Online training and support programs designed to improve mental health and reduce burden among caregivers of people with dementia : a systematic review
Introduction -- Dementia poses a considerable socioeconomic burden to society. On a global scale, family and other unpaid care predominates. Supporting caregivers is crucial, but scalable interventions are currently lacking. Because a growing number of studies have suggested that online training and support programs hold considerable promise for scaling up, we reviewed existing literature. Methods -- We systematically searched 6 databases to identify studies of Internet-based interventions designed to train and support caregivers of people with dementia, and we formally assessed risk of bias. Our prespecified primary outcomes of interest included both mental health and caregiver burden/perceived stress. Our secondary outcomes of interest included knowledge, quality of life of caregivers, quality of care, caregiver response to challenging behaviors, coping, and self-efficacy. Results -- Eight randomized control trials met our inclusion criteria involving over 900 participants. The content and structure of Internet-based interventions, outcome measures, and duration differed markedly, and selection, performance, and reporting biases were varied and on occasion of concern. Six studies reported outcomes in caregivers' mental health outcomes, 3 studies reported burden outcomes. Three studies reported knowledge skills, quality of life and reaction to challenging behaviours, whereas 2 studies reported changes in coping outcomes and self-efficacy. No studies reported outcomes on quality of care. Discussion -- Although there is some evidence that Internet-based interventions can improve mental health outcomes for informal caregivers of people with dementia, marked methodological diversity across studies prevented the robust pooling of the results. A concerted and cohesive approach from all stakeholders is now required to help realize the full potential of this emerging field

Communicative processes of individuals with injuries of the right cerebral hemisphere
Objective: This study aimed to profile the language skills of patients with damage to the right cerebral hemisphere attended at the Ibirapuera Unit (Central) of the Association for the Welfare of Handicapped Children - AACD (Associação de Assistência à Criança Deficiente - AACD/Unidade Ibirapuera - Central), and to discover the perceptions of caregivers and patients regarding the presence or absence of language disorders after a stroke. Method: The descriptive study was conducted from July to September of 2009 with 11 adults through the application of the Montreal Communication Evaluation Battery (known in Brazil as Bateria MAC) tests, the Questionnaire on Awareness of Difficulties, and the Communicative Disorders Screening on individuals with neurological conditions, direct relatives, and/or caregivers. Results: It was found that 90.9% of patients with injuries of the right cerebral hemisphere had a deficit in at least one of the tests comprising the Bateria MAC assessment of language. Conclusion: Findings showing the patient’s absence of awareness of their linguistic and cognitive deficits are also very important, not only about their daily life activities, but about their agnosia.Objetivo: O presente estudo teve como objetivo traçar o perfil de linguagem de pacientes com lesão de hemisfério direito atendidos na Associação de Assistência a Criança Deficiente (AACD/Unidade Ibirapuera - Central), bem como verificar a percepção dos cuidadores e dos pacientes em relação à presença ou não de alterações de linguagem pós-acidente vascular cerebral. Método: O estudo descritivo foi desenvolvido de julho a setembro de 2009 com 11 indivíduos adultos por meio da aplicação da Bateria Montreal de Avaliação da Comunicação/Bateria MAC, prova Questionário sobre a Consciência das Dificuldades e o questionário Triagem de Distúrbios Comunicativos em Indivíduos com quadro neurológico, direcionado aos familiares e/ou cuidadores. Resultados: Verificou-se que 90,9% dos pacientes com Lesão de Hemisfério Direito apresentaram déficit em pelo menos uma das provas que compunham a Bateria MAC de avaliação da linguagem. Conclusão: Mostra-se também de extrema importância o achado relacionado à ausência de percepção dos pacientes em relação as suas próprias alterações linguístico-cognitivas, não pela inexistência de impactos em sua vida diária, mas devido à agnosia
The SPLASH Survey: A Spectroscopic Portrait of Andromeda's Giant Southern Stream
The giant southern stream (GSS) is the most prominent tidal debris feature in
M31's stellar halo. The GSS is composed of a relatively metal-rich, high
surface-brightness "core" and a lower metallicity, lower surface brightness
"envelope." We present Keck/DEIMOS spectroscopy of red giant stars in six
fields in the vicinity of M31's GSS and one field on Stream C, an arc-like
feature on M31's SE minor axis at R=60 kpc. Several GSS-related findings and
measurements are presented here. We present the innermost kinematical detection
of the GSS core to date (R=17 kpc). This field also contains the continuation
of a second kinematically cold component originally seen in a GSS core field at
R=21 kpc. The velocity gradients of the GSS and the second component in the
combined data set are parallel over a radial range of 7 kpc, suggesting a
possible bifurcation in the line-of-sight velocities of GSS stars. We also
present the first kinematical detection of substructure in the GSS envelope.
Using kinematically identified samples, we show that the envelope debris has a
~0.7 dex lower mean photometric metallicity and possibly higher intrinsic
velocity dispersion than the GSS core. The GSS is also identified in the field
of the M31 dSph satellite And I; the GSS in this field has a metallicity
distribution identical to that of the GSS core. We confirm the presence of two
kinematically cold components in Stream C, and measure intrinsic velocity
dispersions of ~10 and ~4 km/s. This compilation of the kinematical (mean
velocity, intrinsic velocity dispersion) and chemical properties of stars in
the GSS core and envelope, coupled with published surface brightness
measurements and wide-area star-count maps, will improve constraints on the
orbit and internal structure of the dwarf satellite progenitor.Comment: Accepted for publication in Ap
Características dos pacientes reinternados em unidade de terapia intensiva / Characteristics of readmitted patients in the intensive care unit
Objetivo: Descrever as características dos pacientes reinternados no serviço de terapia intensiva adulto e sua distribuição entre os anos de 2017 e 2019. Método: Trata-se de um estudo retrospectivo de análise documental, com abordagem quantitativa, realizado entre os anos de 2017 a 2019. Resultados: A amostra constituiu-se de 94 pacientes, sendo 60,6% do sexo masculino e 61,7% idosos. Insuficiência respiratória aguda (23,4%) e choque séptico de foco pulmonar (19,1%) foram as maiores causas de reinternação. 59,6% dos pacientes foram a óbito e com maior ocorrência nos meses de setembro e maio. Os números de reinternações foram maiores em maio, julho e setembro. Conclusões: Constata-se que o desfecho das taxas de reinternação resultou majoritariamente em óbito de pacientes em sua maioria idosos e homens, Não foi encontrado nenhuma associação bivariada relevante ao óbito, a maioria dos pacientes são de procedência cirúrgica e reinternam mais nos meses de maio, julho e setembro, com diagnósticos de insuficiência respiratória e sepse pulmonar

LSST: from Science Drivers to Reference Design and Anticipated Data Products
(Abridged) We describe here the most ambitious survey currently planned in
the optical, the Large Synoptic Survey Telescope (LSST). A vast array of
science will be enabled by a single wide-deep-fast sky survey, and LSST will
have unique survey capability in the faint time domain. The LSST design is
driven by four main science themes: probing dark energy and dark matter, taking
an inventory of the Solar System, exploring the transient optical sky, and
mapping the Milky Way. LSST will be a wide-field ground-based system sited at
Cerro Pach\'{o}n in northern Chile. The telescope will have an 8.4 m (6.5 m
effective) primary mirror, a 9.6 deg field of view, and a 3.2 Gigapixel
camera. The standard observing sequence will consist of pairs of 15-second
exposures in a given field, with two such visits in each pointing in a given
night. With these repeats, the LSST system is capable of imaging about 10,000
square degrees of sky in a single filter in three nights. The typical 5
point-source depth in a single visit in will be (AB). The
project is in the construction phase and will begin regular survey operations
by 2022. The survey area will be contained within 30,000 deg with
, and will be imaged multiple times in six bands, ,
covering the wavelength range 320--1050 nm. About 90\% of the observing time
will be devoted to a deep-wide-fast survey mode which will uniformly observe a
18,000 deg region about 800 times (summed over all six bands) during the
anticipated 10 years of operations, and yield a coadded map to . The
remaining 10\% of the observing time will be allocated to projects such as a
Very Deep and Fast time domain survey. The goal is to make LSST data products,
including a relational database of about 32 trillion observations of 40 billion

Impact of COVID-19 on cardiovascular testing in the United States versus the rest of the world
Objectives: This study sought to quantify and compare the decline in volumes of cardiovascular procedures between the United States and non-US institutions during the early phase of the coronavirus disease-2019 (COVID-19) pandemic.
Background: The COVID-19 pandemic has disrupted the care of many non-COVID-19 illnesses. Reductions in diagnostic cardiovascular testing around the world have led to concerns over the implications of reduced testing for cardiovascular disease (CVD) morbidity and mortality.
Methods: Data were submitted to the INCAPS-COVID (International Atomic Energy Agency Non-Invasive Cardiology Protocols Study of COVID-19), a multinational registry comprising 909 institutions in 108 countries (including 155 facilities in 40 U.S. states), assessing the impact of the COVID-19 pandemic on volumes of diagnostic cardiovascular procedures. Data were obtained for April 2020 and compared with volumes of baseline procedures from March 2019. We compared laboratory characteristics, practices, and procedure volumes between U.S. and non-U.S. facilities and between U.S. geographic regions and identified factors associated with volume reduction in the United States.
Results: Reductions in the volumes of procedures in the United States were similar to those in non-U.S. facilities (68% vs. 63%, respectively; p = 0.237), although U.S. facilities reported greater reductions in invasive coronary angiography (69% vs. 53%, respectively; p < 0.001). Significantly more U.S. facilities reported increased use of telehealth and patient screening measures than non-U.S. facilities, such as temperature checks, symptom screenings, and COVID-19 testing. Reductions in volumes of procedures differed between U.S. regions, with larger declines observed in the Northeast (76%) and Midwest (74%) than in the South (62%) and West (44%). Prevalence of COVID-19, staff redeployments, outpatient centers, and urban centers were associated with greater reductions in volume in U.S. facilities in a multivariable analysis.
Conclusions: We observed marked reductions in U.S. cardiovascular testing in the early phase of the pandemic and significant variability between U.S. regions. The association between reductions of volumes and COVID-19 prevalence in the United States highlighted the need for proactive efforts to maintain access to cardiovascular testing in areas most affected by outbreaks of COVID-19 infection
