    Utilização da caderneta de saúde da pessoa idosa na atenção primária: revisão integrativa / Use of the child's health chair in primary care: integrative review

    Analisar as evidências disponíveis na literatura sobre a utilização da Caderneta da Saúde da Pessoa Idosa (CSPI) na atenção primária. Metodologia: Revisão integrativa, realizada nas bases de dados BVS, Scielo e Google Acadêmico. Os critérios de inclusão foram: artigos em língua portuguesa, publicados nos últimos 10 anos, disponíveis na íntegra. Os descritores foram “Idoso”, “Caderneta de Saúde da Pessoa Idosa” e “Atenção primária à Saúde”. Selecionaram-se 07 artigos. A busca compreendeu o período de novembro de 2020. Resultado e Discussão: Constatou-se que 57% artigos eram com abordagem quantitativa. Foram estabelecidas duas categorias temáticas, a saber: a) Caderneta de saúde da pessoa idosa: ferramenta de cuidado e gestão; b) Entraves na implementação da caderneta. Conclusão: Conclui-se que há dificuldades na implementação da caderneta de saúde da pessoa idosa na Atenção Primária à Saúde. Tal fato se dá pela alta demanda de tarefas que os profissionais necessitam cumprir, causando-os desinteresse, pois os mesmos têm uma visão de que esse processo exigirá muito trabalho, mesmo sabendo da importância que a mesma tem no processo de saúde desses indivíduos

    Viral genetic clustering and transmission dynamics of the 2022 mpox outbreak in Portugal

    Pathogen genome sequencing during epidemics enhances our ability to identify and understand suspected clusters and investigate their relationships. Here, we combine genomic and epidemiological data of the 2022 mpox outbreak to better understand early viral spread, diversification and transmission dynamics. By sequencing 52% of the confirmed cases in Portugal, we identified the mpox virus sublineages with the highest impact on case numbers and fitted them into a global context, finding evidence that several international sublineages probably emerged or spread early in Portugal. We estimated a 62% infection reporting rate and that 1.3% of the population of men who have sex with men in Portugal were infected. We infer the critical role played by sexual networks and superspreader gatherings, such as sauna attendance, in the dissemination of mpox virus. Overall, our findings highlight genomic epidemiology as a tool for the real-time monitoring and control of mpox epidemics, and can guide future vaccine policy in a highly susceptible population.info:eu-repo/semantics/publishedVersio

    A picture of medically assisted reproduction activities during the COVID-19 pandemic in Europe

    STUDY QUESTION: How did coronavirus disease 2019 (COVID-19) impact on medically assisted reproduction (MAR) services in Europe during the COVID-19 pandemic (March to May 2020)? SUMMARY ANSWER: MAR services, and hence treatments for infertile couples, were stopped in most European countries for a mean of 7 weeks. WHAT IS KNOWN ALREADY: With the outbreak of COVID-19 in Europe, non-urgent medical care was reduced by local authorities to preserve health resources and maintain social distancing. Furthermore, ESHRE and other societies recommended to postpone ART pregnancies as of 14 March 2020. STUDY DESIGN, SIZE, DURATION: A structured questionnaire was distributed in April among the ESHRE Committee of National Representatives, followed by further information collection through email. PARTICIPANTS/MATERIALS, SETTING, METHODS: The information was collected through the questionnaire and afterwards summarised and aligned with data from the European Centre for Disease Control on the number of COVID-19 cases per country. MAIN RESULTS AND THE ROLE OF CHANCE: By aligning the data for each country with respective epidemiological data, we show a large variation in the time and the phase in the epidemic in the curve when MAR/ART treatments were suspended and restarted. Similarly, the duration of interruption varied. Fertility preservation treatments and patient supportive care for patients remained available during the pandemic. LARGE SCALE DATA: N/A. LIMITATIONS, REASONS FOR CAUTION: Data collection was prone to misinterpretation of the questions and replies, and required further follow-up to check the accuracy. Some representatives reported that they, themselves, were not always aware of the situation throughout the country or reported difficulties with providing single generalised replies, for instance when there were regional differences within their country. WIDER IMPLICATIONS OF THE FINDINGS: The current article provides a basis for further research of the different strategies developed in response to the COVID-19 crisis. Such conclusions will be invaluable for health authorities and healthcare professionals with respect to future similar situations.peer-reviewe

    Reducing the environmental impact of surgery on a global scale: systematic review and co-prioritization with healthcare workers in 132 countries

    Abstract Background Healthcare cannot achieve net-zero carbon without addressing operating theatres. The aim of this study was to prioritize feasible interventions to reduce the environmental impact of operating theatres. Methods This study adopted a four-phase Delphi consensus co-prioritization methodology. In phase 1, a systematic review of published interventions and global consultation of perioperative healthcare professionals were used to longlist interventions. In phase 2, iterative thematic analysis consolidated comparable interventions into a shortlist. In phase 3, the shortlist was co-prioritized based on patient and clinician views on acceptability, feasibility, and safety. In phase 4, ranked lists of interventions were presented by their relevance to high-income countries and low–middle-income countries. Results In phase 1, 43 interventions were identified, which had low uptake in practice according to 3042 professionals globally. In phase 2, a shortlist of 15 intervention domains was generated. In phase 3, interventions were deemed acceptable for more than 90 per cent of patients except for reducing general anaesthesia (84 per cent) and re-sterilization of ‘single-use’ consumables (86 per cent). In phase 4, the top three shortlisted interventions for high-income countries were: introducing recycling; reducing use of anaesthetic gases; and appropriate clinical waste processing. In phase 4, the top three shortlisted interventions for low–middle-income countries were: introducing reusable surgical devices; reducing use of consumables; and reducing the use of general anaesthesia. Conclusion This is a step toward environmentally sustainable operating environments with actionable interventions applicable to both high– and low–middle–income countries

    A mastocitose é doença rara, definida como a infiltração desordenada de mastócitos em vários tecidos. Compreende diferentes quadros clínicos, variando de formas cutâneas indolentes a condições sistêmicas e malignas. O termo mastocitose bolhosa descreve a variedade na qual a lesão cutânea bolhosa é o aspecto predominante e geralmente indica casos de mastocitose cutânea. É mais comum na criança e possui prognóstico mais reservado. Este relato de caso enfatiza a diversidade de suas manifestações clínicas e as complicações decorrentes.<br>Mastocytosis is a rare disease, defined as a disorderly infiltration with mast cells in several tissues. It comprises many different clinical situations varying from indolent cutaneous forms to malignant and systemic conditions. The term bullous mastocytosis describes all the variety in which the cutaneous bullae represent the predominant feature and generally indicates cases of diffuse cutaneous mastocytosis. Its presence is more common during the childhood and has a more reserved prognosis. This case report emphasizes the extreme diversity of its clinical manifestations and complications

    SARS-CoV-2 in children with cancer in Brazil : results of a multicenter national registry

    Background: Strategies to mitigate the impact of COVID-19 in special populations are complex and challenging. Few studies have addressed the impact of COVID-19 on pediatric patients with cancer in low- and middle-income countries. Methods: Multicenter observational cohort study with prospective records and retrospective analyses starting in April 2020 in 21 pediatric oncology centers distributed throughout Brazil. Participants: Patients under 18 years of age who are infected by the SARS-CoV-2 virus (confirmed diagnosis through reverse transcriptase-polymerase chain reaction [RT-PCR]) while under treatment at pediatric oncology centers. The variables of interest included clinical symptoms, diagnostic and therapeutic measures. The repercussions of SARS-CoV-2 infection on cancer treatment and general prognosis were monitored. Results: One hundred seventy-nine patients were included (median age 6 [4–13] years, 58% male). Of these, 55.9% had acute leukemia and 34.1% had solid tumors. The presence of SARS-CoV-2 was diagnosed by RT-PCR. Various laboratory markers were analyzed, but showed no correlation with outcome. Children with low or high BMI for age had lower overall survival (71.4% and 82.6%, respectively) than those with ageappropriate BMI (92.7%) (p = .007). The severity of presentation at diagnosis was significantly associated with outcome (p < .001). Overall mortality in the presence of infection was 12.3% (n = 22). Conclusion: In children with cancer and COVID-19, lower BMI was associated with worse prognosis. The mortality in this group of patients (12.3%) was significantly higher than that described in the pediatric population overall (~1%)
