    Assessment and management of oral health in liver transplant candidates

    Liver transplantation has become a standard treatment for end-stage liver disease and the number of recipients has grown rapidly in the last few years. Dental care during pre-transplant workup is important to reduce potential sources of infection in the drug-induced immunosuppression phase of liver transplantation. Objectives: The objectives of this study were to document the prevalence of oral abnormalities in patients on a liver transplant waiting list presenting to an urban dental school clinic, discuss the appropriate dental treatment according their systemic conditions and compare their oral manifestations with those of healthy individuals. Material and Methods: A pilot study was conducted involving 16 end-stage liver disease individuals (study group- SG) attending the Special Care Dentistry Center of the University of So Paulo and 16 control individuals (control group- CG) with no liver diseases, receiving dental care at the Dental School of the University of So Paulo. These individuals were assessed for their dental status (presence of oral disease or abnormalities), coagulation status, and dental treatment indications. Results: The patients from SG exhibited a greater incidence of oral manifestations compared with CG (p=0.0327) and were diagnosed with at least one oral disease or condition that required treatment. Coagulation abnormalities reflecting an increased risk of bleeding were found in 93.75% of the patients. However, no bleeding complications occurred after dental treatment. Conclusions: The patients with chronic liver diseases evaluated in this study exhibited a higher incidence of oral manifestations compared with the control group and had at least one oral disease or abnormality which required dental treatment prior to liver transplantation. Careful oral examination and evaluation of the patient, including laboratory tests, will ensure correct oral preparation and control of oral disease prior to liver transplantation

    Adenocarcinoma polimorfo de baixo grau: relato de caso clínico

    O adenocarcinoma polimorfo de baixo grau (APBG) é uma neoplasia maligna incomum em região de cabeça e pescoço e que ocorre quase exclusivamente em glândulas salivares menores. Esta lesão apresenta baixo potencial biológico de malignidade e tem como principais diagnósticos diferenciais tanto clínico quanto histológico o adenoma pleomórfico e o carcinoma adenoide cístico. Descrevemos um caso de APBG em um paciente do sexo masculino com 59 anos de idade, leucoderma e que apresentava um nódulo assintomático de aproximadamente 10 dias de evolução em palato mole. Foi realizada uma biópsia incisional e o exame microscópico confirmou o diagnóstico de APBG. O tratamento dessas lesões envolve a completa excisão cirúrgica e raramente há recorrência ou desenvolvimento de doença metastática. Após três meses da excisão cirúrgica, o paciente não apresentou recorrências

    The question whether taxonomic descriptions naming new animal species without type specimen(s) deposited in collections should be accepted for publication by scientific journals and allowed by the Code has already been discussed in Zootaxa (Dubois & Nemésio 2007; Donegan 2008, 2009; Nemésio 2009a–b; Dubois 2009; Gentile & Snell 2009; Minelli 2009; Cianferoni & Bartolozzi 2016; Amorim et al. 2016). This question was again raised in a letter supported by 35 signatories published in the journal Nature (Pape et al. 2016) on 15 September 2016. On 25 September 2016, the following rebuttal (strictly limited to 300 words as per the editorial rules of Nature) was submitted to Nature, which on 18 October 2016 refused to publish it. As we think this problem is a very important one for zoological taxonomy, this text is published here exactly as submitted to Nature, followed by the list of the 493 taxonomists and collection-based researchers who signed it in the short time span from 20 September to 6 October 2016

    Oral manifestations in patients with hypogammaglobulinemia

    A hipogamaglobulinemia é uma alteração da imunidade humoral caracterizada por baixos níveis séricos de anticorpos podendo ter causas primárias e secundárias. Estes pacientes apresentam uma susceptibilidade a infecções bacterianas de repetição ou crônicas, principalmente do trato respiratório. Além disso, há alta prevalência de doenças gastrointestinais infecciosas e inflamatórias, hepatite C, doenças auto-imunes, doenças linfoproliferativas e granulomatosas. Ainda que a doença seja conhecida desde 1954 há poucos trabalhos na literatura sobre manifestações bucais nestes pacientes. Alguns estudos sugerem uma maior prevalência de lesões liquenóides, doença periodontal, candidíase pseudomembranosa, úlcera aftosa recorrente e hipoplasia de esmalte. Desta forma, o objetivo deste trabalho foi avaliar a prevalência das manifestações e alterações bucais, e caracterizar a saúde bucal de pacientes com hipogamaglobulinemias e correlacioná-las com o estado imunológico do paciente, comparativamente a um grupo de pacientes saudáveis. Para tanto avaliamos 100 pacientes com hipogamaglobulinemias atendidos no Ambulatório de Imunologia e Alergia do Hospital das Clínicas da Faculdade de Medicina da Universidade de São Paulo e 93 pacientes normorreativos da Clínica Odontológica da Faculdade de Odontologia da Universidade de São Paulo, sendo realizado exame clínico bucal, anamnese e compilação de exames laboratoriais recentes. Do total de pacientes com hipogamaglobulinemias, 59 pacientes apresentaram alguma alteração bucal, sendo as mais frequentes lesões de cárie (21), hipoplasia de esmalte (21), gengivite (18), periodontite (8), boca seca (6), língua geográfica (5) e úlcera aftosa recorrente (2). Vinte e sete pacientes apresentaram queixas de úlcera aftosa recorrente com frequência. Dos 93 pacientes do grupo controle, 84 (90,3%) apresentaram alguma alteração bucal, sendo as mais frequentes lesões de cárie (66), gengivite (31), periodontite (44) e candidíase (3). Dezoito pacientes apresentaram queixas de úlcera aftosa recorrente com freqüência. Concluímos que os pacientes com hipogamaglobulinemias, apesar de apresentarem diminuição das imunoglobulinas do sangue, e alguns pacientes apresentarem uma diminuição de células da imunidade celular, não se encontrou relação positiva entre a incidência de cárie e IgA, doença periodontal e IgA e doença periodontal e CD4.The hypogammaglobulinemia is an alteration in humoral immunity characterized by low levels of antibodies may have primary and secondary causes. These patients have a susceptibility to recurrent bacterial infections or chronic diseases, mainly respiratory tract. Moreover, there is a high prevalence of infectious and inflammatory gastrointestinal diseases, hepatitis C, autoimmune diseases, lymphoproliferative diseases and granulomatous disease. Although the disease is known since 1954 there are few available studies on oral manifestations in these patients. Some studies suggest a higher prevalence of lichenoid lesions, periodontal disease, pseudomembranous candidiasis, recurrent aphthous ulcer and enamel hypoplasia. Thus, the objective was to assess the prevalence of manifestations diseases and to characterize the oral health of patients with hypogammaglobulinemia and correlate with the immune status, compared with healthy patients. For that evaluated 100 patients with hypogammaglobulinemia in the Outpatient Immunology and Allergy Hospital of the Faculty of Medicine, University of São Paulo and 93 healthy patients of School of Dentistry, University of São Paulo, and was conducted oral clinical examination, medical history and compilation of recent laboratory tests. Of all patients with hypogammaglobulinemia, 59 patients showed abnormalities of the mouth, with 21 patients exhibited caries, 21 enamel hypoplasia, 18 gingivitis, 8 periodontitis, 6 dry mouth, 5 geographic tongue and 2 recurrent aphthous ulcer. Twenty-seven patients complained of frequent recurrent aphthous ulcer. Of the 93 control group patients, 84 (90.3%) showed abnormalities, being the most frequent caries (66), gingivitis (31), periodontitis (44) and candidiasis (3). Eighteen patients complained of recurrent aphthous ulcers frequently. We conclude that patients with hypogammaglobulinemia, despite showing decreased blood immunoglobulins, and some patients had a decrease of cells in cellular immunity, we did not find a positive relationship between the incidence of caries and IgA, IgA and periodontal disease and periodontal disease and CD4

    Post-extraction wound healing in patients with type 2 diabetes

    A hiperglicemia, bem como o diabetes, é reconhecida como fator de risco para infecções pós-cirúrgicas, assim manter o controle glicêmico perioperatório tem sido padrão de cuidado em saúde. Entretanto há poucos estudos sobre o impacto do controle glicêmico no processo de reparação de cirurgias de extração dentária. Apesar da escassez de estudos e de evidências clínicas e científicas que investiguem o risco de infecções pós-exodônticas em pessoas com diabetes, existem livros, artigos e guias que recomendam aos dentistas o uso de antibióticos profiláticos nesses indivíduos, especialmente para aqueles descompensados. O objetivo deste estudo foi avaliar clinicamente a cicatrização pós-exodôntica, em relação à cronologia dos eventos reparacionais e em relação à ocorrência de complicações pós-operatórias, em indivíduos com diabetes tipo 2 comparados a um grupo controle. Além disso, procuramos relacionar os eventos pós-cirúrgicos com o controle metabólico e com o perfil imunológico dos pacientes. Este estudo prospectivo longitudinal caso controle incluiu 53 indivíduos com diabetes tipo 2 (grupo de estudo=GE) e 29 indivíduos sem diabetes (grupo controle=GC). Foi aplicado questionário sobre a história médica, realizados exames clínicos intraoral e extraoral e exames laboratoriais para conhecimento do controle glicêmico e do perfil imunológico do indivíduo, tais como, hemograma completo, hemoglobina glicada (A1C), glicemia de jejum, IgA, IgG, IgM, C3, C4, linfócitos T CD3+, CD4+, CD8+, quimiotaxia de neutrófilos, oxidação de neutrófilos, fagocitose de neutrófilos e monócitos. Todos os participantes foram submetidos a extrações padronizadas de dentes erupcionados e as avaliações clínicas foram realizadas 3, 7, 21 e 60 dias após a cirurgia. A frequência de complicações pós-operatórias foi maior no GC, no qual 7 de 29 pacientes (24%) exibiu pelo menos uma complicação a saber: trismo (1 caso), queixa de paladar desagradável (3 casos), inapetência (1 caso) e queixa de indisposição (2 casos) (p=0,005). No GE apenas 1 sujeito (1,9%) exibiu complicação 13 uma vez que queixou-se de paladar desagradável, apesar de 36 (67,9%) pacientes desse grupo apresentarem hiperglicemia no momento da cirurgia, da maioria estar com o diabetes descompensado (A1C>6,5%) e apesar da maioria apresentar diminuição da quimiotaxia dos neutrófilos. Apesar da ocorrência de complicações, nenhum participante, de nenhum grupo, precisou tomar antibiótico e após 60 dias da exodontia todos os alvéolos dentais estavam reparados. O atraso na epitelização da ferida cirúrgica foi observado apenas no GE, onde 9 (17%) casos apresentaram incompleta epitelização nesse tempo pós-operatório (p=0,023). Não houve relação deste atraso com nível de A1C, diminuição da quimiotaxia de neutrófilos, diminuição da fagocitose dos neutrófilos e nem com a diminuição de oxidação dos neutrófilos. Nossos resultados permitiram concluir que o diabetes tipo 2 não representa um fator de risco para ocorrência de complicações pós-exodônticas. Podemos inferir que pessoas com diabetes tem risco aumentado de atraso na epitelização da ferida cirúrgica, mas que esse atraso não tem impacto na reparação final do alvéolo.Hyperglycemia and diabetes are recognized as a risk factor for postoperative infections. Thus, maintaining perioperative glucose control has become the standard of care. However, there are scarce data on the appropriate glucose control during minor dental surgery. Despite the paucity of studies investigating the risk of postsurgical oral infections in persons with diabetes, there are text books, papers and guidelines recommending to dentists the use of prophylactic antibiotics for patients with poorly controlled diabetes undergoing invasive oral procedures. The aim of this study was to clinically evaluate the post extraction healing regarding to the chronology and to the occurrence of postoperative complications in patients with type 2 diabetes compared to a control group. Additionally, we associated the postoperative events with metabolic control and the immunological profile of the participants. This prospective case-control study included 53 subjects with type 2 diabetes (SG) and 29 controls without diabetes (CG). A questionnaire on medical history was applied, intraoral and extraoral clinical examinations were conducted and laboratory tests for glycemic control and immunological profile of the individual, such as complete blood count, glycated hemoglobin (A1C), fasting glucose, IgA, IgG, IgM, C3, C4, lymphocytes T CD3+, CD4+, CD8+, neutrophil chemotaxis, oxidation of neutrophil phagocytosis by neutrophils and monocytes, were obtained at the time of the extraction. All participants underwent standardized extractions of erupted teeth and clinical assessments were performed 3, 7, 21 and 60 days after surgery. The frequency of postoperative complications was higher in CG, where 7 of 29 patients (24%) exhibited at least one complication such as: trismus (1 case), complaints of unpleasant taste (3 cases), anorexia (1 case), and malaise (2 cases) (p=0.005). Only one patient from SG (1.9%) complained of bad taste in mouth despite 36 (67,9%) among the subjects from this group had hyperglycemia at the time of surgery, the majority present uncontrolled diabetes (A1C>6.5%), and despite most of them 15 present decreased neutrophils chemotaxis. Regardless of the occurrence of some complications, no participant in any group had to take antibiotics, and after 60 days of extraction all dental alveoli were repaired. The delay in epithelialization of the wound was observed only in SG, where 9 (17%) cases showed incomplete epithelialization at 21 days after surgery (p=0.023). There was no relationship with the epithelization delay and values of A1C, decreased neutrophil chemotaxis, decrease of neutrophil phagocytosis or with decreased oxidation of neutrophils. Our results showed that type 2 diabetes is not a risk factor for the occurrence of postoperative dental extractions. We can infer that people with diabetes have increased risk of delayed epithelialization of the surgical wound but that this delay has no impact on the final repair of the alveoli

    Oral manifestations in patients with hypogammaglobulinemia

    Objective. The overall objective of this study was to assess the oral manifestations and their association with immunologic status and health history, of individuals with hypogammaglobulinemia. Study Design. A case-controlled study of 100 subjects with hypogammaglobulinemia and 93 control individuals was performed. All participants were examined for dental caries, periodontal disease, mucosal lesions/infections, and general oral health problems. Decayed, missing, filled teeth and community periodontal index were recorded. Complete blood count, serum immunoglobulins, and lymphocyte immunophenotyping were measured on the same day of the oral health assessment. Results. Individuals with hypogammaglobulinemia showed higher prevalence of enamel hypoplasia and complaints of dry mouth, and lower prevalence of dental caries and periodontal disease. Conclusions. The systemic conditions associated with hypogammaglobulinemia were not associated with enhanced susceptibility to caries, gingivitis, or periodontitis; however, individuals with hypogammaglobulinemia were more likely to report more episodes of recurrent aphthous ulcers compared with control individuals.     Desempenho cognitivo e transtornos mentais em indivíduos obesos do sul de Santa Catarina

    Obesity is characterized as a serious public health problem, since it is a chronic, multifactorial disease associated with the development of several comorbidities. The pathophysiology of obesity includes alterations in peripheral tissues and also in the Central Nervous System; such as the hypothalamus, hippocampus, and prefrontal cortex, which relate respectively to the center of satiety, memory and learning, and the reward system. In this context, it is necessary to seek to understand the relationship between obesity and cognitive impairment and mental disorders, such as anxiety and depression. In this way, the aim of the present study was to analyze the relationship between cognitive performance and mental disorders in obese and eutrophic patients. For this, control case study was performed, which 30 obese patients (BMI> 30) and 30 eutrophic volunteers (BMI 30) e 30 voluntários eutróficos (IMC 30) and 30 eutrophic volunteers (BMI <30) were evaluated. The case group and the control group participated in the application of a socio-demographic questionnaire, three cognitive evaluation tests: Digits, Sequence of Numbers and Letters, and Wasi, which aims to measure the Intellectual Quotient and respond to an inventory of anxiety and depression. The control group was matched according to age, sex and schooling. All the results were compared and analyzed between groups. Obese patients presented a higher score in the digit tests and Wasi test. There was no statistical difference between the groups in the test sequence of numbers and letters. Patients in the obese group had higher scores on the anxiety and depression inventory scores compared to the eutrophic group. In conclusion, obesity is related to a lower performance in cognitive tests and an increase in the symptoms of anxiety and depression
