Molecular markers of sorafenib efficacy in patients with metastatic renal cell carcinoma (mRCC) are not available. The purpose of this study was to discover genetic markers of survival in patients with mRCC treated with sorafenib. Germline variants from 56 genes were genotyped in 295 patients with mRCC. Variant-overall survival (OS) associations were tested in multivariate regression models. Mechanistic studies were conducted to validate clinical associations. VEGFA rs1885657, ITGAV rs3816375, and WWOX rs8047917 (sorafenib arm), and FLT4 rs307826 and VEGFA rs3024987 (sorafenib and placebo arms combined) were associated with shorter OS. FLT4 rs307826 increased VEGFR-3 phosphorylation, membrane trafficking, and receptor activation. VEGFA rs1885657 and rs58159269 increased transcriptional activity of the constructs containing these variants in endothelial and RCC cell lines, and VEGFA rs58159269 increased endothelial cell proliferation and tube formation. FLT4 rs307826 and VEGFA rs58159269 led to reduced sorafenib cytotoxicity. Genetic variation in VEGFA and FLT4 could affect survival in sorafenib-treated patients with mRCC. These markers should be examined in additional malignancies treated with sorafenib and in other angiogenesis inhibitors used in mRCC. Significance: Clinical and mechanistic data identify germline genetic variants in VEGFA and FLT4 as markers of survival in patients with metastatic renal cell carcinoma.Peer reviewe

Abstract

Background

The recommendations outlined in the latest guidelines published by the Surgical Infection Society (SIS) and the Infectious Disease Society of America (IDSA) regarding the proper duration of antibiotic therapy in patients with intra-abdominal infections are limited and non-specific. This ambiguity is due mainly to the lack of clinical trials on the topic of optimal duration of therapy. It is well known that the overuse of antibiotics has several important consequences such as increased treatment costs, reduced clinical efficacy, and above all, the increased emergence of antibiotic-resistant pathogens. Ampicillin-Sulbactam is a commonly used "first line" antibiotic for intra-abdominal infections. Ertapenem and Ampicillin-sulbactam are recommended as primary treatment agents for localized peritonitis by both the SIS and IDSA guidelines.

Methods/Design

This study is a prospective multi-center randomized investigation. The study will be performed in the Departments of General, Emergency, and Transplant Surgery of Sant'Orsola-Malpighi University Hospital in Bologna, Italy, in the General Surgery Department of the Ospedali Riuniti of Bergamo, Italy, and in the Trauma and Emergency Surgery Department of Maggiore Hospital in Bologna, Italy, and will be conducted by all surgeons willing to participate in the study. The inclusion period of the study will take approximately two years before the planned number of 142 enrolled patients is reached.

Discussion

Ertapenem and Ampicillin-sulbactam are recommended both as primary treatment agents for localized peritonitis by both the SIS and IDSA guidelines. As one of the discussed topic is the optimal duration of the antibiotic therapy and this ambiguity is due mainly to the lack of clinical trials on the topic, the present study aims for obtain precise data.

Trial Registration

ClinicalTrials.gov: NCT00630513</p

This paper presents the data obtained in the framework of the research project UBACyT 2011-2014: “Attitudes regarding professional ethics of the university undergraduate and graduate students, Universidad de Buenos Aires’ School of Psychology. A descriptive exploratory study based on a qualitative and quantitative research method. †This project, directed by PhD. Elizabeth B. Ormart, aims to survey the attitudes regarding professional ethics of the undergraduate students of the Psychology Program, Universidad de Buenos Aires. It is stated that based only on the students’ attitudes is where we can think about what kind of adjustments are needed to educate professionals ethically committed to their profession. To achieve the goal of the investigation, the Ethical Attitudes Scale Hirsch et al. -originally developed in Spanish and validated with Spanish university population was adapted and put into practice. This scale, which is based on the model of the theory of reasoned action by Fishbein and Ajzen (Escámez, 1988 and 1991) is composed of 55 statements referred to four skills: cognitive, social, ethical and affective-emotional. The sample consists of 781 students, of both genders, who belong to the University of Buenos Aires’ School of Psychology. Their ages range from 25 to 53 years. The survey results show that students of psychology of the UBA value primarily cognitive and technical skills, secondly, ethical competences, thirdly affective-emotional skills and, finally, social skills.Â

Modelling the Eddystone Lighthouse response to wave loading

    Get PDF
The Eddystone Lighthouse is an imposing granite structure that has guided mariners through the treacherous waters off the Plymouth coast for nearly 150 years. The General Lighthouse Authorities (GLAs) of the UK and Ireland, recognising the continuing importance of rock-mounted lighthouses as physical aids for navigation, funded a pilot project, commissioning Plymouth University to monitor the tower. The present study aims to provide more information on the structural behaviour of the Eddystone Lighthouse under the impacts from wave loading, through the utilisation of a 3D finite element model. Data from geophones, an offshore wave buoy and video cameras installed on the tower have been used to calibrate and validate the model; in particular, the wave that caused the maximum displacement during the winter 2013/2014 storms has been considered. The point of application of the wave load is important in the tower’s structural response; the lighthouse being especially vulnerable to larger displacements when the wave acts above its cylindrical base. Finite element analysis suggests that the lighthouse is stable with regard to material failure, and for failure mechanisms of overturning and sliding there are factors of safety of 6.3 and 8.0 respectively. A hypothetical unbroken wave of 17.5 m height would be required to overturn the lighthouse, and one of height 17 m would cause cracking at the base, but in such a location these waves would not be possible

Background: Emerging evidence indicates a potential role for monocytes in COVID-19 immunopathology. We investigated two soluble markers of monocyte activation, sCD14 and sCD163, in COVID-19 patients, with the aim of characterizing their potential role in monocyte-macrophage disease immunopathology. To the best of our knowledge, this is the first study of its kind. Methods: Fifty-nine SARS-Cov-2 positive hospitalized patients, classified according to ICU or non-ICU admission requirement, were prospectively recruited and analyzed by ELISA for levels of sCD14 and sCD163, along with other laboratory parameters, and compared to a healthy control group. Results: sCD14 and sCD163 levels were significantly higher among COVID-19 patients, independently of ICU admission requirement, compared to the control group. We found a significant correlation between sCD14 levels and other inflammatory markers, particularly Interleukin-6, in the non-ICU patients group. sCD163 showed a moderate positive correlation with the time lapsed from admission to sampling, independently of severity group. Treatment with corticoids showed an interference with sCD14 levels, whereas hydroxychloroquine and tocilizumab did not. Conclusions: Monocyte-macrophage activation markers are increased and correlate with other inflammatory markers in SARS-Cov-2 infection, in association to hospital admission. These data suggest a preponderant role for monocyte-macrophage activation in the development of immunopathology of COVID-19 patients

First Results from a Broadband Search for Dark Photon Dark Matter in the 4444 to 52μ52\,\mueV range with a coaxial dish antenna

We present first results from a dark photon dark matter search in the mass range from 44 to 52 μeV\mu{\rm eV} (10.712.5GHz10.7 - 12.5\,{\rm GHz}) using a room-temperature dish antenna setup called GigaBREAD. Dark photon dark matter converts to ordinary photons on a cylindrical metallic emission surface with area 0.5m20.5\,{\rm m}^2 and is focused by a novel parabolic reflector onto a horn antenna. Signals are read out with a low-noise receiver system. A first data taking run with 24 days of data does not show evidence for dark photon dark matter in this mass range, excluding dark photon - photon mixing parameters χ1012\chi \gtrsim 10^{-12} in this range at 90% confidence level. This surpasses existing constraints by about two orders of magnitude and is the most stringent bound on dark photons in this range below 49 μ\mueV.Comment: 7 pages, 4 figure
