. In addition, eBOSS contains two major subprograms: TDSS and SPIDERS. This paper describes the first data release from SDSS-IV, Data Release 13 (DR13), which contains new data, reanalysis of existing data sets and, like all SDSS data releases, is inclusive of previously released data. DR13 makes publicly available 1390 spatially resolved integral field unit observations of nearby galaxies from MaNGA,the first data released from this survey. It includes new observations from eBOSS, completing SEQUELS. In addition to targeting galaxies and quasars, SEQUELS also targeted variability-selected objects from TDSS and X-ray selected objects from SPIDERS. DR13 includes new reductions ofthe SDSS-III BOSS data, improving the spectrophotometric calibration and redshift classification. DR13 releases new reductions of the APOGEE-1data from SDSS-III, with abundances of elements not previously included and improved stellar parameters for dwarf stars and cooler stars. For the SDSS imaging data, DR13 provides new, more robust and precise photometric calibrations. Several value-added catalogs are being released in tandem with DR13, in particular target catalogs relevant for eBOSS, TDSS, and SPIDERS, and an updated red-clump catalog for APOGEE.This paper describes the location and format of the data now publicly available, as well as providing references to the important technical papers that describe the targeting, observing, and data reduction. The SDSS website, http://www.sdss.org, provides links to the data, tutorials and examples of data access, and extensive documentation of the reduction and analysis procedures.     Analysis of the Risk of Wear on Cemented and Uncemented Polyethylene Liners According to Different Variables in Hip Arthroplasty

    Wear debris in total hip arthroplasty is one of the main causes of loosening and failure, and the optimal acetabular fixation for primary total hip arthroplasty is still controversial because there is no significant difference between cemented and uncemented types for long-term clinical and functional outcome. To assess and predict, from a theoretical viewpoint, the risk of wear with two types of polyethylene liners, cemented and uncemented, a simulation using the finite element (FE) method was carried out. The risk of wear was analyzed according to different variables: the polyethylene acetabular component’s position with respect to the center of rotation of the hip; the thickness of the polyethylene insert; the material of the femoral head; and the relationship of the cervical–diaphyseal morphology of the proximal end of the femur to the restoration of the femoral offset. In all 72 simulations studied, a difference was observed in favour of a cemented solution with respect to the risk of wear. With regard to the other variables, the acetabular fixation, the thickness of the polyethylene, and the acetabular component positioning were statistically significant. The highest values for the risk of wear corresponded to a smaller thickness (5.3 mm), and super-lateral positioning at 25 mm reached the highest value of the von Mises stress. According to our results, for the reconstruction of the acetabular side, a cemented insert with a thickness of at least 5 mm should be used at the center of rotation

    Decrease of Quality of Life, Functional Assessment and Associated Psychological Distress in Patients with Hypoallergenic Total Knee Arthroplasty

    Total knee arthroplasty (TKA) is the final treatment for knee osteoarthritis, and 15–30% of patients show little or no improvement. This high percentage is related to aspects of the surgical technique, the selected implant, and specific patient characteristics. The aim of this study was to analyze whether there are differences in quality of life (QoL) and functional capacity among patients undergoing TKA with conventional implants compared to those treated with hypoallergenic oxinium implants. A pragmatic clinical study was carried out that included patients who underwent TKA between January 2013 and December 2015. During this period, 245 knees in 228 patients were treated. Eleven patients were excluded, leaving a sample of 161 conventionally treated knees, 72 knees treated with hypoallergenic implants, and one patient who received both implant types. In all patients, QoL and functional capacity were measured with the WOMAC index, the SF-12 questionnaire, and the Euro-Qol-5D L-VAS. We also assessed the psychological distress of each patient and related the findings to the functional results. The differences in QoL were tested using ANCOVA and propensity score matching (PSM) models adjusted for sex, age, weight, psychiatric history and associated complications. Patients who underwent TKA using conventional prostheses had significantly better scores on the total WOMAC index and in the pain domain (p < 0.05) than those who received hypoallergenic prostheses, but no significant differences were observed for the other domains in the ANCOVA. In contrast, with the PSM, we also found statistically significant differences in the difficulty domain of the WOMAC. Significant differences were found for the SF-12 mental health questionnaire results (p = 0.038), but the same did not occur for the physical health domain in the ANCOVA and PSM. We also found statistically significant differences in the Euro-Qol-5D index results (p = 0.041), but not in the VAS scale scores for the same questionnaire in the ANCOVA, and we did not find significant differences in either with the PSM. Patients with metal allergies and those who present psychological distress had WOMAC, SF-12, and Euro-Qol-5D results that were statistically significantly worse than those of patients who received conventional implants. Patients who underwent hypoallergic TKA had lower scores on the QoL and functional capacity scales than patients who received conventional Cr–Co implants. Additionally, patients with psychological distress had worse results on the questionnaires, and those with a metal allergy had even lower scores; the differences were statistically significant

    Wear Risk Prevention and Reduction in Total Hip Arthroplasty. A Personalized Study Comparing Cement and Cementless Fixation Techniques Employing Finite Element Analysis

    The wear rate on Total Hip Arthroplasty (THA) entails a heavy burden for patients. This becomes more relevant with increased wear risk and its consequences such as osteolysis. In addition, osteolysis has been described in cemented and uncemented acetabular implants, and nowadays, controversy remains as to whether or not to cement the acetabular component. A personalized theoretical study was carried out to investigate which parameters have an influence on wear risk and to determine the best fixation method. Liner wear risk was assessed for two different types of fixation (cemented vs uncemented) through Finite Elements Analysis (FEA). The intraoperative variables used to determine the wear risk (cervical-diaphyseal angle, Center of Rotation positioning -COR-, head material, head size, and liner thickness) are vital parameters in surgical planning. Two types of tridimensional liner models of Ultra High Molecular Weight Polyethene (UHMWPE) were simulated through finite element analysis (FEA—over 216 cases were the core of this research). A significant relationship was found between the cervical-diaphyseal angle and wear risk (p < 0.0001), especially in valgus morphology. The acetabular fixation technique (p < 0.0001) and liner thickness (p < 0.0001) showed a significant relationship with wear risk. According to our study, using a cemented fixation with a thick liner in the right center of rotation appears to be the proper stratagy for preventing polyethylene liner wear
