    Le statut logique de la notion de "signification-stimulus" chez Quine

    Dans Word and Object, Quine propose explicitement une définition de la notion de signification-stimulus dans les termes du behaviorisme. Cette caractérisation est commandée par son naturalisme, qui consiste à faire de la philosophie une partie de la science empirique. Il est néanmoins généralement admis que la science entend décrire les phénomènes tandis qu’à la suite du tournant linguistique, la philosophie se concentre sur le langage. Dans ces conditions, il est légitime de chercher à déterminer le statut logique que l’on doit assigner à la notion de signification-stimulus chez Quine. Nous présentons à cet effet trois procédures langagières bien distinctes dans la littérature. Nous ébauchons ensuite le cadre théorique dans lequel se situe la notion de signification-stimulus avant d’en proposer une reconstruction rationnelle en termes neurophysiologiques. Ces considérations apportées, nous montrons que la caractérisation que Quine donne de cette notion constitue non une définition, mais une explication

    Une durée de séjour (DDS) prolongée, estimée représenter 20% des jours en soins aigus, entraîne une augmentation des complications, de la morbidité, de la mortalité et des coûts. Nous avons réalisé une étude de cohorte rétrospective multicentrique avec 11 119 patients victimes d'un traumatisme craniocérébral (TCC) sortis vivants d'un des 57 centres de traumatologie du Québec entre 2007 et 2012 afin de comparer la DDS index et la DDS totale puis d'identifier les déterminants de la DDS. La DDS en soins aigus est légèrement sous-estimée lorsque seule la DDS index est considérée. L'utilisation des ressources est plus importante chez les patients victimes d'un TCC que chez les admissions provinciales générales. Huit variables reliées aux caractéristiques des patients ou aux traitements expliquent 80% de la variation de la DDS. Ces informations peuvent être utilisées afin d'améliorer la qualité et l'efficacité des soins aigus chez ces patients.Prolonged length of stay (LOS), estimated to represent approximately 20% of acute care days, leads to adverse outcomes in terms of complications, morbidity, mortality, and costs. However, we lack comprehensive information on hospital resource use for patients admitted following traumatic brain injury (TBI). We conducted a multicenter, retrospective cohort study based on 11,199 patients discharged alive from the Quebec trauma system between 2007 and 2012 in order to compare acute care index LOS to total LOS and identify the determinants of LOS. Acute care LOS is slightly underestimated when only index LOS is considered. TBI patients have much higher resource use than general provincial admissions. Eight variables related either to patients’ characteristics or treatments explain 80% of the variation in LOS. This information can be used to develop targeted interventions to reduce LOS for TBI admissions to improve the efficiency and quality of acute care for these patients

    L'auteur entreprend dans ce mémoire de proposer une interprétation formaliste de la signification et du statut logique de la critique que mène Quine, dans son article Two Dogmas of Empiricism de 1951, à l'encontre de la distinction classique en philosophie entre les énoncés analytiques et synthétiques. Contre l'interprétation standard qui persiste à concevoir cette critique comme un rejet catégorique de la distinction, l'auteur reconstruit la théorie de la signification quinienne afin de relever la distinction renouvelée qu'introduit Quine entre "analytique" et "synthétique" dans son dernier ouvrage, From Stimulus to Science. Par une étude comparative de la conception de Quine et de celles de Churchland et Searle sont spécifiés le sens et le statut que l'on doit assigner à sa notion de "signification". Réinterprétée à la lumière de ces considérations, la critique quinienne se révèle être une remise en question de niveau métaépistémologique de la pratique philosophique de l'empirisme logique.In this dissertation, the author proposes a formalist interpretation of the meaning and logical status of the critique that Quine put forward against the classical philosophic distinction between analytic and synthetic statements in his 1951 article Two Dogmas of Empiricism. Against the standard interpretation that prevails in the literature, according to which this critique is categorical and without appeal, the author undertakes to rationally reconstruct Quine's theory of meaning in order to reveal the renewed distinction between "analytic" and "synthetic" as it was introduced in his last book, From Stimulus to Science (1995). The meaning and logical status that ought to be assigned to his notion of "meaning" are specified by means of a comparative study between Quine's own conception and that of Churchland and Searle. Reinterpreted in the light of these considerations, the Quinian critique proves to be a metaepistemological reassessment of the logical empiricism's philosophical practice

    Validating intravascular imaging with serial optical coherence tomography and confocal fluorescence microscopy

    Atherosclerotic cardiovascular diseases are characterized by the formation of a plaque in the arterial wall. Intravascular ultrasound (IVUS) provides high-resolution images allowing delineation of atherosclerotic plaques. When combined with near infrared fluorescence (NIRF), the plaque can also be studied at a molecular level with a large variety of biomarkers. In this work, we present a system enabling automated volumetric histology imaging of excised aortas that can spatially correlate results with combined IVUS/NIRF imaging of lipid-rich atheroma in cholesterol-fed rabbits. Pullbacks in the rabbit aortas were performed with a dual modality IVUS/NIRF catheter developed by our group. Ex vivo three-dimensional (3D) histology was performed combining optical coherence tomography (OCT) and confocal fluorescence microscopy, providing high-resolution anatomical and molecular information, respectively, to validate in vivo findings. The microscope was combined with a serial slicer allowing for the imaging of the whole vessel automatically. Colocalization of in vivo and ex vivo results is demonstrated. Slices can then be recovered to be tested in conventional histology

    Impact of Trauma System Structure on Injury Outcomes : A Systematic Review and Meta-Analysis

    The effectiveness of trauma systems in decreasing injury mortality and morbidity has been well demonstrated. However, little is known about which components contribute to their effectiveness. We aimed to systematically review the evidence of the impact of trauma system components on clinically important injury outcomes. We searched MEDLINE, EMBASE, Cochrane CENTRAL, and BIOSIS/Web of Knowledge, gray literature and trauma association Web sites to identify studies evaluating the association between at least one trauma system component and injury outcome. We calculated pooled effect estimates using inverse-variance random-effects models. We evaluated quality of evidence using GRADE criteria. We screened 15,974 records, retaining 41 studies for qualitative synthesis and 19 for meta-analysis. Two recommended trauma system components were associated with reduced odds of mortality: inclusive design (odds ratio [OR] = 0.72 [0.65-0.80]) and helicopter transport (OR = 0.70 [0.55-0.88]). Pre-Hospital Advanced Trauma Life Support was associated with a significant reduction in hospital days (mean difference [MD] = 5.7 [4.4-7.0]) but a nonsignificant reduction in mortality (OR = 0.78 [0.44-1.39]). Population density of surgeons was associated with a nonsignificant decrease in mortality (MD = 0.58 [-0.22 to 1.39]). Trauma system maturity was associated with a significant reduction in mortality (OR = 0.76 [0.68-0.85]). Quality of evidence was low or very low for mortality and healthcare utilization. This review offers low-quality evidence for the effectiveness of an inclusive design and trauma system maturity and very-low-quality evidence for helicopter transport in reducing injury mortality. Further research should evaluate other recommended components of trauma systems and non-fatal outcomes and explore the impact of system component interactions.Peer reviewe

    Low-value clinical practices in adult traumatic brain injury : an umbrella review

    Despite numerous interventions and treatment options, the outcomes of traumatic brain injury (TBI) have improved little over the last 3 decades, which raises concern about the value of care in this patient population. We aimed to synthesize the evidence on 14 potentially low-value clinical practices in TBI care. Using umbrella review methodology, we identified systematic reviews evaluating the effectiveness of 14 potentially low-value practices in adults with acute TBI. We present data on methodological quality (Assessing the Methodological Quality of Systematic Reviews), reported effect sizes, and credibility of evidence (I to IV). The only clinical practice with evidence of benefit was therapeutic hypothermia (credibility of evidence II to IV). However, the most recent meta-analysis on hypothermia based on high-quality trials suggested harm (credibility of evidence IV). Meta-analyses on platelet transfusion for patients on antiplatelet therapy were all consistent with harm but were statistically non-significant. For the following practices, effect estimates were consistently close to the null: computed tomography (CT) in adults with mild TBI who are low-risk on a validated clinical decision rule; repeat CT in adults with mild TBI on anticoagulant therapy with no clinical deterioration; antibiotic prophylaxis for external ventricular drain placement; and decompressive craniectomy for refractory intracranial hypertension. We identified five clinical practices with evidence of lack of benefit or harm. However, evidence could not be considered to be strong for any clinical practice as effect measures were imprecise and heterogeneous, systematic reviews were often of low quality, and most included studies had a high risk of bias

The reconstruction of tracks, leptons, photons, missing energy and jets is investigated, together with the performance of b-tagging and the trigger. The physics potential for a variety of interesting physics processes, within the Standard Model and beyond, is examined. The study comprises a series of notes based on simulations of the detector and physics processes, with particular emphasis given to the data expected from the first years of operation of the LHC at CERN

    OBJECTIVE: Proinsulin is a precursor of mature insulin and C-peptide. Higher circulating proinsulin levels are associated with impaired β-cell function, raised glucose levels, insulin resistance, and type 2 diabetes (T2D). Studies of the insulin processing pathway could provide new insights about T2D pathophysiology. RESEARCH DESIGN AND METHODS: We have conducted a meta-analysis of genome-wide association tests of ∼2.5 million genotyped or imputed single nucleotide polymorphisms (SNPs) and fasting proinsulin levels in 10,701 nondiabetic adults of European ancestry, with follow-up of 23 loci in up to 16,378 individuals, using additive genetic models adjusted for age, sex, fasting insulin, and study-specific covariates. RESULTS: Nine SNPs at eight loci were associated with proinsulin levels (P < 5 × 10(-8)). Two loci (LARP6 and SGSM2) have not been previously related to metabolic traits, one (MADD) has been associated with fasting glucose, one (PCSK1) has been implicated in obesity, and four (TCF7L2, SLC30A8, VPS13C/C2CD4A/B, and ARAP1, formerly CENTD2) increase T2D risk. The proinsulin-raising allele of ARAP1 was associated with a lower fasting glucose (P = 1.7 × 10(-4)), improved β-cell function (P = 1.1 × 10(-5)), and lower risk of T2D (odds ratio 0.88; P = 7.8 × 10(-6)). Notably, PCSK1 encodes the protein prohormone convertase 1/3, the first enzyme in the insulin processing pathway. A genotype score composed of the nine proinsulin-raising alleles was not associated with coronary disease in two large case-control datasets. CONCLUSIONS: We have identified nine genetic variants associated with fasting proinsulin. Our findings illuminate the biology underlying glucose homeostasis and T2D development in humans and argue against a direct role of proinsulin in coronary artery disease pathogenesis

    SARS-CoV-2 susceptibility and COVID-19 disease severity are associated with genetic variants affecting gene expression in a variety of tissues

    Variability in SARS-CoV-2 susceptibility and COVID-19 disease severity between individuals is partly due to genetic factors. Here, we identify 4 genomic loci with suggestive associations for SARS-CoV-2 susceptibility and 19 for COVID-19 disease severity. Four of these 23 loci likely have an ethnicity-specific component. Genome-wide association study (GWAS) signals in 11 loci colocalize with expression quantitative trait loci (eQTLs) associated with the expression of 20 genes in 62 tissues/cell types (range: 1:43 tissues/gene), including lung, brain, heart, muscle, and skin as well as the digestive system and immune system. We perform genetic fine mapping to compute 99% credible SNP sets, which identify 10 GWAS loci that have eight or fewer SNPs in the credible set, including three loci with one single likely causal SNP. Our study suggests that the diverse symptoms and disease severity of COVID-19 observed between individuals is associated with variants across the genome, affecting gene expression levels in a wide variety of tissue types
